便秘是一种常见的疾病,但有关其定义和发病机制的认识尚不统一。由于肠道传输时间并不是一个常数,不同时间测的数值有很大差异,不应作为便秘诊断的参数,其时间延长经常因出口梗阻型便秘所致,而非便秘的初始原因。由于便秘病人需要通过增加腹内压和盆腔内压力,产生排便的刺激,导致道格拉斯窝加深、肠疝、直肠内套叠、膀胱膨出、子宫及阴道脱垂及会阴过度下降等功能性改变,继而出现解剖学改变。直肠内脱垂分为两种类型:直肠长度和结构正常的直肠脱垂;以及伴有直肠肌层延长等结构改变的直肠脱垂,通过经肛吻合器直肠切除术(STARR)切除直肠脱垂及直肠膨出,可以明显改善这些功能性和解剖性变化。

Magnesium is attractive for the application as a temporary bone implant due to its inherent biodegradability,non-toxicity and suitable mechanical *** degradation process of magnesium in physiological environments is complex and is thought to be a diffusion-limited transport *** use a multi-scale imaging approach using micro computed tomography and transmission X-ray microscopy(TXM)at resolutions below 40 ***,we are able to evaluate the nanoporosity of the degradation layer and infer its impact on the degradation process of pure magnesium in two physiological *** samples were degraded in simulated body fluid(SBF)or Dulbecco’s modified Eagle’s medium(DMEM)with 10%fetal bovine serum(FBS)for one to four *** reveals the three-dimensional interconnected pore network within the degradation layer for both *** pore network morphology and degradation layer composition are similar for all *** contrast,the degradation layer thickness in samples degraded in SBF was significantly higher and more inhomogeneous than in DMEM+10%*** features could be observed within the degradation layer of samples degraded in SBF,suggesting the formation of microgalvanic cells,which are not present in samples degraded in DMEM+10%*** results suggest that the nanoporosity of the degradation layer and the resulting ion diffusion processes therein have a limited influence on the overall degradation *** indicates that the influence of organic components on the dampening of the degradation rate by the suppression of microgalvanic degradation is much greater in the present study.

AIM: To evaluate the impact of the diagnosis of hepatitis C infection on lifestyle habits such as smoking, drinking, sports activity and ***: A self-administered, anonymous question-naire was offered to out-patients with HCV infection consecutively attending three clinical centres in ***: Of the 275 respondents, 62.2% (171) were male. Mean age was 51 (range 20-80) years. Overall, after the diagnosis of hepatitis C, 74.5% of drinkers had modified (giving up or reducing) alcohol consumption, 21.3% of smokers had modifi ed their habits and 32.3% of subjects who reported sports activity had either stopped or reduced frequency of activity or chose a less fatiguing sport. Sixty-four percent of the respondents reported having modified their diet, most of them on physician's advice. CONCLUSION: After the diagnosis of hepatitis C many patients correctly modif ied their alcohol consumption and a minority their smoking habits. The reason for reported changes in diet and physical activity needs further investigations.

Associations of High Density Lipoprotein Cholesterol and Framingham cardiovascular Risk with Diabetic Retinopathy in African Type 2 Diabetics

Objectives: To assess the associations of high density lipoprotein cholesterol (HDL-C) and Framingham cardiovascular (CVD) with diabetic retinopathy (DR). Methods: A cross-sectional study of random sample of 200 T2DM Central Africans. Sociobiographical, laboratory and eye examination main outcome measures were investigated using Tertiles of HDL-C (stratification = lowest 10% were the significant independent determinants for DR. In the highest HDL-C group, smoking status and 10-year Framingham risk ≥ 10% were the significantly independent determinants for DR. In 10-year Framingham risk ≥ 10% group, smoking status, insulin resistance and increasing levels of HDL-C were the significantly independent determinants for DR. Conclusion: DR and VD remain a public health problem in T2DM Central Africans. Some Central Africans with DR and VD appear to have higher HDL-C than T2DM Central Africans without DR and VD. HDL-C in T2DM patients with DR, may be tightly controlled by genetic factors (black Bantu ethnicity) than the other lipoproteins as reported among Indians, African-Americans, and Japanese individuals. The most preventable environmental risk factors for DR were smoking status, global cardiovascular disease risk, insulin resistance and oxidative stress.

Urotensin-II （U-II） is a cyclic peptide that acts through a G protein-coupled receptor （urotensin-II receptor [UTR]） mainly involved in cardiovascular function in humans. The urotensinergic system is also implicated in the urogenital tract. Indeed, U-II relaxes human corpus cavernosum strips and causes an increase in intracavernous pressure （ICP） in rats. In light of this, the U-II/UTR pathway can be considered a new target for the treatment of erectile dysfunction. On this hypothesis, herein we report on two new UTR high affinity-agonists, P5U （H-Asp-c[Pen-Phe-Trp-Lys-Tyr-Cys]-Val-OH） and UPG84（H-Asp-c[Pen-Phe-DTrp-Orn-（pNH2） Phe-Cys]-Val-OH）. The effects of P5U and UPG84 were each compared separately with U-II by monitoring the ICP in anesthetized rats. Intracavernous injection of U-II （0.03-1 nmol）, P5U （0.03-1 nmol） or UPG84 （0.03-1 nmol） caused an increase in ICP. P5U, in particular, elicited a significant increase in ICP as compared to U-II. The observed effect by using P5U at a dose of 0.1 nmol per rat was comparable to the effect elicited by U-II at a dose of 0.3 nmol. Moreover, UPG84 at the lowest dose （0.03 nmol） showed an effect similar to the highest dose of U-II （1 nmol）. Furthermore, UPG84 was found to be more effective than P5U. Indeed, while the lowest dose of P5U （0.03 nmol） did not affect the ICP, UPG84, at the same dose, induced a prominent penile erection in rat. These compounds did not modify the blood pressure, which indicates a good safety profile. In conclusion, UPG84 and P5U may open new perspectives for the management of erectile dysfunction.

Chromitite bodies hosted in peridotites typical of suboceanic mantle(***)are found in the northern and central part of the Loma caribe peridotite,in the Cordillera Central of the Dominican *** chromitites are massive pods of small size(less than a few meters across)and veins that intrude both dunite and ***,they are high-Cr chromitites[Cr#=Cr/(Cr+Al)atomic ratio=0.71-0.83]singularly enriched in TiO2(up to 1.25 wt.%),Fe2 O3(2.77-9.16 wt.%)as well as some trace elements(Ga,V,Co,Mn,and Zn)and PGE(up to 4548 ppb in whole-rock).This geochemical signature is unknown for chromitites hosted in oceanic upper mantle but akin to those chromites crystallized from mantle plume derived ***,the melt estimated to be in equilibrium with such chromite from the Loma caribe chromitites is similar to basalts derived from different source regions of a heterogeneous caribbean mantle *** mantle plume is responsible for the formation of the caribbean Large Igneous Province(CLIP).Dolerite dykes with back-arc basin basalt(BABB)and enriched mid-ocean ridge basalt(E-MORB)affinities commonly intrude the Loma caribe peridotite,and are interpreted as evidence of the impact that the caribbean plume had in the off-axis magmatism of the back-arc basin,developed after the caribbean island-arc extension in the Late *** propose a model in which chromitites were formed in the shallow portion of the back-arc mantle as a result of the metasomatic reaction between the supra-subduction zone(SSZ)peridotites and upwelling plume-related melts.
