Spotlight on eltrombopag concentration in pediatric immune thrombocytopenia: A single-center observational study in China

Importance: Eltrombopag has been recommended for pediatric immune thrombocytopenia (ITP). Response and adverse drug reactions (ADRs) varied widely between individuals, even at the same dose of eltrombopag. The appropriate eltrombopag concentration in ITP has not been reported.Objective: This study aims to explore the appropriate eltrombopag concentration in pediatric ITP.Methods: This was a single-center, prospective cohort study. Children diagnosed with refractory persistent/chronic ITP and platelet count < 30×109/L were treated with eltrombopag and followed up for at least 2 months. Concentration was detected by high-performance liquid chromatography-mass spectrometry at least 2 weeks after eltrombopag. The clinical characteristics-concentration, concentration-response, and concentration-ADRs were analyzed.Results: A total of 30 patients were enrolled, comprising 13 males and 17 females, with a median age of 72 (45-94) months. The median dose and concentration were 1.39 (1.09-1.56) mg/kg and 2.70 (2.25-4.13) mg/L, respectively. Of the enrolled patients, 14 responded to treatment, whereas 16 did not. Additionally, five experienced adverse drug reactions. No linear correlation was observed between eltrombopag concentration and clinical characteristics. The concentration was lower in the response group than in the nonresponse group, but there was no significant difference (t = 0.755,P = 0.457). Patients who experienced ADRs had a higher concentration than those without ADRs (t = 2.538,P = 0.017). The area under the receiver operating characteristic curve of ADRs was 0.78 (95% confidence interval: 0.56-1.00). Youden’s index identified the cutoff point as 4.33 mg/L, with a sensitivity of 88% and a specificity of 60%. Logistic regression analysis demonstrated that a higher platelet count before eltrombopag predicted a favorable response.Interpretation: Eltrombopag proves efficacious and well-tolerated for treating pediatric ITP. However, prolonged and high-dose administration may increase the likelihood of ADRs. Thus

Effect of electroacupuncture at Neiguan(PC6)at different time points on myocardial ischemia reperfusion arrhythmia in rats

OBJECTIVE:To observe the effects of electroacupuncture at Neiguan(PC6)at different time points on reperfusion arrhythmia(RA)after myocardial ischemia and reperfusion in rats,and to investigate the correlation of this protective effect with nerve growth factor(NGF),tyrosine kinase A(Trk A),tyrosine hydroxylase(TH),and norepinephrine(NE).METHODS:A total of 72 Sprague-Dawley male rats were randomly divided into six groups(n=12 rats/group):normal group(Norm),sham operation group(Sham),ischemia reperfusion group(I/R),pre-ischemic electroacupuncture group(EAI),pre-reperfusion electroacupuncture group(EAII),post-reperfusion electroacupuncture group(EAIII).The myocardial ischemia-reperfusion injury(MIRI)model was induced by occlusion of left anterior descending coronary artery for 20 min followed by reperfusion for 40 min in rats.With no intervention in the Norm group and only threading without ligation in the Sham group.Electroacupuncture pretreatment at 20 min/d for 7 d before ligation in the EAⅠgroup,20 min of electroacupuncture before reperfusion in the EAII group and 20 min of electroacupuncture after reperfusion in the EAIII group.The electrocardiogram(ECG)of each group was recorded throughout the whole process,and the success of the MIRI model was determined based on the changs of J-point and T-wave in the ECG.The arrhythmia score was used to record premature ventricular contractions,ventricular tachycardia and ventricular fibrillation during the reperfusion period to assess the reperfusion induced arrhythmias.The expression levels of NGF,Trk A,TH protein were measured by Western blot.Moreover,the expression levels of plasma and myocardial NE levels were detected by enzyme linked immunosorbent assay.RESULTS:The differences between Norm group and Sham group were not statistically significant in all indexes.Arrhythmia score,myocardial NGF,Trk A,TH,and NE expression were significantly higher in the I/R group compared with the Sham group.Arrhythmia score,myocardial NGF,Trk A,TH,and NE expression were significantly low

