Background:Vascular inflow occlusion(VIO)during liver resections(Pringle manoeuvre)can be applied to reduce blood loss,however may at the same time,give rise to ischemia-reperfusion injury(IRI).The aim of this study was to assess the characteristics of hepatic microvascular perfusion during VIO in patients undergoing major liver ***:Assessment of hepatic microcirculation was performed using a handheld vital microscope(HVM)at the beginning of surgery,end of VIO(20 minutes)and during reperfusion after the termination of *** microcirculatory parameters assessed were:functional capillary density(FCD),microvascular flow index(MFI)and sinusoidal diameter(SinD).Results:A total of 15 patients underwent VIO;8 patients showed hepatic microvascular perfusion despite VIO(partial responders)and 7 patients showed complete cessation of hepatic microvascular perfusion(full responders).Functional microvascular parameters and blood flow levels were significantly higher in the partial responders when compared to the full responders during VIO(FCD:0.84±0.88 vs.0.00±0.00 mm/mm2,P<0.03,respectively,and MFI:0.69–0.22 vs.0.00±0.00,P<0.01,respectively).Conclusions:An interpatient heterogeneous response in hepatic microvascular blood flow was observed upon *** may explain why clinical strategies to protect the liver against IRI lacked consistency.

Resuscitation incoherence and dynamic circulation-perfusion coupling in circulatory shock

Objective: Poor tissue perfusion/cellular hypoxia may persist despite restoration of the macrocirculation (Macro). This article reviewed the literatures of coherence between hemodynamics and tissue perfusion in circulatory shock. Data sources: We retrieved information from the PubMed database up to January 2018 using various search terms or/and their combinations, including resuscitation, circulatory shock, septic shock, tissue perfusion, hemodynamic coherence, and microcirculation (Micro). Study selection: The data from peer-reviewed journals printed in English on the relationships of tissue perfusion, shock, and resuscitation were included. Results: A binary (coherence/incoherence, coupled/uncoupled, or associated/disassociated) mode is used to describe resuscitation coherence. The phenomenon of resuscitation incoherence (RI) has gained great attention. However, the RI concept requires a more practical, systematic, and comprehensive framework for use in clinical practice. Moreover, we introduce a conceptual framework of RI to evaluate the interrelationship of the Macro, Micro, and cell. The RI is divided into four types (Type 1: Macro-Micro incoherence + impaired cell;Type 2: Macro-Micro incoherence + normal cell;Type 3: Micro-Cell incoherence + normal Micro;and Type 4: both Macro-Micro and Micro-cell incoherence). Furthermore, we propose the concept of dynamic circulation-perfusion coupling to evaluate the relationship of circulation and tissue perfusion during circulatory shock. Conclusions: The concept of RI and dynamic circulation-perfusion coupling should be considered in the management of circulatory shock. Moreover, these concepts require further studies in clinical practice.

In our editorial,we want to comment on the article by Stefanolo et al titled“Effect of Aspergillus niger prolyl endopeptidase in patients with celiac disease on a long-term gluten-free diet”.Celiac disease is an immune-mediated disorder triggered by dietary gluten in genetically predisposed *** avoiding gluten can permit patients to live symptom-free,ongoing voluntary or involuntary exposure to gluten is common and associated with persistent villous atrophy in small bowel *** villous atrophy predisposes patients to life threatening complications,such as osteoporotic fractures or malignancies,therapeutic adjuncts to gluten-free diet become important to improve patients’quality of life and,if these adjuncts can be shown to improve villous atrophy,avoid *** administration of enzyme preparations,such as endopeptidases that digest gluten and mitigate its antigenicity to trigger inflam-mation,is one clinical strategy under *** article is about the utility of one endopeptidase isolated from Aspergillus *** critique findings of this clinical trial and also summarize endopeptidase-based as well as other strategies and how they can complement gluten-free diet in the management of celiac disease.

Effects of maturity stages on textural mechanical properties of tomato

The textural mechanical properties of Jadelo F1 variety tomato were evaluated at three different mature stages(green,pink and red)which were classified according to the colors of the *** compression and the puncture tests were applied on *** cycling and relaxation tests showed that indexes of average firmness,energy absorption and deformation ratio were more sensitive parameters to maturity stages than *** puncture tests indicated that maturity stages were effective on initial and average firmness and skin rupture *** the rigidity and the firmness of the fruit reduced,it was more prone to mechanical damage in the process of *** initial firmness values obtained fromcompressiontest and average firmness values from puncture tests had the highest correlation with each other which indicates that they can be used as representative parameter for ***,it was found that a/b color parameter can be good indicator for evaluating mechanical properties of tomato.

