Background Tuberculosis is one of the most significant infectious diseases for global public health.The reallocation of healthcare resources and the restrictions imposed by the COVID-19 pandemic have hindered access to TB diag-nosis and treatment.Increases in unfavorable outcomes of the disease have been observed in Brazil.The objective of this study was to analyze the spatial distribution of unfavorable TB treatment outcomes in Brazil before and during the pandemic.Methods An ecological study with spatial analysis was conducted with all 5569 municipalities in Brazil.All reported cases of tuberculosis between January 2010 and December 2021,as well as reported cases of COVID-19 from Febru-ary 2020 to December 2021,were included.The outcomes studied encompass loss to follow-up,drug-resistant tuber-culosis,and death.The Getis Ord GI*technique was employed to assess spatial association,and the Kernel density estimator was used to identify areas with concentrated increases or decreases in outcomes.Bivariate Local Moran's/was used to examine the spatial association between outcomes and cOVID-19 incidence.The study was approved by the Research Ethics Committee of Ribeirao Preto Nursing School,University of Sao Paulo.Results There were 134,394 cases of loss to follow-up,10,270 cases of drug resistance,and 37,863 deaths.Clusters of high and low values were identified for all three outcomes,indicating significant changes in the spatial distribu-tion patterns.Increases in concentrations were observed for lost to follow-up cases in the Southeast,while reductions occurred in the Northeast,South,and Midwest.Drug-resistant tuberculosis experienced an increase in the Southern and Southeastern regions and a decrease in the Northeast and South.TB-related deaths showed notable concentra-tions in the Midwest,Northeast,South,and Southeast.There was an increase in high occurrence clusters for deaths after 2020 and 2021 in the Northeast.Conclusions The pandemic has brought additional challenges,emphasizing the importance of enhancing e

Silicon-based electro-optic modulators contribute to easing the integration of high-speed and low-power consumption circuits for classical optical communications and data computations.Beyond the plasma dispersion modulation,an alternative solution in silicon is to exploit the DC Kerr effect,which generates an equivalent linear electro-optical effect enabled by applying a large DC electric field.Although some theoretical and experimental studies have shown its existence in silicon,limited contributions relative to plasma dispersion have been achieved in high-speed modulation so far.This paper presents high-speed optical modulation based on the DC Kerr effect in silicon PIN waveguides.The contributions of both plasma dispersion and Kerr effects have been analyzed in different waveguide configurations,and we demonstrated that the Kerr induced modulation is dominant when a high external DC electric field is applied in PIN waveguides.High-speed optical modulation response is analyzed,and eye diagrams up to 80 Gbit/s in NRZ format are obtained under a d.***.voltage of 30 V.This work paves the way to exploit the Kerr effect to generate high-speed Pockels-like optical modulation.

Functional Roux-en-Y Gastric Bypass (F-RYGB), with Preservation of Duodenal Access: Report of Two Revisional Cases of Sleeve Gastrectomy

