Effectiveness of craniosacral therapy,Bowen therapy,static touch and standard exercise program on sleep quality in fibromyalgia syndrome:A randomized controlled trial

Background:Sleep disturbance is commonly seen in fibromyalgia syndrome (FMS);however,high quality studies involving manual therapies that target FMS-linked poor sleep quality are lacking for the Indian population.Objective:craniosacral therapy (cST),Bowen therapy and exercises have been found to influence the autonomic nervous system,which plays a crucial role in sleep physiology.Given the paucity of evidence concerning these effects in individuals with FMS,our study tests the effectiveness of cST,Bowen therapy and a standard exercise program against static touch (the manual placebo group) on sleep quality in FMS.Design,setting,participants and intervention:A placebo-controlled randomized trial was conducted on132 FMS participants with poor sleep at a hospital in Bangalore.The participants were randomly allocated to one of the four study groups,including cST,Bowen therapy,standard exercise program,and a manual placebo control group that received static touch.cST,Bowen therapy and static touch treatments were administered in once-weekly 45-minute sessions for 12 weeks;the standard exercise group received weekly supervised exercises for 6 weeks with home exercises until 12 weeks.After 12 weeks,all study participants performed the standard exercises at home for another 12 weeks.Main outcome measures:Sleep quality,pressure pain threshold (PPT),quality of life and fibromyalgia impact,physical function,fatigue,pain catastrophizing,kinesiophobia,and positive–negative affect were recorded at baseline,and at weeks 12 and 24 of the intervention.Results:At the end of 12 weeks,the sleep quality improved significantly in the cST group (P=0.037) and Bowen therapy group (P=0.023),and the PPT improved significantly in the Bowen therapy group(P=0.002) and the standard exercise group (Pcompared to the static touch group.These improvements were maintained at 24 weeks.No between-group differences were observed for other secondary outcomes.conclusion:cST and Bowen therapy improved sleep quality,and Bowen therapy and standard

The microbiota-gut-brain axis(MGBA)has emerged as a key prospect in the bidirectional communication between two major organ systems:the brain and the gut.Homeostasis between the two organ systems allows the body to function without disease,whereas dysbiosis has long-standing evidence of etiopathological conditions.The most common communication paths are the microbial release of metabolites,soluble neurotransmitters,and immune cells.However,each pathway is intertwined with a complex one.With the emergence of in vitro models and the popularity of three-dimensional(3D)cultures and Transwells,engineering has become easier for the scientific understanding of neurodegenerative diseases.This paper briefly retraces the possible communication pathways between the gut microbiome and the brain.It further elaborates on three major diseases:autism spectrum disorder,Parkinson’s disease,and Alzheimer’s disease,which are prevalent in children and the elderly.These diseases also decrease patients’quality of life.Hence,understanding them more deeply with respect to current advances in in vitro modeling is crucial for understanding the diseases.Remodeling of MGBA in the laboratory uses many molecular technologies and biomaterial advances.Spheroids and organoids provide a more realistic picture of the cell and tissue structure than monolayers.combining them with the Transwell system offers the advantage of compartmentalizing the two systems(apical and basal)while allowing physical and chemical cues between them.cutting-edge technologies,such as bioprinting and microfluidic chips,might be the future of in vitro modeling,as they provide dynamicity.

Basalt fiber reinforcement in polymer matrix composites is becoming more and more popular because of its environmental friendliness and mechanical qualities that are comparable to those of synthetic fibers.Basalt fiber strengthened vinyl ester matrix polymeric composite with filler addition of nano-and micro-sized silicon carbide(Sic)element spanning from 2 weight percent to 10 weight percent was studied for its mechanical and wear properties.The application of Artificial Neural Network(ANN)to correlate the filler addition composition for optimum mechanical properties is required due to the non-linear mechanical and tribological features of composites.The stuffing blend and composition of the composite are optimized using the hybrid model and Genetic Algorithm(GA)to maximize the mechanical and wear-resistant properties.The predicted and tested ANN–GA optimal values obtained for the composite combination had a tensile,flexural,impact resilience,hardness and wear properties of 202.93 MPa,501.67 MPa,3.460 J/s,43 HV and 0.196 g,respectively,for its optimum combination of filler and reinforcement.It can be noted that the nano-sized Sic filler particle enhances most of the properties of the composite which diversifies its applications.The predicted mechanical and wear values of the developed ANN–GA model were in closer agreement with the experimental values which validate the model.

