The task of using the machine learning to approximate the mapping x→Σi=1^d xi^(2)with Xi∈[-1,1]seems to be a trivial *** the knowledge of the separable structure of the function,one can design a sparse network to represent the function very accurately,or even *** such structural information is not available,and we may only use a dense neural network,the optimization procedure to find the sparse network embedded in the dense network is similar to finding the needle in a haystack,using a given number of samples of the *** demonstrate that the cost(measured by sample complexity)of finding the needle is directly related to the Barron norm of the *** only a small number of samples are needed to train a sparse network,the dense network trained with the same number of samples exhibits large test loss and a large generalization *** control the size of the generalization gap,we find that the use of the explicit regularization becomes increasingly more important as d *** numerically observed sample complexity with explicit regularization scales as G(d^(2.5)),which is in fact better than the theoretically predicted sample complexity that scales as 0(d^(4)).Without the explicit regularization(also called the implicit regularization),the numerically observed sample complexity is significantly higher and is close to 0(d^(4.5)).

Survivors of child Sexual Abuse (cSA) can develop Post-Traumatic Stress Disorder (PTSD), alterations in the prefrontal cortex, changes in electroencephalographic (EEG) activity, and lower performance on working memory tasks. The aim of this study was to characterize brain electrical correlations in girls with PTSD secondary to cSA during a working memory task based on recognizing emotional facial stimuli. Girls aged 8 - 16 years old were evaluated: 12 with PTSD secondary to cSA, and 12 healthy girls with no history of abuse. EEG activity during a working memory task with emotional stimuli was recorded, and the inter- and intra-hemispheric correlations that assessed the functional connectivity among different cortical regions were analyzed. The PTSD group showed lower performance than controls on the working memory task while watching happy faces, while the EEG of this group showed greater intrahemispheric correlation among frontal areas and between frontal and posterior cortical regions. Also, the PTSD group had lower interhemispheric correlations between posterior temporal areas. The higher intrahemispheric correlation in the PTSD group could indicate that those girls used more brain areas when performing the task, likely because it required greater effort. The lower inter-posterior temporal correlation could be attributed to a reduction of the corpus callosum.

Background: Proximal gastrectomy is a rarely performed procedure but a feasible option in benign tumors and malignant neoplasms in the proximal third of the stomach since novel options of reconstructions are available nowadays with fewer long-term sequels. Methods: Report of three cases of proximal gastric gastrointestinal stromal tumors (GIST), with a description of its presentation, histological characteristics, and follow-up after being treated with proximal gastrectomy with cobra head reconstruction. Results: case 1: A 62-year-old woman with epigastric pain of four months with endoscopic evidence of a cardia GIST. The surgery was performed without complications. The histopathological report confirmed a fusiform GIST of 3.2 × 3 × 2.5 cm, stage IA. no adjuvant treatment was considered. An esophagogram showed no evidence of reflux and no stenosis, and no disease recurrence after 40-months follow-up. case 2: A 66-year-old woman with dyspepsia and a palpable tumor in the epigastrium. The surgery was performed without complications. The pathology report confirmed a proximal gastric GIST of 13 × 8 × 7 cm, staged II. She received adjuvant treatment with imatinib for 36 months without recurrence and no reflux or stenosis. case 3: A 55-year-old woman with intermittent hematemesis and right subscapular pain. The surgery was performed without complications. The histopathological report concluded that a GIST tumor of the cardia, 1.4 × 1.2 cm, staged IA. She was left in observation. At a 6-month follow-up, she does not report dysphagia, reflux, or stenosis, and no locoregional recurrence. conclusions: Open and laparoscopic proximal gastrectomy is a safe therapeutic option for GIST. Furthermore, the reconstruction of the esophagus-gastro anastomosis in the cobra head after proximal gastrectomy is feasible and secure in our population, with good functional results in a short follow-up.

Adult hippocampal neurogenesis is a finely tuned process regulated by extrinsic factors. neuroinflammation is a hallmark of several pathological conditions underlying dysregulation of neurogenesis. In animal models, lipopolysaccharide(LPS)-induced neuroinflammation leads to a neurogenic decrease mainly associated to the early inflammatory response. However, it is not well understood how the neuroinflammatory response progresses over time and if neurogenesis continues to be diminished during the late neuroinflammatory response. Moreover, it is unknown if repeated intermittent administration of LPS along time induces a greater reduction in neurogenesis. We administered one single intraperitoneal injection of LPS or saline or four repeated injections(one per week) of LPS or saline to young-adult mice. A cohort of new cells was labeled with three 5-bromo-2-deoxyuridine injections(one per day) 4 days after the last LPS injection. We evaluated systemic and neuroinflammation-associated parameters and compared the effects of the late neuroinflammatory response on neurogenesis induced by each protocol. Our results show that 1) a single LPS injection leads to a late pro-inflammatory response characterized by microglial activation, moderate astrocytic reaction and increased interleukin-6 levels. This response correlates in time with decreased neurogenesis and 2) a repeated intermittent injection of LPS does not elicit a late pro-inflammatory response although activated microglia persists. The latter profile is not accompanied by a continued longterm hippocampal neurogenic decrease. Hereby, we provide evidence that the neuroinflammatory response is a dynamic process that progresses in a milieu-dependent manner and does not necessarily lead to a neurogenic decrease, highlighting the complex interaction between the immune system and neurogenesis.

Background:Hepatectomy in patients with large tumor load may result in postoperative liver failure and associated complications due to excessive liver parenchyma *** two-stage hepatectomy(TSH)and associating liver partition and portal vein ligation for staged hepatectomy(ALPPS)technique are possible solutions to this *** liver metastases(cRLM)is the most frequent indication,and there is a need to assess outcomes for both techniques to improve surgical and long-term oncological outcomes in these ***:A single-center retrospective study was designed to compare TSH with ALPPS in patients with initially unresectable bilateral liver tumors between January 2005 and January *** was performed from January 2012 onwards as the technique of ***-term overall survival(OS)and disease-free survival(DFS)were evaluated as primary outcome in cRLM *** morbidity,mortality and liver growth in all patients were also ***:A total of 38 staged hepatectomies were performed:17 TSH and 21 *** resection rate was 76.5%(n=13)in the TSH group and 85.7%(n=18)in the ALPPS group(P=0.426).Overall major morbidity(clavien-Dindo≥3 a)(stage 1+stage 2)was 41.2%(n=7)in TSH and 33.3%(n=7)in ALPPS patients(P=0.389),and perioperative 90-day mortalities were 11.8%(n=2)vs.19.0%(n=4)in each group,respectively(P=0.654).Intention-to-treat OS rates at 1 and 5 years in cRLM patients for TSH(n=15)were 80%and 33%,and for ALPPS(n=17)76%and 35%,*** rates at 1 and 5 years were 36%and 27%in the TSH group vs.33%and 27%in the ALPPS group,***:ALPPS is an effective alternative to TSH in bilateral affecting liver tumors,allowing higher resection rate,but patients must be carefully *** cRLM patients similar long-term OS and DFS can be achieved with both techniques.
