chronic alcohol consumption is a major risk factor worldwide affecting significantly both mortality and years of life lost （YLL）（1）. ca. 5% of the western world show risky alcohol consumption and in some countries such as china a regional yearly increase of alcohol consumption of over 400% has been observed recently （2,3）. The liver is the major target organ of alcohol. According to the recently published ＇Global Burden of Disease Study 2010, liver cirrhosis and liver cancer are ranked at position 12 and 16 in the global deaths statistics （1）. Thus, in 2010, ca. 1 million people died from liver cirrhosis with one third directly attributable to alcohol. This is a considerable number when comparing with coronary heart disease with 7 million deaths and the leading cause of mortality. In central Europe, liver cirrhosis even ranks at the fourth position in YLL. Hepatocellular cancer （Hcc） is now the most common fatal complication of patients with alcoholic cirrhosis. Moreover, it shows the second fastest increase of all tumors worldwide after kidney tumors and alcohol-associated Hcc ranks on third position after Hccs caused by viral hepatitis B and c.

Background: In patients with pulmonary disease, it is often challenging to distinguish exacerbated pulmonary disease from congestive heart failure(cHF). The impact of B- type natriuretic peptide(BNP) measurements on the management of patients with pulmonary disease and acute dyspnea remains to be defined. Methods: This study evaluated the subgroup of 226 patients with a history of pulmonary disease included in the BASEL Study. Patients were randomly assigned to a diagnostic strategy with(n=119, BNP group) or without(n=107, clinical group) the use of BNP levels provided by a rapid bedside assay. Time to discharge and total cost of treatment were recorded as the primary end points. Results: Baseline characteristics were similar in patients assigned to the BNP and control groups. comorbidity was extensive, including coronary artery disease and hypertension in half of patients. The primary discharge diagnosis was cHF and exacerbated obstructive pulmonary disease in 39% and 33% , respectively. The use of BNP levels significantly reduced the need for hospital admission(81% vs 91% , P=.034). Median time to discharge was 9.0 days in the BNP group as compared with 12.0 days(P=.001) in the clinical group. Median total cost of treatment was ＄ 4841 in the BNP group as compared with ＄ 5671 in the clinical group(P=.008). Inhospital mortality was 8% in both groups. conclusions: cHF is a major cause of acute dyspnea in patients with a history of pulmonary disease. Used in conjunction with other clinical information, rapid measurement of BNP reduced time to discharge and total treatment cost of these patients.

BAcKGROUND Perinatal complications may result in life-long morbidities,among which cerebral palsy(cP)is the most severe motor disability.Once developed,cP is a nonprogressive disease with a prevalence of 1-2 per 1000 live births in developed countries.It demands an extensive and multidisciplinary care.Therefore,it is a challenge for our health system and a burden for patients and their families.Recently,stem cell therapy emerged as a promising treatment option and raised hope in patients and their families.AIM The aim is to evaluate the efficacy and safety of stem cell treatment in children with cP using a systematic review and meta-analysis METHODS We performed a systematic literature search on PubMed and EMBASE to find randomized controlled clinical trials(RcT)investigating the effect of stem cell transplantation in children with cP.After the review,we performed a randomeffects meta-analysis focusing on the change in gross motor function,which was quantified using the gross motor function measure.We calculated the pooled standardized mean differences of the 6-and/or 12-mo-outcome by the method of cohen.We quantified the heterogeneity using the I-squared measure.RESULTS We identified a total of 8 RcT for a qualitative review.From the initially selected trials,5 met the criteria and were included in the meta-analysis.Patients’population ranged from 0.5 up to 35 years(n=282).We detected a significant improvement in the gross motor function with a pooled standard mean difference of 0.95(95%confidence interval:0.13-1.76)favoring the stem cell group and a high heterogeneity(I2=90.1%).Serious adverse events were rare and equally distributed among both intervention and control groups.cONcLUSION Stem cell therapy for cP compared with symptomatic standard care only,shows a significant positive effect on the gross motor function,although the magnitude of the improvement is limited.Short-term safety is present and further highquality RcTs are needed.

