Liver transplantation for critically ill cirrhotic patients:Overview and pragmatic proposals

Liver transplantation for critically ill cirrhotic patients with acute deterioration of liver function associated with extrahepatic organ failures is controversial. While transplantation has been shown to be beneficial on an indivi-dual basis, the potentially poorer post-transplant outcome of these patients taken as a group can be held as an argument against allocating livers to them. Although this issue concerns only a minority of liver transplants, it calls into question the very heart of the allocation paradigms in place. Indeed, most allocation algorithms have been centered on prioritizing the sickest patients by using the model for end-stage liver disease score. This has led to allocating increasing numbers of livers to increasingly critically ill patients without setting objective or consensual limits on how sick patients can be when they receive an organ. Today, finding robust criteria to deem certain cirrhotic patients too sick to be transplanted seems urgent in order to ensure the fairness of our organ allocation protocols. This review starts by fleshing out the argument that finding such criteria is essential. It examines five types of difficulties that have hindered the progress of recent literature on this issue and identifies various strategies that could be followed to move forward on this topic, taking into account the recent discussion on acute on chronic liver failure. We move on to review the literature along four axes that could guide clinicians in their decision-making process regarding transplantation of critically ill cirrhotic patients.

A global analysis of the introduction pathways and characteristics associated with non-native fish species introduction, establishment, and impacts

Background The invasion success of introduced freshwater fishes is influenced by many factors, including ecological, species, and socioeconomic characteristics. Most studies that document the importance of these characteristics are conducted at local scales and/or focus on a single step of the invasion process. In this study, we aim to determine the species characteristics, ecological characteristics, and socioeconomic characteristics of non-native freshwater fish invasions. Our assessment was done at the global scale and considers all three steps of the invasion(i.e., introduction, establishment, and impact). For this purpose, we applied generalized linear models to 20 variables collected for 307 non-native species and modeled them as a function of ecological characteristics(i.e., environmental features), species traits(i.e., functional and morphological), and socioeconomic characteristics(i.e., human use and introduction pathways). We considered the number of countries in which each species was introduced, established, or had ecological impacts as a proxy of invasion step success. We also explored the specifics of species introduced through the aquaculture and the ornamental fish trade *** We found that non-native freshwater fishes with broad diets, high parental care, and multiple introduction pathways are the most widely introduced and established worldwide. The number of countries with impacts reported was best explained by the type of introduction pathway(i.e., aquaculture or fisheries). Current State of Energy Production in Cameroon and Projection for 2035

The low electricity supply rate is a major cause of underdevelopment in Cameroon. To address this issue, Cameroon outlined a strategy in 2003 aiming for a production capacity of 3000 MW by 2020. However, by 2020, production had only reached 1040 MW, leading Cameroon to devise a new national energy sector development strategy targeting 5000 MW by 2035. This paper provides an overview of the current state of energy production and projects future output by 2035. Scientific articles and investigative reports on energy production in Cameroon have enabled an assessment of the current electrical energy production. The 2035 production estimate is based on the Energy Sector Development Projects (PDSEN) report in Cameroon. The current production is estimated at around 1600 MW. Considering the ongoing construction of power plants, future projects, and financing delays, achieving the 5000 MW goal by 2035 appears challenging. Nonetheless, diversifying energy production sources could bring Cameroon closer to this target.

Characterisation of the Bacteria and Archaea Community Associated with Wild Oysters, At Three Possible Restoration Sites in the North Sea

With 85% of the global oyster reefs destroyed, there is an urgent need for large scale restoration to benefit from the ecosystem services provided by biogenic oyster reefs and their associated biodiversity, including microorganisms that drive marine biogeochemical cycles. This experiment established a baseline for the monitoring of the bacterial and archaeal community associated with wild oysters, using samples from their immediate environment of the Voordelta, with cohabiting Crassostrea gigas and Ostrea edulis, Duikplaats with only C. gigas attached to rocks, and the Dansk Skaldyrcentre, with no onsite oysters. The microbial profiling was carried out through DNA analysis of samples collected from the surfaces of oyster shells and their substrate, the sediment and seawater. Following 16S rRNA amplicon sequencing and bioinformatics, alpha indices implied high species abundance and diversity in sediment but low abundance in seawater. As expected, Proteobacteria, Bacteroidetes, Firmicutes and Thaumarchaeota dominated the top 20 OTUs. In the Voordelta, OTUs related to Colwellia, Shewanella and Psychrobium differentiated the oysters collected from a reef with those attached to rocks. Duikplaats were distinct for sulfur-oxidizers Sulfurimonas and sulfate-reducers from the Sva 0081 sediment group. Archaea were found mainly in sediments and the oyster associated microbiome, with greater abundance at the reef site, consisting mostly of Thaumarchaeota from the family Nitrosopumilaceae. The oyster free site displayed archaea in sediments only, and algal bloom indicator microorganisms from the Rhodobacteraceae, Flavobacteriaceae family and genus [Polaribacter] huanghezhanensis, in addition to the ascidian symbiotic partner, Synechococcus. This study suggests site specific microbiome shifts, influenced by the presence of oysters and the type of substrate.

