BAcKGrOUND In recent years,there has been an increase in the number of total hip arthroplasty procedures in the younger patient *** active group has higher expectations of their prosthesis in comparison to the older population,and there is a greater physical demand for the *** femoral stems were in-troduced to retain proximal bone stock and joint biomechanics and became more common to implant in this specific ***,the long-term survival and functional outcomes of various short stems are still being investigated in different *** To determine the 5-year survival of the Optimys hip *** This was a prospective multicenter cohort study of 500 patients conducted in two hospitals in the *** patients received the Optimys short stem(Mathys ltd,Bettlach,Switzerland).The primary outcome measure was survival of the hip stem,with revision as the *** secondary outcome measurements included patient-reported outcome measures(PrOMs).Kaplan-Meier analysis was used to calculate the 5-year survival ***-minus-log transformation was performed to calculate the 95%confidence interval(95%cI).Mixed model analyses were performed to assess the course of the PrOMs during the 1st 2 years after *** were modeled separately for the 1st and 2nd years to calculate the yearly change in PrOMs during both follow-up periods with accompanying 95%*** The mean age of the total 500 patients was 62.3 years(standard deviation:10.6)and 202 were male(40%).At a median follow-up of 5.5 years(interquartile range:4.5-6.7),7 patients were deceased and 6 revisions were registered,for infection(n=3),subsidence(n=2)and malposition(n=1).This resulted in an overall 5-year survival of 98.8%(95%cI:97.3-99.5).If infection was left out as reason for revision,a stem survival of 99.4%(95%cI:98.1-99.8)was *** questionnaires were completed by 471 patients(94%),317 patients(63%)completed the 1-year follow-up questionna

chitin powder and chitin-aerogel were prepared from shrimp wastes and used to uptake Y^(3+)from aqueous solutions and concentrate this rare earth element from phosphogypsum(PG).chitin aerogel displays a specific surface area of 945 m^(2)/g,while chitin powder is 3.6 m^(2)/g,which largely influences its adsorption *** the adsorption in synthetic solutions,the effect of pH on Y^(3+)removal is strong for chitin powder *** contrast,no big pH influence was detected for chitin *** interactions and chelation can highlight the proposed mechanism of Y^(3+)on chitin adsorbents for the powder and ***,in addition to these interactions,pore filling/pore diffusion is the main mechanism of Y^(3+)removal in the chitin *** aerogel is efficient in concentrating 8 times the Y^(3+)from PG,a very complex *** complex chitin aerogel-Y^(3+)can be a secondary source of rare earth elements for other applications.

Early Syphilis Presenting as Thickened Penile Plaques With an Embedded recurrent Ulcer (c.an.re redux): A case report

Introduction: The manifestations of syphilis are varied, and serology can establish the diagnosis early, especially in rare cases. We report a case of c.an.re redux (a rare recurrence of primary syphilis), which was embedded within a secondary syphilitic penile skin plaque. There were missed opportunities at earlier diagnosis as serology for syphilis was not ordered. case presentation: A 56-year-old man presented with thickened penile plaques for five months. He reported a small penile ulcer approximately one month prior. There was no history of other skin lesions or rash. However, clinically there was an asymptomatic, indurated ulcer embedded within a plaque which was swab-positive for syphilis by Pcr. A punch biopsy of a plaque was spirochaete-positive using an immunoperoxidase stain. The patient's lesions resolved three weeks after treatment with intramuscular benzathine penicillin. Discussion: Uncommonly reported even in the preantibiotic era, c.an.re redux is now rare. The ulcer usually recurs at or near the site of the original c.an.re, and has similar morphological features. The skin lesions of secondary syphilis can exhibit remarkable morphological variety. The most common skin manifestation, a generalized macular rash, was not present at any time in this case. rather, there were only a couple of nonspecific penile plaques. If biopsy is performed, histologic findings are variable, though typically the inflammatory infiltrate includes plasma cells. A special immunoperoxidase stain can highlight spirochaetes in biopsy sections. conclusion: This case highlights the importance of considering syphilis in the differential diagnosis of persistent, atypical penile lesions and underscores the need for appropriate serological testing in such instances. copyright © 2023 Hospital for Skin Diseases (Institute of Dermatology),

