Aspiration therapy for acute embolic occlusion of the superior mesenteric artery

BACKGROUND Embolic superior mesenteric artery(SMA) occlusion is associated with high mortality rates. Delayed treatment often leads to serious consequences, including intestinal necrosis, resection, and even patient death. Endovascular repair is being introduced, which can improve clinical symptoms and prognosis and decrease the incidence of exploratory laparotomy. Many reports have described successful endovascular revascularization of embolic SMA occlusion. However,most of those reports are case reports, and there are few reports on Chinese patients. In this paper, we describe the technical and clinical outcomes of aspiration therapy using a guiding catheter and long sheath technique which facilitates the endovascular repair *** To evaluate the complications, feasibility, effectiveness, and safety of endovascular treatment for the acute embolic occlusion of the *** This retrospective study reviewed eight patients(six males and two females)from August 2013 to October 2018 at Xuanwu Hospital, Capital Medical University. The patients presented with acute embolic occlusion of the SMA on admission and were initially diagnosed by computed tomography angiography(CTA). The patients who underwent endovascular treatment with a guiding catheter had no obvious evidence of bowel infarct. No intestinal necrosis was identified by gastrointestinal surgeons through peritoneal puncture or CTA. ●AIM:To introduce the macular hole(MH)hydromassage technique as a potentially beneficial approach for the treatment of large or persistent MH.●METHODS:This retrospective observational case series comprised 16 consecutive patients(17 eyes)diagnosed with *** criteria involved a hole aperture diameter larger than 600μm or the presence of an unclosed MH larger than 600μm following the previous *** MH repair procedures were administered in all cases,involving the manipulation and aspiration of the hole margin through the application of water flow with a soft-tip flute needle.A comprehensive assessment was conducted for each case before and after surgery,and optical coherence tomography(OCT)images were captured at every follow-up point.●RESULTS:The mean preoperative aperture diameter was 747±156μm(range 611-1180μm),with a mean base diameter of 1390±435μm(range 578-2220μm).Following surgery,all cases achieved complete anatomical closure of MH,with 13 cases(76.5%)exhibiting type 1 closure and 4 cases(23.5%)demonstrating type 2 *** significant differences were observed in the preoperative OCT variables between the two closure *** with type 1 closure showed a significantly improved visual acuity(0.70±0.10,range 0.50-0.80)compared to those with type 2 closure(0.90±0.12,range 0.80-1.00,P=0.014).●CONCLUSION:The MH hydromassage technique demonstrates promising results,achieving acceptable closure rates in cases of large or persistent *** technique may serve as an effective adjunctive maneuver during challenging MH surgery.

Deformation features and failure mechanism of steep rock slope under the mining activities and rainfall

Underground mining activities and rainfall have potential important influence on the initiation and reactivation of the slope deformations,especially on the steep rock slope. In this paper,using the discrete element method(UDEC),numerical simulation was carried out to investigate deformation features and the failure mechanism of the steep rock slope under mining activities and rainfall. A steep rock slope numerical model was created based on a case study at the Wulong area in Chongqing city,China. Mechanical parameters of the rock mass have been determined by situ measurements and laboratory measurements. A preliminary site monitoring system has been realized,aiming at getting structure movements and stresses of unstablerock masses at the most significant discontinuities. According to the numerical model calibrated based on the monitoring data,four types of operation conditions are designed to reveal the effect of mining excavation and extreme rainfall on the deformation of the steep rock slope.

Sarl is a small GTPase involved in COPⅡ vesicle transport. Previous studies showed that H79G mutation of Sarl can lock Sarl in its GTP-bound active conformation, stabilize coat assembly, and prevent the disassembly of COP Ⅱ vesicle coats by reducing Sec23/24 GAP- stimulated hydrolysis. We show here that the replacement of His79 by glycine induces a large conformation change in switch Ⅱ and results in the lost of hydrogen bond between His79 and its associated nucleophilic water molecule that was hypothesized to reduce the GAP-stimulated hydrolysis during the COP Ⅱ assembly and disassembly. These results confirm that the switch Ⅱ conformation is important for COP Ⅱ disassembly through coat-controlled GTP hydrolysis.
