Minimal hepatic encephalopathy is a subclinical disorder in patients with liver cirrhosis that may have an impact upon driving capacity. This study in highly selected patients without other confounding factors examines whether there is a relationship between the degree of liver fibrosis as determined by Fibroscan exploration and the driving test scores. Using the ASDE DRIVER-TEST N-845 system, validated for obtaining the driving license in Spain, we assessed the calculation of distances and time, bimanual visual-motor coordination, reaction time to different stimuli, concentrated attention, and resistance to monotony. The data were processed using the SPSS statistical package, with an analysis of the correlation between the degree of fibrosis and the test scores based on the Spearman statistic and the comparison of means (Mann-Whitney U-test). We found no alterations in driving capacity in patients diagnosed of hepatitis C with different degrees of early stage fibrosis or cirrhosis.

A study was made to determine whether HIV-infected patients with prolonged virological control suffer subclinical neurocognitive problems capable of interfering with driving skills, compared with the general population, and to explore the possible existence of differences between those treated with and without efavirenz. Material and Methods: We included 40 patients without history of neoplasm, psychiatric disorders or infections of the central nervous system associated or not to HIV, with stable and effective antiretroviral therapy during at least 48 months. Use was made of the ASDE DRIVER TEST N-845 standardized by the Spanish traffic authorities, and for which data corresponding to the Spanish general population were obtained from the manufacturer of the test battery. The Student t-test was used to compare the different variables with the population standards, and the comparison of proportions Z-statistic was used to determine the proportion of subjects above the accepted limit of normality cutoff point. These analyses were replicated for the two sub-samples (with or without efavirenz therapy), with a 95% confidence level. The SPSS version 15 statistical package and Epidat 3.1 program were used. Results: The scores obtained in the HIV group were significantly poorer in the anticipation speed tests and in one of the multiple reactions test, though better results were obtained in the bimanual visual-motor coordination test. There were no differences in the percentages of patients with scores below the recommended limits. On comparing the treatment subgroups (efavirenz versus protease inhibitor), no differences were recorded in any of the study variables, and the differences with respect to the general population were the same as those described for the global group. Zinc (Zn) is a key micronutrient for correct immune function and its deficiency correction has been shown to be useful in HIV-infected but most of the studies included a significant proportion of patients without adequate virological control. It would be interesting to establish the prevalence, associated factors and clinical repercussions of Zn deficiency in patients with good virological to assessing the usefulness of Zn monitoring in the routine follow-up of well controlled HIV-infected patients, based on the colorimetric techniques commonly used in daily clinical practice. We included the first 100 patients that met the requirements of HAART and viremia levels under 200 copies for at least 6 consecutive months, with no active illicit drug use, active infections or weight loss of any cause during the previous 6 months. Serum Zn concentration was measured using a colorimetric assay (Sentinel Diagnostics?) adapted to the Cobas 8000 analyzer (Roche Diagnostics). The Zn values showed a normal distribution with a mean concentration of 88.7 μg/dl (SD 23.3) and were found to be decreased in 13 patients and in 6 subjects were below 61 μg/dl. Both the simple statistical analysis and the multivariate regression model only identified a significant effect for age and alcohol consumption. In sum an important number of HIV-infected with effective and prolonged HAART and no evidence of active infections or other associated factor show diminished serum Zn concentrations. The inclusion of at least occasional Zn determinations should be considered in the regular follow-up evaluations of HIV-infected patients.

One of the great advantages of electrospun fibers is the large tridimensional area produced, capable of storing any type of material. The aim of our investigation is to study the electrospinning technique to produce polymer membranes for drug delivery applications, given their large surface area and ability to transport a bioactive compound. A mathematical modeling of the delivery system kinetics was also studied to find the mechanism that controls the releasing process. Results showed that electrospinning could provide regular and smooth membranes suitable for drug delivery processes. The mathematical modeling also proved that thicker PLLA membranes exhibited a Fickian diffusion behavior during the drug transport, presenting better control in drug delivery processes.

Objective： Analysis of abstinence rates of smokers per gender at 3, 6, 9 and 12 months in a Smoking Cessation Unit from January 2008 to December 2009. Methods： Descriptive retrospective study. Analysis of socio-demographic variables, smoking patterns, associated comorbidities, continuous abstinence rates, success, relapses, failure and dropping out. Results： 278 smokers started treatment （33%）; 55.4% males and 44.6% females （mean age of 48.3 and 44.06 years, respectively）. The main associated comorbidities were： psychiatric （38.7%）, cardiovascular （dyslipemia 25%） and respiratory （COPD （chronic obstructive pulmonary disease） 9.7%） in females; cardiovascular （dyslipemia 34.4%）, psychiatric （34.4%） and respiratory （COPD 19.5%） in males. VRN （Varenicline） was prescribed in 40.2% males and 32.2% females; NRT （nicotine replacement therapy） was used in 46.6% and 38.7%, respectively; bupropion was employed in 6.5% and 21.8%, respectively. Psychological counseling was offered only to 7.8% males and 8.1% females. The continuous abstinence rates in males at 3, 6, 9 and 12 months were 51.3%, 37.7%, 32.5% and 30.5%, respectively, and were 45.2%, 29.8%, 25.0% and 24.2% for females. Failure was 9.7% for females and 18.8% for males. Success was more frequent for those on VRN （n = 41 males; n = 12 females）. No relapses were indicated for 42 females and 71 males. The percentage of relapses was higher at 3 months （29.0% females, 19.5% males）. Conclusions： The study observed differences in treating abstinence between genders （in the abstinence rates and failure index）. This implies having to consider incorporating the gender variable into the diagnosis, treatment and prevention of smoking.

We studied 28 patients with fixed drug eruption (FDE) caused by sulfonamide antibiotics to investigate cross-reactivity between sulfonamide derivatives and p-amino compounds and to explore the usefulness of patch testing, as an alternative to controlled oral challenge testing (COCT), in diagnosis within this clinical area. COCT with sulfamethoxazole (SMX), sulfadiazine (SDZ), sulfamethizole (SMZ), furosemide (FU), procaine (PRO) and glipizide (GPZ) was performed. Patch testing (PT)-with SMX and SDZ was carried out. In all patients, the diagnosis of FDE was confirmed by positive COCT and allergy to trimethoprim ruled out by COCT. 42.8 and 31.8%of the SMX-induced FDE patients reacted to SMZ and SDZ, respectively. All patients (n = 28) tolerated FU, PRO and GPZ. COCT performed with the 3 sulfonamide antibiotics in 12 patients was positive in 2 subjects with the 3 drugs, in 2 patients only with SMXand SMZ and in the remaining 8, SMXwas the only causative drug. PTwas positive in 5 of 25 patients positive onCOCT. The probability of obtaining a positive PT was higher among patients who had a residual lesion than that among those who lacked this. Cross-reactivity between different sulfonamide antibiotics is thus variable, being most likely between SMX and SMZ. We have found no cross-reactivity between sulfonamide antibiotics and other sulfonamide derivatives or p-amino drugs in FDE. PT is a useful tool in the diagnosis of FDE, especially if there are residual lesions, because it avoided the need for COCT in 20%of patients.