In this editorial,we talk about a compelling case focusing on posterior reversible encephalopathy syndrome(PRES)as a complication in patients undergoing liver transplantation and treated with Tacrolimus.Tacrolimus(FK 506),derived from Streptomyces tsukubaensis,is a potent immunosuppressive macrolide.It inhibits Tcell transcription by binding to FK-binding protein,and is able to amplify glucocorticoid and progesterone effects.Tacrolimus effectively prevents allograft rejection in transplant patients but has adverse effects such as Tacrolimus-related PRES.PRES presents with various neurological symptoms alongside elevated blood pressure,and is primarily characterized by vasogenic edema on neuroimaging.While computed tomography detects initial lesions,magnetic resonance imaging,especially the Fluid-Attenuated Inversion Recovery sequence,is superior for diagnosing cortical and subcortical edema.Our discussion centers on the incidence of PRES in solid organ transplant recipients,which ranges between 0.5 to 5+AcU-,with varying presentations,from seizures to visual disturbances.The case of a 66-year-old male status post liver transplantation highlights the diagnostic and management challenges associated with Tacrolimus-related PRES.Radiographically evident in the parietal and occipital lobes,PRES underlines the need for heightened vigilance among healthcare providers.This editorial emphasizes the importance of early recognition,accurate diagnosis,and effective management of PRES to optimize outcomes in liver transplant patients.The case further explores the balance between the efficacy of immunosuppression with Tacrolimus and its potential neurological risks,underlining the necessity for careful monitoring and intervention strategies in this patient population.

Butyrylcholinesterase(BchE;Ec 3.***.1.8),an enzyme structurally related to acetylcholinesterase,is widely distributed in the human body.It plays a role in the detoxification of chemicals such as succinylcholine,a muscle relaxant used in anesthetic practice.BchE is well-known due to variant forms of the enzyme with little or no hydrolytic activity which exist in some endogamous communities and result in prolonged apnea following the administration of succinylcholine.Its other functions include the ability to hydrolyze acetylcholine,the cholinergic neurotransmitter in the brain,when its primary hydrolytic enzyme,acetylcholinesterase,is absent.To assess its potential roles,BchE was studied in relation to insulin resistance,type 2 diabetes mellitus,cognition,hepatic disorders,cardiovascular and cerebrovascular diseases,and inflammatory conditions.Individuals who lack the enzyme activity of BchE are otherwise healthy,until they are given drugs hydrolyzed by this enzyme.Therefore,BchE is a candidate for the study of loss-of-function mutations in humans.Studying individuals with variant forms of BchE can provide insights into whether they are protected against metabolic diseases.The potential utility of the enzyme as a biomarker for Alzheimer’s disease and the response to its drug treatment can also be assessed.

