Facial nerve monitoring in parotid gland surgery:Design and feasibility assessment of a potential standardized technique

Background:Even though the use of nerve monitoring during parotid gland surgery is not the gold standard to prevent damage to the nerve,it surely offers some advantages over the traditional approach.Different from thyroid surgery,where a series of steps in intraoperative nerve monitoring have been described to confirm not only the integrity but—most importantly—the function of the recurrent laryngeal nerve,in parotid gland surgery,a formal guideline to follow while dissecting the facial nerve has yet to be described.Methods:A five‐year retrospective study was done reviewing the intraoperative records of patients who underwent parotid gland surgery under neural monitoring.The operative findings regarding the neuromonitoring process,particularly in regard to the amplitude of two main branches,were revised.A literature search was done to search for guidelines to follow when a facial nerve loss of signal is encountered.Results:Fifty‐five patients were operated on using the Nim 3 Nerve Monitoring system(Medtronic);31 were female patients,and 47 patients had benign lesions.Minimum changes were observed in the amplitude records after a comparison was made between the first and the last stimulation.There were only three articles discussing the term loss of signal during parotid gland surgery.Conclusion:Today,no sufficient attention has been given to the facial nerve monitoring process during parotidectomy.This study proposes a formal guideline to follow during this procedure as well as an instruction to consider when a loss of signal is observed to develop a uniform technique of facial nerve stimulation.

What is the effect of Tribulus terrestris on testicular morphology and sperm production?An experimental study in the rat model

The objective of this study was to investigate whether treatment with Tribulus terrestris(Tt)has any impact on the testicular morphology and function in a rodent model.Twenty male rats were divided into a control group and a group receiving 100 mg kg^(-1) body weight of Tt supplementation.After 4o days of experiment,the animals were submitted to euthanasia;epididymal tail spermatozoa were collected;and spermatozoa concentration,motility,and viability were analyzed.In addition,testicles were collected and processed for histomorphometrical analyses.Data were compared using the student's t-test and considered significant when Pspermatozoa concentration,motility,and viability showed no difference between the groups.Further,testicular weight and volume,seminiferous tubule diameter,tunica propria surface density,seminiferous epithelium surface density,and intertubular compartment surface density were statistically similar between the groups.However,seminiferous epithelium height and tubular lumen surface density were augmented in animals treated with Tt.Treatment with Tt does not cause a major impact on testicular morphology,promoting only subtle modifications.No difference on spermatozoa parameters was observed.

Background Optimising the immunogenicity of COVID-19 vaccines to improve their protection against disease is necessary.Fractional dosing by intradermal(ID)administration has been shown to be equally immunogenic as intramuscular(IM)administration for several vaccines,but the immunogenicity of ID inactivated whole severe acute respiratory syndrome coronavirus 2(sARs-CoV-2)at the full dose is unknown.This study(NCT04800133)investigated the superiority of antibody and T-cell responses of full-dose CoronaVac by ID over IM administration in adolescents.Methods Participants aged 11–17 years received two doses of IM or ID vaccine,followed by the 3rd dose 13–42 days later.Humoral and cellular immunogenicity outcomes were measured post-dose 2(IM-CC versus ID-CC)and post-dose 3(IMCCC versus ID-CCC).Doses 2 and 3 were administered to 173 and 104 adolescents,respectively.Results spike protein(s)immunoglobulin G(IgG),s-receptor-binding domain(RBD)IgG,s IgG Fcγreceptor IIIa(FcγRIIIa)-binding,sNM[sum of individual(s),nucleocapsid protein(N),and membrane protein(M)peptide pool]-specific interleukin-2(IL-2)+CD4+,sNM-specific IL-2+CD8+,s-specific IL-2+CD8+,N-specific IL-2+CD4+,N-specific IL-2+CD8+and M-specific IL-2+CD4+responses fulfilled the superior and non-inferior criteria for ID-CC compared to IM-CC,whereas IgG avidity was inferior.For ID-CCC,s-RBD IgG,surrogate virus neutralisation test,90%plaque reduction neutralisation titre(PRNT90),PRNT50,s IgG avidity,s IgG FcγRIIIa-binding,M-specific IL-2+CD4+,interferon-γ+CD8+and IL-2+CD8+responses were superior and non-inferior to IM-CCC.The estimated vaccine efficacies were 49%,52%,66%and 79%for IM-CC,ID-CC,IM-CCC and ID-CCC,respectively.The ID groups reported more local,mild adverse reactions.Conclusion This is the first study to demonstrate superior antibody and M-specific T-cell responses by ID inactivated sARsCoV-2 vaccination and serves as the basis for future research to improve the immunogenicity of inactivated vaccines.

