AIM: To study the trends of major causes of visual impairment(VI) in adults in Sichuan,China and evaluate the effect of aging on the trends. ·METHODS: We used data from the National Sample Survey on Disabilities(NSSD) in Sichuan province conducted in 1987 and 2006. The age-adjusted prevalence of major causes of VI and the prevalence stratified by age in each cause were calculated and compared. The association between age and each cause of VI was also analyzed.·RESULTS: Retinal disease increased and became the second leading cause of VI in 2006 while blinding trachoma decreased markedly. Cataract and non-trachomatous corneal diseases were among the leading causes of VI in both years. We found associations between age and causes of VI,with age showing the strongest association with cataract and relatively lower associations with other causes. · CONCluSION: In the last two decades,dramatic changes occurred in the major causes of VI with significantly increased retinal disease and decreased blinding trachoma. Aging of the population might be an important factor accounting for the changed trends of VI. Understanding the prevalence of VI,its major causes and trends over time can assist in prioritizing and developing effective interventional strategies and monitoring their impact.

Background The severity of respiratory distress was associated with neonatal prognosis. This study aimed to explore the clinical characteristics, therapeutic interventions and short-term outcomes of late preterm or term infants who required respiratory support, and compare the usage of different illness severity assessment *** Seven neonatal intensive care units in tertiary hospitals were recruited. From November 2008 to October 2009, neonates born at ≥34 weeks＇ gestational age, admitted at 〈72 hours of age, requiring continuous positive airway pressure （CPAP） or mechanical ventilation for respiratory support were enrolled. Clinical data including demographic variables, underlying disease, complications, therapeutic interventions and short-term outcomes were collected. All infants were divided into three groups by Acute care of at-risk newborns （ACoRN） Respiratory Score 〈5, 5-8, and 〉*** During the study period, 503 newborn late preterm or term infants required respiratory support. The mean gestational age was （36.8±2.2） weeks, mean birth weight was （2734.5±603.5） g. The majority of the neonates were male （69.4%）, late preterm （63.3%）, delivered by cesarean section （74.8%）, admitted in the first day of life （89.3%） and outborn （born at other hospitals, 76.9%）. Of the cesarean section, 51.1% were performed electively. Infants in the severe group were more mature, had the highest rate of elective cesarean section, Apgar score 〈7 at 5 minutes and resuscitated with intubation, the in-hospital mortality increased significantly. In total, 58.1% of the patients were supported with mechanical ventilation and 17.3% received high frequency oscillation. Adjunctive therapies were commonly *** rate of infants in severe group needed mechanical ventilation or high frequency oscillation, volume expansion,bicarbonate infusion or vasopressors therapy （P 〈0.05）. Prostate cancer antigen 3 （PCA3） is a biomarker for diagnosing prostate cancer （PCa） identified in the Caucasian population. We evaluated the effectiveness of urinary PCA3 in predicting the biopsy result in 500 men undergoing initial prostate biopsy. The predictive power of the PCA3 score was evaluated by the area under receiver operating characteristic （ROC） curve （AUC） and by decision curve analysis. PCA3 score sufficed to discriminate positive from negative prostate biopsy results but was not correlated with the aggressiveness of PCa. The ROC analysis showed a higher AUC for the PCA3 score than %fPSA （0.750 vs 0.622, P = 0.046） in patients with a PSA of 4.0-10.0 ng ml-I, but the PCA3-based model is not significantly better than the base model. Decision curve analysis indicates the PCA3-based model was superior to the base model with a higher net benefit for almost all threshold probabilities, especially the threshold probabilities of 25%-40% in patients with a PSA of 4.0-10.0 ng ml-1. However, the AUC of the PCA3 score （0.712） is not superior to %fPSA （0.698） or PSAD （0.773） in patients with a PSA 〉10.0 ng ml-1. Our results confirmed that the RT-PCR-based PCA3 test moderately improved diagnostic accuracy in Chinese patients undergoing first prostate biopsy with a PSA of 4.0-10.0 ng ml-1.

