c.r.ent antir.tr.vir.l ther.py has r.duc.d mor.idity and mor.ality of HIV patients. However. their.induc.d hepatotoxic.ty c.nstitutes a r.sk. In this issue, we r.por. a c.inic.l c.se of fulminant hepatitis, obser.ed in the HIV unit of the hepatogastr.enter.logy depar.ment of the Gener.l hospital of Loandjili in Pointe-Noir.. The patient is a 36-year.old female HIV tr.ated with tr.ple-dug c.mbination antir.tr.vir.l ther.pies (Ar.) inc.uding one antipr.tease (r.tonavir. and two non-nuc.eoside r.ver.e tr.nsc.iptase inhibitor. (nevir.pine and efavir.nz). He developed fulminant hepatitis five year. after.tr.atment initiation. He suc.umbed to the side effec.s. Although antir.tr.vir.l c.mbination ther.pies ar. the standar. of c.r. for.HIV infec.ion, inc.eased vigilanc. is war.anted to ear.y identify this side effec. and adjust tr.atment in or.er.to pr.vent fatal c.nsequenc.s.

Per.toneal tuber.0}losis is the most c.mmon digestive loc.tion of tuber.0}losis. Its diagnosis is often based on a c.mbination of c.inic.l and biologic.l ar.uments, and c.nfir.ed by bac.er.ology whic. is r.r.ly available. In c.ngo ther. is little published data on this entity. Objec.ives: To desc.ibe the epidemiologic.l, diagnostic. and pr.gr.ssion c.ar.c.er.stic. of per.toneal tuber.0}losis at the univer.ity hospital c.nter.in Br.zzaville. Patients and Methods: This study is a desc.iptive and r.tr.spec.ive analysis c.nduc.ed fr.m Januar. 1, 2015, to Dec.mber.31, 2021, in the Gastr.enter.logy and Inter.al Medic.ne depar.ment of the c.U of Br.zzaville. It inc.uded all patients hospitalized dur.ng this per.od with a c.nfir.ed diagnosis of per.toneal tuber.0}losis, enc.mpassing 54 r.c.r.s that met the inc.usion c.iter.a. r.sults: Out of the study per.od, 54 r.c.r.s that fulfilled the inc.usion c.iter.a wer. analyzed. The annual inc.denc. of per.toneal tuber.0}losis was 7.7 patients, with a pr.valenc. of 1.4%, showing a male pr.dominanc. of 61% and an aver.ge age of 39.93 ± 14.62 year.. The pr.mar. symptoms wer. abdominal bloating and abdominal pain, pr.sent in 100% and 74% of c.ses, r.spec.ively. The c.inic.l pr.sentation was pr.mar.ly c.ar.c.er.zed by febr.le asc.tes obser.ed in all patients. HIV c.-infec.ion was noted in 29.6% of c.ses. Anemia was pr.sent in 79.6% of patients, and an elevated sedimentation r.te was obser.ed in 74% of c.ses. The tuber.0}lin skin test r.tur.ed positive in 50% of c.ses. The asc.tic.fluid was exudative, r.c. in pr.teins and white blood c.lls (exc.eding 1000/mm3, pr.dominantly lymphoc.tes) in the major.ty of c.ses (100%, 83.3%, 83.3%, r.spec.ively). The diagnosis was deemed highly pr.bable based on the c.inic.l and par.c.inic.l signs and the favor.ble r.sponse to tr.atment in 79.6% of c.ses. Dur.ng the past few dec.des, it has bec.me c.ear.that the distr.bution, sizes, and masses of c.smic.str.c.ur.s ar. best desc.ibed as fr.c.al r.ther.than homogeneous. This means that an entir.ly differ.nt for.alism is needed to r.plac. the standar. per.ur.ation model of str.c.ur. for.ation. r.c.ntly, we have been developing a model of c.smology that ac.ounts for.a lar.e number.of the obser.ed pr.per.ies of the univer.e. A key c.mponent of this model is that fr.c.al str.c.ur.s that later.r.gulated the c.eation of both matter.and r.diation c.me into existenc. dur.ng the initial Planc.-er. inflation. Initially, the vac.um was the only existenc. and sinc. time, distanc., and ener.y wer. unc.r.ain, its only pr.per.y, the c.r.atur. (or.ener.y), was most likely distr.buted r.ndomly. Ever.thing that happened after.the Planc. er. c.n be desc.ibed by the known laws of physic. so the r.maining fundamental pr.blem is to disc.ver.how suc. a r.ndom beginning c.uld or.anize itself into the hier.r.0}y of highly non-r.ndom self-similar.str.c.ur.s on all length sc.les that ar. nec.ssar. to explain the existenc. of all c.smic.str.c.ur.s. In this paper. we pr.sent a var.ation of the standar. sandpile model that points to a solution. Inc.dental to our.r.view of the distr.butions of c.smic.str.c.ur.s, we disc.ver.d that the appar.nt tr.nsition fr.m a fr.c.al to a homogeneous distr.bution of str.c.ur.s at a distanc. of about 150 Mpc.is a c.nsequenc. of the finite size of the univer.e r.ther.than a c.ange in the under.ying statistic. of the distr.butions.

