An unprecedented outbreak investigation for nosocomial and community-acquired legionellosis in Hong Kong

Background The environmental sources associated with community-acquired or nosocomial legionellosis were not always detectable in the mainland of China and Hong Kong, China. The objective of this study was to illustrate the control measures implemented for nosocomial and community outbreaks of legionellosis, and to understand the environmental distribution of legionella in the water system in Hong Kong, China. Methods We investigated the environmental sources of two cases of legionellosis acquired in the hospital and the community by extensive outbreak investigation and sampling of the potable water system using culture and genetic testing at the respective premises. Results The diagnosis of nosocomial legionellosis was suspected in a patient presenting with nosocomial pneumonia not responsive to multiple beta-lactam antibiotics with subsequent confirmation by Legionella pneumophila serogroup 1 antigenuria. High counts of Legionella pneumophila were detected in the potable water supply of the 70-year-old hospital building. Another patient on continuous ambulatory peritoneal dialysis presenting with acute community-acquired pneumonia and severe diarrhoea was positive for Legionella pneumophila serogroup 1 by polymerase chain reaction （PCR） testing on both sputum and nasopharyngeal aspirate despite negative antigenuria. Paradoxically the source of the second case was traced to the water system of a newly commissioned office building complex. No further cases were detected after shock hyperchlorination with or without superheating of the water systems. Subsequent legionella counts were drastically reduced. Point-of-care infection control by off-boiled or sterile water for mouth care and installation of water filter for showers in the hospital wards for immunocompromised patients was instituted. Territory wide investigation of the community potable water supply showed that 22.1% of the household water supply was positive at a mean legionella count of 108.56 CFU/ml （range 0.10 to 639.30 CFU/ml）. An Overview of Chemical Constituents, Medicinal Properties, Clinical Trials, and Patents of Twigs of Morus alba(Ramulus Mori)

The rich chemical constituents and diverse pharmacological properties of Ramulus Mori(RM) or the twig of Morus alba with evidence supported by clinical trials and patents are reviewed. Known as Sangzhi in Chinese, RM is widely used in traditional Chinese medicine to treat gout, arthritis, and rheumatism. Chemical constituents include flavonoids, benzofurans, alkaloids, stilbenes, chalcones, phenolic acids, and coumarins. Bioactivities of RM include antidiabetic, anti-obesity, anti-inflammatory, antityrosinase, neuroprotective, antioxidant, hepatoprotective, cytoprotective, renoprotective, antihyperuricemic, analgesic, antifungal, DNA protective, anticancer, gastroprotective, cardioprotective, anti-hair aging, anti-arthritic, and antiplatelet aggregation properties. The most active compounds from RM are mulberrin, oxyresveratrol, and mulberroside A. All four clinical trials on RM are focused on the treatment of diabetes. The patents entail using RM extracts as cosmetics for skin whitening and as agents for hypoglycemia. Topics for further research on RM are suggested.

To the Editor:Extraovarian primary peritoneal carcinoma(EOPPC)is an uncommon malignancy with many similarities to epithelial ovarian carcinoma in histological,clinical,and etiological aspects[1].This phenomenon is explained by their common embryonal origin,in which both develop from the coelomic epithelium in the early embryological stage.Despite their similarities,the incidence of EOPPC is significantly lower than that of epithelial ovarian carcinoma(6.78 cases per million vs.120.5 cases per million)[1].

Endometriosis is a common chronic gynecological disease with endometrial cell implantation outside the uterus.Angiogenesis is a major pathophysiology in endometriosis.Our previous studies have demonstrated that the prodrug of epigallocatechin gallate(ProEGCG)exhibits superior anti-endometriotic and anti-angiogenic effects compared to epigallocatechin gallate(EGCG).However,their direct binding targets and underlying mechanisms for the differential effects remain unknown.In this study,we demonstrated that oral ProEGCG can be effective in preventing and treating endometriosis.Additionally,1D and 2D Proteome Integral Solubility Alteration assay-based chemical proteomics identified metadherin(MTDH)and PX domain containing serine/threonine kinase-like(PXK)as novel binding targets of EGCG and ProEGCG,respectively.Computational simulation and BioLayer interferometry were used to confirm their binding affinity.Our results showed that MTDH-EGCG inhibited protein kinase B(Akt)-mediated angiogenesis,while PXK-ProEGCG inhibited epidermal growth factor(EGF)-mediated angiogenesis via the EGF/hypoxia-inducible factor(HIF-1a)/vascular endothelial growth factor(VEGF)pathway.In vitro and in vivo knockdown assays and microvascular network imaging further confirmed the involvement of these signaling pathways.Moreover,our study demonstrated that ProEGCG has superior therapeutic effects than EGCG by targeting distinct signal transduction pathways and may act as a novel antiangiogenic therapy for endometriosis.

