Temporary mechanical circulatory support(tMCS)plays a pivotal role in managing severe or refractory cardiogenic shock(CS),furnishing essential hemodynamic support,and sustaining organ perfusion from hours to weeks,or to even *** the past decade,the utilization of tMCS has witnessed a signicant increase in American and European *** in Asia,the newly developed concepts and devices constituting tMCS have also made rapid ***,the absence of a consensus or standardized guidelines,especially for the Asian population considering inherited polymorphism,is *** address this knowledge gap,eight associations/societies and experts in thiseld came up with the present consensus after repeated discussions,more specically for the Asian *** comprehensively elucidates the commonly employed tMCS devices,criteria for patient selection,indications and contraindications,clinical management,and the optimal timing for implantation and *** goal of this statement is to promote the standardized application of tMCS and make reasonable decisions in clinical ***-length version of the consensus can be seen in Supplementary File,http://***/CM9/B883.

Trauma-induced coagulopathy(TIC)is caused by post-traumatic tissue injury and manifests as hypercoagulability that leads to thromboembolism or hypocoagulability that leads to uncontrollable massive *** studies on TIC have mainly focused on hemorrhagic coagulopathy caused by the hypocoagulable phenotype of TIC,while recent studies have found that trauma-induced hypercoagulopathy can occur in as many as 22.2%–85.1%of trauma patients,in whom it can increase the risk of thrombotic events and mortality by 2-to ***,the chinese People’s Liberation Army professional committee of critical care Medicine and the chinese Society of Thrombosis,Hemostasis and critical care,chinese Medicine Education association jointly formulated this chinese Expert Consensus comprising 15 recommendations for the definition,pathophysiological mechanism,assessment,prevention,and treatment of trauma-induced hypercoagulopathy.

The coronavirus disease 2019 (COVID-19) seriously threatens the health of humans worldwide, and at the time of this writing, there is no specific antiviral treatment for COVID-19. To reduce the case mortality, experts at the frontlines of the COVID-19 pandemic in China, have collected, analyzed, and summarized data on the clinical diagnosis and treatment of severe and critical patients with COVID-19 in Wuhan and other severe disaster areas. Here, we present the chinese experts’ consensus on the diagnosis and treatment of this population. The first edition was released on February 22, 2020, and revised on March 4, 2020.

It has been found that some patients with coronavirus disease 2019(COVID-19)have clinically relevant heart injuries,and the incidence of myocarditis among patients with COVID-19 was reported to be 12%in hospitals and 32%in intensive care units in Wuhan,China.[1]Acute myocardial injuries or myocarditis is a very important clinical issue due to the risk of rapid disease progression to severe arrhythmia,heart failure,cardiac shock,etc.[2,3]The bedside echocardiography examination is the first-line preferential imaging option for the evaluation of myocarditis and other complications,such as acute right heart failure specially due to pulmonary artery embolism,stress cardiomyopathy,acute myocardial infarction and coexisting fundamental chronic cardiovascular disease.

Expert consensus on COVID-19 vaccination in children

Coronavirus disease 2019(COVID-19)is a public health disaster that has not been encountered for a hundred *** January 12,2020,the World Health Organization(WHO)confirmed and named the coronavirus which caused unexplained pneumonia as 2019 novel coronavirus(2019-nCoV).Since then,the disease caused by this virus had been named as coronavirus disease 2019(COVID-19).On February 11,2020,the International committee on Taxonomy of Viruses announced that the English name of the novel coronavirus was severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)[1].At present,SARS-CoV-2 infection is still rampant *** of September 10,2021,there were about 222 million confirmed cases of COVID-19 and more than 4.5 million deaths worldwide[2].According to the American Centers for Disease Control and Prevention,by July 29,2021,4.19 million pediatric COVID-19 cases had been reported in the United States。

严重感染（severe sepsis）及其相关的感染性休克（septicshock）和多器官功能障碍综合征（multiple organ dysfunction syndrome，MODS）是当前重症加强治疗病房（ICU）的主要死亡原因，也是当代重症医学面临的主要焦点及难点。在美国，每年有75万的严重感染病例发生，超过了充血性心力衰竭或乳腺癌、结肠癌和艾滋病的患病数总和，病死率在20％～63％左右，与急性心肌梗死的院外病死率相近，而且患病率以每年1．5％的比例增长，预计到2010年和2020年，严重感染的患病人数将达到93万和110万。美国每年的相关治疗费用大约为167亿美元，而欧洲每年的相关治疗费用大约为94亿美元。在全球范围内，严重感染的患病率、病死率及相关治疗费用也在逐年增加，全球每年有1800万人发生严重感染，每日大约有1400人死于严重感染。尽管国内尚无完整的流行病学资料，但据估计严重感染的患病率、病死率及治疗费用也相当高。人口老龄化和慢性疾病的增加，人类的医疗活动如肿瘤化疗和器官移植后免疫抑制剂的应用，都是导致严重感染患病率增加的重要原因。

一、引言脓毒症（Sepsis）是指感染（确诊或疑似）引起的全身性炎症反应综合征（systemicinflammatoryresponsesyndrome，SIRS），是ICU患儿死亡的主要原因之一，亦是当今危重病医学所面临的棘手难题。2002年国际儿科脓毒症会议明确了儿童SIRS、脓毒症、严重脓毒症、脓毒性休克及器官功能障碍的定义，为大规模、多中心的临床研究奠定了基础。虽然国际拯救脓毒症战役的开展为严重脓毒症诊治提供了指南，但严重脓毒症的病死率仍居高不下。连续血液净化（ContinuousBloodPurification，CBP）又称连续性肾脏替代治疗（continuousrenalreplacementtherapy，CRRT），是所有连续、缓慢经体外循环和滤器清除水分和溶质的治疗方式的总称。CBP可通过滤过、透析和吸附等清除血液中炎症介质、降低组织炎症介质水平、改善重要脏器功能，可能有助于严重脓毒症治疗。