Objective: This case report aimed to demonstrate a possible neuromuscular effect of Latarjet nerves transection or truncal vagotomy, in association with sleeve gastrectomy plus antrojejunostomy, in order to reproduce a Roux-en-Y gastric bypass (RYGB) mechanistic principles, in patients with previous Sleeve Gastrectomy (SG) who had had weight regain, with or without concomitant gastroesophageal reflux disease (GERD). Background: Sleeve gastrectomy (SG) is one of the most frequently performed bariatric operations worldwide. Nevertheless, weight regain and gastroesophageal reflux disease (GERD) have been consistently demonstrated, in association with this technique, which may require a revisional procedure. RYGB is an option in such a situation but, implies in gastrointestinal exclusions, which represents a shortcoming of this revision. Surpassing this inconvenient would be of great value for the patients. Methods: We describe herein two cases of SG revision for weight regain and GERD, with a follow-up of one year. Gastroesophageal reflux disease was evaluated by validated questionnaire, upper endoscopy, seriography, high resolution manometry (HRM) and impedance pHmetry (I-pHmetry), in the pre and postoperative periods. A re-Sleeve Gastrectomy with antrojejunal anastomosis was done in both cases, after informed consents. The Latarjet nerves were transected in one case, due to a bleeding in the left gastric vessels and a truncal vagotomy was required in the other, to appropriately treat an associated hiatal hernia. Results: In the postoperative evaluation it was observed a pyloric spasm in both cases, during seriography and endoscopy, kept until the one-year follow-up. There was unidirectional contrast flow to the gastrointestinal anastomosis, filling the jejunal limb, in radiologic contrast study. No contrast passed through the pylorus. BACKGROUND Radicular cyst is a lesion of odontogenic origin that arises from epithelial remains due to periapical periodontitis caused by inflammatory reactions generated at the apex of affected teeth with infected or necrotic pulps.The therapeutic mana gement of radicular cysts is controversial.There is only one case report of enucleation of a radicular cyst managed with microsurgery and apicoectomy,but without the use of the guided tissue regeneration(GTR)technique in the same surgical procedure.The present clinical case describes the management of a radicular cyst with microsurgical approach,performance of an apicoectomy of the tooth associated with the entity,application of GTR technique,use of a resorbable membrane of type I bovine collagen,and bovine xenograft.CASE SUMMARY A 68-year-old patient presented with a radicular cyst from an upper lateral incisor.The microsurgical management used was aimed at enucleating the chemical membrane,performing apicoectomy of the tooth along with careful and precise retrograde filling,and implementing GTR technique using a resorbable collagen membrane and bovine xenograft.The diagnosis of radicular cyst was confirmed using histopathological analysis.The patient underwent follow-up evaluations at 10 and 30 d postoperatively.At 4 months postoperative evaluation,she remained asymptomatic,and radiographs showed significant periapical healing with adequate bone formation.CONCLUSION These results suggest that microsurgical management using the GTR technique with collagen membrane and xenograft,contributes to bone regeneration.

Pyloric Sympathectomy: A Novel Way to Get Pylorus Relaxation? Report of Two Bariatric Cases

Objective: This case report aimed to demonstrate a possible neuromuscular effect of pyloric sympathectomy on the pylorus motility, leading to its permanent relaxation, observed in two bariatric surgical cases, one primary and the other, revisional. Background: Pyloric sphincter is a physiological barrier to free gastric emptying. In sleeve gastrectomy and in Roux-en-Y gastric bypass revisions, that put the remnant stomach in transit, the pylorus can contribute to generate a high intragastric pressure, which could lead to acute gastric fistulas and/or chronic gastroesophageal reflux disease. The possibility of functionally relaxing the pylorus, promoting intragastric pressure reduction, could represent a protective maneuver against these complications. Methods: We describe herein a surgical maneuver applied in two cases of bariatric surgeries, with the intention of facilitating antrum-duodenal region exposure, in a sleeve gastrectomy and in a Roux-en-Y gastric bypass conversion to SADI-S. For this, a ligation of terminal peripyloric vessels was done, allowing a passage of a tape, which surrounded the pylorus, for its traction. In both cases, 30-day and 1-year endoscopic evaluations were done, to assess esophagitis signs, anastomosis healing and anatomical pouch aspect. A clinical evaluation was also done, to assess gastroesophageal symptoms (validated questionnaire). Results: In the postoperative endoscopic evaluations, no esophagitis were observed in any case and the gastric pouches had normal aspect, with the expected anatomical findings, compatible with the surgeries performed. The anastomosis healed without problems, in the revisional case. Interestingly, in both cases, it was observed a complete pyloric sphincter relaxation, which lasted until 1-year endoscopic evaluation. There were no fistulas, nor esophagitis in any of these cases. Obesity is a significant and escalating health issue both in Brazil and globally, with over 650 million overweight adults worldwide. The treatment of obesity can be performed clinically, endoscopically or surgically;surgical treatment proves to be safe and more effective in terms of weight loss and long-term maintenance. Objective: This study aimed to monitor the progress of weight loss and comorbidity control in patients undergoing sleeve gastrectomy with duodenal bipartition. Methods: This pilot project involved 8 patients divided into 2 arms. In the first arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenoileal transit bipartition (S-RYDITB), while in the second arm, patients underwent sleeve gastrectomy with Roux-en-Y duodenojejunal transit bipartition (S-RYDJTB). Both procedures involved Roux-en-Y reconstruction without duodenal exclusion. In S-RYDITB, the duodenal-ileal anastomosis was performed 300 cm from the ileocecal valve (ICV), creating a 250 cm common channel and a 50 cm alimentary channel. In S-RYDJTB, a biliopancreatic loop was created 200 cm from the angle of Treitz, with a 1 m alimentary channel. Results: Five patients underwent the procedures, with one undergoing S-RYDITB and four undergoing S-RYDJTB. No adverse events such as hospitalizations, readmissions, reoperations, fistulas, bleeding, pulmonary embolism, diarrhea, dumping syndrome, or hypoglycemia occurred during the study period. The mean length of hospital stay was 2 days. The average BMI decreased from 37.27 kg/m2 preoperatively to 29.48 kg/m2 after 6 months. The significant percentage of weight loss was 21.22%, with excess weight loss of 63.6%. Ninety-five percent remission of comorbidities, including hypercholesterolemia, hypertriglyceridemia, diabetes, hypertension, steatosis, and pre-diabetes. 研究激光粉末床熔融增材制造(AM)(又称直接金属激光烧结)技术制备的Ti6Al4V合金等离子体电解氧化(PEO)处理及其电化学行为。通过短时间(<120 s)PEO处理(也称闪速PEO),在AM合金和传统合金表面制备了3~10μm厚、含Ca和P的涂层。然后在改良的α-MEM溶液中,通过动电位极化曲线和电化学阻抗谱(EIS)评估了合金的电化学行为。与传统合金相比,AM合金中形成了细小的层片状α显微组织和层间小尺寸的β相颗粒,这促使了火花的产生,从而促进了PEO涂层的生长。闪速PEO涂层提高了传统合金和AM合金的耐腐蚀性,最薄的涂层(<3μm)提供了高达3倍的保护。AM Ti6Al4V由于其高的晶界密度,易受局部缝隙腐蚀的影响。而即使短至35 s的闪速PEO处理也足以成功避免这种情况。

