Surgical Management of Lumbar and Thoracolumbar Spinal Fractures: Indications, Surgical Technique and Evaluation on a Series of 64 Patients Treated with Percutaneous Posterior Osteosynthesis combined with Kyphoplasty or Anterior Arthrodesis

Introduction: Fractures of the lumbar spine and thoracolumbar junction are common in spinal trauma. The aim of this work is to analyze the nature of the indications, the morbidity as well as the results of these treatment regimens. Patients and Methods: A retrospective, single-center study, based on a review of 64 patients with lumbar spine and thoracolumbar junction fractures (T10-L2) without neurological disorders, was collected in the neurosurgery department of the North Hospital and University Hospital (cHU), Marseille over a period of 2 years from January 2015 to December 2016. Posterior percutaneous osteosynthesis were more or less associated with kyphoplasty preceded anterior arthrodesis. clinical and radiological endpoints were collected at least 6 months later. Results: The mean follow-up was 9.5 months (6 - 24). The clinical evaluation found a mean VAS at last follow-up at 14/100 (0 - 30) and an average Oswestry score at the last follow-up at 88%. The initial average vertebral kyphosis went from 13° to 4° at the last follow-up with a correction loss of 1°, an absolute gain of 8°. No postoperative neurological complications were noted in our series. conclusion: The implementation of a two-step therapeutic strategy with anterior reconstruction in Magerl’s lumbar spine or A3.3 thoracolumbar junction fractures allows effective and long-lasting correction of lumbar lordosis and thoracic kyphosis, and obtaining a balanced spine in the sagittal plane. Our functional results are close to normal, with low morbidity and a low complication rate.

Finite element analysis of the effect of interlayer on interfacial stress transfer in layered graphene nanocomposites

Understanding the roles of interlayers in reinforcement efficiencies by layered graphene is very important in order to produce strong and light graphene based nanocomposites. The present paper uses the finite element method to evaluate the interfacial strain transfers and reinforcement efficiencies in layered graphene-polymer composites. Results indicate that the presence of compliant interlayers in layered graphene plays significant roles in the transfers of strain/stress from matrix to graphene and subsequently the reinforcement effectiveness of layered graphene. In general, the magnitude of shear strain transferred onto the rigid graphene decreases as the thickness of the interlayer increases. This trend becomes insignificant as the graphene becomes sufficiently large(s>25,000). The shear strain at the interface of graphene-matrix is also greatly influenced by the interlayer modulus. A stiffer interlayer would result in a higher shear strain transferred on the graphene. The performance of the interlayers is further affected by the property of the composite and the architecture of the layered graphene stack. If a composite contains more graphene phase, the efficiency of reinforcement by a layered graphene becomes improved. If a graphene stack contains more interlayers, the effectiveness of reinforcement at the edges of the graphene becomes negatively affected.

Analytical modeling of effect of interlayer on effective moduli of layered graphene-polymer nanocomposites

Nanocomposites enhanced with two-dimensional, layered graphene fillers are a new class of engineering materials that exhibit superior properties and characteristics to composites with conventional ***, the roles of ＂interlayers＂ in layered graphene fillers have yet to be fully explored. This paper examines the effect of interlayers on mechanical properties of layered graphene polymer *** an effective filler, the fundamental properties（in-plane Young＇s modulus E（L1）, out-of-plane Young＇s modulus E（L2）; shear modulus G（L12）, major Poisson＇s ratio V（L12）） of the layered graphene were computed by using the Arridge＇s lamellar model. The effects of interlayers on effective moduli of layered graphene epoxy composites were examined through the Tandon-Weng model. The properties of the interlayer show noticeable impact on elastic properties of the composites, particular the out-of-plane properties（Young＇s modulus E2 and shear modulus G（12））. The interlayer spacing is seen to have much great influence on properties of the composites. As the interlayer spacing increases from 0.34 nm to 2 nm, all elastic properties of the composites have been greatly decreased.

