Surgical management of polycystic liver disease

Adult polycystic liver disease (PcLD) is an autosomal dominant condition commonly associated with autosomal dominant polycystic kidney disease (ADPKD). However in the last decade, it has been recognized that there is a distinct form of autosomal dominant PcLD that arises without concomitant ADPKD. Early knowledge of the pathogenesis was gained from the study of hepatic cysts in patients with ADPKD. Bile duct overgrowth after embryogenesis results in cystic hepatic dilatations that are known as biliary microhamartomas or von Meyenburg complexes. Further dilatation arises from cellular proliferation and fluid secretion into these cysts. There is a variable, broad spectrum of manifestations of PcLD. Although PcLD is most often asymptomatic, massive hepatomegaly can lead to disabling symptoms of abdominal pain, early satiety, persistent nausea, dyspnea, ascites, biliary obstruction, and lower body edema. complications of PcLD include cyst rupture and cyst infection. Also, there are associated medical problems, especially intracranial aneurysms and valvular heart disease, which clinicians need to be aware of and evaluate in patients with PcLD. In asymptomatic patients, no treatment is indicated for PcLD. In the symptomatic patient, surgical therapy is the mainstay of treatment tailored to the extent of disease for each patient. Management options include cyst aspiration and sclerosis, open or laparoscopic fenestration, liver resection with fenestration, and liver transplantation. The surgical literature discussing treatment of PcLD, including techniques, outcomes, and complication rates, are summarized in this review.

Eastern Dharwar craton,India:continental lithosphere growth by accretion of diverse plume and arc terranes

Abstract Greenstone belts of the eastern Dharwar craton, India are reinterpreted as composite tecto- nostratigraphic terranes of accreted plume-derived and convergent margin-derived magmatic sequences based on new high-precision elemental data. The former are dominated by a komatiite plus Mg-tholeiitic basalt volcanic association, with deep water siliciclastic and banded iron formation （BIF） sedimentary rocks. Plumes melted at 〈90 km under thin rifted continental lithosphere to preserve intrao- ceanic and continental margin aspects. Associated alkaline basalts record subduction-recycling of Me- soarchean oceanic crust, incubated in the asthenosphere, and erupted coevally with Mg basalts from a heterogeneous mantle plume. Together, komatiites-Mg basalts-alkaline basalts plot along the Phanero- zoic mantle array in Th/Yb versus Nb/Yb coordinate space, representing zoned plumes, establishing that these reservoirs were present in the Neoarchean mantle. convergent margin magmatic associations are dominated by tholeiitic to calc-alkaline basalts compo- sitionally similar to recent intraoceanic arcs. As well, boninitic flows sourced in extremely depleted mantle are present, and the association of arc basalts with Mg-andesites-Nb enriched basalts-adakites documented from cenozoic arcs characterized by subduction of young （〈20 Ma）, hot, oceanic litho- sphere. consequently, cenozoic style ＂hot＂ subduction was operating in the Neoarchean. These diverse volcanic associations were assembled to give composite terranes in a subduction-accretion orogen at -2.7 Ga, coevally with a global accretionary orogen at -2.7 Ga, and associated orogenic gold mineralization.

Incidence of brain metastasis in patients with esophageal carcinoma

AIM:To determine the incidence of brain metastasis in a contemporary group of patients with carcinoma of the ***:Retrospective analysis of 53 patients with esophageal carcinoma who received radiotherapy as a component of treatment between 1998 and 2007,including patient and tumor characteristics,and subsequent diagnosis of brain *** association between the histological type of esophageal cancer and the incidence of brain metastasis was assessed using Fisher's exact ***:Forty-four of the fifty-three patients in this study had adenocarcinoma and nine had squamous cell carcinoma,ranging from stage ⅡA-Ⅳ*** treatment was surgery with neoadjuvant chemoradiotherapy (trimodality therapy) in 19% of patients;chemoradiotherapy in 42%;and surgery and adjuvant radiotherapy in 7%.Twenty-five percent of patients inthis study received palliative *** overall incidence of brain metastasis in this cohort was 13%.Adenocarcinoma was the primary tumor histology in all of the patients who developed brain metastasis,representing an incidence of 16% in this *** patients with squamous cell carcinoma received trimodality *** association between histology and brain metastasis was not statistically ***:The incidence of brain metastasis in this contemporary cohort of patients with esophageal carcinoma is higher than previously reported and was confined to those with adenocarcinoma.