Background:Nonalcoholic fatty liver disease(NAFLD)is associated with impaired renal function,and both diseases often occur alongside other metabolic disorders.However,the prevalence and risk factors for impaired renal function in patients with NAFLD remain unclear.The objective of this study was to identify the prevalence and risk factors for renal impairment in NAFLD patients.Methods:All adults aged 18-70 years with ultrasound-diagnosed NAFLD and transient elastography examination from eight Asian centers were enrolled in this prospective study.Liver fibrosis and cirrhosis were assessed by FibroScan-aspartate aminotransferase(FAST),Agile 3+and Agile 4 scores.Impaired renal function and chronic kidney disease(CKD)were defined by an estimated glomerular filtration rate(eGFR)with value of<90 mL/min/1.73 m^(2) and<60 mL/min/1.73 m^(2),respectively,as estimated by the CKD-Epidemiology Collaboration(CKD-EPI)equation.Results:Among 529 included NAFLD patients,the prevalence rates of impaired renal function and CKD were 37.4%and 4.9%,respectively.In multivariate analysis,a moderate-high risk of advanced liver fibrosis and cirrhosis according to Agile 3+and Agile 4 scores were independent risk factors for CKD(P<0.05).Furthermore,increased fasting plasma glucose(FPG)and blood pressure were significantly associated with impaired renal function after controlling for the other components of metabolic syndrome(P<0.05).Compared with patients with normoglycemia,those with prediabetes[FPG≥5.6 mmol/L or hemoglobin A1c(HbA1c)≥5.7%]were more likely to have impaired renal function(P<0.05).Conclusions:Agile 3+and Agile 4 are reliable for identifying NAFLD patients with high risk of CKD.Early glycemic control in the prediabetic stage might have a potential renoprotective role in these patients.

Genipin and geniposide from Gardenia jasminoides:An overview of their anti-cancer and other pharmacological properties

Genipin and geniposide are iridoids from the fruits of Gardenia jasminoides.Both compounds have a methyl acetate(-COOCH_(3))group at C4 and a hydroxyl(-OH)group at C10.As an iridoid glucoside with a glucose moiety at C1,geniposide is also known as genipin-1-O-β-D-glucoside.Without the glucose moiety,genipin is an iridoid and the aglycone of geniposide.The-OH group at C1 of genipin is responsible for its cytotoxic effects.Geniposide without the-OH group at C1 lacks the cytotoxic effects.There are more publications on the anti-cancer properties of genipin than geniposide.Studies have reported the potentiation of genipin when used in combination with anti-cancer drugs.The anti-cancer properties of geniposide have been investigated using human intestinal microflora that hydrolyzes geniposide to genipin.Both genipin and geniposide exert antiproliferative and apoptotic activities via different molecular targets and pathways.Other pharmacological properties of genipin and geniposide include antidepressant,antidiabetic,anti-inflammatory,anti-obesity,anti-thrombotic,hepatoprotective,and neuroprotective activities.Future research on genipin and geniposide is suggested.

Matrine, oxymatrine, and compound Kushen injection from the roots of Sophora flavescens: an overview of their anticancer activities

In the present review,we updated current information on the chemistry,contents,and anticancer properties of matrine(MT),oxymatrine(OMT),and compound Kushen injection(CKI).The anticancer properties were focused on lung,breast,and liver cancer cells because they are most susceptible.Sources of information were from Google,Google Scholar,PubMed,PubMed Central,Science Direct,PubChem,J-Stage,Directory of Open Access Journals(DOAJ),and China National Knowledge Infrastructure(CNKI).Reference was also made on botanical websites,such as Flora of China and World Flora Online.MT and OMT are dominant quinolizidine alkaloids from the roots of Sophora flavescens(Kushen)of the family Fabaceae.Against lung,breast,and liver cancer cells,MT and OMT inhibit cell proliferation;induce cell cycle arrest,apoptosis,and autophagy;restrict angiogenesis;and inhibit cell metastasis,invasion,and migration.The processes involve various molecular targets and signaling pathways.CKI is a traditional Chinese medicine(TCM)composed of root extracts of S.flavescens and Smilax glabra(Baituling)of the family Smilacaceae.With MT and OMT as major components,CKI has been approved for the treatment of cancer in China more than 20 years ago.In recent years,systematic reviews and meta-analysis have been undertaken to evaluate the anticancer effects of CKI.When CKI is used alone and in combination with chemotherapy of western medicine,there is much to be learned concerning their interactions besides their individual and integrated efficacy.Some perspectives of MT,OMT,and CKI are discussed,and their suggestions for future research are provided.

An overview of flavonoids from Sophora flavescens(kushen)with some emphasis on the anticancer properties of kurarinone and sophoraflavanone G

In this overview,the current knowledge of the constituents of flavonoids isolated from the roots of Sophora flavescens(kushen)is updated.Flavonoids consist of several classes,such as flavanones,flavonols,chalcones,isoflavones,biflavonoids,flavanols,and flavones.The most common compounds are kurarinone(KRN),sophoraflavanone G(SFG),2′-methoxykurarinone,kuraridine,isoxanthohumol,and formononetin.KRN and SFG are two major flavanones with more vital anticancer properties than other flavonoids.From the literature,the cytotoxic values of KRN and SFG are variable and depend on the type of cancer cells tested.The anticancer activities of these two flavonoids involve different molecular mechanisms.Clinical trials are needed before anticancer drugs from KRN and SFG can be developed.

MXene-based hydrogels have drawn considerable attention as flexible and wearable sensors.However,the application of MXene-based hydrogels after sensing failure has rarely been investigated,which is of great significance for expanding their engineering application.In this work,multifunctional mineral MXene hydrogels(MMHs)were synthesized via a simple method inspired by biomineralization.The prepared MMHs were stretchable,self-healable and conductive,and can be used to fabricate wearable tensile strain sensors showing a super-wide sensing range with excellent sensitivity.MMHs-based strain sensors were designed to be directly attached to the skin surface to detect tiny and large human motions.In addition,with the advantages of a large specific area,excellent hydrophilicity and abundant active adsorption sites for MXene nanosheets and hydrogels,dehydrated MMHs were used as highly efficient adsorbents for the removal of strontium ions from aqueous solutions.This work shows the great potential of MXene in promoting the development of nextgeneration functional materials.
