Idiosyncratic drug-induced liver injury(iDILI) encompasses the unexpected harms that prescription and non-prescription drugs,herbal and dietary supplements can cause to the *** remains a major public health problem and a major cause of drug *** the lack of biomarkers for iDILI prediction,diagnosis and prognosis,searching new models to predict and study mechanisms of iDILI is *** of the major limitations of iDILI preclinical assessment has been the lack of correlation between the markers of hepatotoxicity in animal toxicological studies and clinically significant ***,major advances in the understanding of iDILI susceptibility and pathogenesis have come from the study of well-phenotyped iDILI ***,there are many gaps for explaining all the complexity of iDILI susceptibility and ***,there is a need to optimize preclinical human in vitro models to reduce the risk of iDILI during drug ***,the current experimental models and the future directions in iDILI modelling are thoroughly discussed,focusing on the human cellular models available to study the pathophysiological mechanisms of the disease and the most used in vivo animal iDILI *** also comment about in silico approaches and the increasing relevance of patient-derived cellular models.

New Classification and Visual Analysis of the Patella Cartilage during Total Knee Replacement

Objective: The aim of this prospective study is to evaluate how much damage the patellar cartilage presents during a total knee replacement. Methods: The damage of the articular patellar surface was analysed by visual inspection and photographs in 354 primary total knee replacements. The authors graded the degree of cartilage lesion in five groups. The cartilage status was analyzed and correlated with age, gender, side, body mass index (BMI), Kellgren-Lawrence radiographic scale and axial deviation. Results: After statistical analysis, we concluded: there was no evidence of an association between patellar arthrosis and age gender, side, weight and deformity. Conclusions: Articular cartilage was damaged in all 354 knees. Important subchondral bone exposure occurred in 274 knees (77.4%). Obese patients had more severe patellar osteoarthritis.

A spontaneous portosystemic shunt is a rare malformation of the liver vessels. The etiology of these shunts is controversial. They can be divided into acquired (most commonly associated with liver cirrhosis) and congenital. Vascular shunts are asymptomatic in the majority of the patients, and when symptomatic were presented by severe complications. The specific way of management can be selected depended on whether the patient was symptomatic or not, and also on the size of the shunt, shunt ratio and whether it was associated with cancer. We will present a clinical case and discuss the importance of the radiological imaging in the screening, diagnosis and follow up of these anomalies.

Objective:To study the behavioral effects of semi purified fraction prepared from the stem bark of Erythrina velutina on ***:Three chloroformic fractions(CA) at 50 or 100 mg/kg(CA1,CA2 and CA3) were tested in two behavioral tests;plus maze and open ***:In the plus maze test,it was observed a decrease in the number of entries in the closed arms(NECA) CA2 at both doses as compared to the control *** the open field test,CA2 and CA3 fractions decreased the number of squares crossed and the time of permanence on the bar,increased the number of falls at both doses in the rota rod *** CA2 fraction decreased the NECA,but this data did not characterize an anxiolytic *** was the consequence from decrease of locomotor activity(LA) observed in the open field test,that the decrease of LA and rearing,the number of falls and the decrease in time of permanence on the bar in rota rod test were observed after treatment with CA2 and CA3 at both doses,suggesting sedative ***:The present work demonstrated that CA2 and CA3 fractions have impairing effect on LA and motor coordination.

Introduction: The therapeutic success of total hip arthroplasty (THA) is unquestionable. However, in young patients, it is still a controversial topic, not because of the results obtained, but because of the presumed need for revision of the prosthetics components in the future. Objectives: The present work performs a retrospective study of patients who underwent THA surgery in the past, in which the femoral head was transferred to the iliac for use in future revisions of total hip arthroplasty (autograft), and describes the proposed technique. Methods: The research reviewed 27 patients operated on using the femoral head autograft technique for the ipsilateral iliac in total hip arthroplasty, performed from 1996 to 2005;the study considered medical records, X-rays and CT scan images, and photographs taken at the time of surgery. Results: To date, only two revisions of total hip arthroplasty have been performed, in which the patients have benefited from the technique of autografting of the femoral head. No case evolved with infection or any other type of complication. All inserted grafts are consolidated and integrated with the iliac. Conclusion: The autograft technique is cheap, simple and reproducible, making available large amounts of bone for use in future revisions of total hip arthroplasty in young patients.

The aim of this study was to highlight the importance of knowledge of idiopathic dilated cardiomyopathy (IDC) in pregnancy and its complications, with the order to establish early diagnosis and treatment. We report the case of a primigravida aged 17, previously healthy, 18 weeks of gestation with dyspnea at rest, which started 2 weeks ago. During the investigation, the echocardiogram (ECO) showed left ventricle (LV) dilation with impaired systolic and diastolic function, LV ejection fraction of 20%. We did not identify a cause for the patient’s clinical condition, being diagnosed with IDC. The patient remained in obstetrics and cardiology monitoring for up to 23 weeks of gestation, when it was decided by the interruption of pregnancy by caesarean section due to hemodynamic instability. The newborn came to death four days after the procedure. The patient evolved to clinical improvement, 16 days after the resolution of gestation, was discharged for outpatient monitoring.

