Variability of anti-human transglutaminase testing in celiac disease across Mediterranean countries

AIM To verify the precision and accuracy of transglutaminase antibodies(TGA)assays across Mediterranean countries.METHODS This study involved 8 referral centres for celiac disease(CD)in 7 Mediterranean countries.A central laboratory prepared 8 kits of 7 blinded and randomized serum samples,with a titrated amount of Human TGA Ig A.Each sample was analysed three times on three different days,with each centre running a total of 21tests.The results were included in a blindly coded report form,which was sent to the coordinator centre.The coordinator estimated the mean coefficient of Variation(Co Var=σ/μ),the mean accuracy(Accur=Vobserved-Vreal)and the mean percent variation(Var%=[(Vobserved-Vreal)/Vreal]×100).RESULTS The analysis showed that 79.17%of the mean variation fell between-25%and+25%of the expected value,with the accuracy and precision progressively increasing with higher titres of TGA.From values 1.25 times greater than the normal cut-off,the measurements were highly reliable.CONCLUSION TGA estimation is a crucial step for the diagnosis of CD;given its accuracy and precision,clinicians could be confident in establishing a diagnosis.

To the Editor:Immunoglobulin G4(IgG4)-associated autoimmune hepatitis(IgG4-AIH)is a novel and rare disease entity,characterized by sig-nificant infiltration of IgG4-expressing plasma cells in the liver.The classification of of IgG4-AIH as a subtype of AIH or an early manifestation of IgG4-related disease(IgG4-RD)remains controversial.Herein,we discuss an interesting clinical vignette of IgG4-AIH in a gentleman with no significant past medical history,who presented with undifferentiated symptoms and elevated aminotransferases.

Silybin and the liver: From basic research to clinical practice

Herbal products are increasingly used, mainly in chronic liver disease. Extracts of milk thistle, Silymarin and silybin, are the most prescribed natural compounds, with different indications, but with no definitive results in terms of clinical efficacy. This review analyzes the available studies on the effects of the purified product silybin, both as a free and a conjugated molecule, on liver cells or on experimentally induced liver damage, and in patients with liver disease. We searched PUBMED for articles pertaining to the in vitro and in vivo effects of silybin, its antifibrotic, anti-inflammatory, and antioxidant properties, as well as its metabolic effects, combined with the authors’ own knowledge of the literature. Results indicate that the bioavailability of silybin phytosome is higher than that of silymarin and is less influenced by liver damage; silybin does not show significant interactions with other drugs and at doses < 10 g/d has no significant side effects. Experimental studies have clearly demonstrated the antifibrotic, antioxidant and metabolic effects of silybin; previous human studies were insufficient for confirming the clinical efficacy in chronic liver disease, while ongoing clinical trials are promising. On the basis of literature data, silybin seems a promising drug for chronic liver disease.

Human retinal secretome:A cross-link between mesenchymal and retinal cells

In recent years,mesenchymal stem cells(MSC)have been considered the most effective source for regenerative medicine,especially due to released soluble paracrine bioactive components and extracellular vesicles.These factors,collectively called the secretome,play crucial roles in immunomodulation and in improving survival and regeneration capabilities of injured tissue.Recently,there has been a growing interest in the secretome released by retinal cytotypes,especially retinal pigment epithelium and Müller glia cells.The latter trophic factors represent the key to preserving morphofunctional integrity of the retina,regulating biological pathways involved in survival,function and responding to injury.Furthermore,these factors can play a pivotal role in onset and progression of retinal diseases after damage of cell secretory function.In this review,we delineated the importance of cross-talk between MSCs and retinal cells,focusing on common/induced secreted factors,during experimental therapy for retinal diseases.The cross-link between the MSC and retinal cell secretomes suggests that the MSC secretome can modulate the retinal cell secretome and vice versa.For example,the MSC secretome can protect retinal cells from degeneration by reducing oxidative stress,autophagy and programmed cell death.Conversely,the retinal cell secretome can influence the MSC secretome by inducing changes in MSC gene expression and phenotype.

