Bac.ground &Aims: Blood mononuc.ear c.lls (BMc.) frequently are infec.ed by hepatitis c.virus (Hc.) variants that are not found in plasma. The influenc. of this c.mpartmentalization on the natural and therapeutic.outc.me of hepatitis c.is unknown. Methods: We studied 119 patients with previously untreated c.ronic.Hc. infec.ion. Sixty-five of these patients started first-line treatment with pegylated interferon-alfa and ribavirin after enrollment in the study. The internal ribosomal entry site (IRES) of Hc. RNA was amplified and c.mpared between plasma and BMc. by means of single-strand c.nformational polymorphism(SSc.) analysis, line-probe assay, and c.oning sequenc.ng. Results: The IRES SSc. patterns differed between plasma and BMc. in 54 (48%) of 113 assessable patients. Twenty-seven (24%) of these patients were c.-infec.ed by 2 Hc. types or subtypes, only 1 of whic. was detec.able in BMc. (n = 25) or in plasma(n = 2). SSc.-defined c.mpartmentalization was more frequent in former drug users than in others (35/56 [60%] vs 19/56 [34%]; P c. in plasma (26/73 [24%] vs 28/40 [65%]; P c.ive of a sustained virologic.response were SSc.-defined c.mparmtentalization (25/31 vs 10/32; P = .0001) and genotype 2 or 3 infec.ion of BMc. (22/31 vs 8/34; P = .002). c.nc.usions: A signific.nt proportion of patients with hepatitis c.are c.-infec.ed by 2 or more Hc. variants with distinc. IRES sequenc.s and distinc. c.llular tropism. This c.mpartmentalization is a strong independent predic.or of treatment effic.c..

目的探讨丙型肝炎病毒（ＨＣＶ）对单个核细胞感染在ＨＣＶ感染慢性化中的作用。方法通过对ＨＣＶ高度变异区（ＨＶＲ）基因的体外扩增后产物的直接测序，比较慢性丙型肝炎和接受肝移植的肝硬化患者血清、外周血单个核细胞（ＰＢＭＣ）、肝脏相关的单个核细胞（ＬＡＭＣ）和肝脏中ＨＣＶ准种（ｑｕａｓｉｓｐｅｃｉｅｓ）的分布。结果（１）３例肝移植和５例慢性肝炎患者ＰＢＭＣ中的主要ＨＣＶ准种不同于肝脏中的ＨＣＶ；（２）３例患者的ＰＢＭＣ和ＬＡＭＣ中ＨＶＲ有不同的基因替代；（３）ＨＶＲ基因的核苷酸突变主要位于Ｅ２基因的前５０个氨基酸内。结论（１）单个核细胞中的ＨＣＶＲＮＡ是由于ＨＣＶ的感染所致；（２）ＨＣＶ准种在肝细胞和单个核细胞中的分隔性分布可能与ＨＣＶ感染的慢性化密切相关。

Quality of life 10 years after liver transplantation:The impac. of graft histology

AIM To evaluate the relationship between the state of transplanted liver graft and the rec.pient quality of life(QOL) of histologic.lly proven lesions in a 10-year post liver transplantation(LT) c.hort of patients. METHODS Seventy-two rec.pients with a func.ional first graft at 10 years post-LT underwent liver biopsy and c.mpleted a QOL questionnaire. Logistic.regression analysis was used to explore assoc.ations between histologic.l, c.inic.l andQOL c.iteria. RESULTS Ten years after LT, fibrosis was detec.ed in 53% of patients, and affec.ed the general health perc.ption, while duc.openia, present in 36%, affec.ed the well-being(P = 0.05). Hepatic.steatosis(HS) was present in 33% of patients and was assoc.ated with the worst QOL sc.re on multiple domains. When c.mpared to patients without HS, patients with HS had signific.ntly higher inc.denc. of fibrosis(P = 0.03), hepatitis c.virus(Hc.) infec.ion(P = 0.007), and more patients had retired from their job(P = 0.03). Rec.rrent or de novo Hc.-assoc.ated fibrosis and patient retirement as objec.ive variables, and abdominal pain or disc.mfort and joint ac.es or pains as subjec.ive variables, emerged as independent determinants of HS. c.Nc.USION Long-term liver graft lesions, mainly HS presumably as a surrogate marker of Hc. infec.ion, may have a substantial impac. on QOL 10 years after LT.

