Lid scrubbing with a gel combining natural extracts for dry eye treatment

AIM:To evaluate the effect of lid scrubbing using a gel combining different natural extracts on ocular signs and symptoms of dry eye patients.METHODS:A retrospective study was performed on 19 dry eye patients(51.1±16.0y).Non-invasive tear break-up time,tear meniscus height,meibomian gland loss,lipid layer thickness,conjunctival redness,corneal staining,ocular surface disease index(OSDI),and ocular pain intensity were measured before(baseline),1wk,and 2mo after lid scrubbing with a gel containing different natural extracts(okra extract,aloe vera leaf juice,hydrolysed soy protein,caffeine,citrus unshiu peel extract,and raspberry seed oil).RESULTS:Compared with the baseline,there was a statistically significant improvement in both first(P=0.048)and average(P=0.026)non-invasive tear break-up time 2mo after treatment,as well as in corneal staining(P=0.043,0.012),OSDI(P<0.001),and ocular pain intensity(P<0.001)after 1wk and 2mo.In addition,there was no correlation between ocular signs and symptoms.CONCLUSION:The lid scrubbing with a gel combining different natural extracts show beneficial effects on tear film stability,corneal damage,and ocular symptoms,which is the reason why this therapeutic procedure is proposed as an alternative for dry eye management.However,it is not possible to attribute this beneficial effect solely to the presence of the natural extracts in the gel,primarily due to the absence of a negative control group.

Background: Deep brain stimulation (DBS) is an established treatment for patients with advanced Parkinson’s disease (PD). Reports show continued patient satisfaction after surgery despite not maintaining clinical improvement as measured by evolution scales. Objectives: The present study sought to explore expectations and level of satisfaction in patients after DBS surgery with a semi-structured questionnaire and subsequent correlation with functional scales, Quality of Life (QoL), and motor and non-motor symptoms. Methods: We performed descriptive statistics to represent demographic data, Wilcoxon rank tests to determine significant differences, and Spearman correlation between the applied scales. Results: We evaluated 20 patients with a history of DBS surgery. 45% were female, with a mean age of 55.7 ± 14.15 years, a mean disease duration of 13.42 ± 8.3 years, and a mean time after surgery of 3.18 ± 1.86 years. Patients reported surgery meeting expectations in 85.5% and continued satisfaction in 92%. These two variables showed a significant correlation. Conclusions: This sample of patients remained satisfied after DBS surgery, although we found no differences in motor and non-motor clinimetric scales. Further studies are needed to confirm the importance of assessing quality of life in patients with DBS.

The innate immune system has evolved to detect molecular signatures associated with invading microbes and the cellular damage these microbes cause.Recognition of microbial genomes,particularly those of intracellular microbes such as viruses,is crucial in triggering a protective response and preventing infection.Many nucleic acid forms are uniquely associated with pathogens(e.g.,5’ppp-RNA,dsRNA)or localize in abnormal cellular compartments during infection(e.g.,cytosolic DNA),facilitating the distinction between self and nonself[1].

More than 30 years ago,a group of researchers in Tampere-Finland developed a thin foamed polymeric material for capacitive sensors.Such soft-voided films exhibited electrical charging characteristics,forming a powerful combination,which resulted in a smart-material with ferroelectric properties.The discovery of the electro-thermo mechanical film(ETMF)has sparked the curiosity of the electret community,leading to the development of several studies.At that time,ETMF became known as cellular electrets and,later,as ferroelectrets or piezoelectrets regarding their electromechanical properties.This paper provides a timeline review of the research on ferroelectrets produced in Brazil,between the years 1990 and 2020,towards demonstrating how the interest in the electret electrical charging mechanism has resulted in the use of ferroelectrets with well-controlled cavities for ultrasound applications.

