Watch and wait approach in rectal cancer:Current controversies and future directions

According to the main international clinical guidelines,the recommended treatment for locally-advanced rectal cancer is neoadjuvant chemoradiotherapy followed by ***,doubts have been raised about the appropriate definition of clinical complete response(cCR)after neoadjuvant therapy and the role of surgery in patients who achieve a *** resection is associated with significant morbidity and decreased quality of life(QoL),which is especially relevant given the favourable prognosis in this patient subset. Accordingly, therehas been a growing interest in alternative approaches with less morbidity,including the organ-preserving watch and wait strategy, in which surgery isomitted in patients who have achieved a cCR. These patients are managed with aspecific follow-up protocol to ensure adequate cancer control, including the earlyidentification of recurrent disease. However, there are several open questionsabout this strategy, including patient selection, the clinical and radiologicalcriteria to accurately determine cCR, the duration of neoadjuvant treatment, therole of dose intensification (chemotherapy and/or radiotherapy), optimal followupprotocols, and the future perspectives of this approach. In the present review,we summarize the available evidence on the watch and wait strategy in thisclinical scenario, including ongoing clinical trials, QoL in these patients, and thecontroversies surrounding this treatment approach.

AIM To investigate the main current etiologies of cirrhosis in *** We performed a cross-sectional retrospective multicenter study that included eight hospitals in different areas of Mexico. These hospitals provide health care to people of diverse social classes. The inclusion criteria were a histological, clinical, biochemical, endoscopic, or imaging diagnosis of liver cirrhosis. Data were obtained during a 5-year period(January 2012-December 2017). RESULTS A total of 1210 patients were included. The mean age was 62.5 years(SD = 12.1), and the percentages of men and women were similar(52.0% vs 48.0%). The most frequent causes of liver cirrhosis were hepatitis C virus(HCV)(36.2%), alcoholic liver disease(ALD)(31.2%), and nonalcoholic steatohepatitis(23.2%), and the least frequent were hepatitis B virus(1.1%), autoimmune disorders(7.3%), and other conditions(1.0%).CONCLUSION HCV and ALD are the most frequent causes of cirrhosis in Mexico. However, we note that non-alcoholic fatty liver disease(NAFLD) as an etiology of cirrhosis increased by 100% compared with the rate noted previously. We conclude that NAFLD will soon become one of the most frequent etiologies of liver cirrhosis in Mexico.

Tracking of iron-labeled human neural stem cells by magnetic resonance imaging in cell replacement therapy for Parkinson＇s disease

Human neural stem cells（h NSCs） derived from the ventral mesencephalon are powerful research tools and candidates for cell therapies in Parkinson＇s disease. However, their clinical translation has not been fully realized due, in part, to the limited ability to track stem cell regional localization and survival over long periods of time after in vivo transplantation. Magnetic resonance imaging provides an excellent non-invasive method to study the fate of transplanted cells in vivo. For magnetic resonance imaging cell tracking, cells need to be labeled with a contrast agent, such as magnetic nanoparticles, at a concentration high enough to be easily detected by magnetic resonance imaging. Grafting of human neural stem cells labeled with magnetic nanoparticles allows cell tracking by magnetic resonance imaging without impairment of cell survival, proliferation, self-renewal, and multipotency. However, the results reviewed here suggest that in long term grafting, activated microglia and macrophages could contribute to magnetic resonance imaging signal by engulfing dead labeled cells or iron nanoparticles dispersed freely in the brain parenchyma over time.

Light losses in hollow, prismatic light guides related to prism defects: a transmittance model

Hollow, cylindrical, prismatic light guides （CPLGs） are optical components that, using total internal reflection （TIR）, are able to transmit high-diameter light beams in daylight and artificial lighting applications without relevant losses. It is necessary to study the prism defects of their surfaces to quantify the behavior of these optical components. In this Letter, we analyze a CPLG made of a transparent dielectric material. Scanning electron microscopy （SEM） and the topographic optical profilometry by absorption in fluids （TOPAF） imaging tech- nique are conducted to determine if there are defects in the corners of the prisms. A model for light guide trans- mittance that is dependent on prism defects is proposed. Finally, a simulation and an experimental study are carried out to check the validity of the proposed model.

