BACKGROUND Hepatitis B virus(HBV)nucleos(t)ide analog(NA)therapy reduces liver disease but requires prolonged therapy to achieve hepatitis B surface antigen(HBsAg)*** is limited North American real-world data using non-invasive tools for fibrosis assessment and few have compared 1st generation NA or lamivudine(LAM)to tenofovir disoproxil fumarate(TDF).AIM To assess impact of NA on virological response and fibrosis regression using liver stiffness measurement(LSM)(i.e.,FibroScan®).METHODS Retrospective,observational cohort study from the Canadian HBV *** collected included demographics,NA,HBV DNA,alanine aminotransferase(ALT),and *** were HBV monoinfected patients,treatment naïve,and received 1 NA with minimum 1 year *** In 465(median 49 years,37%female,35%hepatitis B e antigen+at baseline,84%Asian,6%White,and 9%Black).Percentage of 64(n=299)received TDF and 166 were LAM-treated with similar median duration of 3.9 and 3.7 years,*** mean baseline LSM was 11.2 kPa(TDF)vs 8.3 kPa(LAM)(P=0.003).At 5-year follow-up,the mean LSM was 7.0 kPa in TDF vs 6.7 kPa in LAM(P=0.83).There was a significant difference in fibrosis regression between groups(i.e.,mean-4.2 kPa change in TDF and-1.6 kPa in LAM,P<0.05).The last available data on treatment showed that all had normal ALT,but more TDF patients were virologically suppressed(<10 IU/mL)(n=170/190,89%)vs LAM-treated(n=35/58,60%)(P<0.05).None cleared *** In this real-world North American study,approximately 5 years of NA achieves liver fibrosis regression rarely leads to HBsAg loss.

Dear Editor,Previous studies had described the adaptation of enterovirus 71(EV-A71)strains that enabled entry and viral replication in Chinese Hamster Ovary(CHO)cell line(Zaini and McMinn 2012;Zaini et al.2012).These adapted strains derived from serial passage of a clinical isolate in CHO cells exhibited an amino acid substitution at VP2149,which enhanced viral replication by 100*1000-fold compared to the clinical *** VP2149 mutation was claimed responsible for adaptation to CHO-K1 cells without performing detailed molecular analyses to support these *** this study,we evaluate various VP1 and VP2 mutations in two CHO-adapted EV-A71 strains derived in our lab to assess their contribution to the phenotype of CHO cell adaptation.

The long-term outcomes of 157 patients affected by Peyronie＇s disease （PD） who underwent penile straightening with Egydio＇s technique between January 2004 and December 2008 are reported. Only patients with PD who were stable for at least 6-12 months prior to surgery were enrolled in this study. Preoperative assessment included a dynamic echo colour Doppler ultrasound scan to evaluate the degree of penile deformity and the peak systolic velocity in the cavernosal arteries and an assessment of erectile function with the administration of the International Index of Erectile Function 5 （IIEF-5） questionnaire. Stretched penile length was recorded pre- and postoperatively. Surgical complications, cosmesis and sexual function, patient satisfaction and postoperative erectile function were assessed postoperatively at 3 months, I year and 2 years, respectively. After a median follow-up period of 20 months （range： 12-24 months）, we found that mild residual curvature （12%） and glans hypoesthesia （3%） were the only causes of partial dissatisfaction. No rejection of the graft was observed. All patients recovered their ability to penetrate with no difficulty. In addition, an intraoperative average increase of 2.5 cm （range： 1.7-4.1 cm） in stretched penile length was recorded, with all patients engaging in penetrative sexual intercourse. In conclusion, this procedure represents a safe and reproducible technique for the correction of penile curvature resulting from PD and yields excellent cosmetic and functional results.

Four sites were selected in a salt marsh in the Bahia Blanca Estuary （Argentina）：（1） low marsh （flooded by the tide twice daily） vegetated by S. alterniflora; （2） non-vegetated low marsh; （3） high marsh （flooded only in spring tides） vegetated by S. alterniflora; （4） non-vegetated high marsh. The pH and Eh were measured in sediments, while dissolved nutrients （ammonium, nitrate, nitrite and phosphate） and particulate organic matter （POM） were determined in pore water, pH （6.2-8.7） was only affected by vegetation in low areas. Eh （from -300 to 250 mV） was lower at low sites than at high ones; in the latter, the values were higher in the non-vegetated sediments. The POM concentration was greater in the high marsh than in the low marsh, with no effect of vegetation. Ammonium was the most abundant nitrogen nutrient species in pore water, except in the non-vegetated high marsh where nitrate concentration was higher. All nitrogen nutrients were affected by both flooding and vegetation. Phosphate was always present in pore water at all sites throughout the year and its concentration varied within narrow limits, with no effect of flooding and greater values always at non-vegetated sites. Our results showed that the variability of the pore water composition within the marsh is greater than the temporal variation, meaning that both tidal flooding and vegetation are important in the dynamics of nutrients and organic matter in the sediment pore water.

In the early part of 2006 the Philippines was hit by a La Niña event which brought floods, flashfloods and landslides. The most significant event was the February 2006 landslide that covered the whole community of Guinsaugon, in Southern Leyte, resulting in 154 deaths with 973 people still missing. Another rare event was the simultaneous presence of two tropical cyclones inside the Philippine Area of Responsibility (PAR) in August - Tropical Depression Bopha and Tropical Storm Saomai - that enhanced the southwest monsoon. With the strong winds and high waves associated with the southwest monsoon, an oil tanker, the M/T Solar 1, sank, resulting in the worst oil spill the country has ever suffered. Lastly, with a weak El Niño phenomenon forming in the Pacific Ocean and affecting the Philippines, a destructive typhoon, Xangsane, made a multi-landfall before hitting Metropolitan Manila. This was followed by the landfall of three supertyphoons - Cimaron, Chebi and Durian - during the last quarter of the year, bringing tremendous destruction of life and property. The occurrence of these weather events are attributed to climate variability the effects of which were exacerbated by a range of factors that include inappropriate land use.

This paper presents a performance-based design of a HVAC plant and controller system of the “Palagio di Parte Guelfa” Palace, a medieval building converted into a museum in the city of Firenze. Transient simulations to evaluate the internal microclimate conditions during the year for the building and the different system plant were carried out. A comparison between the indoor microclimatic parameters obtained and the values suggested by the standards on protection and conservation of historical works of art, suggested a non-invasive and reversible plant system solution. The MuseumofHistorical Footballbuilding portion is the case study. The proposed plant, a constant air flow system coupled with radiant panels dry mounted above the existing flooring, was simulated throughout the year under transient conditions to evaluate its energy performance. This solution ensures indoor temperature and humidity values suitable for the conservation of works of art and visitor comfort. It is an example of a possible course of action for a plant refurbishment in a historic building converted into a museum, located in climatic regions characterized by high thermal and solar loads. The present paper concerns the study of an integrated and reversible architectural-plant solution proposed: reversibility of the proposed system is understood as the possibility for the present conditions being easily restored.