During the production of nitrile rubber, significant amounts of nitrogen in the form of ammonium are generated in the wastewater. The discharge of this high-nitrogen wastewater can lead to serious environmental issues, including eutrophication, disruption of aquatic ecosystems, and groundwater contamination. To mitigate these impacts, this research explored the bioremediation capabilities of the macroalgae Ulva lactuca (Chlorophyta) for removing nitrogen from nitrile rubber production wastewater. The study employed single-phase and Michaelis-Menten decay models based on ammonium consumption, using various dilutions of wastewater to identify the optimal concentration for treatment. The physiological state of the macroalgae was monitored by measuring the photosynthetic capacity and specific growth rate during the experiments. In the presence of U. lactuca, ammonium concentrations decreased in all treatment groups, confirming that the ammonium kinetics conformed to both applied models. Our results show that U. lactuca effectively reduces ammonium concentrations, with an approximate removal rate of 0.020 µM·g−1·min−1 across different wastewater concentrations (70%, 80%, 90%, and 100%). Notably, the treatments with 70%, 80%, and 90% wastewater strength achieved about 67% reduction in ammonium, demonstrating the alga’s capacity to treat high-nitrogen wastewater. The photosynthetic performance of U. lactuca initially declined in control conditions but stabilized across all treatments, highlighting its adaptability. The kinetic analysis using the Michaelis-Menten model indicated a Vmax of 1342 μM·g−1·DMh−1, suggesting a robust capacity for ammonium uptake when fully saturated. Our study underscores the potential of Ulva lactuca as a cost-effective and efficient agent for wastewater bioremediation, particularly in settings with high nitrogen loads.

Occurrence of Extreme Rainfall and Flood Risks in Yopougon, Abidjan, Southeast Côte d’Ivoire from 1971 to 2022

Yopougon, located in the western part of the Autonomous District of Abidjan, is the most heavily populated municipality in Côte d’Ivoire. However, this area is prone to floods and landslides during the rainy season. The study aims to assess recent flood risks in the municipality of Yopougon of the Autonomous District of Abidjan. To achieve this objective, the study analyzed two types of data: daily rainfall from 1971 to 2022 and parameters derived from a Numerical Field and Altitude Model (NFAM). The study examined six rainfall parameters using statistical analysis and combined land use maps obtained from the NFAM of Yopougon. The results indicated that, in 67% of cases, extreme rainfall occurred mainly between week 3 of May and week 1 of July. The peak of extreme rainfall was observed in week 2 of June with 15% of cases. These are critical periods of flood risks in the Autonomous District of Abidjan, especially in Yopougon. In addition, there was variability of rainfall parameters in the Autonomous District of Abidjan. This was characterized by a drop of annual and seasonal rainfall, and an increase of numbers of rainy days. Flood risks in Yopougon are, therefore, due to the regular occurrence of rainy events. Recent floods in Yopougon were caused by normal rains ranging from 55 millimeters (mm) to 153 mm with a return period of less than five years. Abnormal heavy rains of a case study on June 20-21, 2022 in Yopougon were detected by outputs global climate models. Areas of very high risk of flood covered 18% of Yopougon, while 31% were at high risk. Climate information from this study can assist authorities to take in advance adaptation and management measures.

Coverage and Factors Associated with Acceptance and Satisfaction of Seasonal Malaria Chemoprevention in Six Health Zones in Benin

Introduction: Seasonal malaria chemoprevention (SMC) was adopted in 2019 in two health zones in Benin where malaria transmission is very high. Positive results led to the extension of the intervention to other zones with additional financial support. Annual SMC campaigns from 2021 to 2023 were carried out in all six health zones in the Atacora and Alibori departments. In five years of implementation, various approaches have been developed on the basis of a communication plan facilitating buy-in and acceptance by all stakeholders. The aim of this study was to assess the effective coverage and acceptance of the SMC by their beneficiary populations in 2023. Methods: It was a cross-sectional study with an analytical focus. Data collection took place from November 30 to December 13, 2023. The study population consisted of children under 5 years of age residing in the departments of Atacora and Alibori in northern Benin. A total of 3573 children under 5 years of age were included in the study, and their parents or guardians were interviewed. Results: During the 2023 campaign, 87.7% of targets were reached by SMC administration and 100.00% of children had received at least one dose of SMC by the fourth visit. Effective therapeutic coverage of SMC was 70.55%, with 99.60% in BNK, 69.40% in KGS, 16.20% in MK, 56.10% in 2KP, 92.40% in NBT and 89.60% in TMC. This coverage was statistically related to child and respondent ages (p Conclusion: SMC is a strategy accepted by the population, and the main reasons for non-participation in SMC were dominated by the absence of mothers or babysitters when the agents visited.
