This study aimed to describe a Computational Fluid Dynamics(CFD) procedure using the ANSYS CFX software 16.1 and Design of Experiments for the determination of volume and extension of explosive atmospheres due to fugitive emissions of flammable *** multidimensional statistical sampling technique latin Hypercube was used, which defined 100 simulations of random methane gas leak *** CFD model proved to be robust in predicting the extension and volume of the explosive atmosphere for orifice diameters from 0.1 to2.5 mm, pressure from 0.1 MPa to 12 MPa and temperatures from 0 ℃ to 400 ℃.It was found that the calculation domain must be parameterized 8 m in length for each millimeter of the diameter of the source of release to ensure the *** order not to lose precision for very small diameters, the mesh was parameterized with 50 elements along the orifice *** was proved that gravity does not influence the extension and volume of the explosive atmosphere at sonic *** deviation from the ideal gas behavior in the reservoir,achieved by applying the Soave–Redlich–Kwong equation of state, also has not significantly influenced the extension and volume of the explosive *** results showed that the size of the explosive atmosphere varies directly with the diameter of the source emission and reservoir pressure, and inversely with the temperature of the *** diameter of the source is the parameter that has the major effect on the extension of the explosive atmosphere, followed by the pressure and lastly the temperature of the reservoir.

Particulate matter(PM_(2.5))samples were collected in the vicinity of an industrial chemical pole and analysed for organic and elemental carbon(OC and EC),47 trace elements and around 150 organic *** average,OC and EC accounted for 25.2% and 11.4% of the PM_(2.5) mass,*** compounds comprised polycyclic aromatic hydrocarbons(PAHs),alkylated PAHs,anhydrosugars,phenolics,aromatic ketones,glycerol derivatives,aliphatic alcohols,sterols,and carboxyl groups,including aromatic,carboxylic and dicarboxylic *** factors>100 were obtained for Pb,Cd,Zn,Cu,Sn,B,Se,Bi,Sb and Mo,showing the contribution of industrial emissions and nearby major *** component analysis revealed that vehicle,industrial and biomass burning emissions accounted for 66%,11% and 9%,respectively,of the total PM_(2.5)-bound *** of the detected organic constituents are likely associated with plasticiser ingredients and thermal stabilisers used in the manufacture of PVC and other plastics in the industrial *** products of both anthropogenic(e.g.,toluene)and biogenic(e.g.,isoprene and pinenes)precursors were also *** was estimated that biomass burning accounted for 13.8%of the PM_(2.5) concentrations and that secondary OC represented 37.6% of the total *** lifetime cancer risk from inhalation exposure to PM_(2.5)-bound PAHs was found to be negligible,but it exceeded the threshold of 10−6 for metal(loi)s,mainly due to Cr and As.

Alcohol consumption accounts for 3.8% of annual global mortality worldwide, and the majority of these deaths are due to alcoholic liver disease(ALD), mainly alcoholic cirrhosis. ALD is one of the most common indications for liver transplantation(LT). However, it remains a complicated topic on both medical and ethical grounds, as it is seen by many as a "self-inflicted disease". One of the strongest ethical arguments against LT for ALD is the probability of relapse. However, ALD remains a common indication for LT worldwide. For a patient to be placed on an LT waiting list, 6 mo of abstinence must have been achieved for most LT centers. However, this "6-mo rule" is an arbitrary threshold and has never been shown to affect survival, sobriety, or other outcomes. Recent studies have shown similar survival rates among individuals who undergo LT for ALD and those who undergo LT for other chronic causes of end-stage liver disease. There are specific factors that should be addressed when evaluating LT patients with ALD because these patients commonly have a high prevalence of multisystem alcohol-related changes. Risk factors for relapse include the presence of anxiety or depressive disorders, short pre-LT duration of sobriety, and lack of social support. Identification of risk factors and strengthening of the social support system may decrease relapse among these patients. Family counseling for LT candidates is highly encouraged to prevent alcohol consumption relapse. Relapse has been associated with unique histopathological changes, graft damage, graft loss, and even decreased survival in some studies. Research has demonstrated the importance of a multidisciplinary evaluation of LT candidates. Complete abstinence should be attempted to overcome addiction issues and to allow spontaneous liver recovery. Abstinence is the cornerstone of ALD therapy. Psychotherapies, including 12-step facilitation therapy, cognitive-behavioral therapy, and motivational enhancement therapy, help

