Structural, densitometric and biomechanical evaluations of Chinese patients with long-term bisphosphonate treatment

Background The association of long-term bisphosphonate treatment for osteoporosis and related safety problems such as atypical fractures were not clearly defined. This study was to evaluate the structural, densitometric and biomechanical properties of the prolonged bisphosphonate-loaded bones. Methods Bone mineral density （BMD） at hip and femoral midshaft, bone cross-sectional area, moment of inertia of both femurs, bone formation and resorption biochemical markers were compared between 28 elderly with at least 4 years of bisphosphonate treatment from 2002 through 2006 and age-matched group of 37 elderly. Results The total hip BMD and t-score were found not different between two groups. However, bisphosphonate treated patients were found to have significantly lower bone mineral content in the femoral shaft （P 〈0.05）; morphological study showed lower cross-sectional area in subtrochanteric and mid-diaphyseal region and thus significantly lower moment of inertia （P 〈0.01）. High resolution-peripheral quantitative computed tomography showed significantly decreased trabeculardensity, bone volume ratio, trabecular number but increased trabecular spacing in tibia and distal radius. Finite element analysis further confirmed significantly lower stiffness and failure load in tibia. Biochemical studies also showed lower bone resorption and severely suppressed bone formation activity （P 〈0.001）. Conclusions The unchanged total hip BMD between two groups confirmed the beneficial effects of bisphosphonate on trabecular bone, thus preventing osteoporotic fractures at large in previous studies. However, the inferior structural, densitometric and biomechanical properties at cortical bones, especially femur midshaft, need a special attention to look into the association between long-term bisphosphonate intake and the occurrence of stress fractures. When patients taking bisphosphonate complain of proximal thigh pain or discomfort, plain X-ray film can be the first line screening. All

＜正＞One of the critical aspects of modern portfolio theory is to deal with the merits of international diversification, which is closely linked with the co-movements and interdependence between stock markets. This paper aims to investigate the potential of diversifying into U.S. and some Asia Pacific markets by studying the co-movements of the monthly returns of the markets between July 1997 and Dec 2004. The markets considered are: Australia, China, Hong Kong, India, Japan, Malaysia, Singapore, South Korea, Taiwan, Thailand and the U.S. Cluster analysis is first used to classify the markets into groups such that markets within the same group move quite closely in tandem with each other while movements of markets from different groups are not quite closely related. The correlation coefficients between the monthly returns of the markets are used as the measures of similarity between the markets. Then from each group the market with the highest monthly return per unit risk is selected to form a portfolio. An optimum investment strategy is then derived using the Markowitz algorithm, i.e. weight of investment in each market in the portfolio is determined so that the risk is minimized for a given rate of return. We will also look at the stability of the cluster structure by applying the same technique over three sub-periods.

要:本文通过研究深圳华侨城社区居民日常活动特征及其差异,探讨城市居民的社区依赖性。因子分析界定了5种社区日常活动因子:低社区依赖性活动距离、低社区依赖性活动频率、高社区依赖性活动频率、工作的频率与距离以及高社区依赖性活动距离,累积解释率达63.597%。研究进一步表明:女性低社区依赖性活动的时间距离远于男性;居民对社区的依赖性随着年龄的增长而增强;居民对社区的依赖性随着学历的增长而降低;居民对社区的依赖性随着收入的增长而降低;居民对社区的依赖性随着家庭结构的扩大和老龄化而增强;居民高社区依赖性活动频率随着居住时长的增长而提高。

O-GlcNAc对上唇发育过程中上皮间叶转化影响的研究

目的:研究氧联乙酰葡萄糖胺(O-linked N-acetylglucosamine,O-GlcNAc)在上唇发育过程中对面突间上皮间叶转化的影响。方法:通过在鸡胚上唇植入浸有O-GlcNAc促进剂OGT(O-GlcNAc transferase)、OGlcNAc的抑制酶OGA(O-GlcNAcase)、以及OGA的抑制剂STZ(streptozoticin)的琼脂糖微球,构建O-GlcNAc高表达与低表达以及反向高表达实验组并继续孵育24h,茜素红/阿利新蓝骨组织染色观察胚胎骨、软骨发生的变化,免疫组化染色法检测具有间叶组织表达特异性的fibronectin和上皮表达特异性的p63的表达状态。结果:高表达O-GlcNAc实验组OGT和STZ中,发现胚胎上唇出现了骨缺损,且P63、fibronectin持续高表达,上皮、间叶组织保持连续性,凋亡出现延迟,上皮间叶转化出现障碍。正常对照测与低表达OGA组在上皮融合过程中P63表达减弱,fibronectin在上皮中出现表达,上皮层出现断裂,表明上皮间叶转化的正常进行。结论:O-GlcNAc过表达对于上唇形成过程中上皮间叶转化具有负面作用,将影响上唇的正常发育并可能导致唇裂的发生。

