乡村聚落分布是特定地域人地关系的重要表现,探究山区乡村聚落空间分布形态是揭示山区乡村人地关系系统地域特征的重要基础工作,对优化乡村聚落分布、推动乡村振兴具有重要意义。本文以秦巴山区秦岭南段的陕西省洋县为典型案例区,综合采用栅格成本加权距离算法、地理空间分析、可达性与地理探测器方法,系统分析了洋县山区乡村聚落空间分布特征及影响因素。结果表明:洋县可达性存在明显空间差异,呈现出沿交通干线延伸的条带状分布特征,山区乡村聚落分布具有显著的低海拔与集聚指向,洋县乡村聚落多分布在南部低缓地带和中部低平地带,呈现集聚分布态势;山区乡村聚落具有显著的交通可达性空间指向性,乡村绝对数量和相对比重均随着可达性水平的提升呈逐渐增加态势。山区地域系统中,海拔、坡度、降水和气温等自然条件以及教育、医疗等社会经济条件是影响研究区乡村聚落空间分布的关键因素。未来县城和乡镇可达性对洋县乡村聚落空间分布的影响作用有望得到进一步提升,完善乡镇—县城、村域—县城的道路交通对于优化脱贫地区乡村聚落布局具有重要意义。

目的探究柴银亚甲方联合常规西药治疗亚急性甲状腺炎患者的临床疗效及安全性。方法选取2021年1月—2021年12月期间唐山市中医医院收治的亚急性甲状腺炎患者90例作为研究对象,依据简单随机数字表法分为对照组和研究组,每组各45例。对照组采用常规西药治疗,研究组在对照组基础上加用柴银亚甲方。治疗3个月后,观察比较两组患者临床疗效、不良反应及疾病复发率,治疗前后中医证候积分、血沉(Erythrocyte sedimentation rate,ESR)及甲状腺功能指标[促甲状腺激素(Thyroid stimulating hormone,TSH)、游离甲状腺素(Free thyroxine,FT4)、游离三碘甲状腺原氨酸(Free triiodothyronine,FT3)]、炎症因子[干扰素-γ(Interferon-gamma,IFN-γ)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-6(Interleukin-6,IL-6)]。结果治疗后研究组临床总有效率93.33%(42/45)高于对照组77.78%(35/45),差异有统计学意义(P0.05)。随访3个月,研究组疾病复发率2.22%(1/45)明显低于对照组15.56%(7/45),差异有统计学意义(P<0.05)。结论单纯常规西药治疗及联合柴银亚甲方治疗亚急性甲状腺炎均能缓解患者临床症状,改善甲状腺功能,减轻炎性反应程度,提升治疗效果,减少疾病复发,但西药联合柴银亚甲方对相关指标改善效果更佳,利于保证预后。

目的探讨柴银亚甲方治疗热毒壅盛型亚急性甲状腺炎(Subacute thyroiditis,SAT)的临床价值。方法选取2021年1月—2021年12月期间唐山市中医医院收治的80例SAT患者开展研究,采用随机数字表法分为对照组和试验组,每组各40例。两组患者均给予一般治疗,在此基础上,对照组给予常规西医治疗,试验组给予常规西医联合柴银亚甲方治疗,两组患者均治疗4周。观察比较两组患者临床疗效、不良反应发生率、复发率,治疗前后中医主要症状积分、红细胞沉降率(Erythrocyte sedimentation rate,ESR)、血清甲状腺激素[游离三碘甲状腺原氨酸(Free triiodothyronine,FT3)、游离甲状腺素(Free thyroxine,FT4)、促甲状腺激素(Thyroid stimulating hormone,TSH)]、炎症因子[肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、白细胞介素-2(Interleukin-2,IL-2)、白细胞介素-3(Interleukin-3,IL-3)]、补体C(C3、C4)、免疫球蛋白(IgA、IgM、IgG)水平。结果(1)临床疗效:治疗后试验组临床疗效总有效率95.00%(38/40)与对照组89.74%(35/39)比较,差异无统计学意义(P>0.05);但对照组愈显率85.00%(34/40)高于对照组66.67%(26/39),差异有统计学意义(P<0.05)。(2)中医主要症状积分:治疗后,两组患者中医主要症状颈前肿大、疼痛、发热、乏力、急躁易怒、多汗、心悸积分均较治疗前降低,差异有统计学意义(P<0.05);且试验组低于对照组,差异有统计学意义(P<0.05)。(3)ESR及甲状腺激素水平:治疗后,两组患者血清ESR、FT3、FT4水平较治疗前降低,TSH水平较治疗前升高,差异有统计学意义(P<0.05);且试验组血清ESR、FT3、FT4水平低于对照组,TSH水平高于对照组,差异有统计学意义(P<0.05)。(4)炎症因子:治疗后,两组患者血清TNF-α较治疗前降低,IL-2、IL-3水平较治疗前升高,差异有统计学意义(P<0.05);且试验组血清TNF-α低于对照组,IL-2、IL-3水平高于对照组,差异有统计学意义(P<0.05)。(5)补体C及Ig水平:治疗后,两组患者血清C3、C4及IgA、IgM、IgG水平均较治疗前降低,差异有统计学意义(P<0.05);且试验组低于对照组,差异有统计学意义(P<0.05)。(6)安全性:仅对照组发生1例轻度恶心反酸症状,未给予特殊干预自行缓解。(7)复发率:对照组复发率为10.26%,试验组无复发病例,差异有统计学意义(P<0.05)。结论柴银亚甲方治疗SAT效果显著、安全性高,有利于纠正ESR及血清甲状腺激素紊乱,且可降低疾病复发风险,可能与抑制炎症反应、调节免疫功能等机制有关。

