Blood transfusion for the treatment of acute anaemia in inflammatory bowel disease and other digestive diseases

Allogeneic blood transfusion (ABT) is frequently used as the first therapeutic option for the treatment of acute anaemia in patients with inflammatory bowel disease (IBD), especially when it developed due to gastrointestinal or perioperative blood loss, but is not risk-free. Adverse effects of ABT include, but are not limited to, acute hemolytic reaction (wrong blood or wrong patient), febrile non-hemolytic transfusional reaction, bacterial contamination, transfusion-related acute lung injury, transfusion associated circulatory overload, transfusion-related immuno-modulation, and transmission of almost all infectious diseases (bacteria, virus, protozoa and prion), which might result in increased risk of morbidity and mortality. Unfortunately, the main physiological goal of ABT, i.e. to increase oxygen consumption by the hypoxic tissues, has not been well documented. In contrast, the ABT is usually misused only to increase the haemoglobin level within a f ixed protocol [mostly two by two packed red blood cell (PRc) units] independently of the patient' s tolerance to normovolemic anaemia or his clinical response to the transfusion of PRc units according to a "one-by-one" administration schedule. Evidencebased clinical guidelines may promote best transfusion practices by implementing restrictive transfusion protocols, thus reducing variability and minimizing the avoidable risks of transfusion, and the use of autologous blood and pharmacologic alternatives. In this regard, preoperative autologous blood donation (PABD) consistently diminished the frequency of ABT, although its contribution to ABT avoidance is reduced when performed under a transfusion protocol. In addition, interpretation of utility of PABD in surgical IBD patients is hampered by scarcity of published data. However, the role of autologous red blood cells as drug carriers is promising. Using remotely sensed and climate data to predict the current and potential future geographic distribution of a bird at multiple scales: the case of Agelastes meleagrides, a western African forest endemic

Background:Understanding geographic distributions of species is a crucial step in spatial planning for biodiversity conservation, particularly as regards changes in response to global climate *** information is especially important for species of global conservation concern that are susceptible to the effects of habitat loss and climate change. In this study, we used ecological niche modeling to assess the current and future geographic distributional potential of *** Guineafowl (Agelastes meleagrides)(Vulnerable) across West Africa. Methods:We used primary occurrence data obtained from the Global Biodiversity Information Facility and national parks in Liberia and Sierra Leone, and two independent environmental datasets (Moderate Resolution Imaging Spectroradiometer normalized difference vegetation index at 250 m spatial resolution, and Worldclim climate data at 2.5' spatial resolution for two representative concentration pathway emissions scenarios and 27 general circulation models for 2050) to build ecological niche models in Maxent. Results: From the projections, *** Guineafowl showed a broader potential distribution across the region compared to the current IUcN range estimate for the species. Suitable areas were concentrated in the Gola rainforests in northwestern Liberia and southeastern Sierra Leone, the *** corridor in southeastern Liberia and southwestern cote d'lvoire, and the Nimba Mountains in northern Liberia, southeastern Guinea, and northwestern cote d'*** *** projections anticipated minimal range shifts in response to climate change. conclusions: By combining remotely sensed data and climatic data, our results suggest that forest cover, rather than climate is the major driver of the species' current distribution. Thus, conservation efforts should prioritize forest protection and mitigation of other anthropogenic threats (*** pressure) affecting the species.