Anaerobic Mono-Digestion of Turkey Manure: Efficient Revaluation to Obtain Methane and Soil Conditioner

This work demonstrates the possibility to make a full valuation of a solid waste such as turkey manure, to obtain methane and a soil conditioner/fertilizer from turkey manure anaerobic digestion in a mesophilic pilot-scale continuous stirred tank reactor at different organic loading rates (OLR) (from 0.5 to 2.5 kgVS/m3d). The application of the anaerobic mono-digestion for the turkey manure treatment was an efficient alternative, because high volatile solids removal and methane were obtained in addition to obtaining a stabilized solid waste that can be applied as soil conditioner, based on its nutritional parameters and humic substances content. In this way, the turkey manure anaerobic digestion can be applied avoiding the co-digestion of the manure with other wastes and allows a process devoid of pollutant emissions, obtaining two products. The reactor operation depends on the OLR, and its operation does not allow an OLR above 1.5 kgVS/m3d. Higher OLR produced a decrease in the TS and VS removals and methane productivity.

Because incomplete confirmation is available concerning the influential role of atmosphere contamination on conjunctivitis,myopia,asthma,and allergic rhinitis in Brazil,the focus of the present work is to explore the possible relations among atmosphere contamination and eye problems.Rather that a case study on eye diseases,by way of questionnaires supplemented by the investigation of nanoparticles(NPs)on eyeglasses,the study examines the mechanisms in which NPs and ultra-fine particles are deposited on the glasses of children up to 10 years of age in urban and rural area.The important connection between atmosphere contaminants and individual protection equipment justifies improving indoor school properties in order could protect children’s eyes,particularly in high-pollution/high-particulate areas.

Patients with liver cirrhosis,irrespective of etiology,have increased risk of liver-related and all-cause mortality(1).Also,the severity of cirrhosis determines health outcomes.For instance,patients with compensated cirrhosis have a nearly five-fold increased risk of death,while those with decompensated cirrhosis have approximately a 10-fold increased risk compared with the general population(1).Furthermore,the median survival time of patients with compensated cirrhosis is 12 years,while it is only 1.8 years in those with decompensated cirrhosis(2).Moreover,the presence of intercurrent conditions,such as sarcopenia(loss of skeletal muscle mass and function)may also compromise the prognosis of patients with cirrhosis(3).
