AIM To evaluate the ability of Pillcamcolon2 to visualize colonic segments missed by incomplete optical colonoscopy(Oc) and to assess the diagnostic *** This prospective multicentre study included 81 patients from nine centres who underwent second-generation colon capsule endoscopy(ccE) following incomplete Oc performed by an experienced gastroenterologist(> 1000 colonoscopies). Patients with stenosis were excluded. According to patient preferences, ccE was performed the following day(protocol A) after staying on clear liquids and 0.75 L Moviprep in the morning or within 30 d after new split-dose Moviprep(protocol B). Boosts consisted of 0.75 L and 0.25 L Moviprep, and phospho-soda was given as a rescue if the capsule was not excreted after seven *** Seventy-four patients were analysed(51% of them in group A; 49% in group B). Bowel cleansing was adequate in 67% of cases, and ccE could visualize colonic segments missed by incomplete colonoscopy in 90% of patients under protocol A and 97% of patients under protocol B(P = 0.35, n.s.). Significant polyps including adenocarcinoma were detected in 24% of cases. Detection rates for all polyps and significant polyps per patient were similar in both protocols. Polyps were found predominantly in the right colon(86%) in segments that were not reached by Oc. Extracolonic findings-such as reflux esophagitis, suspected Barrett esophagus, upper GI-bleeding, gastric polyps, gastric erosions and angiectasia-were detected in eight patients. Pill cam colon2 capsule was retained in the ileum of one patient(1.4%) without symptoms and removed during an uneventful resection for unknown crohn's disease that was diagnosed as the cause of anemia, which was the indication for colonoscopy. ccE was well tolerated. composite materials having lamellar structure are known to have a good combination of high strength and ductility. They are widely used in the fields of automobiles, civil engineering and construction, machines and many other industries. An application of lamellar microstructure for developing ultra-high strength steel wires was studied and discussed. Based on the experimental results, the relationships between the strength increase and microstructure development during the cold wire drawing were studied to reveal the strengthening mechanism. As cold drawing proceeds, the wire strength extremely increases, the microstructure changes from large single crystal lamellar structure to very fine polycrystalline lamellar one which has nano-sized grains, high dislocation density and amorphous regions. From the results obtained, it is concluded that heavy cold drawing technique is an effective method for lamellar composite to get high strength wires. Furthermore, formation process of the best microstructure for producing the ultra-high strength wires was also discussed.

Objective: Benefits of replacement therapy in growth hormone deficiency (GhD) are well documented in younger and middle-aged patients. The aim of our investigation was to prove the benefit of Gh replacement for patients older than 60 years especially in terms of health-related quality of life (hRQoL) of age as well. Design: Data of 743 consecutively recruited patients (394 men, 349 women) with GhD aged 20 - 49 (n = 606) and 60 - 69 (n = 137) years enrolled from KIMS Germany (Pfizer International Metabolic Database) were compared. Treatment effects over the 12 months dose-finding and the subsequent phase up to three years were analysed using mixed models. Serum insulin-like growth factor I (IGF-I), fasting blood glucose, fasting serum total cholesterol and low-density lipoprotein cholesterol (LDL-c) as well as body mass index (BMI) at baseline and at last visit were studied. hRQoL was assessed using the Quality of Life-Assessment of Growth hormone Deficiency in Adults (QoL-AGhDA). Results: For both age groups and genders the IGF-I level and standardized IGF-I increased over the dose-finding phase. In women, the overall QoL-AGhDA score at the baseline examination was 8.7 (95% cI: 7.7 - 9.7) and decreased to 6.3 (95% cI: 5.1 - 7.6) at the end of the dose-finding phase (p he corresponding values were 8.8 (95% cI: 7.8 - 9.8) and 6.4 (95% cI: 5.1 - 7.6;p hout differences between the age groups. The therapy benefit for elderly was supported by the non-impairment after the dose-finding phase. In total cholesterol, LDL-c and fasting blood glucose, no significant changes were detected, whereas an increase in BMI did not differ between age groups. conclusion: We could show positive effects of Gh replacement on hRQoL in patients older than 60 years of age. Therefore, Gh replacement should be considered in elderly GhD adults without difference compared to younger age groups.

The contribution of chronic peripheral inflammation to the pathogenesis of neurodegenerative diseases is an outstanding question. Sustained activation of the peripheral innate and adaptive immune systems occurs in the context of a broad array of disorders ranging from chronic infectious diseases to autoimmune and metabolic diseases. In addition, progressive systemic inflammation is increasingly recognized during aging. Peripheral immune cells could potentially modulate the cellular brain environment via the secretion of soluble molecules. There is an ongoing debate whether peripheral immune cells have the potential to migrate into the brain under certain permissive circumstances. In this perspective, we discuss the possible contribution of chronic peripheral inflammation to the pathogenesis of age-related neurodegenerative diseases with a focus on microglia, the resident immune cells of the brain parenchyma.
