Diabetic foot syndrome:Immune-inflammatory features as possible cardiovascular markers in diabetes

Diabetic foot ulcerations have been extensively reported as vascular complications of diabetes mellitus associated with a high degree of morbidity and mortality. Diabetic foot syndrome(DFS), as defined by the World Health Organization, is an "ulceration of the foot(distally from the ankle and including the ankle) associated with neuropathy and different grades of ischemia and infection". Pathogenic events able to cause diabetic foot ulcers are *** the commonest causes of this pathogenic pathway it's possible to consider peripheral neuropathy, foot deformity, abnormal foot pressures, abnormal joint mobility, trauma, peripheral artery disease. Several studies reported how diabetic patients show a higher mortality rate compared to patients without diabetes and in particular these studies under filled how cardiovascular mortality and morbidity is 2-4 times higher among patients affected by type 2 diabetes mellitus. This higher degree of cardiovascular morbidity has been explained as due to the observed higher prevalence of major cardiovascular risk factor, of asymptomatic findings of cardiovascular diseases, and of prevalence and incidence of cardiovascular and cerebrovascular events in diabetic patients with foot complications. In diabetes a fundamental pathogenic pathway of most of vascular complications has been reported as linked to a complex interplay of inflammatory, metabolic and procoagulant variables. These pathogenetic aspects have a direct interplay with an insulin resistance, subsequent obesity, diabetes, hypertension, prothrombotic state and blood lipid disorder. Involvement of inflammatory markers such as IL-6 plasma levels and resistin in diabetic subjects as reported by Tuttolomondo et al confirmed the pathogenetic issue of the a "adipo-vascular" axis that may contribute to cardiovascular risk in patients with type 2 diabetes. Gastrointestinal toxicities(GIT), including oral mucositis,nausea and vomiting, and diarrhea, are common side effects of chemotherapy and targeted agents in patients with advanced colorectal cancer and pancreatic cancer. Being often underreported, it is still difficult to precisely establish their burden in terms of both patient's quality of life and cancer care costs. Moreover, with the use of more intensive upfront combination regimens, the frequency of these toxicities is rapidly growing with a potential negative effect also on patient's outcome, as a result of dose reductions, delays or even discontinuation of active treatments. Thus, identifying patients at higher risk of developing GIT as well as an optimal management are paramount in order to improve patient's compliance and outcome. After the description of the main treatment-induced GIT, we discuss the current knowledge on the pathophysiology of these side effects and comment the scales commonly used to assess and grade them. We then provide a critical update on GIT incidence based on the results of key randomized trials conducted in patients with metastatic colorectal cancer and advanced pancreatic cancer.

Current status of magnetic resonance imaging radiomics in hepatocellular carcinoma:A quantitative review with Radiomics Quality Score

BACKGROUND Radiomics is a promising tool that may increase the value of magnetic resonance imaging(MRI)for different tasks related to the management of patients with hepatocellular carcinoma(HCC).However,its implementation in clinical practice is still far,with many issues related to the methodological quality of radiomic *** To systematically review the current status of MRI radiomic studies concerning HCC using the Radiomics Quality Score(RQS).METHODS A systematic literature search of PubMed,Google Scholar,and Web of Science databases was performed to identify original articles focusing on the use of MRI radiomics for HCC management published between 2017 and *** methodological quality of radiomic studies was assessed using the RQS ***’s correlation(ρ)analysis was performed to explore if RQS was correlated with journal metrics and characteristics of the *** level of statistical significance was set at P<*** One hundred and twenty-seven articles were included,of which 43 focused on HCC prognosis,39 on prediction of pathological findings,16 on prediction of the expression of molecular markers outcomes,18 had a diagnostic purpose,and 11 had multiple *** mean RQS was 8±6.22,and the corresponding percentage was 24.15%±15.25%(ranging from 0.0% to 58.33%).RQS was positively correlated with journal impact factor(IF;ρ=0.36,P=2.98×10^(-5)),5-years IF(ρ=0.33,P=1.56×10^(-4)),number of patients included in the study(ρ=0.51,P<9.37×10^(-10))and number of radiomics features extracted in the study(ρ=0.59,P<4.59×10^(-13)),and time of publication(ρ=-0.23,P<0.0072).CONCLUSION Although MRI radiomics in HCC represents a promising tool to develop adequate personalized treatment as a noninvasive approach in HCC patients,our study revealed that studies in this field still lack the quality required to allow its introduction into clinical practice.

Valorisation of industrial hemp (Cannabis sativa L.) residues and cheese whey into volatile fatty acids for single cell protein production

The production of single cell protein(SCP)using lignocellulosic materials stands out as a promising route in the circular bioeconomy ***,multiple steps are necessary for lignocellulosics-to-SCP processes,involving chemical pretreatments and specific aerobic *** there are no studies that investigated the SCP production from lignocellulosics by using only biological processes and microbial biomass able to work both anaerobically and *** this view,the valorisation of industrial hemp(Cannabis sativa L.)biomass residues(HBRs),specifically hurds and a mix of leaves and inflorescences,combined with cheese whey(CW)was investigated through a semi-continuous acidogenic co-fermentation process(co-AF).The aim of this study was to maximise HBRs conversion into VFAs to be further used as carbon-rich substrates for SCP *** process conditions were tested by either removing CW or increasing the amount of HBRs in terms of VS(i.e.,two and four times)to evaluate the performance of the co-AF *** HBRs resulted in a proportional increase in VFA production up to 3115 mg HAc L^(-1),with experimental production nearly 40%higher than theoretical *** synergy between HBRs and CW was demonstrated,proving the latter as essential to improve the biodegradability of the *** produced VFAs were subsequently tested as substrates for SCP synthesis in batch aerobic tests.A biomass concentration of 2.43 g TSS L^(-1) was achieved with a C/N ratio of 5.0 and a pH of 9.0 after two days of aerobic fermentation,reaching a protein content of 42%(g protein per g TSS).These results demonstrate the overall feasibility of the VFA-mediated HBR-to-SCP valorisation process.

