Background/Aims: This study aimed at correlating the p.esence of extrahep.tic manifestations with hep.titis B virus (HBV) genotyp.s in p.tients with chronic HBV infection. Methods: This was a national (France), multicenter, retrosp.ctive, cross-sectional study. HBV genotyp.s were determined in 190 p.tients HBsAg-p.sitive for at least 6 months and documented before any treatment. Results: p.tients were aged 42±15 years and mainly male (77%). Alcohol intake was high in 6%of them, ALT elevated in 73%; 27%were cirrhotic. All HBV genotyp.s were found, mainly A (24%), D (29%), C (11%), and E (10%). Thirty (16%) p.tients had clinical extrahep.tic manifestations, mainly sensory-motor deficiency, sicca syndrome, myalgia, glomerulonep.ritis, and arthralgia-arthritis. Their p.esence was not related to any ep.demiologic, viral (including genotyp.s) or hep.tic factor, but to a higher p.atelet count (p.0.004). Twenty-nine (15%) p.tients had biological extrahep.tic manifestations, mainly anti-smooth muscle, antinuclear, and anti-nucleo-some antibodies. Their p.esence was related only to anti-HBe antibodies p.sitivity (p.0.007) or elevated p.atelet count (p.0.003). Carrying p.ecore mutant HBV increased by 2.8 folds the risk to have at least one extrahep.tic biological manifestation. Conclusions: No relationship. between HBV genotyp.s and the p.esence of extrahep.tic manifestations were evidenced in p.tients with chronic HBV infection.

The relationship.between cryoglobulin and severity of liver lesions is *** study has focused on the relationship.between cryoglobulin,liver steatosis,and *** aim of this study was to determine the relationship.between cryoglobulins and liver lesions(necroinflammation,fibrosis,and steatosis) in p.tients with hep.titis C virus(HCV) *** hundred and thirty-seven consecutive p.tients with untreated chronic hep.titis C who had been admitted for liver biop.y were included in the *** factors for fibrosis and steatosis were *** mean age was 50.9 ± 13.8 years,and 49% were *** was p.esent in 286 p.tients,103 of whom had *** hundred and eighty-six p.tients(43%) had steatosis greater than 10%,and 110(25%) had advanced fibrosis(Metavir score F3-F4) .On multivariate analysis,cryoglobulin increased by nearly threefold the risk of having advanced fibrosis and steatosis greater than 10%.Steatosis greater than 10% was associated with a higher body mass index(p.p. 3(p.p.=.002) ,and advanced liver fibrosis(p.=.009) .Advanced fibrosis(F3-F4) was associated with a higher level of γ-glutamyltransferase(p.=.04) ,cryoglobulin(p..001) ,a high grade of necroinflammation(Metavir score A2-A3) (p.p.=.04) .In conclusion,our study shows an indep.ndent association between cryoglobulin and steatosis as well as advanced fibrosis.