Acute calculous cholecystitis(ACC) is the most frequent complication of cholelithiasis and represents one-third of all surgical emergency hospital admissions, many aspects of the disease are still a matter of debate. Knowledge of the current evidence may allow the surgical team to develop practical bedside decision-making strategies, aiming at a less demanding procedure and lower frequency of complications. In this regard, recommendations on the diagnosis supported by specific criteria and severity scores are being implemented, to prioritize patients eligible for urgency surgery. laparoscopic cholecystectomy is the best treatment for ACC and the procedure should ideally be performed within 72h. Early surgery is associated with better results in comparison to delayed surgery. In addition, when to suspect associated common bile duct stones and how to treat them when found are still debated. The antimicrobial agents are indicated for high-risk patients and especially in the presence of gallbladder necrosis. The use of broad-spectrum antibiotics and in some cases with antifungal agents is related to better prognosis. Moreover, an emerging strategy of not converting to open, a difficult laparoscopic cholecystectomy and performing a subtotal cholecystectomy is recommended by adept surgical teams. Some authors support the use of percutaneous cholecystostomy as an alternative emergency treatment for acute Cholecystitis for patients with severe comorbidities.

INTRODUCTION Seed dispersal is a key process in the life cycle of most plants,with ecological and evolutionary consequences that go beyond vegetation dynamics and cascade to entire ecosystem functioning and stability(Traveset et al.2014).Seed dispersal largely constrains plant recruitment and the distribution of future plant cohorts,thus playing a critical role in the restoration of disturbed sites(Cain et al.2000;Higgins et al.2003).It can also threaten plant diversity by facilitating plant invasions(Vavra et al.2007;Heleno 2020).

Effects of a Carbonated Soft Drink on Epitheial Tumor Incidence in Drosophila melanogaster

Diet represents an important risk factor for tumor development and numerous food dyes have been tested for their carcinogenic potential. Caramel IV colorant and its byproduct, the 4-methylimidazole （4-MI）, are part of the composition of cola soft drinks, although in undefined concentration. Considering that this concentration depends on the manufacturing place, we aimed to evaluate the potential carcinogenic activity of cola soft drinks from Brazil, Canada, Egypt and Netherlands, through the test for detection of epithelial tumor clones （warts） in somatic cells of Drosophila melanogaster. We used 72-hour larvae resulting from the crossing of wts/TM3, Sb^1 virgin females and mwh/mwh males. larvae were exposed to the media containing the cola soft drinks for 48 hours and the analysis was performed in adults after transformation. Cola soft drinks from Brazil significantly induced epithelial tumor in D. melanogaster, while those from Canada, Egypt and Netherlands did not. In fact, the concentration of 4-MI is higher in cola soft drink commercialized in Brazil. Our results highlight the importance of investigating the toxic potential of current drinks, which may correlate with the rising incidence of malignant diseases.

Risk Factors for Fall in Elderly Patients:Follow-Up Study after Hospital Discharge

Introduction: Increased occurrences of falls after discharge are reported, especially in elderly people. Falls are a major cause of disability and identification of risk factors associated with falls is required to plan preventive actions. This study aims to determine the occurrence of falls in the three months after discharge and risk factors in elderly patients. Materials and Methods: 100 patients over 65 admitted to an Internal Medicine Ward participated. Questionnaires were given during hospitalization and three months after discharge. Follow-up information was unavailable for 31 patients (25 deceased, 6 unreachable). Results: Of those analyzed 52% were males with 80 ± 8.1 years (mean ± SD). Polymedication (p = 0.002), use of psychoactive drugs (p = 0.007), analgesics (p = 0.034) and walking devices (p = 0.006) were associated with a higher incidence of falls 6 months before hospitalization. Post-discharge follow-up was obtained for 69 patients: 18 reported falling during the follow-up. There was a higher risk of fall in patients with a history of falls in the 6 months before admission (p = 0.015 RR = 2.76). Patients who had one or more falls after discharge had a significantly shorter length of hospital stay compared to those who didn’t fall (p = 0.012). In multivariate logistic regression, we found that patients who were hospitalized more than 7 days had a lower risk of falling in the post-discharge period (OR = 0.195, p = 0.017) independently of the history of falls 6 months prior to admission. Conclusions: Further studies are required to validate the risk factors identified after discharge and to evaluate preventive measures. Elderly patients discharged from an Internal Medicine Ward should be screened to determine the risk of falls, specifically previous history of falls and medication. These patients should integrate a fall prevention program.
