某院全肠外营养不合理处方的帕累托图分析

目的应用帕累托图分析年龄<60岁和年龄≥60岁的患者(老年患者)全肠外营养(total parenteral nutrition,TPN)不合理处方的情况及原因,为保障患者营养支持的安全性提供参考。方法选取福建医科大学附属泉州第一医院2023年1.至1.月住院患... 详细信息

目的应用帕累托图分析年龄1.至1.月住院患者病历,每月随机抽取30份的TPN处方,应用帕累托图针对不合理处方的主、次要和一般因素进行分析。结果年龄<60岁的患者和老年患者TPN不合理处方类型几乎一致。但造成年龄<60岁的患者TPN不合理处方的主要因素为超适应症、热氮比不合理、糖脂比不合理,次要因素为谷氨酰胺用量不适宜;而造成老年患者TPN不合理处方的主要因素为热氮比不合理、谷氨酰胺用量不适宜、糖脂比不合理,次要因素为超适应症;一般因素均为脂溶性维生素用量不适宜和添加物不稳定。结论应用帕累托图分析年龄<60岁的患者和老年患者TPN不合理处方情况,可及时发现主次要因素,有利于临床药师针对性地进行相应的干预及营养基础知识的培训,以保障临床营养治疗的安全性和有效性。

Background and Aims:We compared lung function parameters in nonalcoholic fatty liver disease(NAFLD)and metabolic dysfunction-associated fatty liver disease(MAFLD),and examined the association between lung function parameters and fibrosis severity in ***:In this cross-sectional study,we randomly recruited 2,543 middle-aged individuals from 25 communities across four cities in china during 2.16 and *** participants received a health check-up including measurement of anthropometric parameters,biochemical variables,liver ultrasonography,and *** severity of liver disease was assessed by the fibrosis(FIB)-4 ***:The prevalence of MAFLD was 20.4%(n=51.)and that of NAFLD was 1..4%(n=469).After adjusting for age,sex,adiposity measures,smoking status,and significant alco-hol intake,subjects with MAFLD had a significantly lower predicted forced vital capacity(FVC,88.27±1..60%vs.90.82.16.85%,p1.s forced expiratory volume(FEV1.79.89±1..34 vs.83.02.16.66%,p1.point increase in FIB-4,FVC was diminished by 0.507(95%CI:-0.840,-0.1.3,p=0.003),and FEV1.was diminished by 0.439(95%CI:-0.739,-0.1.0,p=0.004).The results remained unchanged when the statistical analyses was performed separately for men and ***:MAFLD was significantly asso-ciated with a greater impairment of lung function param-eters than NAFLD.

With the digestive endoscopic tunnel technique(DETT), many diseases that previously would have been treated by surgery are now endoscopically curable by establishing a submucosal tunnel between the mucosa and muscularis propria(MP). Through the tunnel, endoscopic diagnosis or treatment is performed for lesions in the mucosa, in the MP, and even outside the gastrointestinal(GI) *** present, the tunnel technique application range covers the following:(1.Treatment of lesions originating from the mucosal layer, e.g., endoscopic submucosal tunnel dissection for oesophageal large or circular early-stage cancer or precancerosis;(2) treatment of lesions from the MP layer, per-oral endoscopic myotomy, submucosal tunnelling endoscopic resection, etc.; and(3) diagnosis and treatment of lesions outside the GI tract, such as resection of lymph nodes and benign tumour excision in the mediastinum or abdominal cavity. With the increasing number of DETTs performed worldwide, endoscopic tunnel therapeutics, which is based on DETT, has been gradually developed and optimized. However, there is not yet an expert consensus on DETT to regulate its indications, contraindications, surgical procedure, and postoperative *** International DETT Alliance signed up this consensus to standardize the procedures of DETT. In this consensus, we describe the definition, mechanism,and significance of DETT, prevention of infection and concepts of DETTassociated complications, methods to establish a submucosal tunnel, and application of DETT for lesions in the mucosa, in the MP and outside the GI tract(indications and contraindications, procedures, pre-and postoperative treatments, effectiveness, complications and treatments, and a comparison between DETT and other operations).

There is considerable potential for integrating transarterial chemoembolization(TACE),programmed death-(ligand)1.PD-[L]1.inhibitors,and molecular targeted treatments(MTT)in hepatocellular carcinoma(HCC).It is necessary to investigate the therapeutic efficacy and safety of TACE combined with PD-(L)1.inhibitors and MTT in real-world *** this nationwide,retrospective,cohort study,826 HCC patients receiving either TACE plus PD-(L)1.blockades and MTT(combination group,n=376)or TACE monotherapy(monotherapy group,n=450)were included from January 2.18 to May *** primary endpoint was progression-free survival(PFS)according to modified *** secondary outcomes included overall survival(OS),objective response rate(ORR),and *** performed propensity score matching approaches to reduce bias between two *** matching,228 pairs were included with a predominantly advanced disease *** PFS in combination group was 9.5 months(95%confidence interval[CI],8.4-1..0)versus 8.0 months(95%CI,6.6-9.5)(adjusted hazard ratio[HR],0.70,P=0.002).OS and ORR were also significantly higher in combination group(median OS,1..2.16.1.27.3]vs.1..7 months[1..0-20.2];adjusted HR,0.63,P=0.001.ORR,60.1.vs.32.0%;P1..Grade 3/4 adverse events were observed at a rate of 1..8%and 7.5%in combination and monotherapy groups,*** results suggest that TACE plus PD-(L)1.blockades and MTT could significantly improve PFS,OS,and ORR versus TACE monotherapy for Chinese patients with predominantly advanced HCC in real-world practice,with an acceptable safety profile.
