The influence of the blood pressure on the venous cerebral flow measured by magnetic susceptibility (SWI) technique

Susceptibility Weighted Image (SWI) is a magnetic Resonance Imaging (mRI) technique that combines high spatial resolution and sensitivity to magnetic susceptibility differences between tissues, it is extremely sensitive to venous blood due to its iron content of deoxyhemoglobin. The aim of this study was to evaluate the differences in the cerebral venous vasculature produced by the value’s variation of blood pressure. 20 subjects (10 hypertensive and 10 normotensive patients) underwent a mRI system with field strength of 1.5 T using a synergy head coil (7 channels). The obtained sequences were T1w, T2w-FLAIR, T2* and SWI. The value of Contrast-to-Noise Ratio (CNR) was assessed in minIP (minimum Intensity Projection) and magnitude images, through drawing free hand ROIs in venous structures: Superior Sagittal Sinus (SSS) Internal Cerebral Vein (ICV) and Sinus Confluence (SC). The obtained values were presented in descriptive statistics-quartiles and extremes diagrams. The results were compared between groups. CNR shown higher values for normotensive group in minIP (108.89 ± 6.907) to ICV;(238.73 ± 18.556) to SC and (239.384 ± 52.303) to SSS. These values are bigger than images from Hypertensive group about 46 au in average. Comparing the results of magnitude and minIP images, there were obtained lower CNR values for the hypertensive group. There were differences in the CNR values between both groups, being these values more expressive in the large vessels-SSS and SC. The SWI is a potential technique to evaluate and characterize the blood pressure variation in the studied vessels adding a physiological perspective to mRI and giving a new approach to the radiological vascular studies.

Health Promotion and Life Course Dynamics: Transitions of Brazilian Elderly

Background: Understanding life transitions is important to help people to achieve their developmental needs. No studies have examined the patterns of responses to life transitions, eliciting the experiences of growing old of retired Brazilian older adults. Aim: To identify and describe the patterns of responses to life transitions of retired Brazilian older adults. Design: Using a theoretical perspective of transition as a guide, a qualitative, secondary narrative research design was incorporated to obtain data from 11 interviews. method: Initial data collection took place during a broader study on the influence of people’s working life history. Eleven autonomous and independent persons, 64 - 82 years old, were interviewed at a geriatric outpatient unit in Rio de Janeiro to obtain their perceptions of old age. The secondary, narrative holistic-content analysis focused on the patterns of responses to life transitions. Results: The first turning point was a childhood event and the next was their first job. The third turning point was moving to another place/house or another city, the fourth starting a family, and retirement was the last turning point. Conclusions: While older people may feel environmentally disconnected throughout life, their patterns of response, together with the available resources and prevention/interventions influence the transition process. Health Promotion actions are needed at vulnerable points during the transition process, thereby facilitating the health outcomes.

Objective:To investigate the long-term safety of a tetravalent dengue vaccine(CYD-TDV)in children in a phase Ⅱ b follow-up study in ***:In the phase Ⅱb study,children aged 4-11 years were randomized(2:1)to receive three injections of CYD-TDV or serve as control at 6-month intervals,with 25 months’ active follow-up(active phase).This study was an additional four-year passive surveillance for hospitalized virologically-confirmed dengue(VCD;hospital phase).Cases of hospitalized VCD,severe hospitalized VCD,vaccine-related serious adverse events,and deaths were reported for the total population,with post-hoc analyses by enrollment age(mulative RRs in the active phase,hospital phase,and entire six years were 0.28(95% CI:0.08-0.81),0.51(95% CI:0.25-1.05),and 0.42(95% CI:0.24-0.75),*** the overall population,there were ten severe hospitalized VCD cases in the CYD-TDV group and five in the control group over six years(RR:1.00,95% CI:0.31-3.75).Conclusions:Over six years of follow-up,in children aged ≥9 years,CYD-TDV administration is associated with a reduced risk of hospitalized VCD.

