Validation of the Portuguese version of the social isolation scale with a sample of community-dwelling older adults

Objectives The aim of this study was to analyze the psychometric properties(reliability and validity)of the Social Isolation Scale(SIS)in a sample of Portuguese community-dwelling older *** A transversal descriptive study was carried out with a convenience sample of 250 community-dwelling older adults(≥65 years).The survey comprised a sociodemographic questionnaire,SIS,Geriatric depression Scale-4(GdS-4),6-item Lubben Social Network Scale(LSNS-6),a loneliness self-assessment question,and Satisfaction with Life Scale(SWLS).Construct validity(confirmatory factor analysis)and convergent validity were analyzed,and ISI internal reliability(composite reliability),external reliability(test-retest,intraclass correlation coefficient)and inter-rater reliability(Cohen’s kappa coefficient)were *** Confirmatory factor analysis showed a two-factor model with an excellent index of *** SIS showed significant correlations with LSNS-6(rs=0.47),SWLS(rs=0.26),the loneliness self-assessment question(rs=0.35),and GdS-4(rs=−0.16).SIS composite reliability was good(0.708).The inter-class correlation coefficient varied from 0.84 to *** Cohen’s kappa coefficient ranged from 0.936 to *** SIS has been shown to be a valid and reliable instrument for assessing social isolation among Portuguese community-dwelling older *** professionals,particularly nurses working in community settings,can use SIS to assess social isolation in older adults to design,implement,and evaluate interventions.

Prevalence and risk factors for acquired long QT syndrome in the emergency department:a retrospective observational study

BACKGROUNd: Long QT syndrome(LQTS) is a heterogeneous syndrome that may be congenital or, more frequently, acquired. The real-world prevalence of acquired LQTS(aLQTS) in the emergency department(Ed) remains to be determined. The aim of this study was to determine prevalence of aLQTS and its impact on symptoms on Ed ***: Electrocardiograms(ECG) of 5,056 consecutively patients admitted in the Ed of a tertiary hospital between January 28th and March 17th of 2020 were reviewed. All patients with aLQTS were included. Clinical data with a focus on QT prolonging drugs and clinical factors were recorded. Statistical comparison was made between the groups with and without corrected QT(QTc) interval greater than 500 ms(value that is considered severely increased).RESULTS: A total of 383 ECGs with prolonged QTc were recognized, corresponding to a prevalence of aLQTS at admission of 7.82%. Patients with aLQTS were more commonly men(53.3%) with an age of(73.49±14.79) years old and QTc interval of(505.3±32.4) ms. Only 20.4% of these patients with aLQTS were symptomatic. No ventricular arrhythmias were recorded. Patients with QT interval greater than 500 ms were more frequently female(59.5%;Pd were more frequently on QT prolonging drugs(77.3%;P=0.025). Main contributing factor was intake of antibiotics(odds ratio [OR] 4.680) followed by female gender(OR 2.473) and intake of antipsychotics(OR 1.925).CONCLUSION: aLQTS is particularly prevalent in the Ed. Female patients on antibiotics and antipsychotics are at particularly high risk. Efforts must be made to avoid, detect and treat aLQTS as early as possible.

We present major and trace element compositions of mineral concentrates comprising garnet xenocrysts,ilmenite,phlogopite,spinel,zircon,and uncommon minerals(titanite,calzirtite,anatase,baddeleyite and pyrochlore)of a newly discovered Late Cretaceous kimberlite(U-Pb zircon age 90.0±1.3 Ma;2σ)named Osvaldo Franca 1,located in the Alto Paranaíba Igneous Province(APIP),southeastern *** grains are lherzolitic(Lherz-1,Lherz-2 and Lherz-3),harzburgitic(Harz-3)and wehrlitic(Wehr-2).The pyrope xenocrysts cover a wide mantle column in the subcratonic lithosphere(66–143 km;20–43 kbar)at relatively low temperatures(811–875°C).The shallowest part of this mantle is represented by Lherz-1 pyropes(20–32 kbar),which have low-Cr(Cr_(2)O_(3)=1.74–6.89 wt.%)and fractionated middle to heavy rare earth elements(MREE-HREE)*** deepest samples are represented by Lherz-2,Lherz-3,Harz-3,and Wehr-2 pyropes(36–43 kbar).They contain high-Cr contents(Cr_(2)O_(3)=7.36–11.19 wt.%)and are characterized by sinusoidal(Lherz-2 and Wehr-2)and spoon-like(Lherz-3 and Harz-3)REE *** to their REE and trace elements,pyrope xenocrysts have enriched nature(e.g.,Ce and Yb ***_(2)O_(3)),indicating that the cratonic lithosphere has been affected by a silicate melt with subalkaline/tholeiite composition due to their low Zr,Ti and Y *** minerals with typical kimberlitic signatures,such as ilmenite and zircon,the exotic compositions of phlogopite and ulvöspinel suggest an enriched component in the magma *** formation of rare mineral phases with strong enrichment of light-REE(LREE)and high field strength elements(HFSE)is attributed to the late-stage kimberlitic *** propose a tectonic model where a thermal anomaly,represented by the low-velocity seismic anomaly observed in P-wave seismic tomography images,supplied heat to activate the alkaline magmatism from a metasomatized cratonic mantle source during the late-stages of gondwana fragmentation and conse

Clinical Trial demonstrates Efficacy of Transcranial direct Current Stimulation (tdCS) in Improving Pain Management from Post-Laminectomy Syndrome

Chronic pain, a multidimensional experience affecting individuals’ sensory, cognitive, and emotional aspects, significantly impacts their quality of life. Post-laminectomy syndrome, a condition characterized by persistent back pain following spinal surgery, often leads to disability and increased healthcare utilization. Methods: This randomized, controlled, blind clinical trial aimed to investigate the efficacy of Transcranial direct Current Stimulation (tdCS) in managing pain from post-laminectomy syndrome in patients. Twenty-four participants were assigned to three groups: sham stimulation, active stimulation over primary motor cortex (M1), or stimulation over dorsolateral prefrontal cortex (dLPFC). Stimulation was administered for five consecutive days, 20 minutes per session, using a current of 1.5 mA through 25 cm2 electrodes. Pain intensity was assessed using Visual Analog Scale (VAS) before, during, and after intervention. Results: An ANOVA model demonstrates significant reduction in pain intensity compared to baseline in VAS, (F(7, 285) = 12.292;p 0.001;Power = 1.000;η2p = 0.534), in tdCS applied to M1, after five days of intervention. After stimulation, a significant improvement was observed in WHOQoL-Bref Quality of life item 1 (p = 0.04), considering statistical significant difference p 0.05. Correlation between the variables: quality of life, depression, anxiety and pain also demonstrates reduction in depression and anxiety according to Beck’s depression and Anxiety Inventories (BdI and BAI), p 0.05. This effect was not observed in dLPFC stimulation group. Patients who believed they received active stimulation, in sham group, demonstrated potential for effective blinding. Conclusion: The tdCS applied to primary motor cortex effectively improved pain management and psychiatry symptoms in post-laminectomy syndrome patients. The technique’s low cost, ease of use, and high tolerability make it a promising adjuvant therapy for chronic pain conditio