暗色唇鲮肌肉营养成分分析与品质评价

对广西3个不同地区(LS、XB、WM)的暗色唇鲮的肌肉营养成分和肌肉品质进行比较分析,结果表明:3组鱼肉的粗蛋白、粗脂肪含量分别为17.52%~19.76%和0.86%~1.80%,其中LS组的粗蛋白、粗脂肪含量均显著高于XB组的,总糖含量显著低于XB、WM组的;检测出的8种矿物质元素中,常量元素中P的含量最高,Ca、Mg含量次之,5种微量元素含量依次为Fe>Zn>Cu>Se>Mn,且XB组的Cu含量显著高于LS、WM组的;检测出的17种氨基酸中,Glu含量最高,其次为Asp和Lys,除Cys外,LS、WM组的其他氨基酸含量、EAA/TAA和EAA/NEAA均显著高于XB组的,而DAA/TAA显著低于XB组的;3组鱼肉的EAA/TAA和EAA/NEAA分别在37.61%~38.39%和70.83%~72.90%,接近FAO/WHO要求的理想蛋白模式;3组鱼肉检测出的脂肪酸种类均为25种及以上,不饱和脂肪酸含量在48.77%~50.92%,LS组的ALA、EPA、PUFA含量显著高于XB和WM组的。综上表明,暗色唇鲮肌肉营养丰富,品质优良,具有良好的食用价值和保健作用,可作为一种优质经济鱼类进行养殖和推广。

Intramedullary nailing of clavicular midshaft fractures in adults using titanium elastic nail

Objective： Studies showed elastic stable intramedullary nailing （ESIN） of displaced midclavicular fractures has excellent outcomes, as well as high complication rates and specific problems. The aim was to discuss ESIN of midshaft clavicular ***： Totally 60 eligible patients （aged 18-63 years） were randomized to either ESIN group or non-operative group between January 2007 and May 2008. Clavicular shortening was measured after trauma and osseous *** union and complications were assessed. Function analysis including Constant shoulder scores and disabilities of the arm, shoulder and hand （DASH） scores were performed after a 15-month ***： ESIN led to a signifcantly shorter time to union, especially for simple fractures. In ESIN group, all patients got fracture union, of which 5 cases had medial skin irritation and 1 patient needed revision surgery because of implant failure. In the nonoperative group, there were 3 nonunion cases and 2 symptomatic malunions developed requiring corrective osteotomy. At 15 months after intramedullary stabilization, patients in the ESIN group were more satisfied with the appearance of the shoulder and overall outcome, and they benefited a lot from the great improvement of post-traumatic clavicular shortening. Furthermore,DASH scores were lower and Constant scores were significantly higher in contrast to the non-operative ***： ESIN is a safe minimally invasive surgical technique with lower complication rate, faster return to daily activities, excellent cosmetic and better functional results,restoration of clavicular length for treating mid-shaft clavicular fractures, resulting in high overall satisfaction, which can be regard as an alternative to plate fixation or nonoperative treatment of mid-shaft clavicular fractures.

目的监测凉山州高血压患者血压控制达标情况及影响因素,为改进高血压患者管理策略提供依据。方法通过分层整群随机抽样的方法,于2023年抽取凉山州盐源县、德昌县对基本公共卫生服务项目纳入管理的原发性高血压患者进行血压控制达标情况和影响因素调查。数据均使用例数和百分比进行描述,组间差异的比较使用χ^(2)检验,多组比较且总体差异有统计学意义时,使用Bonferroni法进行两两比较,α’=α/[k*(k-1)/2];分组变量为有序分类变量时,组间差异的比较采用趋势χ^(2)检验。高血压患者血压达标情况的危险因素分析使用Logistic回归。统计分析使用SPSS 18.0,以P<0.05为差异有统计学意义。结果共调查盐源县、德昌县原发性高血压患者517人,血压总体控制达标率为44.29%,收缩压控制达标率为53.19%,舒张压控制达标率为61.51%。地区、年龄、民族、吸烟情况、是否定期测量血压、单次购药量、剩余降压药量、服药依从性达标情况、是否服用钙通道阻滞剂、是否服用血管紧张素受体2拮抗剂、是否服用2种及以上降压药对血压控制达标有影响,组间差异均有统计学意义(P均<0.05)。多因素Logistic回归分析分析结果显示,德昌县、年龄为65岁及以上的凉山州在管高血压患者血压控制达标率高,OR及其95%CI分别为:3.33(2.04~5.26)和2.27(1.47~3.45)。结论凉山州德昌县、年龄为65岁及以上的高血压患者血压控制达标率高,应对于不同的高血压群体,针对性的开展对应内容的宣传和入户讲解,加强不良生活方式管理、不良服药行为和遵医嘱服药行为干预、血压监测行为促成、烟酒嗜好干预等服药依从性教育。