Skeletal muscle plays a significant role in both local and systemic energy metabolism. The current investigation aims to explore the role of the Bambi gene in skeletal muscle, focusing on its implications for muscle hypertrophy and systemic metabolism. We hypothesize that skeletal muscle-specific deletion of Bambi induces muscle hypertrophy, improves metabolic performance, and activates thermogenic adipocytes via the reprogramming of progenitor of i WAT, offering potential therapeutic strategies for metabolic syndromes. Leveraging the chromatin immunoprecipitation(chIP)-seq and bioinformatics analysis, Bambi gene is shown to be a direct target of HIF2α, which is further confirmed by chIP-qPcR and promoter luciferase assay. Skeletal muscle-specific Bambi deletion led to significant muscle hypertrophy and improved metabolic parameters, even under high-fat diet conditions. This deletion induced metabolic reprogramming of stromal vascular fractions(SVFs) into thermogenic adipocytes, contributing to systemic metabolic improvements, potentially through the secretory ***, mice with skeletal muscle-specific Bambi deletion demonstrate resistance to high-fat diet-induced metabolic disorders, highlighting a potential therapeutic pathway for metabolic syndrome management. Thus, skeletal muscle-specific deletion of Bambi triggers muscle growth, enhances metabolic performance, and activates thermogenic adipocytes. These findings suggest Bambi as a novel therapeutic target for metabolic syndromes, providing new insights into the interaction between muscle hypertrophy and systemic metabolic improvement. The study underscores the potential of manipulating muscle physiology to regulate whole-body metabolism, offering a novel perspective on treating metabolic disorders.

Hydrocephalies and Vestibular Schwannoma of Great Size: Therapeutic Strategies: About a Series of 32 cases

Introduction: The association hydrocephalus and vestibular schwannoma (VS) has been known for a long time. However, there is no therapeutic consensus, especially the place of drainage of cerebrospinal fluid (cSF). We report the result of our experience on the management of this pathology from a group of patients with a high volume VS (Koos IV) and operated consecutively. After reflections based on current literature data, we propose a therapeutic decisional algorithm. Materials and Methods: This is an analytical, retrospective study of 171 patients operated on KOOS IV vestibular schwannoma from January 2003 to December 2016 at the Marseille University Hospital center. Of these, 32 patients with hydrocephalus and stage IV vestibular schwannoma were included. Radio-diagnostic criteria for hydrocephalus were based on Evans’ index, cortical furrow status, and the presence of trans-ependymal resorption. Our sample was divided into 2 groups. The first consisted of patients first operated on their hydrocephalus and secondarily treated with schwannoma surgery (group I);patients who underwent surgical resection of their first-line tumor were group II. Epidemiological, clinical, radiological, therapeutic and monitoring data were analyzed. The comparison of the quantitative variables was made by Fisher’s test. Results: During our study period (13 years), 171 cases of stage IV SV had been operated. The association between hydrocephalus and SV stage IV of Koos accounted for 18.7%. The average age of our patients was 53 years with a sex ratio of 0.7. The clinical picture was primarily composed of otological signs (90.6%), headache (56.3%) and cerebellar involvement (43.8%). The average diameter of VS inponto-cerebellar angle (PcA) was 31.5 mm. The treatment consisted of placing a first shunt of the cSF in 34.4% (group I). Detection of Breaking Waves Using X-Band Pulse Radar in the Nearshore Region

An X-band pulsed Doppler microwave radar has been used to determine the characteristics of breaking waves. Field experiments were conducted at the Shuang-Si estuary in the north of Taiwan in the winter of 2005. Analyses on maxima radar cross section and Doppler frequency shift are done to characterize wave breaking zones. Based on observations of breaking waves, the wave breaking zones are shown to be located at water depths of 1.8 to 2.2 m in the experimental site. In general, the results indicate that a radar system has the potential to delineate the spatial variation of breaking waves clearly and that this is sufficient to achieve a measurement operation for near-shore air-sea interaction events.

