Introduction: Recent studies showed relation between mortality and bleeding in acute coronary syndromes. Objectiv.: The objectiv. is to analyze the prognosis of patients with acute coronary syndromes with or without in-hospital major and/or minor bleedings. Methods: This was a pro-spectiv. data bank analysis study with 546 patients (39 with bleeding (group I) and 507 without bleeding (group II)) with acute coronary syndromes included between May 2010 and May 2013. Besides, Mehran bleeding risk score was calculated to all patients. The primary endpoint was all causes of in-hospital death and combined ev.nts. Comparison between groups was made by Anov. and Q-square. Multiv.riate analysis was determined by logistic regression and was considered significant when p p = 0.005) and combined ev.nts (35.9% v. 11.4%, p v.d in multiv.riate and long-term analysis. Con-clusions: Almost half of patients with acute coronary syndromes had higher risk of bleeding, and that with major or minor bleedings had greater deaths and combined ev.nts.

2,4,6-trinitrotoluene(TNT) and its deriv.tiv.s are nitrogen-containing aromatic compounds hav.ng chemical and thermal stability at ambient temperature and pressure. TNT has high toxicity and mutagenic activ.ty to humans, plants and animals, thus decontamination processes are necessary. Many microorganisms are capable to bioremediate TNT such as bacteria from the genera Pseudomonas,Enterobacter, Rhodococcus, Mycobacterium, Clostridium and Desulfov.brio: fungus such as Phanerochate and Stropharia species are able to perform TNT biotransformation under aerobic and anaerobic conditions. In this work we rev.ew the state of TNT biodegradation by v.rious routes: aerobic, anaerobic,combined, fungal, enzymatic, and bio-electrochemical.

AIM: To present a new technique of cerv.cal esophagogastric anastomosis to reduce the frequency of fistula formation. METHODS: A group of 31 patients with thoracic and abdominal esophageal cancer underwent cerv.cal esophagogastric anastomosis with inv.gination of the proximal esophageal stump into the stomach tube. In the region elected for anastomosis, a transv.rse myotomy of the esophagus was carried out around the entire circumference of the esophagus. Afterwards, a 4-cm long segment of esophagus was inv.ginated into the stomach and anastomosed to the anterior and the posterior walls. RESULTS: Postoperativ. minor complications occurred in 22 (70.9%) patients. Four (12.9%) patients had serious complications that led to death. The discharge of saliv. was at a lower region, while attempting to leav. the anastomosis site out of the alimentary transit. Three (9.7%) patients had fistula at the esophagogastric anastomosis, with minimal leakage of air or saliv. and with mild clinical repercussions. No patients had esophago- gastric fistula with intense saliv. leakage from either the cerv.cal incision or the thoracic drain. Fibrotic stenosis of anastomoses occurred in sev.n (22.6%) patients. All these patients obtained relief from their dysphagia with endoscopic dilatation of the ***: Cerv.cal esophagogastric anastomosis with inv.gination of the proximal esophageal stump into the stomach tube presented a low rate of esophagogastric fistula with mild clinical repercussions.

AIM:To ev.luate inflammatory activ.ty in patients with Crohn's disease (CD) using technetium-99m-hexamethylpropyleneamine oxime (99mTc-HMPAO) granulocyte ***: Twenty patients (7 male and 13 female) with CD and fiv. healthy v.lunteers were selected for 99mTc-HMPAO granulocyte scintigraphy. The Crohn's Disease Activ.ty Index (CDAI), blood tests and C-reactiv. protein (CRP) of each patient were performed 7 d before the scintigraphic images. The leukocytes were labeled according to the International Society of Radiolabeled Blood Elements (ISORBE) consensus protocol and the scintigraphic images, including single photon emission computed tomography, were obtained 30 min and 2 h after injection of the radiolabeled ***:The labeling yield of the leukocytes with the lipophilic complex 99mTc-HMPAO was 55.0%±10%. Six of the 20 patients (30%) presented congruent results for the three parameters inv.stigated (CDAI, Scintigraphic Index and CRP). On the other hand, 14 patients (70%) did not show congruent results. There was no significant correlation between the indices analyzed according to the Spearman test (P>0.05,n=20).CONCLUSION: The results suggest that 99mTc-HMPAO-labeled leukocyte scintigraphy could be important for determining inflammatory activ.ty in CD ev.n in the absence of clinical symptoms.

BACKGROUND Irritable bowel syndrome(IBS) is a highly prev.lent condition. It is diagnosed on the basis of chronic symptoms after the clinical and/or inv.stigativ. exclusion of organic diseases that can cause similar symptoms. There is no reproducible noninv.siv. test for the diagnosis of IBS, and this raises diagnostic uncertainty among physicians and hinders acceptance of the diagnosis by *** gastrointestinal(GI) syndromes often present with ov.rlapping upper and lower GI tract symptoms, now believ.d to be generated by v.sceral hypersensitiv.ty. This study examines the possibility that, in IBS, a nutrient drink test(NDT) prov.kes GI symptoms that allow a positiv. differentiation of these patients from healthy *** To ev.luate the NDT for the diagnosis of *** This prospectiv. case-control study compared the effect of two different nutrient drinks on GI symptoms in 10 IBS patients(patients) and 10 healthy controls(controls). The 500 kcal high nutrient drink and the low nutrient 250 kcal drink were giv.n in randomized order on separate days. Symptoms were assessed just before and at sev.ral time points after drink ingestion. Global dyspepsia and abdominal scores were deriv.d from indiv.dual symptom data recorded by two questionnaires designed by our group, the upper and the general GI symptomquestionnaires, respectiv.ly. Psycho-social morbidity and quality of life were also formally assessed. Rationale: In a prev.ously published trial on spinal acute non-traumatic pain, peripheral neuro- regenerativ. combination of UTP, CMP and hydroxocobalamin presented unexpected analgesicproperties. Objectiv.: To corroborate analgesiceffects of UTP, CMP and hydroxocobalamin combination in a self-paired ev.lutionary model. Methods: Mean v.S scores from pretreatment, v. (5th treatment day) and v. (10th treatment day) were plotted and statistically analyzed (ANOv.) for differences. PFQ scores from pretreatment, v., and v. were analyzed using the chisquare test. Results: The difference between v. and pretreatment mean v.S scores was statistically significant (p v.ment in PFQ scores throughout the study was found to be statistically significant (p v. analgesic properties in mediumterm use. The complex peripheral neu-roregenerativ. pharmacodynamics of this combination prov.des a plausible basis for this finding. Further randomized studies are needed to explore this combination for the indication of neuropathic pain due to spinal structure inv.lv.ment.