Alcoholic liver disease(ALD) and hepatitis c virus(HcV) infection represent, either alone or in combination, more than two thirds of all patients with liver disease in the Western world.This review discusses the epidemiology and combined impact of ALD and HcV on the progres sion of liver disease.ALD and HcV affect the progres sion of liver disease to liver cirrhosis and hepatocellular carcinoma(Hcc) in a synergistic manner.Thus, the risk for Hcc increases f ive times with a daily alcohol con sumption of 80 g;in the presence of HcV it is increased 20fold, and a combination of both risk factors leads to a more than 100fold risk for Hcc development.Alcohol consumption also decreases the response to interferon treatment which is probably due to a lack of compliance than a direct effect on HcV replication.Several molecu lar mechanisms are discussed that could explain the synergistic interaction of alcohol and HcV on disease progression.They include modulation of the immune response and apoptosis, increased oxidative stress via induction of cYP2E1 and the hepatic accumulation of iron.Thus, both HcV and alcohol independently cause hepatic iron accumulation in > 50% of patients probably due to suppression of the liversecreted systemic iron hormone hepcidin.A better understanding of hepcidin regulation could help in developing novel therapeutic approaches to treat the chronic disease in the future.For now, it can be generally concluded that HcVinfect ed patients should abstain from alcohol and alcoholicsshould be encouraged to participate in detoxification programs.

AIM:To develop and validate a transient micro-elastography device to measure liver stiffness(LS) in mice.METHODS:A novel transient micro-elastography(TME) device,dedicated to LS measurements in mice with a range of measurement from 1-170 kPa,was developed using an optimized vibration frequency of 300 Hz and a 2 mm piston.The novel probe was validated in a classical fibrosis model(ccl4) and in a transgenic murine model of systemic amyloidosis.RESULTS:TME could be successfully performed in control mice below the xiphoid cartilage,with a mean LS of 4.4 ± 1.3 kPa,a mean success rate of 88%,and an excellent intra-observer agreement(0.98).Treatment with ccl4 over seven weeks drastically increased LS as compared to controls(18.2 ± 3.7 kPa vs 3.6 ± 1.2 kPa).Moreover,fibrosis stage was highly correlated with LS(Spearman coefficient = 0.88,P ch higher LS values were obtained,reaching maximum values of > 150 kPa.LS significantly correlated with the amyloidosis index(0.93,P concentration of mutant hapoA-□(0.62,P cONcLUSION:Here,we have established the first non-invasive approach to measure LS in mice,and have successfully validated it in two murine models of high LS.

Background:As chordomas are slow growing and locally invasive with high recurrence rates,initial recommendations include complete surgical resection with or without radiation therapy.A large proportion of recurrences occur years after initial resection necessitating lengthy follow-up.The novel biomarker brachyury and the repurposing of pharmaceutical products have the potential to substantially impact long-term recurrence rates.case presentation:A 43-year-old woman presented with an isolated,cervical extraosseous intradural extramedullary chordoma attached to a nerve root underwent a c3-5 laminectomy,c3-5 lateral mass screw instrumentation,and mass resection.All symptoms resolved by the 12-month postoperative follow-up visit.conclusions:This is the first report of an isolated,cervical extraosseous intradural extramedullary chordoma attached to a nerve root,and this case adds to the previous six TypeⅣchordomas in the literature.Unfortunately,the very rare form of extraosseous intradural chordoma is poorly understood:the lack of detailed knowledge in how they are differentiated from other forms of chordoma confounds the development of optimal treatment strategies and follow-up guidelines.

cachexia is frequently described in patients with pancreatic ductal adenocarcinoma(PDAc)and is associated with reduced survival and quality of life.Unfortunately,the therapeutic options of this multi-factorial and complex syndrome are limited.This is due to the fact that,despite extensive preclinical and clinical research,the underlying pathological mechanisms leading to PDAc-associated cachexia are still not fully understood.Furthermore,there is still a lack of consensus on the definition of cachexia,which complicates the standardization of diagnosis and treatment as well as the analysis of the current literature.In order to provide an efficient therapy for cachexia,an early and reliable diagnosis and consistent monitoring is required,which can be challenging especially in obese patients.Although many substances have been tested in clinical and preclinical settings,so far none of them have been proven to have a long-term effect in ameliorating cancer-associated cachexia.However,recent studies have demonstrated that multidimensional therapeutic modalities are able to alleviate pancreatic cancerassociated cachexia and ultimately improve patients’outcome.In this current review,we propose a stepwise and pragmatic approach to facilitate and standardize the treatment of cachexia in pancreatic cancer patients.This strategy consists of nutritional,dietary,pharmacological,physical and psychological methods.