Metastatic gastric cancer remains a non-curative disease. Palliative chemotherapy has been demonstrated to prolong survival without quality of life compromise. Many single-agents and combinations have been confirmed to be active in the treatment of metastatic disease. Objective response rates ranged from 10-30% for single-agent therapy and 30-60% for polychemotherapy. Results of phase Ⅱ and Ⅲ studies are reviewed in this paper as well as the potential efficacy of new drugs. For patients with localized disease, the role of adjuvant and neoadjuvant chemotherapy and radiation therapy is discussed. Most studies on adjuvant chemotherapy failed to demonstrate a survival advantage, and therefore, it is not considerecl as standard treatment in most centres. Adjuvant immunochemotherapy has been developed fundamentally in Korea and Japan. A meta-analysis of phase Ⅲ trials with OK-432 suggested that immunochemotherapy may improve survival of patients with curatively resected gastric cancer. Based on the results of US Intergroup 0116 study, postoperative chemoradiation has been accepted as standard care in patients with resected gastric cancer in North America. However, the results are somewhat confounded by the fact that patients underwent less than a recommended D1 lymph node dissection and the pattern of recurrence suggested a positive effect derived from local radiotherapy without any effect on micrometastatic disease. Neoadjuvant chemotherapy or chemoradiation therapy remains experimental, but several phase Ⅱ studies are showing promising results. Phase Ⅲ trials are needed.

Detection of KRAS G12D in colorectal cancer stool by droplet digital PcR

AIMTo assess KRAS G12D mutation detection by droplet digital PcR (ddPcR) in stool-derived DNA from colorectal cancer (cRc) *** this study, tumor tissue and stool samples were collected from 70 patients with stage I-IV cRc diagnosed by preoperative biopsy. KRAS mutational status was determined by pyrosequencing analysis of DNA obtained from formalin-fixed paraffin-embedded (FFPE) tumor tissues. The KRAS G12D mutation was then analyzed by ddPcR in FFPE tumors and stool-derived DNA from patients with this point mutation. Wild-type (WT) tumors, as determined by pyrosequencing, were included as controls; analysis of FFPE tissue and stool-derived DNA by ddPcR was performed for these patients as *** the total 70 patients included, KRAS mutations were detected by pyrosequencing in 32 (45.71%), whereas 38 (54.29%) had WT tumors. The frequency of KRAS mutations was higher in left-sided tumors (11 located in the right colon, 15 in the left, and 6 in the rectum). The predominant point mutation was KRAS G12D (14.29%, n = 10), which was more frequent in early-stage tumors (I-IIA, n = 7). In agreement with pyrosequencing results, the KRAS G12D mutation was detected by ddPcR in FFPE tumor-derived DNA, and only a residual number of mutated copies was found in WT controls. The KRAS G12D mutation was also detected in stool-derived DNA in 80% of all fecal samples from cRc patients with this point *** is a reliable and sensitive method to analyze KRAS G12D mutation in stool-derived DNA from cRc patients, especially at early stages. This non-invasive approach is potentially applicable to other relevant biomarkers for cRc management.

cover crops have long been proposed as an alternative soil management for minimizing erosion rates in olive stands while providing additional ecosystem ***,the trade-off between these benefits and the competition for water with the trees makes the definition of optimal management practices a challenging task in semiarid *** work presents an improved version of Olivecan,a process-based simulation model of olive orchards that now can simulate the main impacts of cover crops on the water and carbon balances of olive *** simple in its formulation,the new model components were developed to deal with different cover crop management *** are presented for simulation runs of a traditional olive orchard in the conditions of southern Spain,evaluating the effects of different widths for the strip occupied by the cover crop(Fcc)and two contrasting mowing *** revealed that high Fccresulted in lower olive yields,but only when mowing was applied at the end of *** this regard,late mowing and high Fccwas associated with lower soil water content from spring to summer,coinciding with olive flowering and the earlier stages of fruit *** also negatively correlated with surface runoff irrespective of the mowing *** the other hand,net ecosystem productivity(NEP)was substantially affected by both Fccand mowing *** simulations under future climate scenarios comparing the same management alternatives are also presented,showing substantial yield reductions by the end of the century and minor or negligible changes in NEP and seasonal runoff.