Improved lifestyle is associated with improved depression,anxiety and wellbeing over time in UK healthcare professionals during the COVID-19 pandemic:insights from the CoPE-HCP cohort study

Background One potential modifiable factor to improve the mental health of healthcare professionals(HCPs)during the pandemic is lifestyle.Aims This study aimed to assess whether an improved lifestyle during the pandemic is associated with improved mental health symptoms and mental well-being in HCPs over time.Methods This was a cohort study involving an online survey distributed at two separate time points during the pandemic(baseline(July–September 2020)and follow-up(December 2020–March 2021))to HCPs working in primary or secondary care in the UK.Both surveys assessed for major depressive disorder(MDD)(Patient Health Questionnaire-9(PHQ-9)),generalised anxiety disorder(GAD)(Generalised Anxiety Disorder-7(GAD-7)),mental well-being(Short Warwick-Edinburgh Mental Well-being Score(SWEMWBS))and self-reported lifestyle change(compared with the start of the pandemic)on multiple domains.Cumulative scores were calculated to estimate overall lifestyle change compared with that before the pandemic(at both baseline and follow-up).At each time point,separate logistic regression models were constructed to relate the lifestyle change score with the presence of MDD,GAD and low mental well-being.Linear regression models were also developed relating the change in lifestyle scores from baseline to follow-up to changes in PHQ-9,GAD-7 and SWEMWBS scores.Results 613 HCPs completed both baseline assessment and follow-up assessment.Consistent significant cross-sectional associations between increased lifestyle change scores and a reduced risk of MDD,GAD and low mental well-being were observed at both baseline and follow-up.Over the study period,a whole unit increase in the change in novel scores(ie,improved overall lifestyle)over 4 months was inversely associated with changes in PHQ-9(adjusted coefficient:−0.51,95%confidence interval(CI):−0.73 to−0.30,p<0.001)and GAD-7 scores(adjusted coefficient:−0.32,95%CI:−0.53 to−0.10,p=0.004)and positively associated with the change in SWEMWBS scores(adjusted coefficient:0.37,95%CI:0.18 to 0.55,p<

Fungal spores and pollen in particulate matter collected during agricultural activities in the Po Valley(Italy)

Airborne particulate matter（PM） containing fungal spores and pollen grains was sampled within a monitoring campaign of wheat threshing, plowing and sowing agricultural operations. Fungal spores and pollen grains were detected and identified on morphological basis. No studies were previously available about fungal spore and pollen content in agricultural PM in the Po Valley. Sampling was conducted in a Po Valley farmland in Mezzano（Ferrara, Italy）. The organic particles collected were examined by scanning electron microscopy with energy dispersive X-ray spectrometer. Fungal spores and pollen grains were identified when possible at the level of species. The most frequent components of the organic particles sampled were spores of Aspergillus sp., which could represent a risk of developing allergies and aspergillosis for crop farmers.

Understanding how defect chemistry of oxide material influences the thermal stability of noble metal dopant ions plays an important role in designing high-performance heterogeneous catalytic systems.Here we use in-situ ambient-pressure X-ray photoemission spectroscopy(APXPS)to experimentally determine the role of grain boundary in the thermal stability of platinum doped cerium oxide(Pt/CeO_(2)).The grain boundaries were introduced in Pt/CeO_(2)thin films by pulsed laser deposition without significantly change of the surface microstructure.The defect level was tuned by the strain field obtained using a highly/low mismatched substrate.The Pt/CeO_(2)thin film models having well defined crystallographic properties but different grain boundary structural defect levels provide an ideal platform for exploring the evolution of Pt–O–Ce bond with changing the temperature in reducing conditions.We have direct demonstration and explanation of the role of Ce^(3+)induced by grain boundaries in enhancing Pt2+stability.We observe that the Pt^(2+)–O–Ce^(3+)bond provides an ideal coordinated site for anchoring of Pt^(2+)ions and limits the further formation of oxygen vacancies during the reduction with H_(2).Our findings demonstrate the importance of grain boundary in the atomic-scale design of thermally stable catalytic active sites.

Properties of agricultural aerosol released during wheat harvest threshing,plowing and sowing

This study shows for the first time a chemical and morphological characterization of agricultural aerosols released during three important agricultural operations： threshing, plowing and sowing. The field campaigns were carried out in the eastern part of the Po Valley, Italy, in summer and autumn 2009. The aerosol particles were sampled on quartz fiber filters and polytetrafluoroethylene membranes in order to allow Inductively Coupled Plasma Mass Spectrometry （ICP-MS） analysis and Scanning Electron Microscopy equipped with an Energy Dispersive X-ray Spectrometer （SEM-EDS） investigations, respectively. The organic carbon mass concentrations were measured with a Sunset Laboratory Dual-Optical Organic Carbone/Elemental Carbon （OCEC） Aerosol analyzer. The morphological and chemical analyses by SEM-EDS allowed recognizing four main particle classes： organic, silica, calcite and clay minerals. The organic particles contribute to both fine and coarse aerosol fractions up to ca. 50% for all three agricultural activities. This was also confirmed by OCEC analysis for fine fraction. Most of the agricultural aerosols, about 60%, were single particles and the remaining 40% were agglomerations of particles. The ICP-MS results showed that threshing and plowing produce more aerosol particles than sowing, which was characterized by important amounts of clay minerals produced from land soils.
