Sleep quality in middle-aged and elderly hemodialysis patients:Impact of a structured nursing intervention program

BACKGROUND Poor sleep quality is common among hemodialysis patients and can significantly impact their *** study aimed to evaluate the effectiveness of a structured nursing intervention program in improving sleep quality in middleaged and elderly hemodialysis *** To evaluate the impact of nursing intervention on sleep quality in hemodialysis *** This cross-sectional study was conducted in a tertiary hospital,the First Affiliated Hospital of Nanchang University,in *** study included 105 middle-aged and elderly hemodialysis patients aged≥45 years who underwent maintenance hemodialysis for at least 3 mo,utilizing the Pittsburgh Sleep Quality Index(PSQI)to identify poor *** identified underwent a 12-wk nursing intervention program focusing on education,relaxation techniques,and ***-intervention,sleep quality was reassessed using the *** The study found that 68.6%of hemodialysis patients were poor *** the 12-wk nursing intervention program,there was a significant decrease in the mean global PSQI score from 8.9±3.2 to 5.1±2.7(P<0.001),indicating improved sleep *** demonstrated the effectiveness of the structured nursing intervention in enhancing sleep quality for middle-aged and elderly hemodialysis *** The structured nursing intervention program focusing on sleep hygiene education,relaxation techniques,and counseling effectively improved sleep quality among middle-aged and elderly hemodialysis *** significant decrease in the mean global PSQI score post-intervention indicates the positive impact of tailored nursing interventions in addressing poor sleep quality in this patient *** findings emphasize the importance of implementing targeted nursing interventions to enhance the quality of life for hemodialysis patients by addressing the prevalent issue of poor sleep quality.

Treg Immunomodulation Contributes to the Anti-atherosclerotic Effects of Huxin Formula in ApoE-/- Mice

Objective: To explore the effects of Huxin formula（HXF） in curtailing atherosclerosis and its underlying mechanism. Methods: According to random number table method, 24 specific pathogen free male ApoE-/-mice were randomly divided into model group, HXF low-dose（HXF-L） group（8.4 g/kg daily）, HXF high-dose（HXF-H） group（16.8 g/kg daily）, and pravastatin（8 mg/kg daily） group in Experiment Ⅰ（n=6 per group）. C57BL/6J mice served as the control group（n=6）. ApoE-/-mice in HXF-L, HXF-H, pravastatin groups were fed a Western diet and administered continuously by gavage for 12 weeks, while C57BL/6J mice in the control group were fed conventional lab mouse chow for 12 weeks. Further, Tregs were depleted by weekly intraperitoneal injection of purified anti-mouse CD25 antibody（PC61, 250 μg per mouse） for 4 weeks in Experiment Ⅱ（n=6 per group）. Oil Red O and Masson staining were used to evaluate the plaque area and aortic root fibrosis. The CD4+CD25+Foxp3+Treg counts in the lymph nodes and spleen cells were detected using flow cytometric analysis. The transforming growth factor-β1（TGF-β1）, interleukin（IL）-10, and IL-6 serum levels were examined by MILLIPLEX?MAP technology. Quantitative real-time reverse transcription PCR（qRT-PCR） and Western blot were utilized to assess the expression of TGF-β mRNA and protein in the aorta. The expression of CD4+T lymphocytes, macrophages and smooth muscle cells in the aortic root were detected by immunofluorescence staining. Results: HXF reduced plaque area in ApoE-/-mice（P0.05）. Also, HXF upregulated the expression of TGF-β mRNA and protein in the aorta（P<0.05）. Additionally, HXF attenuated CD4+T lymphocytes, macrophages and smooth muscle cells in aortic root plaque（P<0.01）. Atrium-atrioventricular node block： an unusual complication during catheter ablation of persistent atrial fibrillation

Ablation of persistent atrial fibrillation is still a challenge for the ablationist. Extensive ablation is required under some conditions and could lead to some unintended complications. Here we report a case of atrium-atrioventricular node block complicating multiple catheter ablation procedures for persistent atrial fibrillation. After extensive ablation, including circumferential pulmonary vein ablation, linear ablation at the left atrial roof, mitral isthmus, atrial septum, cavotricuspid isthmus, and complex fractionated atrial electrogram ablation, conduction obstacle was found, and sinus impulse could not travel from the right atrium, atrial septum and left atrium to atrioventricular node. The case indicated that intensive ablation at some key sites, especially the interatrial septum, should be careful during ablation of atrial fibrillation

Study on trifluoromethanesulfonic acid-promoted synthesis of daidzein: Process optimization and reaction mechanism

Daidzein has been widely used in pharmaceuticals,nutraceuticals,cosmetics,feed additives,*** preparation process and related reaction mechanism need to be further investigated.A cost-effective process for synthesizing daidzein was developed in this *** this article,a two-step synthesis of daidzein(Friedel–Crafts acylation and[5+1]cyclization)was developed via the employment of trifluoromethanesulfonic acid(TfOH)as an effective promoting *** effect of reaction conditions such as solvent,the amount of TfOH,reaction temperature,and reactant ratio on the conversion rate and the yield of the reaction,respectively,was systematically investigated,and daidzein was obtained in 74.0%isolated yield under optimal *** to the facilitating effect of TfOH,the Friedel–Crafts acylation was completed within 10 min at 90℃ and the[5+1]cyclization was completed within 180 min at 25℃.In addition,a possible reaction mechanism for this process was *** results of the study may provide useful guidance for industrial production of daidzein on a large scale.

