The Aging Waistline: Impact of the Geriatric Obesity Epidemic on an Urban Emergency Department: Original Communication

Purpose: Reviews adult emergency department (ED) visits for patients age 65 and older during one calendar year;determine the prevalence of weight classifications;identifies trends between BMI and discharge/admitting diagnoses, vital signs, and severity ***: The electronic medical records system and data from the ED billing service was reviewed for an urban academic institution with an annual volume of 125,000 for patients age > 65. Using a random number table, a retrospective cohort of 328 elderly patients was selected for review, representing a convenience sample of 2.6% of elderly ED visits. Body Mass Index (BMI) was calculated, using the Center for Disease Control (CDC) formula with underweight (- 24.9), overweight (25 - 29.9), and obese (≥30). Results: The majority of the cohort in this study was African-American and Hispanic (60% and 27% respectively), and there were a higher percentage of females than males (60% and 40% respectively). Approximately 29% of the patients were classified as normal weight, 35% classified as overweight, and 36% as obese. The older the patient, the more likely that patient belonged to a lower weight classification (p 0.01). Those presenting with neurological, pulmonary or gastrointestinal complaints were more likely to be of a higher weight classification (p 0.05). Patients who were hypertensive on arrival to the ED were more likely to be in a higher weight classification (p 0.01). Conclusion: Those patients with a higher weight classification had a strong correlation with selected abnormal vital signs and disease presentations. EDs are important sources of care for the elderly. EDs can serve as a previously untapped resource for screening and early referral exercise programs aimed at improving physical function/ functional status and quality of life in the elderly patient population.

Effects of liquid feeding of corn condensed distiller's solubles and whole stillage on growth performance, carcass characteristics, and sensory traits of pigs

Background： The immense growth in global bioethanol production has greatly increased the supply of by-products such as whole stillage and condensed distiller＇s solubles, which could be potentially used for animal feeding. The objective of this study was to investigate effects of liquid feeding high levels of corn condensed distiller＇s solubles（CCDS） and whole stillage（CWS） on growth performance, carcass characteristics, belly firmness and meat sensory traits of ***： A total of 256 pigs were blocked by sex and initial BW（13.5 ± 2.5 kg）, and pens of pigs（8 pigs/pen） were randomly allocated to 1 of 4 dietary treatments（8 pens/treatment）： 1） corn-soybean meal based diet as control, 2） 25%CWS ＋ 5% CCDS, 3） 19.5% CWS ＋ 10.5% CCDS, and 4） 19.5, 26, and 32.5% CWS ＋ 10.5, 14, and 17.5% CCDS in phases 1（28 d）, 2（38 d）, and 3（60 d）, respectively. Inclusion levels of CCDS and CWS for Treatments 1, 2, and 3 were fixed during all the three phases of the experiment. Inclusion levels of CWS and CCDS were on 88% dry matter basis. The liquid feeding system delivered feed from the mixing tank to feed troughs by high-pressure air, had sensors inside feed troughs, and recorded daily feed intake on the basis of a reference feed intake curve. The pigs were fed 5 to 10 times per day with increasing frequency during the ***： Control pigs had greater（P 0.10） dressing percentage, loin muscle depth, and lean percentage were observed among the four treatments. Inclusion of CWS and CCDS reduced（P 0.10） the overall like,flavor, tenderness and juiciness of loin chops when compared with the control ***： In conclusion, our results indicate that including 30–50% of a mixture of whole stillage and condensed distiller＇s solubles in the growing-finishing diets may reduce growth performance, carcass weight and belly firmness, but does not affect pork sensory traits.

Comparison of population estimators and indices for monitoring house mice in sorghum crops

House mice(Mus musculus domesticus Schwarz&Schwarz,1943)are monitored in Australia and China to track changes in mouse population densities and forecast their potential damage to cereal *** present study compared population indices based on the number of different mice caught and overall trap success from live-trapping with an oil card index(OC)and a tracking index(T)for monitoring mice in sorghum crops immediately before crop maturation.T was measured as the percentage of track board covered with mouse footprints night-1,and OC as the percentage of card removed by mice *** reliability of these abundance indices was quantified by Pearson correlation coefficient with the trappable population size(Ñ),which was estimated by capture-recapture over eight consecutive nights on 175×5 trapping grids,in sorghum crops on two properties on the Darling Downs,*** of differences among individual mice in capture probability,Model Mh of program MARK was used to account for such heterogeneity and to estimate the size of each mouse *** number of individual animals caught was more strongly correlated withÑthan trap success and,therefore,might be a more reliable index;the data suggest that three trapping occasions provide optimal precision for this index.T correlated significantly withÑonly at sites where the canopy of sorghum plants was closed,and its use should,therefore,be restricted to this *** did not correlate withÑbecause none or very little of the cards was eaten at low to moderate mouse densities.T and the number of animals caught over three trapping nights are recommended for monitoring mice in sorghum crops immediately prior to crop maturation.

