clinical outcomes of tricuspid valve repair accompanying left-sided heart disease

AIM To determine whether the need for additional tricuspid valve repair is an independent risk factor when surgery is required for a left-sided heart *** One hundred and eighty patients(68 ± 12 years,79 males) underwent tricuspid *** proportionalhazards regression model for multivariate analysis was performed for variables found significant in univariate *** Tricuspid regurgitation etiology was functional in 154 cases(86%),organic in 16 cases(9%),and mixed in10 cases(6%),*** mortality at 30 days was 11.7%.Mean follow-up was 51.7 mo with survival at 5 years of 73.5%.Risk factors for mortality were acute endocarditis [hazard ratio(HR) = 9.22(95%cI:2.87-29.62),P chemic heart disease requiring myocardial revascularization [HR = 2.79(1.26-6.20),P = 0.012],and aortic valve stenosis [HR = 2.6(1.15-5.85),P = 0.021].Significant predictive factors from univariate analyses were double-valve replacement combined with tricuspid annuloplasty [HR = 2.21(1.11-4.39),P = 0.003] and preoperatively impaired ejection fraction [HR = 1.98(1.04-3.92),P = 0.044].However,successful mitral valve repair showed a protective effect [HR = 0.32(0.10-0.98),P = 0.046].Additionally,in instances where tricuspid regurgitation required the need for concomitant tricuspid valve repair,mortality predictor scores such as Euroscore 2 could be shortened to a simple Euroscore-tricuspid comprised of only 7 *** explanation may lie in the fact that significant tricuspid regurgitation following leftsided heart disease represents an independent risk factor encompassing several other factors such as pulmonary arterial hypertension and *** Tricuspid annuloplasty should be used more often as a concomitant procedure in the presence of relevant tricuspid regurgitation,although it usually reveals an overly delayed correction of a left-sided heart disease.

Post-operative protein supplementation following orthopaedic surgery: A systematic review

Decreased mechanical loading after orthopaedic surgery predisposes patients to develop muscle *** purpose of this review was to assess whether the evidence supports oral protein supplementation can help decrease postoperative muscle atrophy and/or improve patient outcomes following orthopaedic surgery.A systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and MetaAnalysis(PRISMA).PubMed(MEDline),Embase,Scopus,and Web of Science were searched for randomized controlled trials that assessed protein or amino acid supplementation in patients undergoing orthopaedic *** investigators independently conducted the search using relevant Boolean *** outcomes included functional or physiologic measures of muscle atrophy or *** studies including 611 patients(224 males,387 females)were *** studies evaluated protein supplementation after AcL reconstruction(AcLR),3 after total hip arthroplasty(THA),5 after total knee arthroplasty(TKA),and 3 after surgical treatment of hip *** supplementation showed beneficial effects across all types of *** primary benefit was a decrease in muscle atrophy compared to placebo as measured by muscle cross sectional *** authors also demonstrated improved functional measures and quicker achievement of rehabilitation *** supplementation has beneficial effects on mitigating muscle atrophy in the postoperative period following AcLR,THA,TKA,and surgical treatment of hip *** effects often correlate with improved functional measures and quicker achievement of rehabilitation *** research is needed to evaluate long-term effects of protein supplementation and to establish standardized population-specific regimens that maximize treatment efficacy in the postoperative period.

Development of a Simple Extraction Method for Tetracycline Analogues from Bovine Milk

Tetracycline and analogues are among the most used antibiotics in the dairy industry. Besides the therapeutic uses, tetracyclines are often incorporated into livestock feed as growth promoters. A considerable amount of antibiotics is released unaltered through milk from dairy animals. The presence of antibiotic residues in milk and their subsequent consumption can lead to potential health impacts, including cancer, hypersensitivity reactions, and the development of antibiotic resistance. Thus, it is important to monitor residual levels of tetracyclines in milk. The purpose of this study is to develop a quick and simple method for simultaneously extracting five tetracycline analogues from bovine milk. Specifically, five tetracycline analogues: chlortetracycline (cTc), demeclocycline (DEM), doxycycline (Dc), minocycline (Mc), and tetracycline (Tc) were simultaneously extracted from milk using trifluoroacetic acid. Subsequently, the extracted analogues were separated by reverse-phase high-performance liquid chromatography (RP-HPLc) and detected at 355 nm using UV/Vis. calibration curves for all five tetracycline analogues show excellent linearity (r2 value > 0.99). Percent recovery for Mc, Tc, DEM, cTc, and Dc were: 31.88%, 96.91%, 151.29, 99.20%, and 85.58% respectively. The developed extraction method has good precision (RSD creening test.