cD4^(+)T cells can"help"or"license" conventional type 1 dendritic cells(cDc1s)to induce cD8^(+)cytotoxic T lymphocyte(cTl)anticancer responses,as proven in mouse *** recently identified cDc1s with a transcriptomic imprint of cD4^(+)T-cell help,specifically in T-cell-infiltrated human cancers,and these cells were associated with a good prognosis and response to PD-1-targeting ***,we delineate the mechanism of cDc1 licensing by cD4^(+)T cells in *** cD4^(+)T cells produce IFNβvia the STING pathway,which promotes MHc-I antigen(cross-)presentation by cDc1s and thereby improves their ability to induce cTl anticancer *** cooperation with cD40 ligand(l),IFNβalso optimizes the costimulatory and other functions of cDc1s required for cTl response ***-I-producing cD4^(+)T cells are present in diverse T-cell-infiltrated cancers and likely deliver“help”signals to cTls locally,according to their transcriptomic profile and colocalization with“helped/licensed”cDcs and tumor-reactive cD8^(+)T *** agreement with this scenario,the presence of IFN-I-producing cD4^(+)T cells in the TME is associated with overall survival and the response to PD-1 checkpoint blockade in cancer patients.

Background The study objective was to test the hypothesis that low crude protein(cP)diet with crystalline amino acids(cAA)supplementation improves lys utilization efficiency for milk production and reduces protein turnover and muscle protein *** lactating multiparous Yorkshire sows were allotted to 1 of 2 isocaloric diets(10.80 MJ/kg net energy):control(cON;19.24%cP)and reduced cP with“optimal”AA profile(OPT;14.00%cP).Sow body weight and backfat were recorded on d 1 and 21 of lactation and piglets were weighed on d 1,14,18,and 21 of *** d 14 and 18,a subset of 9 sows(cON=4,OPT=5)was infused with a mixed solution of 3-[methyl-2H3]histidine(bolus injection)and[13c]bicarbonate(priming dose)first,then a constant 2-h[13c]bicarbonate infusion followed by a 6-h primed constant[1-13c]lysine *** blood and milk sampling were performed to determine plasma and milk lys enrichment,lys oxidation rate,whole body protein turnover,and muscle protein *** Over the 21-d lactation period,compared to cON,sows fed OPT had greater litter growth rate(Pcompared to cON,sows fed OPT had greater efficiency of lys(Plys mammary flux(Ple-body protein turnover efficiency(Pcompared to cON,sows fed OPT tended to have lower whole body protein breakdown rate(P=0.069).Muscle protein breakdown rate did not differ between OPT and cON(P=0.197).conclusion Feeding an improved AA balance diet increased efficiency of lys and reduced whole-body protein turnover and protein *** results imply that the lower maternal N retention observed in lactating sows fed improved AA balance diets in previous studies may be a result of greater partitioning of AA towards milk rather than greater body protein breakdown.

In vivo measurement of NADH fluorescence lifetime in skeletal muscle via -ber-coupled time-correlated single photon counting

Nicotinamide adenine dinucleotide(NADH)is a cofactor that serves to shuttle electrons during metabolic processes such as glycolysis,the tricarboxylic acid cycle,and oxidative phosphorylation(OXPHOS).NADH is autofluorescent,and itsfluorescence lifetime can be used to infer metabolic dynamics in living ***-coupled time-correlated single photon counting(TcSPc)equipped with an implantable needle probe can be used to measure NADH lifetime in vivo,enabling investigation of changing metabolic demand during muscle contraction or tissue *** study illustrates a proof of concept for point-based,minimally-invasive NADHfluorescence lifetime measurement in *** muscle loss(VMl)injuries were created in the left tibialis anterior(TA)muscle of male Sprague Dawley *** lifetime measurements were collected before,during,and after a 30 s tetanic contraction in the injured and uninjured TA muscles,which was subsequently-t to a biexponential decay model to yield a metric of NADH utilization(cytoplasmic vs protein-bound NADH,the A11/A22 ratio).On average,this ratio was higher during and after contraction in uninjured muscle compared to muscle at rest,suggesting higher levels of free NADH in contracting and recovering muscle,indicating increased rates of *** injured muscle,this ratio was higher than uninjured muscle overall but decreased over time,which is consistent with current knowledge of inflammatory response to injury,suggesting tissue regeneration has *** data suggest that-ber-coupled TcSPc has the potential to measure changes in NADH binding in vivo in a minimally invasive manner that requires further investigation.