The Major Facilitator superfamily(MFs)is ubiquitous in living organisms and represents the largest group of secondary active membrane transporters.In plants,significant research efforts have focused on the role of specific families within the MFs,particularly those transporting macronutrients(C,N,and P)that constitute the vast majority of the members of this superfamily.Other MFs families remain less explored,although a plethora of additional substrates and physiological functions have been uncovered.Nevertheless,the lack of a systematic approach to analyzing the MFs as a whole has obscured the high diversity and versatility of these transporters.Here,we present a phylogenetic analysis of all annotated MFs domaincontaining proteins encoded in the Arab/dops/s fA?a//ana genome and propose that this superfamily of transporters consists of 218 members,clustered in 22 families.In reviewing the available information regarding the diversity in biological functions and substrates of Arab/dops/s MFs members,we provide arguments for intensified research on these membrane transporters to unveil the breadth of their physiological relevance,disclose the molecular mechanisms underlying their mode of action,and explore their biotechnological potential.

Assessing radiation dose for postoperative radiotherapy in prostate cancer: Real world data

BACKGROUND Approximately 30%of patients with localized prostate cancer(PCa)who undergo radical prostatectomy will develop biochemical recurrence.In these patients,the only potentially curative treatment is postoperative radiotherapy(PORT)with or without hormone therapy.However,the optimal radiotherapy dose is unknown due to the limited data available.AIM To determine whether the postoperative radiotherapy dose influences biochemical failure-free survival(BFFs)in patients with PCa.METHODs Retrospective analysis of patients who underwent radical prostatectomy for PCa followed by PORT-either adjuvant radiotherapy(ART)or salvage radiotherapy(sRT)-between April 2002 and July 2015.From 2002 to 2010,the prescribed radiation dose to the surgical bed was 66-70 Gy in fractions of 2 Gy;from 2010 until July 2015,the prescribed dose was 70-72 Gy.Patients were grouped into three categories according to the total dose administered:66-68 Gy,70 Gy,and 72 Gy.The primary endpoint was BFFs,defined as the post-radiotherapy prostatespecific antigen(PsA)nadir+0.2 ng/mL.secondary endpoints were overall survival(Os),cancer-specific survival(Css),and metastasis-free survival(MFs;based on conventional imaging tests).Treatment-related genitourinary(GU)and gastrointestinal(GI)toxicity was evaluated according to Radiation Therapy Oncology Group/European Organization for Research and Treatment of Cancer criteria.Finally,we aimed to identify potential prognostic factors.BFFs,Os,Css,and MFs were calculated with the Kaplan-Meier method and the log-rank test.Univariate and multivariate Cox regression models were performed to explore between-group differences in survival outcome measures.REsULTs A total of 301 consecutive patients were included.Of these,93(33.6%)received ART and 186(66.4%)sRT;22 patients were excluded due to residual macroscopic disease or local recurrence in the surgical bed.In this subgroup(n=93),43 patients(46.2%)were Gleason score(Gs)≤6,44(47.3%)Gs 7,and 6(6.5%)Gs≥8;clinical stage was cT1 in 51(54.8%),cT2 in 35(39.3%),and cT3 i

Objective： Vascular calcification is the consequence of the complex interaction between genetic, environmental, and vascular factors, which ultimately lead to the deposition of calcium in the tunica intima （atherosclerotic calcification） or tunica media （M6nckenberg＇s sclerosis）. Vascular calcification is also closely related to other pathologies, such as diabetes mellitus, dyslipidemia, and chronic kidney disease. It has been concluded that the degree of vascular calcification may vary from person to person, even if the associated pathologies and environmental factors are the same. Therefore, this suggests an important genetic contribution to the development of vascular calcification. This review aimed to find the most recent evidence about vascular calcification pathophysiology regarding the genetic aspects and molecular pathways. Data sources： We conducted an exhaustive search in scopus, EBsCO, and PubMed with the keywords ＂genetics and vascular calcification＂, ＂molecular pathways, genetic and vascular calcification＂ and included the main articles from January 1995 up to August 2016. We focused on the most recent evidence about vascular calcification pathophysiology regarding the genetic aspects and molecular pathways. study selection： The most valuable published original and review articles related to our objective were selected. Results： Vascular calcification is a multifactorial disease; thus, its pathophysiology cannot be explained by a single specific factor, rather than by the result of the association of several genetic variants, molecular pathway interactions, and environmental factors that promote its development. Conclusion： Although several molecular aspects of this mechanism have been elucidated, there is still a need for a better understanding of the factors that predispose to this disease.