The differential diagnosis between benign and malignant biliary strictures is challenging and requires a multidisciplinary approach with the use of serum biomarkers,imaging techniques,and several modalities of endoscopic or percutaneous tissue sampling.The diagnosis of biliary strictures consists of laboratory markers,and invasive and non-invasive imaging examinations such as computed tomography(CT),contrast-enhanced magnetic resonance cholangiopancreatography,and endoscopic ultrasonography(EUS).Nevertheless,invasive imaging modalities combined with tissue sampling are usually required to confirm the diagnosis of suspected malignant biliary strictures,while pathological diagnosis is mandatory to decide the optimal therapeutic strategy.Although EUS-guided fine-needle aspiration biopsy is currently the standard procedure for tissue sampling of solid pancreatic mass lesions,its diagnostic value in intraductal infiltrating type of cholangiocarcinoma remains limited.Moreover,the“endobiliary approach”using novel slim biopsy forceps,transpapillary and percutaneous cholangioscopy,and intraductal ultrasound-guided biopsy,is gaining ground on traditional endoscopic retrograde cholangiopancreatography and percutaneous transhepatic cholangiography endobiliary forceps biopsy.This review focuses on the available endobiliary techniques currently used to perform biliary strictures biopsy,comparing the diagnostic performance of endoscopic and percutaneous approaches.

Biotechnological techniques provide a viable alternative to help improve and increase the production of plant species of agricultural and economic importance,which have been affected over the years by climate change,increasing their susceptibility to pests and/or diseases,generating losses in production as well as a decrease in their regenerative and genetic diversity.The application of biotechnological techniques such as in vitro mutagenesis offers a viable option for the generation of crops that are resistant to the different factors caused by abiotic and biotic stress.In vitro mutagenesis has been used in an efficient way to generate genetic changes in different plant species.However,these methods have not been studied thoroughly in crops of agro-industrial interest,such as agave,which represents an economic resource of national importance and are considered as endemic species of Mexico.Therefore,this literary review aimed to focus on the studies that have been used for the genetic improvement of this species via mutagenesis techniques in plants in the agave genus.Therefore,the objective was to set a precedent for future genetic studies that aim to obtain more productive regenerants for various industries,such as food and pharmaceutical.It is also of great interest to compile information from basic research that helps understand and elucidate a model of possible defense mechanisms that are activated in the Agave genus.

Mortality in cirrhosis is mostly associated with the development of clinical decompensation,characterized by ascites,hepatic encephalopathy,variceal bleeding,or jaundice.Therefore,it is important to prevent and manage such complications.Traditionally,the pathophysiology of decompensated cirrhosis was explained by the peripheral arterial vasodilation hypothesis,but it is currently understood that decompensation might also be driven by a systemic inflammatory state(the systemic inflammation hypothesis).Considering its oncotic and nononcotic properties,albumin has been thoroughly evaluated in the prevention and management of several of these decompensating events.There are formal evidence-based recommendations from international medical societies proposing that albumin be administered in individuals with cirrhosis undergoing large-volume paracentesis,patients with spontaneous bacterial peritonitis,those with acute kidney injury(even before the etiological diagnosis),and those with hepatorenal syndrome.Moreover,there are a few randomized controlled trials and meta-analyses suggesting a possible role for albumin infusion in patients with cirrhosis and ascites(long-term albumin administration),individuals with hepatic encephalopathy,and those with acute-on-chronic liver failure undergoing modest-volume paracentesis.Further studies are necessary to elucidate whether albumin administration also benefits patients with cirrhosis and other complications,such as individuals with extraperitoneal infections,those hospitalized with decompensated cirrhosis and hypoalbuminemia,and patients with hyponatremia.

A comprehensive thermo-economic model combining a geothermal heat mining system and a direct supercritical CO_(2) turbine expansion electric power generation system was proposed in this paper.Assisted by this integrated model,thermo-economic and optimization analyses for the key design parameters of the whole system including the geothermal well pattern and operational conditions were performed to obtain a minimal levelized cost of electricity(LCOE).Specifically,in geothermal heat extraction simulation,an integrated wellbore-reservoir system model(T2Well/ECO_(2)N)was used to generate a database for creating a fast,predictive,and compatible geothermal heat mining model by employing a response surface methodology.A parametric study was conducted to demonstrate the impact of turbine discharge pressure,injection and production well distance,CO_(2) injection flowrate,CO_(2) injection temperature,and monitored production well bottom pressure on LCOE,system thermal efficiency,and capital cost.It was found that for a 100 MWe power plant,a minimal LCOE of$0.177/kWh was achieved for a 20-year steady operation without considering CO_(2) sequestration credit.In addition,when CO_(2) sequestration credit is$1.00/t,an LCOE breakeven point compared to a conventional geothermal power plant is achieved and a breakpoint for generating electric power generation at no cost was achieved for a sequestration credit of $2.05/t.