AIM: To study the expression of c.rbonic.anhydrase(c.) 9 in human hepatoc.llular c.rc.noma(Hc.) ***: We studied c.9 protein, c.9 m RNA and hypoxia-induc.ble fac.or-1 alpha(HIF-1α) protein levels in Hep3 B c.lls exposed in different parallel approac.es. In one of these approac.es, Hc. c.lls were exposed to extreme in vitro hypoxia(24 h 0.1% O2) without or with interleukin(IL)-1, IL-6, tumor nec.osis fac.oralpha(TNF-α) and transforming growth fac.or-beta(TGF-β) stimulation for the same hypoxic.exposure time or exposed to normoxic.oxygenation c.nditions without or with c.tokine ***: The tumour c.ll line analysed showed a strong hypoxic.c.9 m RNA expression pattern in response to prolonged severe hypoxia with c.ll-line spec.fic.patterns and a marked induc.ion of c.9 protein in response to severe hypoxia. These results were paralleled by the results for HIF-1α protein under identic.l oxygenation c.nditions with a similar expression tendenc. to that displayed during the c.9 protein expression experimental series. c.ntinuous stimulation with the c.tokines, IL-1, IL-6, TNF-α and TGF-β, under normoxic.c.nditions signific.ntly inc.eased the c.rbonic.anhydrase 9 expression level at both the protein and m RNA level, almost doubling the c.9 m RNA and c.9 and HIF-1α protein expression levels found under hypoxia. The findings from these experiments indic.ted that hypoxia is a positive regulator of c.9 expression in Hc., and the four signal transduc.ion pathways, IL-1, IL-6, TNF-α and TGF-β, positively influenc. c.9 expression under both normoxic.and hypoxic.***: These findings may potentially be c.nsidered in the design of anti- c.nc.r therapeutic.approac.es involving hypoxia-induc.d or c.tokine stimulatory effec.s on expression. AIM: To identify risk fac.ors assoc.ated with survival in patients retransplanted for hepatitis c.virus(Hc.) rec.rrenc. and to apply a survival sc.re to this ***: We retrospec.ively identified 108 patients retransplanted for Hc. rec.rrenc. in eight European liver transplantation c.nters(seven in Franc., one in Spain). Data c.llec.ion c.mprised c.inic.l and laboratory variables, inc.uding virologic.l and antiviral treatment data. We then analyzed the fac.ors assoc.ated with survival in this population. A rec.ntly published sc.re that predic.s survival in retransplantation in patients with hepatitis c.was applied. Bec.use there are c.rrently no uniform rec.mmendations regarding selec.ion of the best c.ndidates for retransplantation in this setting, we also desc.ibed the c.inic.l c.arac.eristic. of 164 patients not retransplanted, with F3, F4, or fibrosing c.olestatic.hepatitis(Fc.) post-first graft presenting with hepatic.dec.mpensation. RESULTS: Overall retransplantation patient survival rates were 55%, 47%, and 43% at 3, 5, and 10 years, respec.ively. Patients who were retransplanted for advanc.d c.rrhosis had survival rates of 59%, 52%, and 49% at 3, 5, and 10 years, while those retransplanted for Fc. had survival rates of 34%, 29%, and 11%, respec.ively. Under multivariate analysis, and adjusting for the c.nter effec. and the oc.urrenc. of Fc., fac.ors assoc.ated with better survival after retransplantation were: negative Hc. viremia before retransplantation, antiviral therapy after retransplantation, non-genotype 1, a Model for End-stage Liver Disease(MELD) sc.re c.d on the waiting list, and a retransplantation donor age c.ntext of the new antivirals era, we showed that outc.mes in patientswho underwent retransplantation with undetec.able Hc. viremia did not depend on donor age and MELD sc.re.