Pupil dynamics are the important characteristics of face spoofing detection.The face recognition system is one of the most used biometrics for authenticating individual identity.The main threats to the facial recognition system are different types of presentation attacks like print attacks,3D mask attacks,replay attacks,etc.The proposed model uses pupil characteristics for liveness detection during the authentication process.The pupillary light reflex is an involuntary reaction controlling the pupil’s diameter at different light intensities.The proposed framework consists of two-phase methodologies.In the first phase,the pupil’s diameter is calculated by applying stimulus(light)in one eye of the subject and calculating the constriction of the pupil size on both eyes in different video frames.The above measurement is converted into feature space using Kohn and clynes model-defined parameters.The Support Vector Machine is used to classify legitimate subjects when the diameter change is normal(or when the eye is alive)or illegitimate subjects when there is no change or abnormal oscillations of pupil behavior due to the presence of printed photograph,video,or 3D mask of the subject in front of the camera.In the second phase,we perform the facial recognition process.Scale-invariant feature transform(SIFT)is used to find the features from the facial images,with each feature having a size of a 128-dimensional vector.These features are scale,rotation,and orientation invariant and are used for recognizing facial images.The brute force matching algorithm is used for matching features of two different images.The threshold value we considered is 0.08 for good matches.To analyze the performance of the framework,we tested our model in two Face antispoofing datasets named Replay attack datasets and cASIA-SURF datasets,which were used because they contain the videos of the subjects in each sample having three modalities(RGB,IR,Depth).The cASIA-SURF datasets showed an 89.9%Equal Error Rate,while the Replay Attack datasets showed

Dear Editor,I would like to express my views on“Amendments to the Abortion Law in India:Is it Progressive Enough?[1]”Dr.Rani stated that“The law also permits abortions on the ground of contraceptive failure even among unmarried women,which were earlier restricted to the married couple…Reproductive freedom of women continues to remain submissive to medical and legal rules.[1]”In India,it is agreed that abortion laws are progressive and women’s reproductive rights are subject to medical and legal procedures.[2]However,Section 3(2)(a)read with Explanation I of Section 3(2)of the Medical Termination of Pregnancy Act,1971(MTP Act,1971)(as amended in 2021),allowed all women(including unmarried women)to have abortion rights up to 20 weeks of pregnancy due to contraceptive failure used by her or her partner based on doctor’s opinion that such pregnancy poses a risk to the mother’s life;or grave injury to physical or mental health.

The use of Artificial intelligence(AI)has evolved from its mid-20th century origins to playing a pivotal tool in modern medicine.It leverages digital data and computational hardware for diverse applications,including diagnosis,prognosis,and treatment responses in gastrointestinal and hepatic conditions.AI has had an impact in diagnostic techniques,particularly endoscopy,ultrasound,and histopathology.AI encompasses machine learning,natural language processing,and robotics,with machine learning being central.This involves sophisticated algorithms capable of managing complex datasets,far surpassing traditional statistical methods.These algorithms,both supervised and unsupervised,are integral for interpreting large datasets.In liver diseases,AI's non-invasive diagnostic applications,particularly in non-alcoholic fatty liver disease,and its role in characterizing hepatic lesions is promising.AI aids in distinguishing between normal and cirrhotic livers and improves the accuracy of lesion characterization and prognostication of hepatocellular carcinoma.AI enhances lesion identification during endoscopy,showing potential in the diagnosis and management of early-stage esophageal carcinoma.In peptic ulcer disease,AI technologies influence patient management strategies.AI is useful in colonoscopy,particularly in detecting smaller colonic polyps.However,its applicability in nonacademic settings requires further validation.Addressing these issues is vital for harnessing the potential of AI.In conclusion,while AI offers transformative possibilities in gastroenterology,careful integration and balancing of technical possibilities with ethical and practical application,is essential for optimal use.

Dear Editor,Around a billion individuals worldwide have hypertension.Of these,95%have essential hypertension,a type of undiagnosed hypertension.1 The regulation of blood pressure(BP)involves numerous signaling pathways.Among them,the Renin Angiotensin System is well known.All these pathways are regulated by modulation of renal salt handling and tone of vascular smooth muscle(VSM)tissue.Any of these mechanisms can become faulty and alter the resistance arteries’VSM tone,which can elevate BP.However,since the exact origin of PH and its pathophysiology are unknown,less effective,and generic treatments are used.2 The fact that more than 50%of hypertension patients in the USA do not have their BP under good control serves as an illustration of this.Antihypertensive treatment resistance affects an additional 5 million people and is defined as the inability to regulate BP despite the use of at least three antihypertensive drugs in combination.