Currently, engineering processes require reduced manufacturing time and low cost, in addition to the growing demand for workpieces with high accuracy. Workpieces with complex geometries and free forms has been a common practice in industries from different sectors such as： automotive, aeronautics, bioengineering among others. One way to satisfy the market requirements satisfactorily is making measurements more efficient to make the production process faster, in other words, it is necessary to make the inspection system more accurate and flexible. The coordinate measuring evolved over the past three decades and today it is the technology that best meets the requirements of modem manufacturing through CMMs （coordinate measurement machines）. The CMMs are important tool for design, fabrication and inspection of manufactured products, also used in the application of reverse engineering. These machines are also used by engineers in order to produce an accurate digital model in a virtual space for later use in CAD （computer-aided design）/CAM （computer-aided manufacturing）. It is worth mentioning that the accuracy of the modeling process of given piece depends on the number of control points that are captured on the workpiece surface. Consequently, the laser inspection systems are the best tools for use in reverse engineering, but more expensive when compared to contact measurement systems that use the TTP （touch trigger probe）, also used by CMMs. In this case, this paper aims to present an approach based on NURBS （non-uniform rational B-splines） to obtain free form curves and surfaces from a group of points obtained by using a contact sensor, the touch trigger probe. NURBS is an important mathematical tool and consists of generalizations of Bezier curves and surfaces and B-splines. Postoperative Endophthalmitis Caused by Candida Parapsilosis: A Case Report

Purpose: To report an unusual case of postcataract endophthalmitis secondary to Candida parapsilosis. Case Presentation: A 77-year-old female patient was admitted to the emergency ward with reduced visual acuity (VA), and redness and pain in the right eye (RE) with a few hours of evolution. The patient underwent cataract surgery in the right eye 50 days earlier;no abnormalities were discovered in postsurgery check-ups on the following days. Biomicroscopy revealed corneal oedema, tyndall ++++, and flare +++. Capsule-shaped whitish deposits and posterior synechiae were also reported. Fundoscopy: not visible due to anterior segment opacity. RE ultrasound: mild vitritis, attached retina. Due to a strong suspicion of endophthalmitis, an aqueous humour biopsy was performed and the presence of Candida parapsilosis sensitive to amphotericin B and voriconazole was revealed. In view of a poor prognosis, we decided to carry out a vitrectomy, removal of the IOL + capsular bag complex and intravitreal injection of amphotericin B. After several check-ups, the patient reported being diagnosed with a fungal infection in her fingernails some time ago. She was referred for a microbiological testing, which confirmed the presence of Candida in the fingernails. Conclusions: Fungal endophthalmitis is uncommon in the postoperative setting, with most of the cases being attributable to the Candida species. The combination of pars plana vitrectomy, removal of the IOL + capsular bag complex and antifungal agents was able to successfully treat this case of postoperative endophthalmitis due to Candida parapsilosis. The relationship between postoperative Candida endophthalmitis and Candida nail infection is dramatic. To date, only one case of endogenous endophthalmitis associated with complicated onychomycosis has been reported.

AIM： To determine which IIRC scheme was used by retinoblastoma centers worldwide and the percentage of D eyes treated primarily with enucleation versus globe salvaging therapies as well as to correlate trends in treatment choice to IIRC version used and geographic region. METHODS： An anonymized electronic survey was offered to 115 physicians at 39 retinoblastoma centers worldwide asking about IIRC classification schemes and treatment patterns used between 2008 and 2012. Participants were asked to record which version of the IIRC was used for classification, how many group D eyes were diagnosed, and how many eyes were treated with enucleation versus globe salvaging therapies. Averages of eyes per treatment modality were calculated and stratified by both IIRC version and geographic region. Statistical significance was determined by Chi-square, ANOVA and Kruskal-Wallis tests using Prism. RESULTS： The survey was completed by 29% of physicians invited to participate. Totally 1807 D eyes were diagnosed. Regarding IIRC system, 27% of centers used the Children＇s Hospital of Los Angeles （CHLA） version, 33% used the Children＇s Oncology Group （COG） version, 23% used the Philadelphia version, and 17% were unsure. The rate for primary enucleation varied between 0 and 100% and the mean was 29%. By IIRC version, primary enucleation rates were： Philadelphia, 8%; COG, 34%; and CHLA, 37%. By geographic region, primary enucleation rates were： Latin America, 57%; Asia, 40%; Europe, 36%; Africa, 10%, US, 8%; and Middle East, 8%. However, systemic chemoreduction was used more often than enucleation in all regions except Latin America with a mean of 57% per center （P〈0.0001）. CONCLUSION： Worldwide there is no consensus on which IIRC version is used, systemic chemoreduction was the most frequently used initial treatment during the study period followed by enucleation and primary treatment modality, especially enucleation, varied greatly with regards to IIRC version used and geographic r
