We enrolled 245 subjec.s, c.mprising 65 subjec.s with ga.lstones (36 women, 29 men) a.d 180 c.ntrols (79women a.d 101 men without ga.lstones). Body ma.s index, wa.st c.rc.mferenc., blood pressure, pla.ma.insulin, a.d serum lipids a.d lipoproteins levels were mea.ured. Insulin resista.c. wa. c.lc.la.ed by homeosta.is model a.sessment. Unc.nditiona. logistic.regressiona.a.ysis (univa.ia.e a.d multiva.ia.e) wa. used to c.lc.la.e the risk of ga.lstone disea.e a.soc.a.ed with the presenc. of a. lea.t three of the c.iteria.(a.ult Trea.ment Pa.el Ⅲ). a.a.yses were a.justed for a.e a.d sex.REsULTs: a.ong 245 subjec.s, meta.olic.syndrome wa. present in 40% of ga.lstone disea.e subjec.s, c.mpa.ed with 17.2% of the c.ntrols, a.justed by a.e a.d gender (odds ra.io (OR) = 2.79; 95%c., 1.46-5.33; P = 0.002),a.dose-dependent effec. wa. observed with ea.0} c.mponent of meta.olic.syndrome (OR = 2.36, 95%c., 0.72-7.71;P = 0.16 with one c.mponent a.d OR = 5.54, 95%c.,1.35-22.74; P = 0.02 with four c.mponents of meta.olic.syndrome). Informa.ion c.nter (1966-Ma.c. 2006), Libra.y, Informa.ion sc.enc. & Tec.nology a.stra.0}s (1967-Ma.c. 2006), c.c.ra.e Da.a.a.e of systema.ic.Reviews, Da.a.a.e of a.stra.0}s of Reviews of Effec.s (1994-2006), disserta.ions in ProQuest a.d Firstsea.c. da.a.a.es. Ma.ua. sea.c.es were ma.e in the a.stra.0}s from meetings of the a.eric.n Ga.troenterologic.l a.soc.a.ion (1999-2006), a.d the a.eric.n a.soc.a.ion for the study of Liver Disea.es (2003-2005). studies were retrieved using the following selec.ion c.iteria. (1) c.inic.l tria.s using insulin sensitizers in subjec.s with Na.LD, (2) a.ult pa.ients, (3) published a. full ma.usc.ipts or a.stra.0}s, a.d (4) English, spa.ish, Germa., a.d Frenc. la.gua.es only. Da.a.were a.stra.0}ed independently by two reviewers following sta.da.dized proc.dures. a.fa.0}-to- fa.0} c.mpa.ison of da.a.wa. c.nduc.ed to ensure the c.mpleteness a.d relia.ility of the a.stra.0}ion proc.ss. REsULTs: Nine studies were inc.uded, six using metformin a.d three using thia.olidinediones. Only two studies were pla.0}bo-c.ntrolled tria.s. The media.sa.ple size for a.l studies wa. 18 subjec.s. In the pla.0}bo-c.ntrolled tria.s, metformin improved insulin resista.c. ma.kers a.d liver func.ion tests, but not histologic.l sc.res. 背景:目前对肝移植患者胆管括约肌疾病的认识严重不足。本研究旨在描述基于最新修订的罗马Ⅳ标准,肝移植患者乳头狭窄(Ps)和胆管括约肌功能障碍(FBsD)的发生率和临床结局。方法:筛选2003年1月至2019年12月间接受内镜逆行性胆胰管造影(ERc.)的肝移植患者,从电子病历和内镜数据库中提取其临床资料及内镜发现,并收集ERc.检查时及ERc.术后1个月的实验室检查结果和临床结局。结果:在1,307例肝移植患者中,336例接受了849次ERc.。13例(1.0%)符合罗马Ⅳ的Ps诊断标准[既往称为Oddi括约肌功能障碍(ODs)Ⅰ型],14例(1.0%)符合FBsD诊断标准(既往称为sOD Ⅱ型)。13例Ps患者和10例FBsD患者接受了胆管狭窄成形术,术后1个月,分别有85%、61%和92%的Ps患者总胆红素(P=0.019)、γ谷氨酰转移酶(P=0.087)和碱性磷酸酶(P=0.003)下降,而在FBsD患者,上述指标下降的患者比例分别为50%(P=0.721)、70%(P=0.013)和80%(P=0.093)。所有最初疑似BFsD的14例患者在随访中均调整了最终诊断。结论:肝移植术后Ps的发生并不常见,发生率仅1%。我们的数据并不支持肝移植术后FBsD的诊断。狭窄成形术治疗肝移植术后Ps患者安全有效。

Body ma.s index (BMI), fa.ting pla.ma.leptin, insulin, serum lipid, a.d lipoprotein levels were mea.ured. Insulin resista.c. wa. c.lc.la.ed by homeosta.is model a.sessment (HOMa.IR). Unc.nditiona. logistic.regression a.a.ysis (univa.ia.e a.d multiva.ia.e)stra.ified by BMI wa. used to c.lc.la.e the risk of GD.REsULTs: The multiva.ia.e c.nditiona. regression a.a.ysis revea.ed a.model for those pa.ients with BMI ＜30. The selec.ed va.ia.les in the model were HOMa.IR index with OR = 1.31, P= 0.02 a.d leptin higher tha. media. with OR = 2.11, P= 0.05. In the stra.um of BMI ≥30, we did not find a.useful model.c.Nc.UsION: We c.nc.uded tha. insulin resista.c. a.d the development of GD a.pea.s to be a.soc.a.ed with serum leptin levels in subjec.s with overweight, but not in obese subjec.s with simila. meta.olic.profiles.
