Nonopioid perioperative analgesia in head and neck cancer surgery:A systematic review

Objective:Management of postoperative pain after head and neck cancer surgery is a complex issue,requiring a careful balance of analgesic properties and side *** objective of this review is to discuss the efficacy and safety of multimodal analgesia(MMA)for these ***:Pubmed,cochrane,Embase,Scopus,and *** were systematically searched for all comparative studies of patients receiving MMA(nonsteroidal anti-inflammatory drugs(NSAIDs),acetaminophen,anticonvulsants,local anesthetics,and corticosteroids)for head and neck cancer *** primary outcome was additional postoperative opioid usage,and secondary outcomes included subjective pain scores,complications,adverse effects,and 30-day ***:A total of five studies representing 592 patients(MMA,n=275;non-MMA,n=317)met inclusion *** most commonly used agents were gabapentin,NSAIDs,and acetaminophen(n=221),NSAIDs(n=221),followed by corticosteroids(n=35),dextromethorphan(n=40),and local nerve block(n=19).Four studies described a significant decrease in overall postoperative narcotic usage with two studies reporting a significant decrease in hospital *** pain scores widely varied with two studies reporting reduced pain at postoperative day *** were no differences in surgical outcomes,medical complications,adverse effects,or 30-day mortality and readmission ***:MMA is an increasingly popular strategy that may reduce dependence on opioids for the treatment of postoperative pain.A variety of regimens and protocols are available for providers to utilize in the appropriate head and neck cancer patient.

Objective:As the population ages,facial plastic providers must remain aware of the treatments to prevent and reverse the external signs of *** the mandibular region,skin laxity and soft tissue sagging in the jawline may lead to jowling and chin ptosis along with reduced chin *** surgical procedures,including chin implantation,may be performed,nonsurgical procedures are becoming increasingly popular due to their temporary,noninvasive,yet effective *** review covers the use of hyaluronic acid,calcium hydroxylapatite,poly-L-lactic acid,and polymethyl methacrylate in the ***:PubMed was searched for data on the mechanism of action,pertinent anatomy,indications,contraindications,technique,and evidence supporting the safety and efficacy of the ***:There are a wide variety of fillers available for use in the lower face with unique characteristics and application *** the advantages of injectable fillers include relatively affordable cost,minimal patient discomfort,and limited recovery times,taking measures to prevent short-and long-term complications is necessary for optimal ***:Understanding the benefits and limitations of injectable fillers in the jawline can help providers appropriately counsel and treat patients.

Objective:The aim of this study was to identify and analyze the gender breakdown of first authorship contributing to the most-cited papers in the field of otolaryngology,with a goal of identifying trends in gender representation in ***:The top 150 most-cited papers were identified using the Science citation Index of the Institute for Scientific *** the first authors,gender,h-index,percentage of first,last,and corresponding authorship positions,total publications,and citations were ***:The majority of papers were in the English language,from the United States,of clinical nature,and on otologic ***-one percent of papers(n=122)had men who were first authors,although there was no difference inh-index score,authorship position,number of publications,citations,and average citations/year between men and women first *** subgroup analysis by decade(1950s-2010s),there was no difference in the number of articles by women first authors(P=0.11);however,there was a statistically significant increase in the percentage of women authors(P=0.001)in papers published later compared to those published ***:While a promising number of women otolaryngologists are publishing high-powered articles,future initiatives to promote academic inclusivity of women should be considered.

Experimental and numerical analysis of multi-chamber oscillating water column devices

A review of multi-chamber oscillating water column(OWc)device designs is *** significant variations of these devices are discussed,onshore OWc(OOWc)and a floating OWc(FOWc).The efficiency results of several theoretical studies based on low-and high-fidelity numerical models are presented and compared with the model scale ***,low-fidelity numerical models are very fast to run,but their accuracy is limited compared with high-fidelity numerical *** model experiments usually give results much more accurate than numerical models,but they need adequate facilities and are very *** the case of the OOWc,all models show a similar trend of total efficiency,but while the analytical model shows a maximum value of around 90%efficiency,the cFD model shows 60%,and the experiments only go up to 40%.The main reason is connected with the mathematical simplifications and assumptions that do not represent all the hydrodynamic and aerodynamic processes between the water,air,and *** the case of the FOWc,interestingly,the experimental results show a maximum efficiency of almost 100%,while the analytical model only predicts a maximum of 80%.The efficiency seems highly dependent on the heave motion resonance of the entire device,where the analytical model fails to predict this natural frequency.

The perturbation theory of matrices is applied to ray transfer matrices （RTMs） to describe an optical component with aberration. A quantitative description of the perturbation extent corresponding to aberration strength is provided using condition numbers and absolute errors for the perturbed RTM. An application to a single small aberration is presented, and the results are compared with those of the diffraction theory of aberrations.