Osteoarthritis(OA)is a common degenerative disease worldwide and new therapeutics that target inflammation and the crosstalk between immunocytes and chondrocytes are being developed to prevent and treat OA.These attempts involve repolarizing pro-inflammatory M1 macrophages into the anti-inflammatory M2 phenotype in synovium.In this study,we found that phosphoglycerate mutase 5(PGAM5)significantly increased in macrophages in OA synovium compared to controls based on histology of human samples and single-cell RNA sequencing results of mice models.To address the role of PGAM5 in macrophages in OA,we found conditional knockout of PGAM5 in macrophages greatly alleviated OA symptoms and promoted anabolic metabolism of chondrocytes in vitro and in vivo.Mechanistically,we found that PGAM5 enhanced M1 polarization via AKT-mTOR/p38/ERK pathways,whereas inhibited M2 polarization via STAT6-PPARγpathway in murine bone marrow-derived macrophages.Furthermore,we found that PGAM5 directly dephosphorylated Dishevelled Segment Polarity Protein 2(DVL2)which resulted in the inhibition ofβ-catenin and repolarization of M2 macrophages into M1 macrophages.Conditional knockout of both PGAM5 andβ-catenin in macrophages significantly exacerbated osteoarthritis compared to PGAM5-deficient mice.Motivated by these findings,we successfully designed mannose modified fluoropolymers combined with siPGAM5 to inhibit PGAM5 specifically in synovial macrophages via intra-articular injection,which possessed desired targeting abilities of synovial macrophages and greatly attenuated murine osteoarthritis.Collectively,these findings defined a key role for PGAM5 in orchestrating macrophage polarization and provides insights into novel macrophage-targeted strategy for treating OA.

Diabetic retinopathy(DR),the main cause of irreversible blindness,is one of the most common complications of diabetes.At present,deep convolutional neural networks have achieved promising performance in automatic DR detection tasks.The convolution operation of methods is a local cross-correlation operation,whose receptive field de-termines the size of the local neighbourhood for processing.However,for retinal fundus photographs,there is not only the local information but also long-distance dependence between the lesion features(e.***.hemorrhages and exudates)scattered throughout the whole image.The proposed method incorporates correlations between long-range patches into the deep learning framework to improve DR detection.Patch-wise re-lationships are used to enhance the local patch features since lesions of DR usually appear as plaques.The Long-Range unit in the proposed network with a residual structure can be flexibly embedded into other trained networks.Extensive experimental results demon-strate that the proposed approach can achieve higher accuracy than existing state-of-the-art models on Messidor and EyePACS datasets.

Endowing flexible and adaptable fiber devices with light-emitting capabilities has the potential to revolutionize the current design philosophy of intelligent,wearable interactive devices.However,significant challenges remain in developing fiber devices when it comes to achieving uniform and customizable light effects while utilizing lightweight hardware.Here,we introduce a mass-produced,wearable,and interactive photochromic fiber that provides uniform multicolored light control.We designed independent waveguides inside the fiber to maintain total internal reflection of light as it traverses the fiber.The impact of excessive light leakage on the overall illuminance can be reduced by utilizing the saturable absorption effect of fluorescent materials to ensure light emission uniformity along the transmission direction.In addition,we coupled various fluorescent composite materials inside the fiber to achieve artificially controllable spectral radiation of multiple color systems in a single fiber.We prepared fibers on mass-produced kilometer-long using the thermal drawing method.The fibers can be directly integrated into daily wearable devices or clothing in various patterns and combined with other signal input components to control and display patterns as needed.This work provides a new perspective and inspiration to the existing field of fiber display interaction,paving the way for future human–machine integration.

