Adrenomedullin in cirrhotic and non-cirrhotic portal hypertension

AIM:Adrenomedullin (ADM) is a potent vasodilator peptide. ADM and nitric oxide (NO) are produced in vascular endothelial *** ADM level has been linked to hyperdynamic circulation and arterial vasodilatation in cirrhotic portal hypertension (cPH).The role of ADM in non-cirrhotic portal hypertension (NcPH) is unknown,plasma ADM levels were studied in patients with NcPH,compensated and decompensated cirrhosis in order to determine its contribution to portal hypertension (PH) in these groups. METHODS:There were 4 groups of *** 1 consisted of 27 patients (F/M:12/15) with NcPH due to portal and/or splenic vein thrombosis (mean age:41±12 years),group 2 consisted of 14 patients (F/M:6/8) with compensated (child-Pugh A) cirrhosis (mean age:46±4), group 3 consisted of 16 patients (F/M:6/10) with decompensated (child-Pugh c) cirrhosis (mean age:47±12). Fourteen healthy subjects (F/M:6/8) (mean age:44±8) were used as controls in Group *** level was measured by *** was determined as nitrite/nitrate level by chemoluminescence. RESULTS:ADM level in Group 1 (236±61.4 pg/mL) was significantly higher than that in group 2 (108.4±28.3 pg/mL) and group 4 (84.1±31.5 pg/mL) (both Pcantly higher than that in group 2 (19.8±2.8 μmol/L) and group 4 (16.9±1.6 μmol/L) but was lower than that in Group 3 (39±3.6 μmol/L) (for all three Pcorrelation was observed between ADM and NO levels (r=0.827,PcONcLUSION:Adrenomedullin and NO levels were high in both non-cirrhotic and cirrhotic portal hypertension and were closely correlated,Adrenomedullin and NO levels increased proportionally with the severity of cirrhosis,and were significantly higher than those in patients with NcPH. Background:This study was undertaken to evaluate demographics,clinical manifestations,laboratoryfi ndings and outcomes of children with inflammatory bowel disease(IBD)in ***:We analyzed the medical records of 127 children diagnosed with IBD(under 18 years old)between January 2004 and January 2012 in 8 pediatric gastroenterology ***:Of the 127 patients,90(70.9%)suffered from ulcerative colitis(Uc),29(22.8%)from crohn's disease(cD),and 8(6.3%)from IBD *** mean age of the 127 patients was 11.6±4.1 years,and 11.8%of the patients were below 5 years *** the patients,49.6%were male,and males were more predominant in patients with cD than in those with Uc(72.4%vs.42.2%,P=0.008;a male/female ratio of 2.62 in cD,P=0.0016).Approximately one fifth of the patients had extra-intestinal manifestations and 13.3%of the patients had associated *** manifestations and associated diseases were more common in early onset disease[P=0.017,odds ratio(OR)=4.02;P=0.03,OR=4.1].Of the patients,15%had normal laboratory parameters including anemia,high platelet count,hypoalbuminemia,hypoferritinemia,and high sedimentation *** under receiver operation characteristics was used to predict pancolitis in patients with *** values of c-reactive protein,sedimentation rate and pediatric ulcerative colitis activity were 0.61(P=0.06),0.66(P=0.01)and 0.76(P=0.0001),***(4.4%)patients with Uc underwent colectomy,andfi nally two(1.5%,95%confidence interval:0-3.7%)patients died from primary disease or ***:IBD is an increasing clinical entity in *** of IBD are similar to those in other populations,but prospective multicenter studies are needed to analyze the true incidence of IBD in Turkish children.

Does Anti-Vascular Endothelial Growth Factor Treatment Delay Photoreceptor Maturation in Preterm Infants?

Purpose: To investigate the effects of intravitreal anti-vascular endothelial growth factor (VEGF) for treatment of type-1 or aggressive posterior retinopathy of prematurity (ROP) on photoreceptor maturation in preterm infants using spectral domain optical coherence tomography (SD-OcT). Methods: A study group of 36 patients with ROP treated with anti-VEGF therapy was compared to a control group of 80 patients with ROP. All patients had undergone SD-OcT imaging at least once between 40 and 44 weeks post-menstrual age (PMA). Two imaging specialists evaluated the images for ellipsoid zone (EZ) development, cystoid macular edema (cME), sub-retinal fluid (SRF) at the fovea, and cone outer segment tips (cOST). The outer nuclear layer thickness and the distance from the EZ to the central fovea was measured. Results: EZ development was observed in 18 (50%) patients in the study group and 68 (85%) in the control group (p = 0.002). cME was observed in 17 (47.2%) patients in the study group and 17 (21.2%) in the control group. The distance between the EZ and central fovea was 868 ± 440 μm in the study group and 725 ± 380 μm in the control group (p = 0.02). cOST was not observed in any patient. Regression analysis indicated that anti-VEGF treatment and cME affect EZ development. conclusions: Early photoreceptor maturation is correlated with preterm birth week but not ROP stage. Anti-VEGF treatment delays photoreceptor maturation in preterm infants.

Endoscopic and histopathological evaluation of acute gastric injury in high-dose acetaminophen and nonsteroidal anti-inflammatory drug ingestion with suicidal intent

AIM： To evaluate endoscopic and histopathologic aspects of acute gastric injury due to ingestion of high-dose acetaminophen and nonsteroidal antiinflammatory drugs （NSAIDs） with respect to some risk factors and patient characteristics. METHODS： The study group consists of 50 patients admitted to emergency department with high dose analgesic ingestion （group Ⅰ ） with suicidal intent. Thirty patients with or without mild complaints of dyspepsia （group Ⅱ） were selected as the control group. The study group was stratified according to the use of type and number of analgesics. Endoscopic findings were evaluated according to the Lanza score （LS）, expressing the severity of the gastroduodenal damage and biopsies according to a scoring system based on histopathologic findings of acute erosive gastritis. RESULTS： Gastroduodenal damage was significantly more severe in group Ⅰ compared to group Ⅱ （P 〈 0.01）. The LS was similar in both groups Ⅰ a and Ⅰb. However LS was significantly higher in patients who had ingested multiple NSAIDs （group Ⅰ c） compared to other patients （P 〈 0.01）. The LS was correlated to age （P 〈 0.01） and total amount of drug ingested （P 〈 0.05） in group Ⅰ ; but it was not correlated with Helicobacter pylori （H pylori） infection or duration of exposure （P 〉 0.05）. The biopsy score （BS） was higher in group Ⅰ than group Ⅱ （P 〈 0.01）, and higher in group Ⅰb than group Ⅰa （P 〈 0.05）. cONcLUSION： The histopathologic damage was more severe among NSAID ingesting patients compared to those ingesting only acetaminophen and there is no significant difference in the endoscopic findings between the groups. There is no significant difference in the LS between the groups. This lack of significance is remarkable in terms of the gastric effects of highdose acetaminophen.