Ophiolites:Identification and tectonic significance in space and time

Ophiolites are fragments of oceanic lithosphere that are produced at spreading centers at ocean ridges,back arc basins,or forearcs during subduction initiation,and are key indicators of plate *** it is widely agreed that ophiolites are remnants of oceanic crust and associated depleted mantle preserved in orogens,the recognition of ophiolites and their tectonic significance is still a topic of discussion and *** propose that ophiolites can be recognized in the geologic record by some combination of genetically related pillow basalt,layered gabbro,sheeted dykes,podiform chromite,harzburgite or/and *** igneous rocks have either ocean-ridge basalt or immature oceanic arc basalt chemical *** a scoring system of 1–11 for ophiolite confidence level,scores of8 are considered confident,6–8 probable,andcores6 areconfident ophiolite(score of 8)is the Zunhua ophiolite in eastern china at 2550 Ma,and the oldest well documented sheeted dykes occur in the Jormua and Purtuniq ophiolites at 2000–1950 *** do not become geographically widespread until after 900 Ma,and most ophiolites of all ages formed in a forearc(subduction initiation)tectonic *** ophiolite production requires plate tectonics,subduction must have begun at least locally by 2700 Ma but did not become widespread until after 2000 *** abundance of ophiolites after 900 Ma may reflect better preservation of subduction-related ophiolites,or to an increasing global network of interconnected *** frequency peaks in the geologic record partially reflect geographic regions where ophiolites have been extensively studied rather than monitoring the production rate of ophiolites,but the scarcity of>900 Ma ophiolites is probably real.

Impact of carbon-based nutrient enhancement on biofiltration performance for drinking water treatment

Incorporation of a carbon-based nutrient enhancementstrategy for drinking water biofiltration is an attractive option, especially for source waters which contain recalcitrant organics. This study compared biofilters that were operated in parallel and individually enhanced with amino acids(including alanine, phenylalanine, and tryptophan), inulin, and sucrose to increase biomass concentration and promote biodegradation of dissolved organic carbon(DOc) in the source water, including disinfection by-product(DBP) precursors. Biomass activity was characterized by measuring adenosine tri-phosphate(ATP), dissolved oxygen(DO) consumption, and through the use of laccase and esterase enzyme assays. Performance was evaluated in terms of headloss,turbidity, p H, DOc, UV254, and DBP formation potential(DBP FP). The introduction of carbonbased nutrients significantly increased biomass activity, where ATP values peaked at 976 ng/g of filter media, 853 ng/g, and 513 ng/g for amino acids, inulin, and sucrose-spiked biofilters,respectively, while a non-spiked control only reached 104 ng/g. DO utilization by the enhanced biofilters was significantly higher than the control, with a strong correlation between ATP and DO uptake observed for all filters(R^2> 0.74). Laccase and esterase enzyme activities of enhanced biofilters were also elevated(p > 0.05), suggesting greater biodegradation potential. Operational parameters such as headloss development and turbidity control were not impaired by carbon supplementation strategies or an increase in biomass concentration and activity. However, the enhancement strategy did not provide improvement in terms of source water carbon removal(DOc and UV254) or DBP FP when treated filters were compared to a control.

Deep Reinforcement Learning for Power System Applications: An Overview

Due to increasing complexity, uncertainty and data dimensions in power systems, conventional methods often meet bottlenecks when attempting to solve decision and control prob- lems. Therefore, data-driven methods toward solving such prob- lems are being extensively studied. Deep reinforcement learning (DRL) is one of these data-driven methods and is regarded as real artificial intelligence (AI). DRL is a combination of deep learning (DL) and reinforcement learning (RL). This field of research has been applied to solve a wide range of complex sequential decision-making problems, including those in power systems. This paper firstly reviews the basic ideas, models, algorithms and techniques of DRL. Applications in power systems such as energy management, demand response, electricity market, operational control, and others are then considered. In addition, recent advances in DRL including the combination of RL with other classical methods, and the prospect and challenges of applications in power systems are also discussed.

comparison of three-dimensional fluorescence analysis methods for predicting formation of trihalomethanes and haloacetic acids