A quiet place:The impact of the word"quiet"on clinical workload

Purpose:This study aimed to determine the impact of uttering the word"quiet"on clinical workload during the overnight otolaryngology call shift and understand the factors contributing to resident *** and Methods:A multicenter,single-blind,randomized-controlled trial was conducted.A total of 80 overnight call shifts covered by a pool of 10 residents were randomized to the quiet or to the control *** the start of shift,residents were asked to state aloud,"Today will be a quiet night"(quiet group)or"Today will be a good night"(control group).clinical workload,as measured by number of consults,was the primary *** measures included number of sign-out tasks,unplanned inpatient and operating room visits,number of phone calls and hours of sleep,and self-perceived ***:There was no difference in the number of total(P=0.23),nonurgent(P=0.18),and urgent(P=0.18)*** at signout,total phone calls,unplanned inpatient visits,and unplanned operating room visits did not differ between the control and quiet *** there were more unplanned operating room visits in the quiet group(29,80.6%)compared to the control group(34,94.4%),this was not found to be significant(P=0.07).The majority of residents reported feeling"not busy"during control nights(18,50.0%)compared to feeling"somewhat busy"during quiet nights(17,47.2%;P=0.42).conclusion:contrary to popular belief,there is no clear evidence that uttering the word"quiet"significantly increases clinical workload.

Direct visualization of the atomic structure in scanning transmission electron microscopy has led to a comprehensive understanding of the structure-property ***,a reliable characterization of the structural transition on a picometric scale is still challenging because of the limited spatial resolution and ***,we demonstrate that the primary segmentation of atomic signals from background,succeeded by a denoising process,enables structural analysis in a sub-pixel *** noise is eliminated using the block matching and three-dimensional filtering with Anscombe transformation,and remnant noise is removed via morphological filtering,which results in an increase of peak signal-to-noise ratio from 7 to 11 *** the centroids of atomic columns segmented via K-means clustering,an unsupervised method for robust thresholding,achieves an average error of less than 0.7 pixel,which corresponds to 4.6 *** study will contribute to a profound understanding of the local structural dynamics in crystal structures.

Effects of Sn addition on the microstructure and mechanical properties of extruded Mg-Bi binary alloy

We investigated the effects of Sn addition on the microstructural characteristics and mechanical properties of an extruded Mg-Bi binary alloy by comparing Mg-5 Bi(B5)and Mg-5 Bi-4 Sn(BT54).Both the extruded alloys exhibit a partially recrystallized grain structure with a strong extrusion fiber texture and numerous Mg3Bi2 ***,the addition of Sn significantly decreases the average grain size of the extruded alloy from 123.9 to 75.2μ*** Sn solute atoms inhibit the activity of dislocation slip,which reduces the internal strain energy accumulated in the dynamically recrystallized(DRXed)grains during ***,this reduced strain energy leads to the decrease in the DRXed grain size owing to weakened grain growth during natural *** extruded BT54 alloy exhibits higher tensile strength and ductility than the extruded B5 *** improvement in the strength by the Sn addition is attributed to the combined effects of grain refinement,Sn solute atoms,and increased dislocation *** formation of{10-11}and{10-11}-{10-12}twins during tension is suppressed by the grain refinement,thereby improving the tensile elongation considerably.

The extrudability,microstructural characteristics,and tensile properties of the Mg–5Bi–3Al(BA53)alloy are investigated herein by comparing them with those of a commercial Mg–8Al–0.5 Zn(AZ80)*** AZ80 is extruded at 400℃,severe hot cracking occurs at exit speeds of 4.5 m/min or *** contrast,BA53 is successfully extruded without any surface cracking at 400℃ and at high exit speeds of 21–40 m/*** extruded at 3 m/min(AZ80–3)and 40 m/min(BA53–40),both AZ80 and BA53 exhibited completely recrystallized microstructures with abasal ***,BA53–40 has a coarser grain structure owing to grain growth promoted by the high temperature in the deformation ***80–3 contains a continuous network of Mg_(17)Al_(12) particles along the grain boundaries,which form via static precipitation during natural air-cooling after the material exits the extrusion ***53–40 contains coarse Mg_(3)Bi_(2) particles aligned parallel to the extrusion direction along with numerous uniformly distributed fine Mg_(3)Bi_(2) ***80–3 has higher tensile strength than BA53–40 because the relatively finer grains and larger number of solute atoms in AZ80–3 result in stronger grain-boundary and solid-solution hardening effects,*** BA53 is extruded at a high temperature and extrusion speed of 400℃ and 40 m/min,respectively,the extruded material has a high tensile yield strength of 188 *** can be primarily attributed to the large particle hardening effect resulting from the numerous fine Mg_(3)Bi_(2) particles.
