Enhanced recovery after surgery-based evidence-based care plus ice stimulation for thirst management in convalescent patients following digestive surgery under general anesthesia

BACKGROUND Thirst management in convalescent patients recovering from a digestive surgery performed under general anesthesia requires attention.A simple,practical,and safe method can effectively relieve thirst symptoms in such *** To evaluate the enhanced recovery after surgery(ERAS)-based evidence-based care(EBC)plus ice stimulation therapy for thirst management of convalescent patients following digestive surgery performed under general *** A total of 191 patients convalescing after digestive surgery performed under general anesthesia between March 2020 and February 2023 and experiencing thirst were *** total,89 patients and 102 patients in the control and research groups received routine care and ERAS-based EBC plus ice stimulation therapy,*** following data were comparatively analyzed:(1)Thirst degree(thirst intensity numerical rating scale)and thirst distress(TD)degree(TD scale);(2)Oral mucosal wetness;(3)Unstimulated whole salivary flow rate(UWSFR);(4)Adverse reactions(palpitation,fatigue,chapped lips,and nausea and vomiting);and(5)Nursing *** After nursing,thirst degree and distress were statistically lower in the research group than in the control ***,compared with the control group,the research group exhibited a lower degree of oral mucosal wetness,higher UWSFR,fewer adverse reactions,and more total nursing *** ERAS-based EBC plus ice stimulation therapy can effectively alleviate thirst in convalescent patients recovering from a digestive surgery performed under general *** can alleviate xerostomia symptoms,reduce adverse reactions,and improve patient comfort.

The Study of Ultrasound-Guided Central Venous Catheterization in the Teaching of Anesthesia Residents

Objective: To evaluate the application effect of ultrasound-guided central venous catheterization in the teaching of anesthesia residents. Methods: Forty anesthesia resident companions who received standardized residency training in our department from July 2018 to July 2020 were randomly divided into an ultrasound group and a control group, with 20 participants in each group. The ultrasound group was taught by ultrasound-guided central venipuncture, while the control group was taught by traditional anatomy. After ten training punctures, all trainees were assessed twice. Results: Both groups could master the technique of central venipuncture. The success rate of first puncture and the overall success rate of puncture in the ultrasound group were significantly higher than those in the control group (p p < 0.05). Conclusion: Compared with the traditional anatomical localization teaching, the use of ultrasound-guided technology can improve the success rate of puncture, save puncture time, reduce related complications, and have a better training effect.

为了解贵州小车河不同水文期浮游植物生态位及种间联结性,于2020~2022年采集了2个不同水文期(枯水期和丰水期)的浮游植物样品.利用生态位宽度(B_(i))、生态位重叠(O_(ik))、方差比率法(VR)、卡方检验(χ^(2))、AC检验和Jaccard指数(JI)分析了浮游植物优势种的生态位和种间联结.结果表明,小车河共鉴定出浮游植物7门50种,其中优势种为4门12种.生态位宽度与FG功能群结合分析表明,小车河水体环境为中富营养状态;优势种可划分为广生态位种、中生态位种和窄生态位种,优势种生态位宽度在1.278~4.849之间变化.生态位重叠值3 a差异不大,O_(ik)>0.6的占比分别为50.00%(2020年)、42.86%(2021年)、53.49%(2022年),表明有接近或超过一般的种对占用环境资源的方式高度相似.优势种间总体呈显著正关联,群落较稳定;卡方检验显示66对优势种对仅有4对有显著联结性,AC检验结果显示有33对有显著联结性,Jaccard指数显示有41对有显著联结性,均为负关联多于正关联,表明小车河浮游植物群落正处于演替初期.

Plants have mechanisms to transport secondary metabolites from where they are biosynthesized to the sites where they function,or to sites such as the vacuole for ***,current research has mainly focused on metabolite biosynthesis and regulation,and little is known about their ***,a class diterpenoid with medicinal properties,is biosynthesized in the periderm of Salvia miltiorrhiza ***,we discovered that tanshinone can be transported out of peridermal cells and secreted into the soil environment and that the ABC transporter SmABCG1 is involved in the efflux of tanshinoneⅡA and tanshinoneⅠ.The SmABCG1 gene is adjacent to the diterpene biosynthesis gene cluster in the *** *** temporal–spatial expression pattern of SmABCG1 is consistent with tanshinone accumulation ***1 is located on the plasma membrane and preferentially accumulates in the peridermal cells of *** *** expression in Xenopus laevis oocytes demonstrated that SmABCG1 can export tanshinoneⅡA and tanshinoneⅠ.CRISPR/Cas9-mediated mutagenesis of SmABCG1 in *** hairy roots resulted in a significant decrease in tanshinone contents in both hairy roots and the culture medium,whereas overexpression of this gene resulted in increased tanshinone ***76AH3 transcript levels increased in hairy roots overexpressing SmABCG1 and decreased in knockout lines,suggesting that SmABCG1 may affect the expression of CYP76AH3,indirectly regulating tanshinone ***,tanshinoneⅡA showed cytotoxicity to Arabidopsis *** findings offer new perspectives on plant diterpenoid transport and provide a new genetic tool for metabolic engineering and synthetic biology research.

Catalytic atroposelective synthesis of indolyl quinazolinones bearing N-N/C-C diaxes

The first catalytic atroposelective synthesis of indole derivatives bearing N-N/C-C diaxes has been established by the strategy of de novo ring formation. By this strategy, various indolyl quinazolinones bearing vicinal N-N/C-C diaxes were synthesized in high yields with overall good diastereoselectivities and excellent enantioselectivities via chiral phosphoric acid-catalyzed onepot asymmetric formal (5+1) cycloaddition/oxidation reaction. Moreover, such indolyl quinazolinones bearing vicinal N-N/CC diaxes have good configurational stability and could be converted into new chiral ligands with applications in asymmetric catalysis. Therefore, this work not only offered a new member for the family of diaxial atropisomers, but also provided a powerful strategy for catalytic asymmetric synthesis of atropisomers bearing N-N/C-C diaxes.

