Analysis of the therapeutic evolution in the management of airway infantile hemangioma

AIM: To analyze the evolution in the management of airway infantile hemangioma(AIH) and to report the results from 3 pediatric tertiary care institutions.METHODS: A retrospective study of patients with diagnosis of AIH and treated in 3 pediatric tertiary care institutions from 1996 to 2014 was performed. RESULTS: Twenty-three patients with diagnosis of AIH were identified. Mean age at diagnosis was 6 mo(range, 1-27). Single therapy was indicated in 16 patients and 7 patients received combined therapy. Two therapeutic groups were identified: Group A included 14 patients who were treated with steroids, interferon, laser therapy and/or surgery; group B included 9 patients treated with oral propranolol. In group A, oral corticosteroids were used in 9 patients with a good response in 3 cases(no requiring other therapeutic option), the other patients required additional treatment options. Cushing syndrome was observed in 3 patients. One patient died of a fulminant sepsis. Open surgical excision and endoscopic therapy were performed in 11 patients(in 5 of them as a single treatment) with a response rateof 54.5%. Stridor persisted in 2 cases, and one patient died during the clinical course of bronchial aspiration. In group B, oral propranolol was used in 9 patients(in 8 of them as a single treatment) with a response rate of 100%, with an mean treatment duration of 7 mo(range, 5-10); complications were not observed. CONCLUSION: Our experience and the medical literature support the use of propranolol as a first line of treatment in AIH.

Hypersensitivity to Aspirin as a Factor for Poor Control in Hereditary Angioedema

Background: Hereditary angioedema (HAE) is a primary immunodeficiency disorder characterized by C1 complement inhibitor deficiency and unregulated activation of complement. Aspirin hypersensitivity is related to an increase in the amount of leukotrienes with eosinophil and mast cell activation and increased levels of glandular kallikrein with upregulated local conversion of bradykinin. Both conditions can be present in the same patient. Objectives: We present five patients with HAE;they were all being treated in similar ways according to the therapeuthic options available in our institute (danazol). However, three of them had recurrent episodes of angioedema;in these cases, it was identified aspirin hypersensitivity as a cause of poor disease control. A review of the literature is included. Case Presentation: We present the cases of four females and one male (age range 21 - 58 years) with type I HAE. Subjects were all ISSSTE beneficiaries (state workers) treated at the National Medical Center “20 de Noviembre”. Aspirin hypersensitivity was identified in three patients. Elimination of NSAIDs along with dietary elimination of high salicylate-containing foods improved control of angioedema crisis (severe and/ or recurrent episodes). Discussion: Aspirin hypersensitivity was identified as a factor for poor control in our patients with HAE. Such cases improved with dietary elimination of high salicylate-containing foods and avoidance of NSAIDs. Conclusions: This is the first report of patients with HAE and aspirin hypersensitivity as a cause of poor control. We recommend a deliberate search of these comorbidities, especially in cases of poor disease control. Further studies are needed to continue the investigation on this topic.

This work presents a study about the chemical structure of titanium oxide (TiO) particles synthesized by plasmas using titanium tetrapropoxide, Ti(-O-CH2-CH2-CH3)4. In plasmas, practically all chemical bonds are susceptible to participate in the reactions producing different results than those obtained by the traditional chemical routes. The particles obtained this way are semispheres and fibers grouped in random and layered structures in the 120-500 nm interval and mean diameter of 86.4 nm (fibers) and 308.6 nm (semispheres). The analysis of the resulting TiO structure was performed by IR and XPS finding that the main chemical state of Ti in these conditions was O2-Ti-O2 (Ti surrounded by O) which is part of the precursor structure, however, in O, the main chemical state was Ti-O-Ti formed with the rupture of the pre- cursor Ti-O-C bonds. These last bonds reduce the conjugation between the structure of both elements, O2-Ti-O2 and Ti-O-Ti, to produce organometallic compounds. Other chemical states appeared showing consecutive dehydrogenation steps during the synthesis with the formation of multiple bonds as a consequence of the continuous collisions in the plasma. These results allow us to follow the chemical reactions promoted by this kind of plasmas to produce TiO nanoparticles which are essentially conformed of intensive dehydrogenation.

Does Adult Intestinal Invagination Need Surgery? Report of a Case and Review of Literature

Benign intestinal tumors are the most frequent cause of small bowel intussusception in adults. This report presents a case of a 67-year-old male with abdominal pain, leukocytosis and C-reactive protein elevation. After ultrasonography and double contrast CT-scan consistent with ileal intussusception patient underwent laparoscopy, which confirmed diagnose. Intestinal resection and anastomosis were performed via minilaparotomy. Further histological study showed a fibroid inflammatory polyp or Vanek’s tumor as intussusception’s lead point.

