Risk factors for endophthalmitis following cataract surgery-our experience at a tertiary eye care centre in India

AIMTo determine the risk factors for acute endophthalmitis after cataract extraction in a tertiary care centre in *** performed a nested case control study within a retrospective cohort. The surgical records of all patients with clinically diagnosed endophthalmitis within one month after cataract surgery, performed between January 2006 and December 2009, were reviewed. These were compared with randomly selected age and gender-matched controls, from patients having routine cataract surgery within ±1wk of the endophthalmitis case. Univariable and multivariable analysis were performed to identify risk factors for *** the total 33 856 cataract surgeries performed during this period, there were 57 cases of postoperative acute endophthalmitis that met our study criteria. Thus, the overall incidence of endophthalmitis in our cohort was 1.6 per 1000 cataract extractions performed. Mean age of cases was 55.9y (SD: 10.9y) and for controls was 55.6y (SD: 9.8y). Thirty-five cases (61.4%) and 133 controls (59.6%) were males. Median time of onset of endophthalmitis was 4d (IQR 2-9d; range: 1-30d). Thirty-nine cases (68.4%) presented within 7d and 27 cases (47.4%) were culture positive. Two hundred and twenty-three age and gender matched controls were selected. In multivariate analysis, endophthalmitis was associated with posterior capsular rupture (PcR) during surgery (OR 6.98, 95%cI: 2.22-21.98), phacoemulsification via scleral incision with a foldable intraocular lens (IOL) implantation (OR 3.02, 95%cI: 1.13-8.04) and ocular co-morbidity (OR 2.32, 95%cI: 1.11-4.87).cONcLUSIONPcR, presence of ocular co-morbidity, and phacoemulsification via scleral incision with foldable-IOL were found to be independent risk factors for acute endophthalmitis.

In the present study, we evaluated laser Doppler flowmetry technique using LDF100c (Biopac systems) by determining blood flow changes following acute application of capsaicin on 12 healthy human subjects. capsaicin applied locally (topical application) at a dose of 0.075% produced significant increase in mean dermal blood flow from 31.4 ± 3.1 Blood Perfusion Units (BPU) to 115. 7 ± 24.6 Blood Perfusion Units (BPU) after 30 minutes, also there was significant difference in dermal blood flow change between placebo (32.1 ± 2.7 BPU) and capsaicin (115.7 ± 24.6 BPU) applica- tion. capsaicin application produced significant percentage change in dermal blood flow by 291.0 ± 85.3% from baseline, while the change was insignificant with placebo (13.2 ± 7.4%). Therefore, it is suggested that this technique which is technically sound, non-invasive and inexpensive can be adopted in various fields of research to determine blood flow changes and this technique can also be utilized to determine the antagonists of the mediators involved in capsaicin induced vasodilatation.

Relapsing polychondritis is a rare autoimmune disease characterized by inflammation and degeneration of cartilaginous tissue. We describe the first reported cases of relapsing poly-chondritis in patients with human immunodeficiency virus (HIV) and no associated connective tissue diseases. The relationship between autoimmune and HIV diseases is complex and unclear. Treatment of HIV disease with antiretroviral therapy and subsequent immune restoration may lead to the development of autoimmune diseases in genetically susceptible individuals.

Time-series observations were conducted in the Dowleiswaram dam reservoir that was constructed on the largest monsoonal river in India to understand the source of inorganic carbon, and fluxes to the atmosphere. The reservoir stores water during dry period of six months and water increases during the period when Indian subcontinent receives significant rainfall. Significant modification of organic matter was noticed during storage period indicated by decrease in pH from 7.5 to 6.4 and oxygen saturation from ~95% to 65%. The relationship of dissolved inorganic carbon (DIc) with oxygen saturation, dissolved organic carbon (DOc) and isotopic ratios of DIc suggests that heterotrophic activities are the major source of inorganic carbon to the reservoir. In addition to this, ground water exchange also contributes significantly to the inorganic carbon pool in the reservoir. Nutrients released due to decomposition of organic matter in the reservoir supports both autotrophic and heterotrophic activities. The pcO2 levels in the reservoir varied between 3944 and 16,042 μatm and higher pcO2 levels were noticed during peak discharge period. The annual mean cO2 fluxes from the reservoir amounted to 112 ± 126 mmolc m-2·d-1 and ~6 times higher fluxes were noticed during discharge period compared to dry period and such high fluxes during discharge period were contributed by both high pcO2 levels and winds. It was further noticed that dam reservoir is a strong source of pcO2 to the estuary wherein 2 during dry period and >15,000 μatm during discharge period were observed. Our study also indicates that Dowleiswaram dam reservoir is a strong source of cO2 to atmosphere, even though it is much smaller than Brazilian (tropical) reservoir but higher than European reservoirs.

La0.75Sr0.25cr0.5Mn0.5O3-δ (LScM) perovskite nanoparticles for use as anode material in intermediate temperature solid oxide fuel cells (IT-SOFcs) were synthesized using 3,3’,3”- nitrilotripropionic acid (NTP), citric acid and oxalic acid as carriers via a combustion method. The influence of the carrier on phase and morphology of the obtained pristine products was characterized using X-ray diffraction (XRD), thermal gravimetric analysis (TGA), and scanning electron microscopy (SEM). XRD results showed, that the LScM had rhombohedral symmetry with R-3c space group;a single phase LScM perovskite formed after calcination of fired gel at 1200°c for 7 h. Scanning electron microscopy analysis of the pristine powders showed spherical shape and particle sizes in the range of 50 – 500 nm.
