目的应用TaqMan技术设计PCR联合探针一次性可检测多种软组织肉瘤主要驱动基因的表达,结合病理诊断和FISH结果探讨该联合探针能否较好辅助临床病理诊断需求。方法基于TaqMan技术设计软组织肉瘤相关32对融合基因联合探针,涉及十多种相关肉瘤类型;对福建医科大学附属漳州市医院70例相关融合基因软组织肉瘤患者的病理组织学样本进行一次性融合基因联合检测,并设置30例非相关融合基因其他软组织肉瘤患者的病理组织学样本作为对照;常见肉瘤类型结合FISH探针检测结果进行分析;同时对该联合探针检测性能进行多种方法学评价。结果课题组设计的软组织肉瘤相关融合基因探针检测已确诊的软组织肉瘤,结果显示最高敏感性高达100%;选取隆突性皮肤纤维肉瘤、滑膜肉瘤、黏液性脂肪肉瘤3种类型肿瘤结合FISH技术验证,两种方法检测结果一致性高,差异无统计学意义(P>0.05);选取隆突性皮肤纤维肉瘤、滑膜肉瘤、黏液性脂肪肉瘤进行重复性实验,结果完全一致;对所有融合基因RNA阳性质粒进行3种不同浓度检测限检测,确定25拷贝/μL为最低浓度检测下限。结论结合已知病理诊断结果,应用TaqMan技术设计针对软组织肉瘤的PCR联合探针,具备高敏感性和高特异性,方法学性能良好,该探针设计相关融合基因的覆盖位点能满足基层医疗机构一次性快速辅助常见软组织肉瘤病理诊断的需求,为临床软组织肉瘤多种融合基因检测提供了一种快速可靠的检测方法。

目的探讨C57BL/6J亚系小鼠经腹腔注射匹罗卡品建立颞叶癫痫模型的方法,总结可用于预测造模成功的癫痫发作急性期行为学表现,为癫痫研究提供可行的造模方案。方法选择30只C57BL/6J亚系小鼠(主要研究对象)和40只C57BL/6N亚系小鼠(对照),通过单次腹腔注射匹罗卡品的方法诱导小鼠癫痫发作从而建立颞叶癫痫模型。两种亚系的小鼠各分为3组,分别经腹腔注射300 mg/kg、330 mg/kg或360 mg/kg的匹罗卡品,观察并比较两种亚系小鼠的运动性癫痫发作的行为学表现,并在造模后第7天开始连续监测小鼠的自发性癫痫发作(spontaneous recurrent seizures,SRS)行为。造模后28 d,处死小鼠并观察其海马的病理改变情况。结果注射匹罗卡品后,C57BL/6N亚系小鼠表现出典型的运动性癫痫发作,而后进入癫痫持续状态(status epilepticus,SE);C57BL/6J小鼠较少观察到典型的运动性癫痫发作及后续的SE,而更多表现为单侧肢体抽搐后持续数秒至数十秒的全身颤抖,本研究将此行为学表现定义为严重痫样发作(severe seizure,SS)。腹腔注射330 mg/kg和360 mg/kg匹罗卡品后,在癫痫急性期出现过SS的C57BL/6J小鼠,经过潜伏期后可出现SRS,C57BL/6J亚系小鼠造模后SRS的比例(70%)和经历过SE后出现SRS的C57BL/6N亚系小鼠(75%)相近。造模后28 d,C57BL/6J小鼠海马出现颞叶癫痫的特征性病理改变,包括苔藓纤维出芽和神经元丢失。结论C57BL/6J亚系小鼠经腹腔注射匹罗卡品诱导癫痫模型时,造模成功的行为学标准可以是SE的发生,也可以是SS的出现。

某院全肠外营养不合理处方的帕累托图分析

目的应用帕累托图分析年龄1月至12月住院患者病历,每月随机抽取30份的TPN处方,应用帕累托图针对不合理处方的主、次要和一般因素进行分析。结果年龄<60岁的患者和老年患者TPN不合理处方类型几乎一致。但造成年龄<60岁的患者TPN不合理处方的主要因素为超适应症、热氮比不合理、糖脂比不合理,次要因素为谷氨酰胺用量不适宜;而造成老年患者TPN不合理处方的主要因素为热氮比不合理、谷氨酰胺用量不适宜、糖脂比不合理,次要因素为超适应症;一般因素均为脂溶性维生素用量不适宜和添加物不稳定。结论应用帕累托图分析年龄<60岁的患者和老年患者TPN不合理处方情况,可及时发现主次要因素,有利于临床药师针对性地进行相应的干预及营养基础知识的培训,以保障临床营养治疗的安全性和有效性。

Background and Aims:We compared lung function parameters in nonalcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease(MAFLD),and examined the association between lung function parameters and fibrosis severity in ***:In this cross-sectional study,we randomly recruited 2,543 middle-aged individuals from 25 communities across four cities in china during 2.16 and *** participants received a health check-up including measurement of anthropometric parameters,biochemical variables,liver ultrasonography,and *** severity of liver disease was assessed by the fibrosis(FIB)-4 ***:The prevalence of MAFLD was 20.4%(n=519)and that of NAFLD was 18.4%(n=469).After adjusting for age,sex,adiposity measures,smoking status,and significant alco-hol intake,subjects with MAFLD had a significantly lower predicted forced vital capacity(FVC,88.27±17.60%vs.90.82.16.85%,p1 s forced expiratory volume(FEV1,79.89±17.34 vs.83.02.16.66%,p1-point increase in FIB-4,FVC was diminished by 0.507(95%CI:-0.840,-0.173,p=0.003),and FEV1 was diminished by 0.439(95%CI:-0.739,-0.140,p=0.004).The results remained unchanged when the statistical analyses was performed separately for men and ***:MAFLD was significantly asso-ciated with a greater impairment of lung function param-eters than NAFLD.