Dear Editor,Phenotypic drug discovery,which observes compound effects on physiology,has given way to targeting specific molecules,but has revived by using modern methods to find drugs based on therapeutic outcomes in disease models(Vincent et al.,2022).Osteoarthritis(OA)is a prevalent chronic joint disorder,primarily affecting knee joints,with osteoclast activation due to low-grade inflammation resulting in increased bone resorption and structural changes in the joint(Katz et al.,2021).This imbalance and the discharge of inflammatory mediators perpetuate the cycle of inflammation and tissue ***,controlling inflammation and modulating osteoclastogenesis are critical therapeutic targets for managing OA.

Efficacy of water infusion combined with defoamers in colonoscopy

BACKGROUND Currently,colonoscopy still needs continuous optimization and exploration of novel alternative approaches to enhance the experience of patients during *** To analyze the efficacy of water infusion combined with defoamers in *** This study included 97 patients undergoing colonoscopy from January 2024 to June *** participants were categorized into two groups,namely,the control group(n=47),who underwent conventional colonoscopy,and the experimental group(n=50),who received colonoscopy using water injection combined with defoamers.A comparative analysis was then conducted on the disease detection rate(colonic polyps,colonorrhagia,colonic ulcers,colonic mucosal lesions,and others),colonoscopy duration,abdominal pain[visual analog scale(VAS)],Boston bowel preparation scale(BBPS),self-rating anxiety scale(SAS),bowel preparation comfort,complications(intestinal perforation,bleeding,nausea and vomiting,abdominal pain,and abdominal distension),and patient *** The experimental group demonstrated a significantly higher total disease detection rate,BBPS scores,and patient satisfaction compared with the control ***,the research group exhibited shorter colonoscopy duration,lower VAS and SAS scores and total complication rate,and better patient comfort and *** These results indicate that the combination of water injection and defoamers exhibited an overall better therapeutic effect than conventional colonoscopy,mainly reflected in higher disease detection rate,faster examination ef-ficiency,lower abdominal pain,anxiety,and complication incidences,and significantly better bowel preparation,comfort,and patient satisfaction.

Norcantharidin:A potential antiangiogenic agent for gallbladder cancers in vitro and in vivo

Our objective was to explore the antiangiogenic activity of norcantharidin ( NCTD ) as an angiogenic inhibitor for gallbladder cancers. In vitro and in vivo experiments to determine the effects of NCTD on HUVECs,chicken CAM capillaries and gallbladder cancer xenograft angiogenesis in nude mice were respectively done. The MTT method was used to assay the cytotoxicity of NCTD on HUVECs. Immunofluorescence was used to evaluate HUVEC apoptosis. The scraping line method,Matrigel invasion assay and tube formation assay were used to detect the migration,invasion and tube formation of HUVECs. A digital camera was used to observe chicken CAM capillaries. Experiments with NCTD in a xenograft model were used to observe the effect of NCTD on xenograft growth and survival of mice with xenografts. CD34 immunohistochemistry,flow cytometry and micro-MRA were used, respectively,to determine MVD,cell apoptosis and hemodynamic analysis of the xenografts. Immunohistochemistry and RT-PCR were used,respectively,to detect the expression of VEGF,Ang-2,TSP,TIMP-2 proteins/mRNAs of the xenografts. The xenograft MVD associated with tumor volume,the PCNA/apoptosis ratio and related-protein expression was evaluated simultaneously. We found that NCTD effectively inhibited the proliferation,migration,invasion and capillary-like tube formation of HUVECs in vitro; it reduced angiogenesis and directly destroyed the formed CAM capillaries in vivo. In the experiments in mice,NCTD not only inhibited significantly xenograft proliferation and growth,prolonged survival time of mice with xenografts,decreased the xenograft MVD and vascular perfusion,but also,similarly to ES,decreased significantly the expression of VEGF or Ang-2 protein/mRNA, increased the expression of TSP or TIMP-2 protein/mRNA. Moreover,the xenograft MVD was positively related with tumor volume,PCNA/apoptosis ratio,and VEGF or Ang-2 expression,respectively ( all P < 0. 05) ,but negatively correlated with TSP or TIMP-2 expression ( both P

Both 20S and 19S proteasome components are essential for meiosis in male mice

The proteasome, an evolutionarily conserved proteolytic complex comprising the 20S core particle and 19S regulatory particles, performs both shared and distinct ***, a highly complex developmental process, relies on proteasome activity at multiple stages to regulate protein turnover. In this study, we selected the20S subunit PSMA1 and 19S regulatory subunit PSMD2 to investigate the potential functions of the proteasome in spermatogenesis. Using Psma1-EGFP and Psmd2-mCherry knock-in mouse models, we confirmed the expression of both subunits in all spermatogenic cell types,with pronounced presence in early germ cell *** further clarify their functional significance, we specifically knocked out Psma1 and Psmd2 in germ *** of either PSMA1 or PSMD2 led to disrupted spermatogenesis, characterized by the complete absence of sperm in the epididymis. Subsequent analysis indicated that loss of these proteasome components impaired meiotic initiation. Psma1 and Psmd2 knockout germ cells showed accumulation of DMRT1, a key regulator of mitosis-to-meiosis transition, leading to a reduction in STRA8 levels and consequent disruption of meiosis initiation. This study sheds light on the molecular mechanisms that govern meiotic initiation and identifies potential genes associated with male infertility.