A Risk-Adjusted Retrospectiv. Data Analysis between Younger and Elderly Patients with Acute Coronary Syndromes—Long-Term Prognosis

Purpose: To compare the demographic data and outcomes of younger v.rsus elderly patients with acute coronary syndromes. Methods: This was a retrospectiv. data bank analysis study with 966 patients (268 in the younger group (less than 55 years) and 698 in the elderly group (more than 55 years)). Data were obtained about clinical characteristics, angiography, and medication used at hospital and coronary definitiv. treatment. The primary endpoint was all cause of in-hospital death and combined ev.nts. Comparison between groups was made by Anov. and Q-square. Multiv.riativ. analysis was determined by logistic regression and was considered significativ. when p v.nts were studied using Kaplan- Meyer curv.s with median follow-up of 11.21 months. Results: The median age in the younger group was 48 years v.rsus 69 years in the elderly group. In the younger group 26% was ST-myocardial infarction v.rsus 18% in the elderly group. About 7% of younger patients were submitted to coronary bypass surgery and 42% to percutaneous coronary angioplasty v.rsus 12% and 25% in elderly group, respectiv.ly. Significant difference was observ.d between the younger v.rsus elderly groups in deaths (1.5% × 7.5%, p = 0.004), combined ev.nts (14.9% × 26.3%, p = 0.02) and killip III/Iv.(3.7% × 8.3%, p = 0.04). Long-term mortality was 3.7% × 10.2%, p = 0.01). Conclusions: In patients with acute coronary syndromes age was an important predictor factor of mortality and complications. Significativ. differences in outcomes were observ.d between the two groups in-hospital and long-term follow-up.

Natural organic matter quantification in the waters of a semiarid freshwater wetland (Tablas de Daimiel, Spain)

Dissolv.d organic matter （DOM） concentrations hav. been measured in the waters of a semiarid freshwater wetland, the Tablas de Daimiel, Spain, when the system-characterised by v.riable hydroperiodicity conditions, was completely flooded （February 2011）. Fluxes of DOM from the wetland soils to the ov.rlying waters were measured by using a passiv. diffusion sampler （peeper）. Not only dissolv.d organic carbon （DOC） concentrations were measured but refractory organic matter （ROM, usually known as humic substances） was also quantified using a nov.l v.ltammetric method. Fluorescence spectra were recorded to help in selecting the appropriate standard for ROM quantification, test the homogeneity of DOM in the waters and get an indication of their source. The results obtained show a 7-fold increase in measured ROM concentrations from the Gigtiela Riv.r to the outlet, which points to a net exportation of ROM from the wetland and to the existence of an internal source of ROM in the system, probably diffusion from the wetland soils. This hypothesis is confirmed by the flux of ROM from the soils to the water column measured with the peeper and by the common fluorescence characteristics of colunm and interstitial waters. The smaller increase in DOC concentrations along the wetland, in spite of the higher DOC fluxes from soils, suggests that there is significant turnov.r of organic carbon （OC） in the water column. The system acts as a major carbon sink but, when flooded, exports .OC as DOM.

L-Arginine Supplementation and Nitric Oxide Production:No Additional Effect When Associated to Exercise

L-arginine is an amino acid semiessencial considered a precursor of nitric oxide, a gas mainly produced in endothelial cells. Nutritional supplements based on the amino acid L-arginine hav. been broadly marketed in order to increase v.sodilation and the blood supply to muscle in order to optimize metabolic responses induced by exercise. The main objectiv. of this study was to ev.luate the effect of L-arginine supplementation on nitric oxide production in response to exercise. Furthermore, the biochemical parameters of muscle fatigue were assessed. Fourteen trained runners were div.ded in two groups, supplemented with L-arginine (ARG) and placebo (PLA). Blood samples were collected before supplementation (T0), immediately after the first exercise session (T1), immediately after the second exercise session (T2), and after 20 minutes of rest (T3). Plasma cyclic guanosine monophosphate was assessed as a marker of nitric oxide production. The biochemical parameters of muscle fatigue analyzed were plasma lactate and ammonia. There was significant increase in plasma cyclic guanosine monophosphate in both groups in response to exercise: ARG (T0: 3.6 ± 1.4;T1: 17.9 ± 5.8;T2: 15.9 ± 5.3;T3: 7.3 ± 2.5 pmol/mL) and PLA (T0: 4.1 ± 1.1;T1: 18.8 ± 9.9;T2: 16.1 ± 3.5;T3: 9.3 ± 3.7 pmol/mL). A significant reduction in plasma lactate and ammonia were observ.d in the recov.ry period after exercise (T3). Howev.r, no significant difference was observ.d between groups in the v.riables studied. Therefore, L-arginine supplementation was unable to increase the effects of exercise on nitric oxide production and did not improv. the metabolic responses to exercise in runners.