The temporal and spatial patterns and potential evaluation of China＇s energy resources development

The issue of China＇s energy supply security is not only the key problem which af- fects China＇s rapid and sustainable development in the 21st century, but also the one which international attention focuses on. Based on the notable characteristic of spatial imbalance between energy production and consumption in China, this paper takes the evolution of China＇s primary energy resources development（excluding hydropower） from 1949 to 2007 as the study object, with the aim to sum up the evolutive characteristics and laws of China＇s energy resources development in the past nearly 60 years. Then, based on comprehensive considerations of coal＇s, oil＇s and natural gas＇s basic reserves, qualities, geological conditions production status, and ecological service function of every province, this paper adopts development potential index （DP）to evaluate the development potential of every province＇s en- ergy resources, and divide them into different ranks. Conclusions are drawn as follows：（1） Generally speaking, China＇s gross energy production was increasing in waves from 1949 to 2007. From the viewpoint of spatial patterns, China＇s energy resources development has shown a characteristic of ＂concentrating to the north and central areas, and evolving from linear-shaped to ＂T-shaped＂ pattern gradually since 1949. （2） The structure evolution of China＇s energy resources development in general has shown a trend of ＂coal proportion is dominant but decreasing, while oil and gas proportions are increasing＂ since 1949. （3） At the provincial scale, China＇s energy resources development potential could be divided into large, sub-large, general and small ranks, four in all. In the future, the spatial pattern of China＇s energy production will evolve from ＂T-shaped＂ to ＂R-shaped pattern＂. These conclusions will help to clarify the temporal and spatial characteristics and laws of China＇s energy resources development, and will be beneficial for China to design scientific and rational energy development strategies and plans, coordinate spatial im

Urban Fresh Water Resources Consumption of China

From the point of view of urban consumption behavior, urban fresh water consumption could be classified as three types, namely, direct, indirect and induced water consumption. A calculation approach of urban fresh water consumption was presented based on the theory of urban basic material consumption and the input-output method, which was utilized to calculate urban fresh water consumption of China, and to analyze its structural change and causes. The results show that the total urban fresh water consumption increased 561.7×109m3, and the proportion to the total national fresh water resources increased by 20 percentage points from 1952 to 2005. The proportion of direct and induced water consumption had been continuously rising, and it increased by 15 and 35 percentage points separately from 1952 to 2005, while the proportion of indirect water consumption decreased by 50 percentage points. Urban indirect water consumption was mainly related to urban grain, beef and mutton consumption, and urban induced water consumption had a close relationship with the amount of carbon emission per capita. Finally, some countermeasures were put forward to realize sustainable utilization of urban fresh water resources in China.

Taking social statistic data as basic data,this paper extended the meaning of urban land uses,highlighted the meaning of urban land uses in modern urbanization,which includes direct,indirect and induced land uses,quantitatively simulated the indirect and induced land uses by the substitution method of agricultural consumption and urban carbon emission and then,analyzed the spatiotemporal evolution of urban land uses in China during 1952–2005 by spatial analysis tool of Geographic Information System. 为了解长江口江心青草沙水库的鱼类群落结构,于2021年对该水库进行了鱼类资源调查和群落结构分析。共采集鱼类3209尾,分属于9目13科36属46种,以鲤科淡水鱼类为主;鲢(Hypophthalmichthys molitrix)、鳙(Aristichthys nobilis)、花(Hemibarbus maculatus)、鲫(Carassius auratus)、刀鲚(Coilia nasus)、翘嘴鲌(Culter alburnus)、长蛇(Saurogobio dumerili)、花鲈(Lateolabrax japonicus)的IRI均超过1000,为优势种;优势种生态位分析显示肉食性和杂食性鱼类在食物资源和生存环境存在较高的生态位重叠,这两种鱼类种群规模扩大受到食物、空间等资源的限制会比生态位重叠低的更大;鲢、鳙与其他鱼类生态位重叠较低,在水库内受到的食物、空间等资源的竞争较小。年度的Margalef种类丰富度指数(D)为5.70,Shannon-Wiener多样性指数(H')为3.01,Pielou均匀度指数(J)为0.78,Simpson优势度指数(C)为0.81,与2015—2019年水库的调查相比,青草沙水库鱼类物种多样性提升明显,ABC曲线表明鱼类群落结构受外界因素干扰小,总体而言,青草沙水库鱼类更加丰富,群落处于稳中向好的状态。