The Angolan strain of Marburg virus （MARV/Ang） can cause lethal disease in humans with a case fatality rate of up to 90%, but infection of immunocompetent rodents do not result in any observable symptoms. Our previous work includes the development and characterization of a MARV/Ang variant that can cause lethal disease in mice （MARV/Ang-MA）, with the aim of using this tool to screen for promising prophylactic and therapeutic candidates. An intermediate animal model is needed to confirm any findings from mice studies before testing in the gold-standard non-human primate （NHP） model. In this study, we serially passaged the clinical isolate of MARV/Ang in the livers and spleens of guinea pigs until a variant emerged that causes 100% lethality in guinea pigs （MARV/Ang- GA）. Animals infected with MARV/Ang-GA showed signs of filovirus infection including lymphocytopenia, thrombocytopenia, and high viremia leading to spread to major organs, including the liver, spleen, lungs, and kidneys. The MARV/Ang-GA guinea pigs died between 7-9 days after infection, and the LD50 was calculated to be 1.1x10-1 TCID50 （median tissue culture infective dose）. Mutations in MARV/Ang-GA were identified and compared to sequences of known rodent-adapted MARV/Ang variants, which may benefit future studies characterizing important host adaptation sites in the MARV/Ang viral genome.

The relevance of serum albumin among elderly patients with acute decompensated heart failure

Thin Cell Layer (TCL) Culture System for Herbal Biomass Production and Genetic Transformation of Bacopa monnieri L. Wettst.

Bacopa monnieri (L.) Wettst. (Scrophulariaceae) is a highly sought after medicinal plant with therapeutic properties as cognition enhancer as well as for other brain and body functions. Research was conducted to optimize a thin cell layer explant based micropropagation system to assist mass propagation. Thin cell layers (TCL) derived from leaf and internode segments were used as explants. Murashige and Skoog medium was used to formulate shoot induction, elongation, and rooting media. Shoot induction media were prepared by supplementing three concentrations (0.1, 1.0, and 10.0 μM) of four cytokinins 6-benzylaminopurine, 2-isopentenyl-adenine, 6-3-Hydroxybenzylaminopurine, and thidiazuron to study adventitious shoot bud induction response. An optimum shoot bud induction response was observed on MS medium supplemented with 10.0 μM 6-benzylaminopurine for both leaf and stem transverse thin cell layer (tTCL) explants. The average number of shoot buds from leaf tTCL explants was 59, whereas, on an average, 33 shoot buds were regenerated from internode tTCL explants. Elongation of adventitious shoot buds was achieved best in a liquid medium using Liquid Lab Rocker® system. Elongated shoots recorded 100% rooting in MS medium supplemented with 5 μM indole butyric acid. Bacopa micropropagation employing tTCL explants for initial shoot bud induction and using LLR® boxes in subsequent elongation step can achieve cost effective way to regenerate high volume of plantlets and biomass required for herbal industry. Leaf and stem tTCL explants both were suitable for Agrobacterium tumefaciens (EHA105) mediated genetic transformation. Successful transformation was scored within three days of co-cultivation with Agrobacterium suspension on the basis of Enhanced Green Fluorescent Protein (EGFP) expression as an early and non-destructible screening device. BACKGROUND Hepatorenal syndrome(HRS)is a life-threatening condition among patients with advanced liver *** trends specific to hospital mortality and hospital admission resource utilization for HRS remain *** To assess the temporal trend in mortality and identify the predictors for mortality among hospital admissions for HRS in the United *** We used the National Inpatient Sample database to identify an unweighted sample of 4938 hospital admissions for HRS from 2005 to 2014(weighted sample of 23973 admissions).The primary outcomes were temporal trends in mortality as well as predictors for hospital *** estimated odds ratios from multilevel mixed effect logistic regression to identify patient characteristics and treatments associated with hospital *** Overall hospital mortality was 32%.Hospital mortality decreased from 44%in 2005 to 24%in 2014(P<0.001),while there was an increase in the rate of liver transplantation(P=0.02),renal replacement therapy(P<0.001),length of hospital stay(P<0.001),and hospitalization cost(P<0.001).On multivariable analysis,older age,alcohol use,coagulopathy,neurological disorder,and need for mechanical ventilation predicted higher hospital mortality,whereas liver transplantation,transjugular intrahepatic portosystemic shunt,and abdominal paracentesis were associated with lower hospital *** Although there was an increase in resource utilizations,hospital mortality among patients admitted for HRS significantly *** predictors for hospital mortality were identified.
