背景新辅助免疫治疗在局部晚期非小细胞肺癌（non-small cell lung cancer, NsCLC）的应用仍存在争议。因此，本研究旨在报道通过免疫联合化疗将初始不可切除c IIIB期NsCLC患者转化为根治性微创手术的系列病例，评估手术效果和术后的生存获益。方法回顾性分析2018年5月-2020年8月期间接受程序性细胞死亡受体1(programmed cell death-1,PD-1)抑制剂联合含铂双药化疗的51例c IIIB期NsCLC患者。收集入组患者手术相关数据和肿瘤相关数据。结果31例患者最终接受手术治疗，其中23例（74.2%）患者行肺叶切除术，1例（3.2%）患者行全肺切除术，5例（16.1%）患者行袖状肺叶切除术，2例（6.5%）患者行双肺叶切除术。中位手术时间为205 min（范围100-520），平均失血量为185 m L（范围10-1100）。15例患者出现致密粘连或纤维化。术后平均住院时间为6 d（范围3-13）。无手术相关死亡，仅5例（16.1%）患者发生术后并发症（无3级以上并发症）。10例（32.3%）患者达到主要病理缓解，22/31例（71.0%）患者出现纵隔病变降期。中位随访15.4个月，31例手术患者的中位无进展生存期/无病生存期（progression-free survival/disease-free survival, PFs/DFs）相对于未进行手术的无缓解或缓解的患者（分别为27.5个月vs 4.7个月vs 16.7个月）更长。结论初始不可切除的IIIB期NsCLC接受免疫联合化疗治疗后行根治性手术相对安全，手术相关死亡率和并发症发病率较低，与未手术患者相比DFs/PFs更长。

The aging of the population and improvements in outcomes after cardiovascular surgery have resulted in a worldwide growing demand of complex surgical intervention for elderly patients. We briefly review the up-to-date English-language literature with particular focus on cardiovascular surgery in elderly patients. With earlier referral, careful preoperative evaluation, strategic planning, and the continuing efforts in optimizing surgical techniques, operative mortality and morbidity following primary or reoperative coronary artery bypass grafting and valvular interventions are expected to fall in this high-risk patient subset. Importantly, accumulating evidence indicates that elderly patients may benefit from improved functional status and quality of life after cardiovascular surgical therapy(J Geriatr Cardiol 2005,2(2): 123-128).

hepatitis B remains a leading cause of cirrhosis, hepatocellular carcinoma and liver transplantation worldwide. Management of chronic hepatitis B during pregnancy is challenging. Transmission of hepatitis B to infants still occurs perinataily although immunoprophylaxis is widely available for infants born to mothers with chronic hepatitis B infection. The emerging data suggest that initiation of antiviral therapy in the beginning of the third trimester in highly viremic mothers can prevent immunoprophylaxis failure in their infants. The available drug safety data show that lamivudine, telbivudine and tenofovir are generally safe to be used during the pregnancy. In order to minimize the fetal exposure to the antiviral medication, antiviral therapy during the pregnancy should be limited to a selected group of patients with cirrhosis, high hepatitis B viral load, or prior history immunoprophylaxis failure. An elective Caesarean section may reduce the risk of perinatal transmission. For those females planning for pregnancy or in early stage of pregnancy, communication and follow-up among obstetrician, gastroenterologist, and primary care physician are important. In this article, we will review the features of hepatitis B infection before, during and after the pregnancy; the risk factors that increase mother-tochild transmission; safety data on antiviral drug use during pregnancy; and the potential role of Caesarean section in selected cases.

In the purpose to design novel antituberculosis (anti-TB) drugs agents against Mycobacterium tuberculosis (Mtb), we have built a molecular library around 42 halimane Diterpenoids isolated from natural sources. Two Mtb enzymes drug targets (Mtb Mycothiol s-transferase and Mtb homoserine transacetylase) have been adopted. The pharmacological potential was investigated through molecular docking, molecular dynamics simulation, density functional theory (gas phase and water) and ADMET analysis. Our results indicate that (2R,5R,6s)-1,2,3,4,5,6,7,8-octahydro-5-((E)-5-hydroxy-3-methylpent-3-enyl)-1,1,5,6-tetramethylnaphtha-lene-2-ol (compound 20) has displays higher docking score with each of the selected drug targets. In addition, this molecule exhibits a satisfactory drug potential activity and a good chemical reactivity. Its improved kinetic stability in the Mtb Mycothiol s-transferase enzyme reflects its suitability as a novel inhibitor of Mtb growth. This molecule has displayed a good absorption potential. Our results also show that its passive passage of the intestinal permeability barrier is more effective than that of first-line treatments (ethambutol, isoniazid). In the same way, this anti-TB druglikeness has shown to be able to cross the blood brain barrier.

Pediatric origin of cancer stem cell hypothesis holds great promise and potential in adult cancer treatment, however; the road to innovation is full of obstacles as there are plenty of questions left unanswered. First, the key question is to characterize the nature of such stem cells (concept). second, the quantitative imaging of pediatric stem cells should be implemented(technology). Conceptually, pediatric stem cell origins of adult cancer are based on the notion that plasticity in early life developmental programming evolves local environments to cancer. Technologically, such imaging in children is lacking as all imaging is designed for adult patients. We postulate that the need for quantitative imaging to measure space-time changes of plasticity in early life developmental programming in children may trigger research and development of the imaging technology. such quantitative imaging of pediatric origin of adulthood cancer will help develop a spatiotemporal monitoring system to determine cancer initiation and progression. Clinical validation of such speculative hypothesis-that cancer originates in a pediatric environment-will help implement a wait-andwatch strategy for cancer treatment.