450 nm diode laser:A new help in oral surgery

AIM To describe the performance of 450 nm diode laser in oral surgery *** The case described consisted of the removal of a lower lip fibroma through a blue diode laser(λ = 450 nm). RESULTS The efficacy of this device, even at very low power(1W, CW), allows us to obtain very high intra and postoperative comfort for the patient, even with just topical anaesthesia and without needing suture. The healing process was completed in one week and, during the follow-up, the patient did not report any problems, pain or discomfort even without the consumption of any kind of drugs, such as painkillers and antibiotics. The histological examination performed by the pathologist showed a large area of fibrous connective tissue with some portions of epithelium-connective detachments and a regular incision with very scanty areas of *** The 450 nm diode laser proved of being very efficient in the oral soft tissue surgical procedures, with no side effects for the patients.

Quality control,commissioning and diagnostic tests on insulation systems of electrical apparatus are necessary to reduce the failure risk under operation,eliminate infant mortality failures and reduce the impact of ageing on insulation system *** study tries to establish a path which,based on the statistical-phenomenological modelling of the hazard rate,or bathtub curve,drives in the choice of test overvoltage and *** choice of the test voltage and its waveform should be made carefully to improve reliability(reduce failure hazard)in any section of the bathtub *** may require,however,different choices depending on the section which corresponds to the planned *** example,large overvoltages could be acceptable to remove the infant mortality to ensure the highest reliability of newly commissioned apparatus,but they can incept highly energetic,and,thus,fast degradation mechanisms on apparatus operating on the field for a period of time,that can worsen,rather than improving,the insulation system *** to partial discharge measurement during any overvoltage test seems to be the optimal solution to find the most appropriate overvoltage for any type of testing need,leveraging the Latin concept that‘in medio stat virtus’.

Numerical investigation of the dynamics of a rigidspherical particle in a vortical cross-slot flow atmoderate inertia

The study of flow and particle dynamics in microfluidic cross-slot channels is of high relevance for lab-on-a-chipapplications. In this work, we investigate the dynamics of a rigid spherical particle in a cross-slot junction for a channelheight-to-width ratio of 0.6 and at a Reynolds number of 120 for which a steady vortex exists in the junction *** an in-house immersed-boundary-lattice-Boltzmann code, we analyse the effect of the entry position of theparticle in the junction and the particle size on the dynamics and trajectory shape of the particle. We find that thedynamics of the particle depend strongly on its lateral entry position in the junction and weakly on its vertical entryposition;particles that enter close to the centre show trajectory oscillations. Larger particles have longer residencetimes in the junction and tend to oscillate less due to their confinement. Our work contributes to the understanding ofparticle dynamics in intersecting flows and enables the design of optimised geometries for cytometry and particlemanipulation.

Late-onset hypogonadism： beyond testosterone

Late-onset hypogonadism is defined as a combination of low testosterone （T） levels and typical symptoms and signs. A major area of uncertainty is whether T concentrations are always really sufficient to fully reflect Leydig cell （dys）function. Mild testicular alteration could be diagnosed only by additional biochemical markers, such as luteinizing hormone （LH） and 25-hydroxyvitamin D levels. These markers help in identifying the so-called ＂subclinical＂ hypogonadism （normal T, high LH levels）. Patients with hypogonadism have frequently low levels of 25-hydroxyvitamin D due to impairment of the hydroxylating enzyme CYP2R1 in the testis. However, no data have been published dealing with the best treatment option （cholecalciferol - the Vitamin D precursor, or calcidiol - 25-hydroxylated form of Vitamin D） in these patients. We studied 66 patients with classic hypogonadism （total T [TT] 〈12 nmol I-~, LH 〉 8 IU 1-1）（n = 26） and subclinical hypogonadism （TT 〉 12 nmol I-＊, LH 〉 8 IU I-~）（n = 40） and low 25-hydroxyvitamin D （〈50 nmol I-1）. Subjects received cholecalciferol （5000 IU per week）（n = 20） or calcidiol （4000 IU per week）（n -- 46）, and 25-hydroxyvitamin D and parathyroid hormone （PTH） were evaluated after 3 months of therapy. Supplementation with calcidiol significantly increased 25-hydroxyvitamin D and significantly decreased PI＂H levels in both groups of men with hypogonadism （primary, n = 16 and subclinical, n = 30）, whereas supplementation with cholecalciferol did not modify their levels. This study shows for the first time that the administration of the 25-hydroxylated form of Vitamin D （calcidiol）, and not the administration of the precursor cholecalciferol, restores 25-hydroxyvitamin D levels in subjects with hypogonadism.