This work investigated the application of several fluorescence excitation–emission matrix analysis methods as natural organic matter（NOM） indicators for use in predicting the formation of trihalomethanes（THMs） and haloacetic acids（HAAs）. Waters from four different sources（two rivers and two lakes） were subjected to jar testing followed by 24 hr disinfection by-product formation tests using chlorine. NOM was quantified using three common measures： dissolved organic carbon, ultraviolet absorbance at 254 nm, and specific ultraviolet absorbance as well as by principal component analysis, peak picking,and parallel factor analysis of fluorescence spectra. Based on multi-linear modeling of THMs and HAAs, principle component（Pc） scores resulted in the lowest mean squared prediction error of cross-folded test sets（THMs： 43.7（μg/L）^2, HAAs： 233.3（μg/L）^2）. Inclusion of principle components representative of protein-like material significantly decreased prediction error for both THMs and HAAs. Parallel factor analysis did not identify a protein-like component and resulted in prediction errors similar to traditional NOM surrogates as well as fluorescence peak picking. These results support the value of fluorescence excitation–emission matrix–principal component analysis as a suitable NOM indicator in predicting the formation of THMs and HAAs for the water sources studied.

The revitalization of downtown Richmond,Virginia,in the 21st century has been a slow process,beginning in the financial center near the State capitol Building and migrating slowly westward along Broad Street,the traditional retail avenue of the *** by one over the course of the past several years,large,iconic buildings have been rehabilitated for new and exciting *** buildings have long been associated with the history of the city itself:the Miller&Rhoads Department Store,the John Marshall Hotel,the First National Bank Building,and the Hotel Richmond among *** central National Bank(cNB)Building was built at the dawn of the Great Depression and eventually became one of the last Art Deco style skyscrapers remaining in downtown *** location in the neglected western fringe area of Broad Street made it the next logical target for *** Douglas Development purchased the vacant building in 2005,they were buying the crowning piece of architecture that they hoped would become the linchpin project to spur the revitalization of the surrounding *** lofty goal was not without challenges,of course,and it took 8 years to put the project together and start the building’s *** complications inherent in the rehabilitation of any iconic 75-year old building listed on the National Register of Historic Places to suit continued use for contemporary life also clearly came into play.

Modern imaging techniques for preoperative detection of distant metastases in gastric cancer

A substantial portion of patients with newly diagnosed gastric cancer has distant metastases(M1 disease).These patients have a very poor prognosis and it is generally accepted that they should be treated with noncurative intent. Because it dramatically changes prognosis and treatment plans, it is very important to diagnose distant metastases. In this article, the definition, pathways, incidence and sites of distant metastases in gastric cancer are ***, the current performance of imaging in detecting distant metastases in newly diagnosed gastric cancer is outlined and future prospects are discussed.

Osteopontin: a leading candidate adhesion molecule for implantation in pigs and sheep

Osteopontin （OPN; also known as Secreted Phosphoprotein 1, SPP1） is a secreted extra-cellular matrix （EcM） protein that binds to a variety of cell surface integrins to stimulate cell-cell and cell-EcM adhesion and communication. It is generally accepted that OPN interacts with apically expressed integrin receptors on the uterine luminal epithelium （LE） and conceptus trophectoderm to attach the conceptus to the uterus for implantation. Research conducted with pigs and sheep has significantly advanced understanding of the role（s） of OPN during implantation through exploitation of the prolonged peri-implantation period of pregnancy when elongating conceptuses are free within the uterine lumen requiring extensive paracrine signaling between conceptus and endometrium. This is followed by a protracted and incremental attachment cascade of trophectoderm to uterine LE during implantation, and development of a true epitheliochorial or synepitheliochorial placenta exhibited by pigs and sheep, respectively. In pigs, implanting conceptuses secrete estrogens which induce the synthesis and secretion of OPN in adjacent uterine LE. OPN then binds to αvβ6 integrin receptors on trophectoderm, and the αvβ3 integrin receptors on uterine LE to bridge conceptus attachment to uterine LE for implantation. In sheep, implanting conceptuses secrete interferon tau that prolongs the lifespan of cL. Progesterone released by cL then induces OPN synthesis and secretion from the endometrial GE into the uterine lumen where OPN binds integrins expressed on trophectoderm （αvβ3） and uterine LE （identity of specific integrins unknown） to adhere the conceptus to the uterus for implantation. OPN binding to the αvβ3 integrin receptor on ovine trophectoderm cells induces in vitro focal adhesion assembly, a prerequisite for adhesion and migration of trophectoderm, through activation of： 1） P70S6K via crosstalk between FRAP1/MTOR and MAPK pathways; 2） MTOR, PI3K, MAPK3/MAPK1 （Erk1/2） and MAPK14 （p38） signalin
