Endoscopic identification of endoluminal esophageal landmarks for radial and longitudinal orientation and lesion location

AIM To characterize esophageal endoluminal landmarks to permit radial and longitudinal esophageal orientation and accurate lesion location.METHODS Distance from the incisors and radial orientation were estimated for the main left bronchus and the left atrium landmarks in 207 consecutive patients using white light examination. A sub-study was also performed using white light followed by endoscopic ultrasound(EUS) in 25 consecutive patients to confirm the findings.The scope orientation throughout the exam was maintained at the natural axis,where the left esophageal quadrant corresponds to the area between 6 and 9 o'clock. When an anatomical landmark was identified, it was recorded with a photograph and its quadrant orientation and distance from the incisors were determined. The reference points to obtain the distances and radial orientation were as follows: the midpoint of the left main bronchus and the most intense pulsatile zone of the left atrium. With the video processor system set to moderate insufflation, measurements were obtained at the end of the patients' air expiration.RESULTS The left main bronchus and left atrium esophageal landmarks were identified using white light in 99% and 100% of subjects at a mean distance of 25.8 cm(SD2.3), and 31.4 cm(SD 2.4) from the incisors, respectively. The left main bronchus landmark was found to be a tubular, concave, non-pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. The left atrium landmark was identified as a round, convex, pulsatile, esophageal external compression, occupying approximately 1/4 of the circumference. Both landmarks were identified using white light on the anterior esophageal quadrant. In the substudy, the left main bronchus was identified in 24(92%) patients at 25.4 cm(SD2.1) and 26.7 cm(SD 1.9) from the incisors, by white light and EUS, respectively.The left atrium was recognized in all patients at 30.5 cm(SD 1.9), and 31.6 cm(SD2.3) from the incisors, by both white light and EUS, respectively. Comparison between Classical and Intelligent Identification Systems for Classification of Gait Events

Gait event detection is important for diagnosis and evaluation. This is a challenging endeavor due to subjectivity, high amount of data, among other problems. ANFIS (Artificial Neural Fuzzy Inference Systems), ARX (Autoregressive Models with Exogenous Variables), OE (Output Error models), NARX (Nonlinear Autoregressive Models with Exogenous Variables) and models based on NN (neural networks) were developed in order to detect gait events without the problems mentioned. The objective was to compare developed models" performance and determinate the most suitable model for gait events detection. Knee joint angle, heel foot switch and toe foot switch during normal walking in a treadmill were collected from a healthy volunteer. Gait events were classified by three experts in human motion. Experts" mean classification was obtained and all models were trained and tested with the collected data and experts" mean classification. Fit percentage was obtained to evaluate models performance. Fit percentages were: ANFIS: 79.49%, ARX: 68.8%, OE: 71.39%, NARX: 88.59%, NNARX: 67.66%, NNRARX: 68.25% and NNARMAX: 54.71%. NARX had the best performance for gait events classification. For ARX and OE, previous filtering is needed. NN"s models showed the best performance for high frequency components. ANFIS and NARX were able to integrate criteria from three experts for gait analysis. NARX and ANFIS are suitable for gait event identification. Test with additional subjects is needed.

There is controversy in the literature on where to place the tourniquet(thigh,calf,ankle)for foot and ankle surgery.While some authors prefer the ankle tourniquet to the calf tourniquet,others state that the surgeon can decide between using the thigh tourniquet or the ankle tourniquet,since there was no difference in postoperative pain between them.Where to place the tourniquet during foot and ankle surgery to cause the least possible postoperative pain to the patient as a result of the tourniquet is a common question in clinical practice.The reality is that,unfortunately,there is no consensus on this issue.Perhaps the only possible way to answer this question would be to conduct a comparative study with sufficient statistical power to reach scientifically sound conclusions.It does not seem easy to carry out such a study,but it would be important to be able to answer the question posed in the title of this Editorial once and for all.

Mortality rate after total knee arthroplasty or total hip arthroplasty in patients with a history of liver transplant

In this editorial I comment on the article by Ahmed et al published in a recent issue of the World J Orthop 2023;14:784-790.It is well known that patients who have undergone a liver transplant(LT)may need to have a total hip arthroplasty(THA)or total knee arthroplasty(TKA)implanted.Ahmed et al stated that the mortality rate in these patients was similar to the one of the general population.However,there are three articles previously published that found higher mortality in LT patients who experienced THA/TKA than in the general population(individuals without LT).Therefore,in this Editorial I would like to point out that there is controversy in the literature regarding whether LT patients undergoing THA/TKA have higher mortality than the general population.Therefore,future research should attempt to resolve this controversy.

In Amazonia,small-oriented family businesses or communities cultivate freshwater organisms as a source of income.Although effective management is fundamental for optimizing cultivated stocks,Amazonian aquaculture belittles the importance of genetic diversity,relying primarily on phenotypical characteristics during breeders’selection.To provide a“proof of concept”built on the premise that implementing modern technologies is crucial to optimizing fish stocks in rural Amazonian fish farms,we conducted a pilot study assessing genetic diversity on cultivated Piaractus brachypomus from Caquet´a,Colombia.We used mitochondrial control region(CR)sequences to evaluate heterogeneity.Our data revealed low genetic diversity and suggested that most sampled individuals probably originated from a single maternal lineage.Haplotype diversification resulted from discrete point mutations not associated with geographical location.Polymorphisms accumulate in non-conserved CR regions,yet the most divergent haplotypes display mutations in conserved CR sequences with roles in mitochondrial metabolisms and proliferation.Overall,our results indicate a need in the local industry to design breeding strategies to enhance diversity to avoid deleterious effects caused by inbreeding and domestication.