In the cultural stone heritage,progressive deteriorations develop over time with the effect of atmospheric *** deteriorations can reach to a significant degree that threaten the integrity of the monuments built from weak-strength *** this study,it is aimed to determine the deteriorations caused by atmospheric processes on the monument surface in cultural heritage works built by carving into low-strength pyroclastic rocks by nondestructive tests(NDTs).To this end,two historic structures in the Kilistra Ancient City of Konya(Turkey)were *** index,strength,mineralogical and petrographic properties of the rocks,in which the monuments were carved,were first ***,contour scaling,crack,efflorescence and biodeterioration types were determined on the facades of the ***,NDT deterioration change maps were prepared based on the data obtained by performing the relative humidity,Schmidt hammer rebound(SHR),and P-wave velocity(Vp)measurements on the facades of the *** the deterioration maps,it was determined that the SHR and Vp values of the rock were low in the capillary,infiltration,and crack zones with water penetration in the monuments built on low-strength pyroclastic ***,deterioration was less in the regions with more limited water access according to zones.

Minimum Cross Fuzzy Entropy Problem, The Existence of Its Solution and Generalized Minimum Cross Fuzzy Entropy Problems

In the present study we have formulated a Minimum Cross Fuzzy Entropy Problem （Minx（F）EntP） and proposed sufficient conditions for existence of its solution. Mentioned problem can be formulated as follows. In the set of membership functions satisfying the given moment constraints generated by given moment functions it is required to choose the membership function that is closest to a priori membership function in the sense of cross fuzzy entropy measure. The existence of solution of formulated problem is proved by virtue of concavity property of cross fuzzy entropy measure, the implicit function theorem and Lagrange multipliers method. Moreover, Generalized Cross Fuzzy Entropy Optimization Methods in the form of MinMinx（F）EntM and MaxMinx（F）EntM are suggested on the basis of primary phase of minimizing cross fuzzy entropy measure for fixed moment vector function and on the definition of the special functional with Minx（F）Ent values of cross fuzzy entropy measure. Next phase for obtaining mentioned distributions consists of optimization of defined functional with respect to moment vector functions. Distributions obtained by mentioned methods are defined as （MinMinx（F）Ent）m and （MaxMinx（F）Ent）m distributions.

Ethyl pyruvate protects against experimental acute-on-chronic liver failure in rats

AIM： To investigate the protective effects of ethyl py- ruvate （EP） on acute-on-chronic liver failure （ACLF） in rats. METHODS： An ACLF model was established in rats, and animals were randomly divided into normal, mod- el and EP treatment groups. The rats in EP treatment group received EP （40 mg/kg） at 3 h, 6 h, 12 h and 24 h after induction of ACLF. Serum endotoxin, high mobility group box-1 （HMGB1）, alanine transaminase （ALT）, tumor necrosis factor-α （TNF-α）, interferon-α （IFN-γ）, interleukin （IL）-10 and IL-18 levels, changes of liver histology and HMGB1 expressions in liver tis- sues were detected at 48 h after induction of ACLF. 目的分析安徽省食源性疾病与气温的关系,探讨滞后效应及识别易感人群。方法收集2016―2019年安徽省各市食源性疾病监测数据以及同期气象数据。采用类Poisson回归的广义线性模型分析各市日均气温和食源性疾病的关系,然后采用Meta分析合并效应值。结果研究期间,安徽省共填报348958例食源性疾病病例,年均发病率0.13%。日均气温与食源性疾病发病呈线性关系,且存在一定的滞后效应。单日滞后中,其效应在当天(lag0)最大,效应值为1.0096(95%CI:1.0047~1.0190),即日均气温每增加1℃,当日食源性疾病发生风险增加1.0096倍。到滞后第3天及以后其效应无统计学意义。累积滞后中,lag05对应的RR值最大,为1.0199(95%CI:1.0126~1.0272)。亚组分析结果显示,<65岁年龄组较≥65岁年龄组更易受到影响。结论气温升高会增加食源性疾病的发病风险,且存在滞后效应,应加强对易感人群的预防。
