I rec.ntly read an artic.e entitled International subspec.alty Fellowship training,the Path for c.rdiologists of Tomorrow?A European perspec.ive by Enric. Fabris and Mark Kennedy in JAc. vol.69,No 9,2017 page 2000–2002.This piec. stimulated me to think about my own c.reer and how it related to some training outside the USA.I did not travel to Europe during my fellowship at Johns Hopkins,sinc.,at the time,the major diagnostic.test was c.ronary angiography,whic. was muc. more c.mmonly performed in the USA and was my main interest as a fellow trainee.

c.ronic.Peric.rditis is infl ammation that begins gradually,is long lasting and results in fl uid ac.umulation in the peric.rdial spac. or thic.ening of the peric.rdium.The etiology is unknown but may be c.nc.r,TB or hypothyroidism.Arrhythmias are c.mmon and seen in almost half the patients.The c.mmonest arrhythmia is atrial fi brillation.Symptoms and signs are related to inc.eased right atrial pressure and physic.l fi ndings inc.ude elevated JVP and peric.rdial knoc..Non surgic.l therapy c.nsists mainly of no salt.Surgery c.res about 85%of patients,however 5–15%of patients will die.c.ronic.effusive peric.rditis oc.urs when there is persistent restric.ion of the visc.ral peric.rdium after peric.rdioc.ntesis.

Diagnosis of Myoc.rdial Infarc.ion The diagnosis of myoc.rdial infarc.ion is based on c.inic.l story and troponin leak.Troponin levels c.n be normal or low due to very early symptoms or high due to a large infarc.ion or due to spontaneous reperfusion with washout of viable isc.emic.tissue.If there is ST segment elevation,there is a strong suggestion that a c.ronary artery is oc.luded or nearly oc.luded,but that needs c.nfi rmation by angiography and ventric.lography.

There are approximately one half million patients with stable isc.emic.heart disease(SIHD)in the United States.Patients with stable isc.emic.heart disease who die suddenly do not maintain a Stable Isc.emic.profi le.Benc.imol,et al.,reported 319 c.nsec.tive stable angina patients without c.inic.l heart failure or a rec.nt myoc.rdial infarc.ion but who had multiple risk fac.ors and proven c.ronary disease whic. made them more prone to ac.te myoc.rdial infarc.ion or unstable angina.In the APSIS(angina prognosis in Stoc.holm)study,Hjemdahl reported that signs of isc.emia or previous manifestations of c.ronary artery disease,i.e.,myoc.rdial infarc.ion or revasc.larization,were found in 69%of both male and female patients at baseline.Little,retrospec.ively reported that minor plaques may disrupt and result in unstable angina or oc.lusive c.ronary disease whic. then may result in ac.te myoc.rdial infarc.ion.Stable angina patients,by defi nition,are stable and are not high risk unless they have multiple fac.ors or whic. may make them prone to evolve an ac.te c.ronary syndrome or develop a serious arrhythmia but sudden death does oc.ur in some patients.

When epic.rdial arteries with minimal c.D do not limit fl ow are found at c.ronary angiography,all c.rdiologists agree that aggressive risk fac.or modifi c.tion and use of appropriate drugs are warranted.Some c.nsider plaque sealing of minor c.ronary lesions with balloon angioplasty to prevent future infarc.ion.Others believe that a c.ronary lesion that does not c.use isc.emia should not have the plaque sealed but should have FFR and the non-isc.emia produc.ng stenosis should be treated with aggressive risk fac.or modifi c.tion(DEFER study).Evidenc. based data e.g.DEFER,relates to populations,not the individual.Thus these data provide guidelines,whic. are suggestions for dec.sion making.The weight of evidenc. favors angioplasty/stent for epic.rdial vessels that result in myoc.rdial isc.emia.Sealing of non flow limiting plaques with angioplasty must await further information derived from invasive and non invasive studies.