目的反复喘息患者多为2岁以下的婴幼儿。在热带国家,对该人群住院期间接受呼吸支持治疗的风险的临床预测模型研究较少。该研究旨在评估就诊于哥伦比亚急诊科的反复喘息婴幼儿需要住院并接受呼吸支持治疗的临床预测因素。方法该研究是一项回顾性队列研究,纳入了2019年1~12月期间在哥伦比亚Rionegro的两个三级中心医院就诊的所有患有2次或2次以上喘息发作的婴幼儿(年龄均小于2岁)。主要结局指标是住院加呼吸支持治疗。采用多因素logistic回归模型确定需要住院并接受呼吸支持治疗的独立预测因素。结果共85名婴幼儿住院并接受呼吸支持治疗,其中34名(40%)予以高流量鼻导管吸氧,2名(2%)予以无创通气,6名(7%)予以机械通气,43名(51%)予以常规氧疗。多因素logistic回归模型分析显示,早产(OR=1.79,95%CI:1.04~3.10)、喂养困难(OR=2.22,95%CI:1.25~3.94)、鼻煽和/或咕噜声(OR=4.27,95%CI:2.41~7.56)和既往有1次以上喘息发作需要住院治疗(OR=3.36,95%CI:1.86~7.08)是需要住院并接受呼吸支持治疗的预测因素。该模型特异度高(99.6%),鉴别度中等,曲线下面积为0.70(95%CI:0.60~0.74)。结论该研究表明,早产、喂养困难、鼻煽和/或呼噜声,以及有1次以上需要住院治疗的喘息发作史,是急诊科就诊的反复喘息婴幼儿需要住院并接受呼吸支持治疗的独立预测因素。然而,还需收集更多的其他热带国家的证据来验证这个结论。

Lid scrubbing with a gel combining natural extracts for dry eye treatment

AIM:To evaluate the effect of lid scrubbing using a gel combining different natural extracts on ocular signs and symptoms of dry eye patients.METHODS:A retrospective study was performed on 19 dry eye patients(51.1±16.0y).Non-invasive tear break-up time,tear meniscus height,meibomian gland loss,lipid layer thickness,conjunctival redness,corneal staining,ocular surface disease index(OSDI),and ocular pain intensity were measured before(baseline),1wk,and 2mo after lid scrubbing with a gel containing different natural extracts(okra extract,aloe vera leaf juice,hydrolysed soy protein,caffeine,citrus unshiu peel extract,and raspberry seed oil).RESULTS:Compared with the baseline,there was a statistically significant improvement in both first(P=0.048)and average(P=0.026)non-invasive tear break-up time 2mo after treatment,as well as in corneal staining(P=0.043,0.012),OSDI(P<0.001),and ocular pain intensity(P<0.001)after 1wk and 2mo.In addition,there was no correlation between ocular signs and symptoms.CONCLUSION:The lid scrubbing with a gel combining different natural extracts show beneficial effects on tear film stability,corneal damage,and ocular symptoms,which is the reason why this therapeutic procedure is proposed as an alternative for dry eye management.However,it is not possible to attribute this beneficial effect solely to the presence of the natural extracts in the gel,primarily due to the absence of a negative control group.

Gone with the wind： Seasonal trends in foraging movement directions for a central-place forager

Lesser kestrels Falco naumanni are migratory central-place foragers that breed in dynamic arable landscapes. After arriving from migration, kestrels have no knowledge of the distribution of crops, and consequently prey, around their colony. The energy demand of pairs increases as breeding season progresses, but at the same time prey abundance, and their knowledge on prey distribution, also increases. Wind can have a strong influence on flight cost and kestrels should try to reduce energy expendi- ture when possible. When prey abundance is low, kestrels have little knowledge of prey distribution, and pairs have no chicks, they could reduce foraging flight cost by leaving the colony with tailwinds. When prey is abundant, knowledge on prey distribu- tion has increased, and chick demand is high, kestrels should fly to the most favorable foraging patches. We analyzed foraging trips directions in a lesser kestrel colony along the breeding season and in relation to wind speed and direction. We recorded 664 foraging trips from 19 individuals using GPS-dataloggers. We found that outward flights direction changed from uniform to a concentrated distribution along the season, as prey abundance and individual experience increased. We also found a temporal trend in the angular difference between outward flights and wind directions, with low values early in the season and then increa- sing as expected, but again low values at the end, contrary to expectation. Results suggest changes in kestrels foraging strategy along the season in relation to wind. Kestrels depart more with tailwinds in exploratory flights early in the season, while there is a spurious coincidence in direction to preferred foraging patches and dominant wind direction at the end [Current Zoology 60 (5):604-615, 2014].