The atomic mass table presents zones where the structure of the states changes rapidly as a function of the neutron or proton *** them,notable examples are the A≈100 Zr region,the Pb region around the neutron midshell(N=104),and the N≈90 rare-earth *** observed phenomena can be understood in terms of either shape coexistence or quantum phase *** objective of this study is to find an observable that can distinguish between both shape coexistence and quantum phase *** an observable to be analyzed,we selected the two-neutron transfer intensity between the 0+states in the parent and daughter *** framework used for this study is the Interacting Boson Model(IBM),including its version with configuration mixing(IBM-cM).To generate wave functions of isotope chains of interest needed for calculating transfer intensities,previous systematic studies using IBM and IBM-cM were used without changing the *** results of two-neutron transfer intensities are presented for Zr,Hg,and Pt isotopic chains using ***,for Zr,Pt,and Sm isotopic chains,the results are presented using IBM with only a single configuration,i.e.,without using configuration *** Zr,the two-neutron transfer intensities between the ground states provide a clear observable,indicating that normal and intruder configurations coexist in the low-lying spectrum and cross at A=98→*** can help clarify whether shape coexistence induces a given quantum phase *** Pt,in which shape coexistence is present and the regular and intruder configurations cross for the ground state,there is almost no impact on the value of the two-neutron transfer *** is the situation with Hg,where the ground state always has a regular *** the Sm isotope chain,which is one of the quantum phase transition paradigms,the value of the two-neutron transfer intensity is affected strongly.

ccL2-2518 A/G and ccR2 190 A/G do not influence the outcome of hepatitis c virus infection in the Spanish population

AIM： To assess whether ccL2 or interactions between this chemokine and its receptor （ccR2） are associated with outcomes of chronic hepatitis c and with responses to antiviral therapy. METHODS： Two hundred and eighty-four patients with chronic hepatitis c and 193 non-infected matched controls were included in this study. Patients were categorized according to their Scheuer score of hepatic fibrosis as F0-F2 （/7 = 202） or F3-F4 （/7 = 82） and according to their response to anti-Hepatitis c virus （HcV） therapy as sustained response （SR, n = 201） or non-sustained response （NSR, n = 98）. Genotyping of the -2518 （A/G） ccL2 was performed using PcR-RFLP, genotyping of the 190 （A/G） ccR2 using a PcR-ARMS system, and genotyping of the rs3138042 （G/A） ccR2 using Taqman probes. RESULTS： Univariate analyses identified 4 parameters （infection duration time, viral genotype, gender and AST levels） that tended to influence fibrosis and 7 parameters （ccL2G, ccL2AccR2A, viremia levels, fibrosis stage, viral genotype, infection duration time and AST levels） that significantly influenced or tended to influence response to treatment. Multivariate analysis identified gender and AST levels as parameters that independently influenced fibrosis stage and viral genotype and infection duration time were the two parameters that independently influenced response to treatment. cONcLUSION： Our results indicate that the mutations studied in the gene pair ccL2/ccR2 do not play a major role in the outcome and response to treatment for HcV infection in the Spanish population.

Soil-vegetation relationships in Mediterranean forests after fire

carbon nanotubes targeted to the tumor microenvironment inhibit metastasis in a preclinical model of melanoma

Despite notable progress in cancer therapy,metastatic diseases continue to be the primary cause of cancer-related ***-walled carbon nanotubes(MWcNTs)can enter tissues and cells and interfere with the dynamics of the cytoskeletal nanofilaments *** endows them with intrinsic anti-tumoral effects comparable to those of microtubule-binding chemotherapies such as Taxol®.In this study,our focus was on exploring the potential of oxidized MWcNTs in selectively targeting the vascular endothelial growth factor receptor(VEGFR).Our objective was to evaluate their effectiveness in inhibiting metastatic growth by inducing anti-proliferative,anti-migratory,and cytotoxic effects on both cancer and tumor microenvironment *** findings demonstrated a significant reduction of over 80%in malignant melanoma lung metastases and a substantial enhancement in overall animal welfare following intravenous administration of the targeted biodegradable ***,the combination of these nanomaterials with the conventional chemotherapy agent Taxol®yielded a remarkable 90%increase in the antimetastatic *** results highlight the promising potential of this combined therapeutic approach against metastatic disease and are of paramount importance as metastasis is responsible for nearly 60,000 deaths each year.