巴彦浩特臭氧污染特征及潜在源分析

基于2022年6-8月份巴彦浩特环境监测站臭氧浓度及气象数据,结合后向轨迹及潜在源分析对近地面臭氧高值进行分析。结果表明,6月份三个国控站点的臭氧浓度均较高,8月份较低,6月16-18日、6月24-25日是三个国控站点臭氧浓度的高值时段。湿度、温度是影响巴彦浩特臭氧浓度的重要气象因子。白天和夜间,三个国控站点的臭氧浓度均为东南风时达到最高值,且臭氧浓度高值时段的主导风向为东南风。后向轨迹分析显示,6月24日22:00-0:00高值时刻的臭氧污染传输主要受东南气流的影响,6月16日-18日、6月24日-25日浓度权重轨迹分析(WCWT)高值区(>100μg/m^(3))主要出现在宁夏、甘肃东部以及陕西省。

BACKGROUND Aorto-esophageal injury is a rare but life-threatening complication of esophageal foreign bodies,which typically requires open *** best way to treat patients with this condition remains *** date,few reports have described an aortic wall directly penetrated by a sharp foreign ***,we present a rare case of a fishbone completely embedded in the esophageal muscularis propria and directly piercing the aorta,which was successfully treated by endoscopy and thoracic endovascular aortic repair(TEVAR).CASE SUMMARY We report the case of a 71-year-old man with a 1-d history of retrosternal pain after eating *** abnormal findings were observed by the emergency *** tomography showed a fishbone that had completely pierced through the esophageal mucosa and into the *** patient refused to undergo surgery for personal reasons and was *** days after the onset of illness,he was readmitted to our *** examination showed a nodule with a smooth surface in the middle of the *** ultrasonography confirmed a fishbone under the *** performing TEVAR,we incised the esophageal mucosa under an endoscope and successfully removed the *** patient has remained in good condition for 1 *** Incising the esophageal wall under endoscope and extracting a foreign body after TEVAR may be a feasible option for cases such as ours.

Background:Lipid abnormalities are prevalent among people living with human immunodeficiency virus(HIV)(PLWH)and contribute to increasing risk of cardiovascular *** study aims to investigate the incidence of dyslipidemia and its risk factors in PLWH after receiving different first-line free antiretroviral ***:PLWH who sought care at the Third People’s Hospital of Shenzhen from January 2014 to December 2018 were included,and the baseline characteristics and clinical data during the follow-up were collected,including total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C)and high-density lipoprotein cholesterol(HDL-C).The risk factors of dyslipidemia after antiretroviral therapy were analyzed with the generalized estimating equation ***:Among the 7623 PLWH included,the mean levels of TC,HDL-C and LDL-C were 4.23±0.85 mmol/L,1.27±0.29 mmol/L and 2.54±0.65 mmol/L,respectively,and the median TG was 1.17(IQR:0.85-1.68)mmol/*** with that in PLWH receiving tenofovir disoproxil fumarate(TDF)+lamivudine(3TC)+ritonavir-boosted lopinavir(LPV/r),zidovudine(AZT)+3TC+efavirenz(EFV),and AZT+3TC+LPV/r,the incidence of dyslipidemia was lower in PLWH receiving TDF+3TC+*** multivariate analysis,we found that the risks of elevations of TG,TC,and LDL-C were higher with TDF+3TC+LPV/r(TG:odds ratio[OR]=2.82,95%confidence interval[CI]:2.55-3.11,P<0.001;TC:OR=1.24,95%CI:1.14-1.35,P<0.001;LDL:OR=1.06,95%CI:1.00-1.12,P=0.041),AZT+3TC+EFV(TG:OR=1.41,95%CI:1.28-1.55,P<0.001;TC:OR=1.43,95%CI:1.31-1.56,P<0.001;LDL:OR=1.18,95%CI:1.12-1.25,P<0.001),and AZT+3TC+LPV/r(TG:OR=3.08,95%CI:2.65-3.59,P<0.001;TC:OR=2.40,95%CI:1.96-2.94,P<0.001;LDL:OR=1.52,95%CI:1.37-1.69,P<0.001)than with TDF+3TC+EFV,while treatment with TDF+3TC+LPV/r was less likely to restore HDL-C levels compared with TDF+3TC+EFV(OR=0.95,95%CI:0.92-0.97,P<0.001).In addition to antiretroviral regimens,antiretroviral therapy duration,older age,overweight,obesity and other tr