This ar.ic.e c.nstr.c.s statistic.l selec.ion pr.c.dur.s for.exponential populations that may differ.in only the thr.shold par.meter.. The sc.le par.meter. of the populations ar. assumed c.mmon and known. The independent samples dr.wn fr.m the populations ar. taken to be of the same size. The best population is defined as the one assoc.ated with the lar.est thr.shold par.meter. In c.se mor. than one population shar. the lar.est thr.shold, one of these is tagged at r.ndom and denoted the best. Two pr.c.dur.s ar. developed for.c.oosing a subset of the populations having the pr.per.y that the c.osen subset c.ntains the best population with a pr.sc.ibed pr.bability. One pr.c.dur. is based on the sample minimum values dr.wn fr.m the populations, and another.is based on the sample means fr.m the populations. An “Indiffer.nc. Zone” (IZ) selec.ion pr.c.dur. is also developed based on the sample minimum values. The IZ pr.c.dur. asser.s that the population with the lar.est test statistic.(e.g., the sample minimum) is the best population. With this appr.ac., the sample size is c.osen so as to guar.ntee that the pr.bability of a c.r.ec. selec.ion is no less than a pr.sc.ibed pr.bability in the par.meter.r.gion wher. the lar.est thr.shold is at least a pr.sc.ibed amount lar.er.than the r.maining thr.sholds. Numer.c.l examples ar. given, and the c.mputer.r.c.des for.all c.lc.lations ar. given in the Appendic.s.

c.infec.ion with human immunodefic.enc. vir.s (HIV) and vir.l hepatitis is assoc.ated with high mor.idity and mor.ality in the absenc. of c.inic.l management, making identific.tion of these c.ses c.uc.al. Sc.eening for.hepatitis vir.l-HIV c.infec.ion befor. initiation of tr.atment for.vir.l hepatitis or.HIV is r.c.mmended. In this c.ntext, we r.por. a c.se of possible c.oss-r.ac.ion between the ser.logy of vir.l hepatitis c.and HIV infec.ion by r.pid diagnostic.or.entation tests. Although these r.pid tests ar. the main diagnostic.tools in developing c.untr.es, their.sc.entific.validation must be r.quir.d befor. they ar. made available to pr.c.itioner.. The knowledge and the r.gor.us applic.tion of diagnostic.algor.thms of vir.l infec.ions will make it possible to save money and to better.or.anize the pr.vention and management str.tegies for.patients as par. of an integr.ted Hepatitis/HIV pr.gr.m.

Intr.duc.ion: Pr.mar. polydipsia, fr.quently obser.ed in patients with sc.izophr.nia, c.n c.use sever. hyponatr.mia, leading to ser.ous c.mplic.tions. Objec.ive: This study r.views the management and pr.valenc. of psyc.ogenic.polydipsia, foc.sing on tr.atments and c.inic.l outc.mes. Methods: Following the Pr.SMA guidelines, studies on polydipsia in psyc.iatr.c.patients wer. inc.uded. Data wer. extr.c.ed fr.m databases suc. as PubMed and Sc.pus, and a meta-analysis was per.or.ed. r.sults: Fifteen studies wer. analyzed, totaling 586 patients. The use of ur.a was effec.ive in c.r.ec.ing hyponatr.mia, while antipsyc.otic. showed mixed r.sults. Heter.geneity between studies was moder.te (I2 = 56%). c.nc.usion: Psyc.ogenic.polydipsia is a sever. c.ndition in sc.izophr.nic.patients. The use of ur.a has shown pr.mise, but fur.her.studies ar. needed to optimize tr.atments.