In July 2023,the center of Excellence in Respiratory Pathogens organized a two-day workshop on infectious diseases modelling and the lessons learnt from the covid-19 *** report summarizes the rich discussions that occurred during the *** workshop participants discussed multisource data integration and highlighted the benefits of combining traditional surveillance with more novel data sources like mobility data,social media,and wastewater *** advancements were noted in the development of predictive models,with examples from various countries showcasing the use of machine learning and artificial intelligence in detecting and monitoring disease *** role of open collaboration between various stakeholders in modelling was stressed,advocating for the continuation of such partnerships beyond the pandemic.A major gap identified was the absence of a common international framework for data sharing,which is crucial for global pandemic ***,the workshop underscored the need for robust,adaptable modelling frameworks and the integration of different data sources and collaboration across sectors,as key elements in enhancing future pandemic response and preparedness.

context: Breast cancer is a high-mortality disease. Early detection has considerably reduced mortality in developed countries. Objectives: To assess the knowledge, attitudes and practices of congolese women in Kinshasa on breast cancer, their levels and to identify factors associated with insufficient knowledge, negative attitudes and non-beneficial practices. Material and Methods: This was a descriptive cross-sectional study with analytical aims, carried out from June 1 to September 30, 2023, in Kinshasa. It involved 1170 female respondents, aged 18 to 65, who agreed to take part in the survey. Data were collected using a pre-established questionnaire. Data analysis was performed using SPSS software version 20.0. The study variables were socio-demographics, morbid history, knowledge, attitudes and practices of respondents regarding breast cancer. Statistical measures and tests used were Mean ± standard deviation, median, proportions, logistic regression Backward stepwise and p Results: 60% had insufficient knowledge, 75% had negative attitudes and 80% had non-beneficial practices. The risk factors common to insufficient knowledge, negative attitudes and non-beneficial practices towards breast cancer were low level of education, membership of revivalist or Muslim churches, and primiparity. conclusion: 60% of congolese women in Kinshasa have insufficient knowledge about breast cancer, 75% have negative attitudes and 80% have non-beneficial practices, with a positive association between insufficient knowledge and negative attitudes on the one hand, and insufficient knowledge and non-beneficial practices on the other. In this context, increasing the population’s literacy and access to information are essential.

目的探究浙江省某县县域医共体内部抗菌药物管理政策和措施实施中的经验和问题。方法研究团队于2019年10月及2020年7月赴浙江省某县开展现场调查,通过目的性抽样收集医共体内两家医疗机构的抗菌药物相关数据进行描述性分析,并对四家医疗机构中的15名关键知情人进行访谈,了解医共体内抗菌药物管理制度与抗菌药物使用状况。结果案例县县域医共体内机构已成立架构清晰的抗菌药物管理系统;设有前置审方、事后点评、绩效奖惩等管理措施;近年来抗菌药物使用相关指标总体呈下降趋势,县医院门诊、住院抗菌药物使用率均达到国家标准,但县医院住院患者抗菌药物使用强度、乡镇卫生院门诊抗菌药物使用率仍高于国家标准。结论案例县依托县域医共体制度优势,促进医疗机构抗菌药物自上而下管理,管理组织架构清晰,措施较为全面,注重过程管理,动态调整考核指标;但仍需加大对临床药师等专业技术人员的培养,完善抗菌药物管理信息系统。

We study the long time formation of rarefaction waves appearing in balance laws by means of singular perturbation methods. The balance laws are non standard because they contain a variable u that appears only in the flux terms. We present a concrete example occurring in flow of steam, nitrogen and water in porous media and an abstract example for a class of systems of three equations. In the concrete example the zero-order equations resulting from the expansion yield a type of conservation law system called compositional model in Petroleum Engineering. In this work we show how compositional models originate from physically more fundamental systems of balance laws. Under appropriate conditions, we prove that certain solutions of the system of balance laws decay with time to rarefaction wave solutions in the compositional model originating from the system of balance laws.

Tidal triggering of seismicity in the region of Palu, central Sulawesi, Indonesia

This paper investigates the correlation between tidal stress and earthquakes for periods ranging from hours to months in the limited zone of the Palu region(central Sulawesi,Indonesia).Through Schuster and binomial tests,we examined the relation between the seismicity(time density of seismic events)and tidal potential arising from the Moon and Sun,using all tidal components simultaneously and focusing on the estimation of specific *** results show significant correlations between the seismicity and tidal potential for S2(0.5 d)and O1(1.075 d)tidal components in the case of solely isolated earthquake events,particularly for shallow ***,there is a strong relationship between aftershocks and tidal components larger than the Mf period(13.661 d).Finally,the analysis of the temporal variation of the earthquake-tide relation reveals an optimal correlation for about six years before the 2018 great Palu *** correlation becomes insignificant afterwards.