Background:Universal vaccination for hepatitis B virus(HBV)and migratory movements have changed the demographic characteristics of this disease in Spain and in ***,we evaluated the characteristics of the disease and the possible differences according to origin(immigrants vs non-immigrants)and access to ***:This is a multicenter cross-sectional study(June 2014 to May 2015)in which outpatients with a positive HBsAg were seen and followed in four Hepatology *** and clinical data and indication and access to treatment were collected in two different regions of Catalonia(Spain)where there are no barriers to treatment due to a comprehensive coverage under the National Health ***:A total of 951 patients were evaluated(48.1%men).Of these,46.6%were immigrants(58.7%of them were born in Africa)and were significantly younger compared to *** proportions of patients with alcohol consumption,being overweight,and other indicators of metabolic co-morbidities were significantly higher in *** the 937 patients receiving HBeAg examination,91.7%were *** HBeAg-positive infection was significantly higher in immigrants(3.9%vs 0.6%,P=0.001)and chronic HBeAg-negative hepatitis was higher non-immigrants(31.7%vs 21.4%,P<0.001).Not only was the proportion of patients who met treatment criteria significantly higher among nonimmigrants(38.4%vs 29.2%,P=0.003),but also the proportion of those with indication of effectively receiving therapy at the time of data collection(83.2%vs 57.8%,P<0.001).Conclusions:The immigrant population with HBV is younger and has a lower prevalence of metabolic co-morbidities and a higher frequency of chronic HBeAg *** having access to care and an indication for treatment,some do not get adequately treated due to several factors including local adaptation that precludes access to treatment.

Open Cows in a Beef Cattle Enterprise Managed in a Seasonal Breeding Program? An Appraisal of Their Reproductive Performance Based on Body Fat Reserves

A comparison between two distinct levels of back fat thickness in open cows, females not becoming pregnant during the previous breeding season, was carried out in order to evaluate their reproductive performance prior to the next natural mating in a seasonal breeding program. Thirty open cows from the previous breeding season were divided evenly by age and back fat thickness (BFT) in two groups. HBFT (high back fat thickness ≥ 70 mm) and LBFT (low back fat thickness ≤ 70 mm), values statistically different between groups (P < 0.05). The study was divided into three phases in which the formation of CL and follicular dynamics were recorded by ultrasound and progesterone. Also, serial monitoring of glucose, urea and triglycerides and back fat thickness were studied. A fertile bull previously approved from a breeding soundness evaluation was introduced after the third phase and remained with the herd for 90 days. No differences were found in the follicular dynamics;the number of animals cycling in the two groups and fertility were also similar (80%). Differences in urea and triglycerides were found in cows with high scores of BFT. In conclusion, the decision of keeping open cows to the next breeding season must be based on an economical advantage to the farmer.

The study of the relationship between crude oil and its refined products prices may be perceived as an important tool for testing how are the dynamics and the type of integration of the petro-derivatives market in the United *** this sense,we have applied a set of causality tests to study the possible presence of asymmetries in the relationship between WTI crude oil and each refined product price and to explore the type of market ***,the application of these causality tests lets us explore the validation of different hypotheses in the literature,such as the Rocket and Feathers hypothesis and the Verleger *** findings reveal that Reformulated Gasoline Blendstock for Oxygen Blending(RBOB),heating oil,diesel and kerosene are supply-driven integrated and conventional gasoline and kerosene are demand-driven integrated when linear effects are *** behaviour changes deeply when the existence of asymmetries is tested,noticing that the Rocket and Feathers hypothesis is not fulfilled when a negative shock ***,the Verleger hypothesis is supported when a negative shock appears for conventional gasoline and *** results provide important policy implications for investors,energy policymakers and refiners.

Introduction The Gram-positive bacterium,Streptococcus pneumoniae(***),often colonizes the nasopharynx;however,it is one of the major microorganisms that cause pneumonia and meningitis.[[1],[2],[3]]It presents several virulence factors,such as surface proteins and enzymes,pneumolysin toxin,and a polysaccharide capsule,which is mainly responsible for the immune response;the capsule also facilitates classification of the bacterium into several serotypes.[1,2]In vulnerable individuals,the bacterium may lead to the development of invasive pneumococcal disease(IPD),which is defined as an infection of normally sterile locations.

On-chip wireless silicon photonics: from reconfigurable interconnects to lab-on-chip devices

Photonic integrated circuits are developing as key enabling components for high-performance computing and advanced networkon-chip,as well as other emerging technologies such as lab-on-chip sensors,with relevant applications in areas from medicine and biotechnology to *** demanding applications will require novel features,such as dynamically reconfigurable light pathways,obtained by properly harnessing on-chip optical *** this paper,we introduce a broadband,high directivity(4150),low loss and reconfigurable silicon photonics nanoantenna that fully enables on-chip radiation *** propose the use of these nanoantennas as versatile building blocks to develop wireless(unguided)silicon photonic devices,which considerably enhance the range of achievable integrated photonic *** examples of applications,we demonstrate 160 Gbit s−1 data transmission over mm-scale wireless interconnects,a compact low-crosstalk 12-port crossing and electrically reconfigurable pathways via optical beam ***,the realization of a flow micro-cytometer for particle characterization demonstrates the smart system integration potential of our approach as lab-on-chip devices.