Objective In prenatal diagnostics,both microarray comparative genomic hybridization (array CGH) and single nucleotide polymorphism(SNP) genotyping have proven to be powerful genomic technologies utilized for the evaluation of fetal structural *** only SNP arrays enable the detection of copy number neutral regions of absence of heterozygosity(AOH), they have limited ability to detect small copy number variants(CNVs) due to the uneven distribution of SNPs across the *** provide comprehensive prenatal testing for both CNVs and copy neutral *** We improved our target designed high-resolution oligonucleotide array that has targeted coverage of 1,650 genes with 15,000 SNP probes,referred to as Fetal DNA chip v2.0(8x60K format).Results Of the 240 cases evaluated by this array, clinically significant CNVs were detected in 25 cases including 21 cases with pathogenic CNV *** addition,31 cases(13.0%) showed at least one AOH region >3 Mb,four of them with AOH >10 Mb demonstrated to be of clinical *** Our data demonstrate that even though this array has a lower density of SNP probes than commercially available SNP arrays, it reliably detected AOH events >3 Mb as well as Pathogenic ***,combining targeted array CGH with SNP probes into one platform provides clinically useful prenatal genetic screening in a more efficient manner.

Objectives The predisposition of intracranial atherosclerotic disease(ICAD)to East Asians over Caucasians infers a genetic basis which,however,remains largely *** prevalence of vascular risk factors(VRFs)in Chinese over Caucasian patients who had a stroke,and shared risk factors of ICAD with other stroke subtypes indicate genes related to VRFs and/or other stroke subtypes may also contribute to *** Unrelated symptomatic patients with ICAD were recruited for genome sequencing(GS,60-fold).Rare and potentially deleterious single-nucleotide variants(SNVs)and small insertions/deletions(InDels)were detected in genome-wide and correlated to genes related to VRFs and/or other stroke *** aneuploidies,copy number variants(CNVs)and chromosomal structural rearrangements were also ***,candidate genes were used for pathway and gene ontology enrichment *** Among 92 patients(mean age at stroke onset 61.0±9.3 years),GS identified likely ICAD-associated rare genomic variants in 54.3%(50/92)of ***-eight patients(52.2%,48/92)had 59 rare SNVs/InDels reported or predicted to be deleterious in genes related to VRFs and/or other stroke *** of the 59 rare variants were identified in local subjects without ICAD(n=126).31 SNVs/InDels were related to conventional VRFs,and 28 were discovered in genes related to other stroke *** study also showed that rare CNVs(n=7)and structural rearrangement(a balanced translocation)were potentially related to ICAD in 8.7%(8/92)of ***,candidate genes were significantly enriched in pathways related to lipoprotein metabolism and cellular lipid catabolic *** Our GS study suggests a role of rare genomic variants with various variant types contributing to the development of ICAD in Chinese patients.

目的探讨氧联乙酰葡萄糖胺(O-linked N-acetylglucosamine,O-Glc NAc)影响上唇发育时上皮层的消失过程中,Wnt信号通路在其中发挥的作用。方法通过在鸡胚唇部植入浸有O-GlcNAc促进剂氧联乙酰葡萄糖胺转移酶(O-GlcNAc transferase,OGT)、O-GlcNAc的抑制酶氧联乙酰葡萄糖胺水解酶(O-Glc NAcase,OGA)以及OGA的抑制剂链脲佐菌素(streptozoticin,SZT)的琼脂糖微球,构建O-Glc NAc高表达与低表达以及反向高表达实验组,并继续孵育24 h,茜素红—阿利新蓝骨组织染色观察胚胎骨、软骨发生的变化,TUNEL法检测细胞凋亡情况,免疫组化染色法检测Wnt信号系包括GSK-3β、β-catenin在内的一系列信号因子。结果 O-GlcNAc过表达组OGT和STZ中,上皮层的消失出现延迟,骨染色发现胚胎唇部出现骨缺损,TUNEL检测与正常组相比未发现明显细胞凋亡,但GSK-3β在细胞质内表达增加,β-catenin细胞核内表达减少;正常侧及O-GlcNAc低表达组OGA中上唇发育未受影响。结论 O-GlcNAc过表达时引起上皮层的消失发生延迟,这并非由细胞凋亡减少引起,而是可能通过抑制Wnt信号通路,导致GSK-3β、β-catenin在内的相关信号因子变化,这些变化将影响上唇发育并可能导致唇腭裂的发生。