Liver ischemia-reperfusion injury(IRI) is an inherent feature of liver surgery and liver transplantation in which damage to a hypoxic organ(ischemia) is exacerbated following the return of oxygen delivery(reperfusion). IRI is a major cause of primary nonfunction after transplantation and may lead to graft rejection, regardless of immunological considerations. The immediate response involves the disruption of cellular mitochondrial oxidative phosphorylation and the accumulation of metabolic intermediates during the ischemic period, and oxidative stress during blood flow restoration. Moreover, a complex cascade of inflammatory mediators is generated during reperfusion, contributing to the extension of the damage and finally to organ failure. A variety of pharmacological interventions(antioxidants, anticytokines, etc.) have been proposed to alleviate graft injury but their usefulness is limited by the local and specific action of the drugs and by their potential undesirable toxic effects. Polyethylene glycols(PEGs), which are non-toxic water-soluble compounds approved by the FDA, have been widely used as a vehicle or a base in food, cosmetics and pharmaceuticals, and also as adjuvants for ameliorating drug pharmacokinetics. Some PEGs are also currently used as additives in organ preservation solutions prior to transplantation in order to limit the damage associated with cold ischemia reperfusion. More recently, the administration of PEGs of different molecular weights by intravenous injection has emerged as a new therapeutic tool to protect liver grafts from IRI. In this review, we summarize the current knowledge concerning the use of PEGs as a useful target for limiting liver IRI.

Importance of the Role of Gibberellic Acid and Potassium Nitrate in Seed Germination Habanero

The habanero chili (Capsicum chinense Jacq.) is a species of great demand in the domestic and international markets. One problem in the cultivation is low percentage of seed germination. There is little information on the use of promoters and imbibition time to increase this parameter, without compromising the quality of seedlings. In this context, authors assessed the independent effect of GA3 (gibberellic acid) and KNO3 at concentrations of 0, 100, 200, 300, 400 and 500 mg·L-1. Seeds without preconditioning were the witness. The experimental unit was a Petri dish with 50 seeds on filter paper placed equidistant. There were three replicates per treatment and the experimental design was randomized complete block. Imbibition time was 24 h. The experimental units were kept in an incubator at 22 ± 2 °C. From the time of planting and for periods of 24 h for 14 d, experimental units were reviewed to determine the time at the beginning of germination and the total cumulative percentage. The authors also determined the number of normal and abnormal seedlings, the length of hypocotyl and radicle, weight of fresh and dry matter of seedlings. On the fifth day after sowing in vitro, concentrations of 500 mg·L-1 of GA3, 300 and 400 mg·L-1 KNO3, increased 208% the percentage of seed germination of habanero chile. On day 14 after sowing in vitro concentrations of 500 mg·L-1 GA3, 300 and 400 mg·L-1 KNO3, increased 23% normal seedlings and 40% dry matter thereof, with respect to seeds obtained without preconditioning. Unconditioned seeds before sowing produced 125% more seedlings.

Poly-histidine peptides such as H6(HHHHHH)are used in protein biotechnologies as purification tags,protein-assembling agents and endosomal-escape entities.The pleiotropic properties of such peptides make them appealing to design protein-based smart materials or nanoparticles for imaging or drug delivery to be produced in form of recombinant proteins.However,the clinical applicability of H6-tagged proteins is restricted by the potential immunogenicity of these segments.In this study,we have explored several humanized histidine-rich peptides in tumor-targeted modular proteins,which can specifically bind and be internalized by the target cells through the tumoral marker CXCR4.We were particularly interested in exploring how protein purification,self-assembling and endosomal escape perform in proteins containing the variant histidine-rich tags.Among the tested candidates,the peptide H5 E(HEHEHEHEH)is promising as a good promoter of endosomal escape of the associated fulllength protein upon endosomal internalization.The numerical modelling of cell penetration and endosomal escape of the tested proteins has revealed a negative relationship between the amount of protein internalized into target cells and the efficiency of cytoplasmic release.This fact demonstrates that the His-mediated,proton sponge-based endosomal escape saturates at moderate amounts of internalized protein,a fact that might be critical for the design of protein materials for cytosolic molecular delivery.

Brugada syndrome is a major cause of sudden death in young adults.Fever has been described to induce a Brugada-type electrocardiogram in asymptomatic patients with a negative family history,to disclose Brugada syndrome and to increase the risk of death and induce T wave alternans in patients with diagnosed Brugada syndrome.Risk stratification is challenging and demands a careful evaluation.Here we present 2 case reports and review the literature.