Objective:To determine the effects of Chinese medicine(CM)involving triple rehabilitation therapy on the progression of knee osteoarthritis(KOA).Methods:A total of 722 patients recruited from 38 community health service centers located in china from March 2.13 to March 2.17 were randomly divided into treatment and control groups equally,using a cluster randomization *** education combined with CM involving triple rehabilitation therapy for KOA(electro-acupuncture,Chinese medicinal herb fumigating-washing,and traditional exercises)was administered in the treatment group while conventional rehabilitation therapy(physical factor therapy,joint movement training,and muscle strength training)was administered in the control *** with a visual analog scale(VAS)scores>A were treated with dispersible meloxicam tablets(7.5 mg,once daily).The Lequesne index scores,VAS scores,range of motion(ROM),lower limb muscle strength,knee joint circumference,quantitative scores of KOA symptoms,and the short-form 36 item health survey questionnaire(SF-36)scores were measured for each patient at 5 checkpoints(before treatment,at the 2nd week and the 4th week during the 4-week treatment period,at 1 month and 3 months after end of treatment),and adverse reactions were observed ***:A total of 696 patients completed the entire process,with 351 in the treatment group and 345 in the control *** all treatment checkpoints,the treatment group demonstrated better outcomes than the control group with regard to the total Lequesne index scores,effective rate and improvement rate of the total Lequesne index scores,VAS scores,lower limb muscle strength,knee circumference,quantitative scores of KOA symptoms,and SF-36 scores as well(Plitation therapy can alleviate KOA-related pain and swelling,improve lower limb muscle strength,promote flexion and activity of the knee a

BACKGROUND Improved adenoma detection at colonoscopy has decreased the risk of developing colorectal ***,whether image-enhanced endoscopy(IEE)further improves the adenoma detection rate(ADR)is *** To compare IEE with white-light imaging(Wli)endoscopy for the detection and identification of colorectal *** This was a multicenter,randomized,controlled *** were enrolled between September 2.19 to April 2021 from 4 hospital in *** were randomly assigned to an IEE group with Wli on entry and IEE on withdrawal(n=2.13)or a Wli group with Wli on both entry and withdrawal(n=2098).The primary outcome was the *** secondary endpoints were the polyp detection rate(PDR),adenomas per colonoscopy,adenomas per positive colonoscopy,and factors related to adenoma *** A total of 42.1 patients(966 adenomas)were included in the analysis(mean age,56.7 years,47.1%male).There were 2.13 patients(508 adenomas)in the IEE group and 2098 patients(458 adenomas)in the Wli *** ADR in two group were not significantly different[24.0%vs 21.8%,1.10,95%confidence interval(CI):0.99-1.23,P=0.09].The PDR was higher with IEE group(41.7%)than with Wli group(36.1%,1.16,95%CI:1.07-1.25,P=0.01).Differences in mean withdrawal time(7.90±3.42 min vs 7.85±3.47 min,P=0.30)and adenomas per colonoscopy(0.33±0.68 vs 0.28±0.62,P=0.06)were not *** analysis found that with narrowband imaging(NBI),between-group differences in the ADR,were not significant(23.7%vs 21.8%,1.09,95%CI:0.97-1.22,P=0.15),but were greater with linked color imaging(30.9%vs 21.8%,1.42,95%CI:1.04-1.93,P=0.04).the second-generation NBI(2G-NBI)had an advantage of ADR than both Wli and the first-generation NBI(27.0%vs 21.8%,P=0.01;27.0%vs 21.2.0%,P=0.01).CONCLUSION This prospective study confirmed that,among Chinese,IEE didn’t increase the ADR compared with Wli,but 2G-NBI increase the ADR.

寒区隧道围岩的最大冻结深度是隧道抗冻胀结构设计、防排水与保温隔热层设计的重要基础参数。基于准稳态假定,采用积分法推导了能够考虑衬砌、保温层及冻结围岩中未冻水含量的寒区隧道围岩最大冻结深度的解析计算公式;利用围岩温度场数值模拟结果反演得出了解析计算公式中的参数影响半径与冻结半径比?的取值,从而得到了围岩最大冻结深度的半解析解。将所得解的计算结果与现场实测数据及数值模拟结果进行了对比,验证了所得解的合理性。利用得到的半解析解分析了工程冻土段围岩冻结深度的影响因素,结果表明:初始地温、年平均气温对冻结深度的影响最为明显,岩体骨架导热系数、岩体孔隙率的影响次之。
