Prevalence and risk factors for acquired long QT syndrome in the emergency department:a retrospective observational study

BACKGROUND: Long QT syndrome(LQTS) is a heterogeneous syndrome that may be congenital or, more frequently, acquired. The real-world prevalence of acquired LQTS(aLQTS) in the emergency department(ED) remains to be determined. The aim of this study was to determine prevalence of aLQTS and its impact on symptoms on ED ***: Electrocardiograms(ECG) of 5,056 consecutively patients admitted in the ED of a tertiary hospital between January 28th and March 17th of 2020 were reviewed. All patients with aLQTS were included. Clinical data with a focus on QT prolonging drugs and clinical factors were recorded. Statistical comparison was made between the groups with and without corrected QT(QTc) interval greater than 500 ms(value that is considered severely increased).RESULTS: A total of 383 ECGs with prolonged QTc were recognized, corresponding to a prevalence of aLQTS at admission of 7.82%. Patients with aLQTS were more commonly men(53.3%) with an age of(73.49±14.79) years old and QTc interval of(505.3±32.4) ms. Only 20.4% of these patients with aLQTS were symptomatic. No ventricular arrhythmias were recorded. Patients with QT interval greater than 500 ms were more frequently female(59.5%;P<0.001) and were more frequently on QT prolonging drugs(77.3%;P=0.025). Main contributing factor was intake of antibiotics(odds ratio [OR] 4.680) followed by female gender(OR 2.473) and intake of antipsychotics(OR 1.925).CONCLUSION: aLQTS is particularly prevalent in the ED. Female patients on antibiotics and antipsychotics are at particularly high risk. Efforts must be made to avoid, detect and treat aLQTS as early as possible.

Degradation processes affect a vast area of arid and semi-arid lands around the world and damage the environment and people′s health. Degradation processes are driven by human productive activities that cause direct and indirect effects on natural resources, such as species extinction at regional scale, reduction and elimination of vegetation cover, soil erosion, etc. In this context, ecological rehabilitation is an important tool to recover key aspects of the degraded ecosystem. Rehabilitation trials rely on the use of native plant species with characteristics that allow them to obtain high survival and growth rates. The aim of this work was to assess the survival and growth of native woody species in degraded areas of northeastern Patagonia and relate them to plant functional traits and environmental variables. We observed high early and late survival rates, and growth rates in Prosopis flexuosa DC. var. depressa F.A. Roig and Schinus johnstonii F.A. Barkley, and low values in Condalia microphylla Cav. and Geoffroea decorticans(Gillies ex Hook. & Arn.) Burkart. Early survival rates were positively associated with specific leaf area(SLA) and precipitation, but negatively associated with wood density, the maximum mean temperature of the warmest month and the minimum mean temperature of the coldest month. Late survival rates were positively associated with SLA and soil organic matter, but negatively associated with plant height and precipitation. The temperature had a positive effect on late survival rates once the plants overcame the critical period of the first summer after they were transplanted to the field. Prosopis flexuosa and S. johnstonii were the most successful species in our study. This could be due to their functional traits that allow these species to acclimatize to the local environment. In this paper, we provide a method based on quantiles to estimate the parameters of a finite mixture of Fréchet distributions, for a large sample of strongly dependent data. This is a situation that appears when dealing with environmental data and there was a real need of such method. We validate our approach by means of estimation and goodness-of-fit testing over simulated data, showing an accurate performance.

In this article we improve a goodness-of-fit test, of the Kolmogorov-Smirnov type, for equally distributed- but not stationary-strongly dependent data. The test is based on the asymptotic behavior of the empirical process, which is much more complex than in the classical case. Applications to simulated data and discussion of the obtained results are provided. This is, to the best of our knowledge, the first result providing a general goodness of fit test for non-weakly dependent data.

Freshwater reservoirs are essential owing because of their ecological, economic, and social *** are particularly vulnerable to contamination, as of metal and metalloids, derived from anthropogenic activities like mining. The temporal variations in trace element concentrations(arsenic(As),mercury(Hg), chromium(Cr), copper(Cu), nickel(Ni), lead(Pb), and zinc(Zn)), enrichment, fluxes, and possible sources were evaluated by studying two sediment cores from the La Angostura(ANG) Reservoir(northwest Mexico), using210Pb geochronology. The enrichment factors showed from null to minor enrichment for most elements, but moderate to severe enrichment of mercury(Hg). Most trace element concentrations had a detrital origin, and notable Hg concentration increases since the past decade were associated with severe drought periods, likely resulting from wildfires. The observed sediment concentrations of As and Hg can cause adverse effects on biota in the ecosystem since they are above the probable effect level(PEL). Development of strategies for metal attenuation in this reservoir is recommended and metals should be controlled until specific ecotoxicological studies are performed.