Photoinduced ligand-to-metal charge transfer(LMCT) has emerged as an effective strategy for synthesizing organic molecules in a sustainable manner. However, the majority of existing reports on selective C(sp^(3))–H bond functionalization via photoinduced LMCT focus on the use of late transition metals or rare-earth metals for radical additions or cross-couplings. In contrast, the utilization of photoinduced LMCT with 3d early transition metals poses a significant challenge. Herein, we describe an unprecedented approach to allylic C(sp^(3))–H addition to aldehydes, employing chromium(Cr) complexes as catalysts through visible-light-induced LMCT. By investigating the reaction pathway through various mechanistic studies, including radical trapping, kinetic isotope effect(KIE) analysis, and transient absorption spectroscopy, valuable insights have been gained. The proposed mechanism suggests the intermediacy of bromine radicals through homolysis of the Cr–Br bond. Notably, this protocol expands our understanding of the photochemical properties of earth-abundant Cr complexes.

Accurate characterization of bubble mixing uniformity in a circular region using computational geometric theory

The present study proposes a novel method based on the geometric theory for measuring the distribution of bubble swarms in the circular region of a direct-contact heat *** was determined that the mixing is uniform when the average distance between the bubble swarms in the unit circular region is approximately 0.9054,which is the standard reference *** effect of sample size(i.e.,the number of bubbles)on mixing uniformity was investigated to determine the optimal sample *** was verified that the metric's accuracy and stability were higher with a sample size of ***,it was proposed to increase the sample size by filling irregular bubbles using a segmentation method,which enabled a further accurate assessment of the mixing *** mixing uniformity of bubble swarms in the circular region and its maximum internal connection with the square region was accurately *** was revealed that the relative average error increased by approximately 3.47% due to information *** proposed method was demonstrated to be rotationally *** present study provided novel insights into evaluating mixing uniformity,which would guide enhanced heat transfer and the effective evaluation of the spatiotemporal characteristics of transient mixing in circular regions or the cross-sections of chemical transport pipelines.

Converting carbohydrates into 5-hydroxymethylfurfural(5-HMF) is an attractive and promising route for value-added utilization of agricultural and forestry biomass resource. As an important platform compound, 5-HMF possesses high active furan structure with hydroxymethyl and aldehyde group for production of various bio-chemicals and materials, meanwhile, which suffer from low stability and poor yield during the industrial biorefinery process. Hence, selective production of 5-HMF with high-yield and low-cost has attracted extensive attention from scientific and industrial researchers. This review sorted and described the latest advanced research on solvent and catalyst system, as well as energy field effect for production of 5-HMF with different feedstock in detail, emphatically discussing the solvent effect and its synergistic effect with other aspects. Besides, the future prospects and challenges for production of 5-HMF from carbohydrates were also presented, which provide a profound insight into industrial 5-HMF process with economic and environmental feature.

A spatiotemporal deep learning method for excavation-induced wall deflections

Data-driven approaches such as neural networks are increasingly used for deep excavations due to the growing amount of available monitoring data in practical ***,most neural network models only use the data from a single monitoring point and neglect the spatial relationships between multiple monitoring ***,most models lack flexibility in providing predictions for multiple days after monitoring *** study proposes a sequence-to-sequence(seq2seq)two-dimensional(2D)convolutional long short-term memory neural network(S2SCL2D)for predicting the spatiotemporal wall deflections induced by deep *** model utilizes the data from all monitoring points on the entire wall and extracts spatiotemporal features from data by combining the 2D convolutional layers and long short-term memory(LSTM)*** S2SCL2D model achieves a long-term prediction of wall deflections through a recursive seq2seq *** excavation depth,which has a significant impact on wall deflections,is also considered using a feature fusion *** excavation project in Hangzhou,China,is used to illustrate the proposed *** results demonstrate that the S2SCL2D model has superior prediction accuracy and robustness than that of the LSTM and S2SCL1D(one-dimensional)*** prediction model demonstrates a strong generalizability when applied to an adjacent *** on the long-term prediction results,practitioners can plan and allocate resources in advance to address the potential engineering issues.

Modeling of multiphase flow in low permeability porous media:Effect of wettability and pore structure properties

Multiphase flow in low permeability porous media is involved in numerous energy and environmental ***,a complete description of this process is challenging due to the limited modeling scale and the effects of complex pore structures and *** address this issue,based on the digital rock of low permeability sandstone,a direct numerical simulation is performed considering the interphase drag and boundary slip to clarify the microscopic water-oil displacement *** addition,a dual-porosity pore network model(PNM)is constructed to obtain the water-oil relative permeability of the *** displacement efficiency as a recovery process is assessed under different wetting and pore structure *** show that microscopic displacement mechanisms explain the corresponding macroscopic relative *** injected water breaks through the outlet earlier with a large mass flow,while thick oil films exist in rough hydrophobic surfaces and poorly connected *** variation of water-oil relative permeability is significant,and residual oil saturation is high in the oil-wet *** flooding is extensive,and the residual oil is trapped in complex pore networks for hydrophilic pore surfaces;thus,water relative permeability is lower in the water-wet *** the displacement efficiency is the worst in mixed-wetting systems for poor water *** negatively correlates with invading oil volume fraction due to strong capillary resistance,and a large microporosity corresponds to low residual oil *** work provides insights into the water-oil flow from different modeling perspectives and helps to optimize the development plan for enhanced recovery.