This work aimed to determine the effect of native probiotic administration on body weight gain, nutritional characteristics and fatty acid profile of meat of creole goats. Equally distributed forty postweaned goats (75 days old) divided into control (n=20) and probiotic (n=20) groups were fed ad libitum for 60 days with native pasture and commercial diet. Lactobacillus reuteri DDL19, Lactobacillus alimentarius DDL48, Enterococcus faecium DDE39, and Bifidobacterium bifidum DDBA mixed culture (108 CFU/ml, 1:1:1:1 ratio) were daily orally delivered to the probiotic group. At the end of the study, muscle samples (Longissimus dorsi, LD, and Bicep femoris, BF) were analyzed for their chemical composition (protein, fat, moisture, pH, water-holding capacity, cholesterol) and fatty acid profile. Probiotic supplementation significantly increased body weight gain, reduced the incidence of diarrhea and improved the fatty acid profile of goat meat. Probiotic-fed animals exhibited increased monounsaturated and polyunsaturated fatty acids and a significant decrease in saturated fatty acid content. Conjugated linoleic acid content was significantly higher in probiotics than in the control group (P muscles from probiotics-treated goats showed healthier lipid indexes and a significant decrease in the n-6/n-3 ratio (~2.50), atherogenic (~0.30) and thrombogenic (~0.75) indexes. In addition, increases in desirable fatty acids (~79.40 g/100 g of FAmE) and nutritional value index (~2.86) were observed compared to control groups. The native mix probiotic could be used as growth promoters to reduce the antibiotics, and to improve its derived-foods.

Cholangiosepsis Caused by Neutrophilic Cholangitis in Plaque Psoriasis

Psoriasis is a common inflammatory skin disease with many comorbid conditions. We present a 37-year-old male patient with a history of plaque psoriasis, status febrilis, violent umbilical pain, elevated inflammatory markers and liver parameters. Blood cultures were tested positive for E. coli. Diagnostic findings indicated that mechanical icterus and cholangiosepsis in the context of neutrophilic cholangitis were caused by inflammatory stenosis. Neutrophilic cholangitis is often found in combination with skin diseases with intense cutaneous infiltration with polymorphonuclear leucocytes and peripherial blood neutrophilia. Interleukin-8 may play a role in the pathogenesis of neutrophilic cholangitis occurring in patients with psoriasis [1]. We showed that complications of psoriasis can also occur at unusual locations. To date no case of neutrophilic cholangitis as an elicitor of cholangiosepsis has been reported.

Diabetic neuropathy is a common complication of both type 1 and type 2 diabetes,which affects over 90% of the diabetic *** pain is one of the main symptoms of diabetic neuropathy,its pathophysiological mechanisms are not yet fully *** is widely accepted that the toxic effects of hyperglycemia play an important role in the development of this complication,but several other hypotheses have been *** management of diabetic neuropathic pain consists basically in excluding other causes of painful peripheral neuropathy,improving glycemic control as a prophylactic therapy and using medications to alleviate *** line drugs for pain relief include anticonvulsants,such as pregabalin and gabapentin and antidepressants,especial y those that act to inhibit the reuptake of serotonin and *** addition,there is experimental and clinical evidence that opioids can be helpful in pain control,mainly if associated with first line *** agents,including for topical application,such as capsaicin cream and lidocaine patches,have also been proposed to be useful as adjuvants in the control of diabetic neuropathic pain,but the clinical evidence is insufficient to support their *** conclusion,a better understanding of the mechanisms underlying diabetic neuropathic pain will contribute to the search of new therapies,but also to the improvement of the guidelines to optimize pain control with the drugs currently available.

Assessing adequacy of emergency provider documentation among interhospital transferred patients with acute aortic dissection

BACKGROUND: Acute aortic dissection(AoD) is a hypertensive emergency often requiring the transfer of patients to higher care hospitals; thus, clinical care documentation and compliance with the Emergency medical Treatment and Active Labor Act(EmTALA) is crucial. The study assessed emergency providers(EP) documentation of clinical care and EmTALA compliance among interhospital transferred AoD ***: This retrospective study examined adult patients transferred directly from a referring emergency department(ED) to a quaternary academic center between January 1, 2011 and September 30, 2015. The primary outcome was the percentage of records with adequate documentation of clinical care(ADoCC). The secondary outcome was the percentage of records with adequate documentation of EmTALA compliance(ADoEmTALA). RESULTS: There were 563 electronically identified patients with 287 included in the final analysis. One hundred and five(36.6%) patients had ADoCC while 166(57.8%) patients had ADoEmTALA. Patients with inadequate documentation of EmTALA(IDoEmTALA) were associated with a higher likelihood of not meeting the American Heart Association(AHA) ED Departure SBP guideline(OR 1.8, 95% CI 1.03–3.2, P=0.04). male gender, handwritten type of documentation, and transport by air were associated with an increased risk of inadequate documentation of clinical care(IDoCC), while receiving continuous infusion was associated with higher risk of ***: Documentation of clinical care and EmTALA compliance by Emergency Providers is poor. Inadequate EmTALA documentation was associated with a higher likelihood of patients not meeting the AHA ED Departure SBP guideline. Therefore, Emergency Providers should thoroughly document clinical care and EmTALA compliance among this critically ill group before transfer.