The high effectiveness of the third dose of BNT162b2 in healthy adolescents against Omicron BA.1 has been reported in some studies,but immune responses conferring this protection are not yet elucidated.In this analysis,our study(NCT04800133)aims to evaluate the humoral and cellular responses against wild-type and Omicron(BA.1,BA.2 and/or BA.5)sARs-CoV-2 before and after a third dose of BNT162b2 in healthy adolescents.At 5 months after 2 doses,s IgG,s IgG Fc receptor-binding,and neutralising antibody responses waned significantly,yet neutralising antibodies remained detectable in all tested adolescents and s IgG avidity increased from 1 month after 2 doses.The antibody responses and s-specific IFN-γ^(+)and IL-2^(+)CD8^(+)T cell responses were significantly boosted in healthy adolescents after a homologous third dose of BNT162b2.Compared to adults,humoral responses for the third dose were non-inferior or superior in adolescents.The s-specific IFN-γ^(+)and IL-2^(+)CD4^(+)and CD8^(+)T cell responses in adolescents and adults were comparable or non-inferior.Interestingly,after 3 doses,adolescents had preserved s IgG,s IgG avidity,s IgG FcγRIIIa-binding,against Omicron BA.2,as well as preserved cellular responses against BA.1 s and moderate neutralisation levels against BA.1,BA.2 and BA.5.sera from 100 and 96%of adolescents tested at 1 and 5 months after two doses could also neutralise BA.1.Our study found high antibody and T cell responses,including potent cross-variant reactivity,after three doses of BNT162b2 vaccine in adolescents in its current formulation,suggesting that current vaccines can be protective against symptomatic Omicron disease.

Comparison between Classical and Intelligent Identification systems for Classification of Gait Events

Gait event detection is important for diagnosis and evaluation. This is a challenging endeavor due to subjectivity, high amount of data, among other problems. ANFIs (Artificial Neural Fuzzy Inference systems), ARX (Autoregressive Models with Exogenous Variables), OE (Output Error models), NARX (Nonlinear Autoregressive Models with Exogenous Variables) and models based on NN (neural networks) were developed in order to detect gait events without the problems mentioned. The objective was to compare developed models" performance and determinate the most suitable model for gait events detection. Knee joint angle, heel foot switch and toe foot switch during normal walking in a treadmill were collected from a healthy volunteer. Gait events were classified by three experts in human motion. Experts" mean classification was obtained and all models were trained and tested with the collected data and experts" mean classification. Fit percentage was obtained to evaluate models performance. Fit percentages were: ANFIs: 79.49%, ARX: 68.8%, OE: 71.39%, NARX: 88.59%, NNARX: 67.66%, NNRARX: 68.25% and NNARMAX: 54.71%. NARX had the best performance for gait events classification. For ARX and OE, previous filtering is needed. NN"s models showed the best performance for high frequency components. ANFIs and NARX were able to integrate criteria from three experts for gait analysis. NARX and ANFIs are suitable for gait event identification. Test with additional subjects is needed.

Feeding Protected Lysine and Methionine Modifies Milk Protein Profile in Grazing Dairy Cows

The experiment was designed to determine the effect of protected lysine (Lys) and methionine (Met) supply on milk protein profile in grazing dairy cows specifically in the caseins (CNs) and α-lactalbumin fractions. Twelve multiparous mid lactation Holstein cows producing 24 (±4.76) kg of milk were assigned to one of two treatments (six cows per treatment) during an experimental period of 21 days. In the control (C) group, cows grazed a Pennisetum clandestinum pasture and were supplemented with a commercial concentrate according to milk production. In the Met-Lys treatment, cows received the same ration supplemented with protected Lys and Met. Milk yield and composition and milk protein profile were measured at the start and the end (21st day) of the experimental period. The Tricine-sDs-PAGE and the Gel-Quant Express Analysis (Invitrogen) software were used to determine milk protein composition. statistical analysis was performed using the sAs’s PROC MIXED procedure through a mixed model that included the animal as a random effect and the treatments as a fixed effect adjusted by covariables. Milk production averaged 23.7 (±2.0) kg cow-1 day-1 without differences between treatments (P P -1 day-1) compared to C (24.2 kg cow-1 day-1). Milk protein content (g/kg) did not differ (C = 30.4;Met-Lys = 31.1) and lactose content tended (P P β-CN also increased (P supply (C = 9.58;Met-Lys = 10.35). It can be concluded that milk protein composition was improved by protected Lys-Met supply without altering other compositional parameters of milk composition. Milk nutritional quality and its potential yield for cheese-making were positively enhanced.