New method for rekindling the explosive waves in Maxwellian space plasmas

Our interest is to study the nonlinear wave phenomena in complex plasma constituents with Maxwellian electrons and ions. The main aim is to use a new method known as the(G′/G)method to exhibit the effects of dust charge fluctuations on the evolution of nonlinear waves. The coherent features of the shock and solitary waves along with the generation of high-energy waves have been amplified through the solution of the Korteweg–de Vries–Burgers equation,and the different natures of the waves were found successfully. Results are discussed graphically with the thoughtful choice of typical plasma parameters from different space plasma environments, exactly those found in cosmic dusty plasmas laden in ionospheric auroral region,radial spokes of Saturn's rings, planetary nebulae and solar F-corona region. All conclusions are in good accordance with the actual occurrences and could be of interest to further investigations of space. Moreover, the applicability of the present method is hoped to be a great enhancement by its use as ingenious mechanism used to evaluate the soliton dynamics and Burgers shock waves.

Pushover analysis of reinforced concrete frames considering shear failure at beam-column joints

Since most current seismic capacity evaluations of reinforced concrete （Rc） frame structures are implemented by either static pushover analysis （PA） or dynamic time history analysis, with diverse settings of the plastic hinges （PHs） on such main structural components as columns, beams and walls, the complex behavior of shear failure at beam-column joints （BcJs） during major earthquakes is commonly neglected. This study proposes new nonlinear PA procedures that consider shear failure at BcJs and seek to assess the actual damage to Rc structures. Based on the specifications of FEMA-356, a simplified joint model composed of two nonlinear cross struts placed diagonally over the location of the plastic hinge is established, allowing a sophisticated PA to be performed. To verify the validity of this method, the analytical results for the capacity curves and the failure mechanism derived from three different full-size Rc frames are compared with the experimental measurements. By considering shear failure at BcJs, the proposed nonlinear analytical procedures can be used to estimate the structural behavior of Rc frames, including seismic capacity and the progressive failure sequence of joints, in a precise and effective manner.

Roles of soil-structure interaction and damping in base-isolated structures built on numerous soil layers overlying a half-space

This study examines the roles of soil-structure interaction （SSI）, higher modes, and damping in a base-isolated structure built on multiple layers of soil overlying a half space. closed-form solutions for the entire system, including a superstructure, seismic isolator, and numerous soil layers overlying a half-space, were obtained. The formulations obtained in this study simply in terms of well-known frequencies and mechanical impedance ratios can explicitly interpret the dynamic behavior of a base-isolated structure interacting with multiple soil layers overlying a half-space. The key factors influencing the performance of the isolation system are the damping ratio of the isolator and the ratio of the natural frequency of the fixed-base structure to that of the isolated structure by assuming that the superstructure moves as a rigid body. This study reveals that higher damping in the base isolator is unfavorable to higher mode responses that usually dominate the responses of the superstructure and that the damping mechanism plays an important role in transmitting energy in addition to absorbing energy. It is also concluded that it is possible to design a soft soil layer as an isolation system for isolating vibration energy.

Equivalent series system to model a multiple friction pendulum system with numerous sliding interfaces for seismic analyses

current structural analysis software programs offer few if any applicable device-specifi c hysteresis rules or nonlinear elements to simulate the precise mechanical behavior of a multiple friction pendulum system(MFPS) with numerous sliding *** on the concept of subsystems,an equivalent series system that adopts existing nonlinear elements with parameters systematically calculated and mathematically proven through rigorous derivations is *** aim is to simulate the characteristics of sliding motions for an MFPS isolation system with numerous concave sliding interfaces without prior knowledge of detailed information on the mobilized forces at various sliding *** MFPS with numerous concave sliding interfaces and one articulated or rigid slider located between these interfaces is divided into two subsystems: the fi rst represents the concave sliding interfaces above the slider,and the second represents those below the *** equivalent series system for the entire system is then obtained by connecting those for each subsystem in *** equivalent series system is validated by comparing numerical results for an MFPS with four sliding interfaces obtained from the proposed method with those from a previous study by Fenz and ***,these numerical results demonstrate that an MFPS isolator with numerous concave sliding interfaces,which may have any number of sliding interfaces,is a good isolation device to protect structures from earthquake damage through appropriate designs with controllable mechanisms.
