Is bactibilia a predictor of poor outcome of pancreaticoduodenectomy?

BAcKGROUND:Although bile infection has been proposed to increase infective complications following pancreaticoduodenectomy,its association with infective complications and non-infective complications like pancreatic fistula is still ***:Seventy-six patients who had undergone pancreaticoduodenectomy between July 2007 and December 2008 were included in a prospective database and their data *** all patients intraoperative bile from the bile duct was ***,intra-operative,and post-operative variables were recorded and ***:Bile culture showed positive growth in 35 patients and negative growth in *** patients in the positive group underwent ERcP and *** patients with a positive bile culture had a higher incidence of infective complications including intra-abdominal abscess (n=8),wound infection (n=27),bacteremia (n=10),and renal insufficiency (n=9).There was no increase in the rate of non-infective complications of pancreaticoduodenectomy including pancreatic fistula (n=7),delayed gastric emptying (n=9),and post-operative hemorrhage (n=3).The hospital stay was significantly prolonged in the patients with a positive bile culture (P=0.0002).cONcLUSIONS:Pre-operative biliary drainage is significantly associated with bile infection,and bile infection increases the overall rates of infective complications and renal *** of the high incidence of complications is associated with infected bile,routine intra-operative bile culture is recommended in patients undergoing ***-operative prophylaxis is dependent on sensitivity of cases to perioperative antibiotics and intraoperative bile culture *** of its significant association with infected bile,biliary stenting should be used in strictly selected cases.

In this study, the evolution of c60F18 molecules on a cu（001） surface was studied by means of scanning tunneling microscopy and density functional theory calculations. The results showed that fluorinated fuUerenes （tortoise-shaped polar c60F18） decay on cu（001） surfaces by a step-by-step detachment of F atoms from the c60 cage. The most favorable adsorption configuration was realized when the F atoms of c60F18 pointed towards the cu surface and six F atoms were detached from it. The results also showed that a further decay of c60F12 molecules strongly depended on the initial c60F18 coverage. The detached F atoms initially formed a two-dimensional （2D） gas phase which then slowly transformed into F-induced surface structures. The degree of contact between the c60F12 molecules and the cu（001） surface depended on the density of the 2D gas phase. Hence, the life-time of fluorinated fullerenes was determined by the density of the 2D gas phase, which was affected by the formation of new F-induced structures and the decay of c60F12 molecules.

Analysis of convective-radiative heat transfer in dovetail longitudinal fins with shape-dependent hybrid nanofluids: a study using the Hermite wavelet method

A distinguished category of operational fluids, known as hybrid nanofluids,occupies a prominent role among various fluid types owing to its superior heat transfer properties. By employing a dovetail fin profile, this work investigates the thermal reaction of a dynamic fin system to a hybrid nanofluid with shape-based properties, flowing uniformly at a velocity U. The analysis focuses on four distinct types of nanoparticles,i.e., Al2O3, Ag, carbon nanotube (cNT), and graphene. Specifically, two of these particles exhibit a spherical shape, one possesses a cylindrical form, and the final type adopts a platelet morphology. The investigation delves into the pairing of these *** examination employs a combined approach to assess the constructional and thermal exchange characteristics of the hybrid nanofluid. The fin design, under the specified circumstances, gives rise to the derivation of a differential equation. The given equation is then transformed into a dimensionless form. Notably, the Hermite wavelet method is introduced for the first time to address the challenge posed by a moving fin submerged in a hybrid nanofluid with shape-dependent features. To validate the credibility of this research, the results obtained in this study are systematically compared with the numerical simulations. The examination discloses that the highest heat flux is achieved when combining nanoparticles with spherical and platelet shapes.

Axonal regeneration following surgical nerve repair is slow and often incomplete,resulting in poor functional recovery which sometimes contributes to lifelong ***,there are no FDA-approved therapies available to promote nerve *** accelerates axonal regeneration,but systemic side effects presently outweigh its potential benefits for peripheral nerve *** authors describe herein a biodegradable polyurethane-based drug delivery system for the sustained local release of tacrolimus at the nerve repair site,with suitable properties for scalable production and clinical application,aiming to promote nerve regeneration and functional recovery with minimal systemic drug *** is encapsulated into co-axially electrospun polycarbonate-urethane nanofibers to generate an implantable nerve wrap that releases therapeutic doses of bioactive tacrolimus over 31 *** and drug loading are adjustable for applications in small and large caliber nerves,and the wrap degrades within 120 days into biocompatible *** released from the nerve wrap promotes axon elongation in vitro and accelerates nerve regeneration and functional recovery in preclinical nerve repair models while off-target systemic drug exposure is reduced by 80%compared with systemic *** its surgical suitability and preclinical efficacy and safety,this system may provide a readily translatable approach to support axonal regeneration and recovery in patients undergoing nerve surgery.

In this article,we explored the role of adipose tissue,especially mesenteric adipose tissue and creeping fat,and its association with the gut microbiota in the pathophysiology and progression of crohn’s disease(cD).cD is a form of inflammatory bowel disease characterized by chronic inflammation of the gastrointestinal tract,influenced by genetic predisposition,gut microbiota dysbiosis,and environmental *** microbiota plays a crucial role in modulating immune response and intestinal inflammation and is associated with the onset and progression of ***,visceral adipose tissue,particularly creeping fat,a mesenteric adipose tissue characterized by hypertrophy and fibrosis,has been implicated in cD pathogenesis,inflammation,and *** bacteria from the gut microbiota may translocate into mesenteric adipose tissue,contributing to the formation of creeping fat and influencing cD *** creeping fat may be a protective barrier against bacterial invasion,its expansion can damage adjacent tissues,leading to *** gut microbiota through interventions such as fecal microbiota transplantation,probiotics,and prebiotics has shown potential in managing ***,more research is needed to clarify the mechanisms linking gut dysbiosis,creeping fat,and cD progression and develop targeted therapies for microbiota modulation and fat-related complications in patients with cD.

Objective:To evaluate phytochemicals and antimicrobial activity of Randia spinosa(Thunb.)*** Dillenia pentagyna *** extracts against human pathogens such as Gram positive bacteria(Streptococcus faecalis,Bacillus subtilis and Staphyllococcus aureus),Gram negative bacteria(Escherichia coli,Klebsiella pneumoniae and Pseudomonas aeruginosa),and a fungus candida ***:Antimicrobial activity of methanol,acetone and ethyl acetate extracts of the two plants against the human pathogens was investigated by agar well diffusion method,and qualitative phytochemical screening was conducted for the presence of phytoconstituents such as alkaloids,catechins,coumarins,flavonoids,phenols,quinones,saponins,steroids,tannins and ***:The qualitative phytochemical analysis showed the presence of diverse range of compounds like alkaloids,flavonoids,phenols,steroids,tannins,terpenoids and *** plants exhibited broad spectrum of antimicrobial activities against all the tested bacterial and fungal *** maximum zone of inhibition was observed in methanol extracts when compared with acetone and ethyl acetate *** present study demonstrated that the selected plants had promising effect on the bacterial and fungal ***:The phytochemicals in the plants may be potentially responsible for the antimicrobial efficacy of these medicinal plants.
